[Choice of anaesthesia for translumbar aortography (author's transl)].

In a retrospective study development and complications of anaesthetic procedures for translumbar aortography (TLA) were investigated in 370 patients. In a prospective study the influence of general anaesthesia and spinal anaesthesia for the same diagnostic procedure was evaluated in 32 patients. There was no difference as regards. Forced Vital Capacity, Peak Flow, Forced Exspiratory Volume (1 sec. abs. and rel.) neither before and after TLA, nor between spinal- and regionalanaesthesia. Bronchopulmonary complications intra- and immediately postoperatively appeared more often in general anaesthesia, whereas allergenic reactions were registered more often in regional anaesthesia. Cardiocirculatory complications were about the same in both groups. The advantages and disadvantages of both procedures are discussed.

[Electrostimulation anaesthesia and neurolept anaesthesia for cholecystectomy. A comparative study (author's transl)].

In 26 patients, undergoing cholecystectomy under electrostimulation anaesthesia (ESA) (n = 14) and neurolept anesthesia (NLA) (n = 12) the effects on consumption of anaesthetic agents, mean arterial pressure, heart rate, blood gases, lung function, serum cortisol and awareness are compared. Consumption of anaesthetic agents was 21.6 per cent in ESA compared to 100 per cent in NLA. Mean arterial pressure, heart rate and serum cortisol were elevated in ESA compared to NLA, probably due to elevated stress. Awareness 4 hours after anaesthesia was higher in ESA, probably due to less persistence of anaesthetics. Blood gases 1 hour after operation were slightly better after ESA, as also were forced vital capacity and peak flow one day after operation. The results are discussed.

[Diagnostic value of sonography (B-scan) in the upper abdominal region].

Minimal risk, easy applicability and high efficiency of ultrasonography, makes this method a valuable screening procedure in upper abdominal diagnostics. Case history, physical examination and laboratory diagnostics should be followed by ultrasound procedures as the first directed examination. This review intends to evaluate the efficacy of ultrasonography in different gastroenterological, oncological, hematological and angiological diseases of the upper abdomen.

Fast and slow myosin within single skeletal muscle fibres of adult rabbits.

There is good evidence for the coexistence of different myosin types both in developing muscles and in Purkinje cells from adult chicken hearts. In skeletal muscle fibres of adult animals, however, coexistence of fast (FM) and slow (SM) myosin has only been demonstrated after long-term electrical stimulation. The term 'promiscuity' has recently been coined to describe the coexistence of different myosin isoenzymes within a single fibre. Using novel, refined immunological methods we demonstrate here the presence of both FM and SM within single fibres of the musculus tibialis anterior of adult rabbits. Essentially identical results were also obtained with other muscles. Our findings imply that the genes coding for FM and SM can be expressed simultaneously within the same cell throughout an animal's entire life, and not only during development or after artificial electrical stimulation.

[Differences in postoperative metabolism in reference to pre- and postoperative initiation of total parenteral nutrition. I].

We investigated the postoperative metabolism of patients undergoing gastric resection by beginning total parenteral nutrition pre- (group 1) and postoperatively (group 2). A third group remained fasting until 3 h after the surgical intervention. Because of the high serum glucose concentrations and the high glucose losses in the 24-h urine on the operation day in group 1 and 2 only the base glucose requirement (150--250 g/24/h) should be given in the early postoperative period. The low blood glucose concentration in group 3 and the elevated lactate values in group 2 underline this recommandation. Compared with group 2 and 3, group 1 had low free fatty acid concentrations of 250 micronVal/l because of the continuous pre-, intra-, and postoperative infusion. Compared with normal metabolic conditions, the free fatty acids do not seem to have the same insulin-antagonizing effect in the posttraumatic state. The fall of cholesterol after the surgical trauma reached its maximum after 12--15 h and amounted to about 40 mg% in the first two groups. Albumin and total protein fell continuously in group 1 and 2. The electrophoresis in group 1 showed a rise of alpha 1- and alpha 2-globulin to more than double the initial value, the beta-globulin showed only slight changes, the gamma-globulin dropped only slightly up to the 4th postoperative day. As a sign of an augmented catabolism the serum urea concentration rose during the postoperative state. Group 1 and 2 had a favourable nitrogen balance. The postoperative bilirubin rise could be held lower in group 1 compared to group 2.

The demonstration of antibody specificity by a new technique. The gel electrophoresis-derived enzyme-linked immunosorbent assay (GEDELISA) and its application to antibodies specific for feline leukemia virus.

By combining the high resolution of sodium dodecylsulfate polyacrylamide gel electrophoresis with the sensitivity of enzyme-linked immunosorbent assay (ELISA) antibodies specific for different feline leukemia virus components are characterized. Based on the same principle, Concanavalin A binding sites of FeLV components are also detected.

Identification of immunoreactive sites in bovine parathyroid cells to antibodies raised against the NH2-terminal sequence of parathyroid hormone.

The NH2-terminal sequence of bovine parathyroid hormone (1-84) was localized with different immunocytochemical methods on the light and electron microscopic level in bovine parathyroid glands and in isolated bovine parathyroid parenchymal cells. The peroxidase labeled staphylococcal protein A and the peroxidase anti-peroxidase method were found to be advantageous for light and electron microscopic localization, respectively. Reaction product was found light microscopically in the cytoplasma of the parenchymal cells and electron microscopically largely over the secretion granules of the parenchymal cells. The immunoreactive sites were subsequently identified to represent only intact parathyroid hormone (1-84) by gel electrophoresis derived enzyme linked immunosorbent assay.

[The influence of sedation with diazepam and flunitrazepam during regional anaesthesia upon postoperative pulmonary performance (author's transl)].

In 32 patients between 53 and 86 years of age, undergoing transurethral prostatectomy, the influence of intraoperative sedation with Diazepam (5-10 mg) and Flunitrazepam (0,4-0,8 mg) on postoperative forced vital capacity, forced exspiratory volume (1 sec.) and peakflow, were measured, compared to placebo. We could not find a depression of these ventilatory parameters, in the three groups except peak-flow after sedation with Flunitrazepam in the evening after operation (p less than or equal to 0,05). We conclude that sedation during regional anesthesia does not impair the most important advantage of local anesthesia, the minor effect on ventilation, compared with general anesthesia.
