De novo-developed T cells have compromised response to existing alloantigens: using Ld-specific transgenic 2C T cells as tracers in a mouse heart transplantation model.

In this study, the phenotype, TCR signaling events, and function of T cells developed de novo during adulthood in the presence of extrathymic alloantigen were investigated. C57BL/6 mice(H-2b) were first transplanted heterotopically with BALB/c hearts (H-2d) and treated with rapamycin for 2 wk to create a tolerant status. Three weeks postoperation, the mice were whole body irradiated and transplanted with bone marrow cells from 2C mice, which are transgenic for TCR, and most of their T cells are Ld-specific CD8 cells. The 2C T cells developed de novo in the C57BL/6 mice were not able to reject the heart allograft. No clonal deletion, TCR down-regulation, or CD8 down-regulation was found in the tolerized 2C T cells. There was no characteristic phenotype of these cells in terms of CD25, ICAM-1, CD44, and MEL-14 expression. Early TCR signaling events such as intracellular calcium concentration flux, tyrosine phosphorylation, Lck and Fyn kinase activities, and Lck and Fyn protein levels in the tolerized 2C T cells were comparable to their normal counterparts, but the tolerized T cells were defective in IL-2 production and proliferation upon H-2d alloantigen stimulation in vitro. Exogenous IL-2 could not reverse the compromised proliferation. The results of this study indicate that during adulthood, the de novo-developed T cells become tolerant to extrathymic Ag without clonal deletion. These newly minted T cells are functionally defective although they are indistinguishable from normal T cells in phenotypes and in some early signaling events.

Enhancement of thrombolysis in vivo without skin and soft tissue damage by transcutaneous ultrasound.

Previous studies have shown that transcutaneous ultrasound enhances thrombolysis by streptokinase in animals in vivo; however, skin and soft tissue damage induced by ultrasound energy has been a major limitation. The objective of this study was to examine the efficacy of thrombolysis and damage to skin and soft tissues using a newly designed concentrated ultrasound system with a cooling manifold. Using a rabbit model with iliofemoral arterial thrombotic occlusions, 15 pairs of arteries were randomized to receive ultrasound treatment or no ultrasound treatment. Streptokinase (25,000 unit/kg) was given intravenously. Skin temperature was maintained at 25-33 degrees C when ultrasound energy was applied. The serum level of creatine kinase, lactate dehydrogenase, red blood cell counts, and platelet counts were checked at baseline, after thrombus induction, and after ultrasound treatment. Fifteen of fifteen (100%) iliofemoral arteries were angiographically recanalized after ultrasound treatment. In contrast, only 1/15 (6.7%) contralateral arteries were patent after 1 hour. After the subsequent hour with heparin the patency was 14/15 in the ultrasound treated group and 3/15 in the control group. Histologically, the patent arteries had only minimal focal mural thrombus, whereas the angiographically occluded arteries had occlusive thrombi. There was no histologic evidence of ultrasound induced damage to overlying skin, soft tissues, or arteries. In addition, there was no significant rise of creatine kinase, lactate dehydrogenase, or decrease in red blood cell counts and platelet counts induced by ultrasound. In conclusion, transcutaneous concentrated ultrasound which significantly enhances streptokinase induced thrombolysis in vivo can be delivered without concomitant tissue damage. This simple combination therapy has clinical potential for safely treating patients with arterial or venous thromboses.

Noninvasive in vivo clot dissolution without a thrombolytic drug: recanalization of thrombosed iliofemoral arteries by transcutaneous ultrasound combined with intravenous infusion of microbubbles.

BACKGROUND: Previous in vivo studies have shown that microbubbles not only enhance the effectiveness of thrombolytic agents in the presence of ultrasound but may also augment clot dissolution without thrombolytic drugs. METHODS AND RESULTS: The objective of this study was to examine the efficacy of arterial clot disruption by a noninvasive, nonlytic approach with intravenous administration of perfluorocarbon-exposed sonicated dextrose albumin (PESDA) and transcutaneous delivery of ultrasound alone. Pairs of iliofemoral arteries in 10 rabbits were randomized to receive transcutaneous ultrasound treatment or no ultrasound treatment after an acute artery thrombotic occlusion and intravenous PESDA infusion. Five arteries from 3 additional rabbits served as controls (ultrasound alone). All 10 iliofemoral arteries treated with PESDA + ultrasound were recanalized by angiography after ultrasound treatment. None of the 10 contralateral arteries treated with PESDA alone and none of the 5 arteries treated with ultrasound alone were patent after 1 hour. D-Dimer levels did not change after intravenous PESDA + ultrasound-mediated reperfusion. CONCLUSIONS: In vivo arterial clot dissolution can be achieved with intravenous microbubbles and transcutaneous ultrasound delivery alone. This technique has potential for clinical application in patients with acute arterial and venous thrombotic occlusions.

Role of proteasomes in T cell activation and proliferation.

The role of proteasomes in T cell activation, proliferation, and apoptosis was investigated using a proteasome-specific inhibitor lactacystin (LAC). Inhibition of the proteasome activity by LAC repressed the mitogen-induced T cell proliferation. The proteasome activity was definitively required for the T cells to progress from the G0 to S phase. It was necessary to optimize the progress from the G1/S boundary to the G2/M phase, but not for the progress from the G2/M phase to the next G1 phase. Probably as a result of a blockage of cell cycle progress, the cycling, but not the resting, T cells underwent apoptosis when treated with LAC. Mechanistically, we have found that cyclin-dependent kinase-2 (CDK2) and the cyclin E-associated kinase (largely CDK2), but not CDK4, in the G1 phase were strongly inhibited by LAC. This could be an important mechanism for the proteasome to regulate the cell cycle. The degradation of cyclin E in the late G1 and early S phases was dependent on the proteasome, although it was unlikely that this accounted for the observed inhibition of T cell proliferation. There was a reduced decay of p27Kip1 in the late G1 phase when the proteasome activity was suppressed, and this might be a contributing mechanism for the observed inhibition of CDK2 activity. Interestingly, p21Cip1 was up-regulated during the G1 phase, and the up-regulation was inhibited by LAC. Our study shows that the proteasome plays pivotal roles in regulating T cell activation and proliferation, and its effect is probably exerted through multiple mechanisms.

Catecholamines potentiate amyloid beta-peptide neurotoxicity: involvement of oxidative stress, mitochondrial dysfunction, and perturbed calcium homeostasis.

Oxidative stress and mitochondrial dysfunction are implicated in the neuronal cell death that occurs in physiological settings and in neurodegenerative disorders. In Alzheimer's disease (AD) degenerating neurons are associated with deposits of amyloid beta-peptide (A beta), and there is evidence for increased membrane lipid peroxidation and protein oxidation in the degenerating neurons. Cell culture studies have shown that A beta can disrupt calcium homeostasis and induce apoptosis in neurons by a mechanism involving oxidative stress. We now report that catecholamines (norepinephrine, epinephrine, and dopamine) increase the vulnerability of cultured hippocampal neurons to A beta toxicity. The catecholamines were effective in potentiating A beta toxicity at concentrations of 10-200 microM, with the higher concentrations (100-200 microM) themselves inducing cell death. Serotonin and acetylcholine were not neurotoxic and did not modify A beta toxicity. Levels of membrane lipid peroxidation, and cytoplasmic and mitochondrial reactive oxygen species, were increased following exposure to neurons to A beta, and catecholamines exacerbated the oxidative stress. Subtoxic concentrations of catecholamines exacerbated decreases in mitochondrial energy charge and transmembrane potential caused by A beta, and higher concentrations of catecholamines alone induced mitochondrial dysfunction. Antioxidants (vitamin E, glutathione, and propyl gallate) protected neurons against the damaging effects of A beta and catecholamines, whereas the beta-adrenergic receptor antagonist propanolol and the dopamine (D1) receptor antagonist SCH23390 were ineffective. Measurements of intracellular free Ca2+ ([Ca2+]i) showed that A beta induced a slow elevation of [Ca2+]i which was greatly enhanced in cultures cotreated with catecholamines. Collectively, these data indicate a role for catecholamines in exacerbating A beta-mediated neuronal degeneration in AD and, when taken together with previous findings, suggest roles for oxidative stress induced by catecholamines in several different neurodegenerative conditions.

Cooling System Permits Effective Transcutaneous Ultrasound Clot Lysis In Vivo Without Skin Damage.

Previous in vivo studies have shown that transcutaneous ultrasound enhances clot dissolution in the presence of either streptokinase or microbubbles. However, ultrasound-induced skin damage has been a major drawback. The objective was to evaluate the effect of a cooling system to prevent the skin damage that has heretofore been associated with transcutaneous low-frequency, high-intensity ultrasound clot dissolution. After thrombi were induced in both iliofemoral arteries in 15 rabbits, streptokinase (25,000 U/kg) was given intravenously and dodecafluoropentane was injected slowly (2 mL/15 min) through an infusion catheter into the abdominal aorta. One iliofemoral artery was randomized to receive ultrasound treatment, and the contralateral artery was treated as a control (receiving streptokinase and dodecafluoropentane alone). In six rabbits (group 1), the skin below the ultrasound transducer was protected by the use of a balloon cooling system, and in the other nine rabbits (group 2), ultrasound was used without a cooling system. Seven of nine (78%) arteries treated without the cooling system, and six of six (100%) arteries treated with the cooling system were angiographically recanalized after ultrasound + streptokinase + dodecafluoropentane treatment. Thermal damage was present in the skin and soft tissues of all nine rabbits treated without a cooling system. However, the skin and soft tissues were grossly and histologically normal in the six rabbits in which the transcutaneous ultrasound was used with the cooling system. Low-frequency, high-intensity ultrasound energy can be delivered transcutaneously for clot dissolution without concomitant tissue damage when coupled with the use of a cooling system to prevent thermal injury.

Par-4 is a mediator of neuronal degeneration associated with the pathogenesis of Alzheimer disease.

Prostate apoptosis response-4 (Par-4) is a protein containing both a leucine zipper and a death domain that was isolated by differential screening for genes upregulated in prostate cancer cells undergoing apoptosis. Par-4 is expressed in the nervous system, where its function is unknown. In Alzheimer disease (AD), neurons may die by apoptosis, and amyloid beta-protein (A beta) may play a role in this. We report here that Par-4 expression is increased in vulnerable neurons in AD brain and is induced in cultured neurons undergoing apoptosis. Blockade of Par-4 expression or function prevented neuronal apoptosis induced by Ab and trophic factor withdrawal. Par-4 expression was enhanced, and mitochondrial dysfunction and apoptosis exacerbated, in cells expressing presenilin-1 mutations associated with early-onset inherited AD.

Clinical research on the therapeutic effect of the electro-acupuncture treatment in patients with depression.

Electroacupuncture (EA) stimulation has been found to influence the brain (norepinephrine metabolism in experimental animals). Preliminary clinical research has shown that EA treatment is as effective as amitriptyline for patients with depression. In this study, two consecutive clinical studies on the treatment of depression with EA are conducted. The first study was double blind placebo controlled, in which 29 depressed inpatients were recruited. Patients were randomly divided into three groups: EA + placebo; amitriptyline; and EA + amitriptyline. They received EA and/or amitriptyline treatment for 6 weeks. The Hamilton Rating Scale for Depression, Clinical Global Impression and ASBERG scales for the side effect of antidepressants were used to evaluate the therapeutic efficacy and side effects. Based on the results and research protocol of the first study, a multi-centered collaborative study was conducted, in which 241 inpatients with depression were recruited. Patients were randomly divided into two treatment groups: the EA + placebo and the amitriptyline groups. The results from both studies showed that the therapeutic efficacy of EA was equal to that of amitriptyline for depressive disorders (P > 0.05). Electro-acupuncture had a better therapeutic effect for anxiety somatization and cognitive process disturbance of depressed patients than amitriptyline (P < 0.05). Moreover, the side effects of EA were much less than that of amitriptyline (P < 0.001). The article suggested that EA treatment was an effective therapeutic method for depressive disorders. Particularly, it was a treatment of choice for depressed patients who were unable to comply with the classic tricyclic antidepressants because of their anticholinergic side effects. The possible mechanism of EA treatment is discussed.

[Clinical analysis of early complications in 132 low body weight infants performed open-heart surgery].

A clinical analysis of early complications in 132 low body weight infants undergoing open-heart surgery is reported. There were a higher risk of early complication associated with severe illness and/or experienced a longer duration time of cardiopulmonary bypass and endotracheal tube. The total complication rate was 42.4% and the major three complications were low cardiac output, acute respiratory distress syndrome and problems from endotracheal tube (occlusion and/or in proper position of tube). The author emphasized the importance of management of endotracheal tube after operation.

[A longitudinal study on growth model and velocity of term small for gestation age].

To probe into the vegetal pattern and find out the key period of promoting normal growth of term small for gestational age (TSGA), a longitudinal study on growth model and velocity of TSGA was conducted from Jan 1993 to June 1997. The body weight and length of 150 children of TSGA (57 boys, 93 girls) and 152 children as controls (58 boys, 94 girls) were measured from the 1st month to 36th month. The growth model was analysed by multilevel models. The result showed that there was a difference between the growth models of TSGA and controls. During 8 and a half months (center month), the weight, length of controls were 1.19 kg, 3.46 cm greater than those of TSGA; the weight, length of boys were 0.34 kg, 1.08 cm more than those of girls respectively. The growth velocity of TSGA was similar to that of controls. The maximal growth velocity was observed during the first 6 monthes after birth. The order of growth velocity from high to low was 1 mo. > 2 mo. > 3 mo. in length and weight, showing that children of TSGA have their own growth model. Their growth velocity should be monitored attentively. If low velocity appears one should search for the cause and adopt apropriate measures to ensure their growth in accordance with their "own track".

[Total cavo-pulmonary connection for complex congenital heart disease].

Between April 1994 and January 1997, 10 patients with complex congenital heart disease were treated with total cavo-pulmonary connection(TCPC). They included 7 males and 3 females, aged 2-17 years(mean 10.1 years). These cases were single ventricle in 5, tricuspid atresia in 4, severe Ebstein anomaly in 1. For construction of the right atrial lateral tunnel, Gore-Tex patch was used in 9 cases and autologous right atrial wall in another, 4 of the 10 patients underwent TCPC without aortic crossclamping. There were no operative deaths. The follow-up is 2-36 months(mean 12.3 months). All of these patients did well and were in NYHA class I. The advantages, operative indication and technique of TCPC and the experience of improvement of surgical results are briefly discussed in this paper.

[Study of experimental mixed bone marrow transplantation].

OBJECTIVE: To explore strategies for preventing graft versus host disease (GVHD) while preserving graft versus leukemia (GVL) effects in allogeneic bone marrow transplantation(BMT). METHODS: Allogeneic bone marrow and spleen cells mixed with syngeneic spleen/bone marrow cells were transplanted into lethally irradiated inbred mice. The mortality of the recipient mice from GVHD, the number of CFU-S, the origin of the bone marrow cells responsible for the reconstitution at different time after transplantation and the proliferative reaction against the stimulating cells from the syngeneic or allogeneic mice were observed and compared with those in the conventional syngeneic and allogeneic BMT. L615 leukemia model was used to evaluate the GVL effect of mixed BMT. RESULTS: Lethal GVHD was effectively prevented by the syngeneic spleen cells which showed no harmful effect on the CFU-S reconstitution. The medium survival time of the mixed transplanted L615 mice was significantly prolonged. CONCLUSION: The grafts composed of allogeneic bone marrow and spleen cells mixed with syngeneic spleen cells, make the recipient mice have transient GVL effects resulted from the MHC mismatched immunocompetent cells and alleviated the severe GVHD of allogeneic BMT.

[Clinical study of pulmonary encephalopathy with integrated traditional Chinese and Western medicine].

OBJECTIVE: To explore the clinical effectiveness of integrated traditional Chinese and Western medicine (TCM-WM) on pulmonary encephalopathy. METHODS: Eight three patients of pulmonary encephalopathy were allocated randomly into the TCM-WM group and the control group. The therapeutic method for the control group including continuous low flow oxygen intake with mechanical ventilation available when necessary, antiasthmatics and expectorants improve ventilation, sufficient and combined intravenous infusion of antibiotics, and prescription of diuretics for dehydration under the therapeutic principle of mild, small dosage, combined and intermittent in use. For the TCM-WM group, besides above-mentioned medication, Chinese medicine of clear up the Heat and toxin, tranquilizing, restore consciousness and induce resuscitation, bactericiding and expelling their toxin. The anti-pulmonary-encephalopathy injection was given as soon as possible. RESULTS: The total effective rate of the TCM-WM group reached 83.0% and was higher than that of the control group (63.9%) significantly (P < 0.05). Time for restoring consciousness in the former was also shorter than that of the latter significantly (P < 0.01). CONCLUSION: This study provided both method and experience for the treatment of pulmonary encephalopathy with TCM-WM.

[Studies on the biological and clinical characteristics of acquired pneumonia caused by K. planticola].

OBJECTIVE: To investigate the characteristics of biological, and clinical epidemiology of acquired pneumonia caused by a new type of Klebsiella, K. planticola. METHOD: 9 strains of K. planticola were isolated from respiratory samples of patients, in vitro, biological and serologic identification were done, in vivo, infected animal models were also evaluated. Clinical epidemiological inquiries were also performed. RESULT: 9 strains' biological features were different from those of the other Klebsiella. The 9 patients' ages were over forty and the primary disease was the third type tuberculosis of lungs. Using of antituberculosis drugs for long term as well as penicillins might be the predisposing factors, corticosteroids were used in 3 of the 9 patients, Combined infection with other pathogens in addition to K. planticola occurred in 8 patients, in 7/8, Candida albicans was identified. Drug sensitive tests show that all of them were resistant to penicillins and sensitive to the second and third generation cephalosporins. CONCLUSION: The new strain of Klebsiella was studied and this information will be useful for diagnosing and treating K. platicola.

[Assessment of responsiveness of spontaneous baroreceptor-heart rate reflex under gravitational stress].

Beat by beat arterial blood pressure was measured using Finapres 2300 monitor. Sequences of 3 or more beats with systolic pressure and next beat interval changed in the same direction were identified as baroreflex sequences from 6-min data segments. A linear regression was applied to each sequence and the slope (correlation coefficient > or = 0.85) was taken as a measure of the baroreflex response slope (BRS). The changes in mean BRS during + 65 degrees head-up tilt (HUT), -6.67 kPa lower body negative pressure (LBNP) and 16 days -6 degrees head-down tilt (HDT) bed rest were observed. The results showed that there was a significant decrease in the mean BRS during LBNP or HUT as compared with that of the supine control, though the blood pressure responses differed. The mean BRS was also significantly decreased during bed rest. It suggests that the responsiveness of spontaneous baroreceptor-heart rate reflex can be evaluated in humans under gravitational stress.

[Study on the chemical components of Alpinia officinarum].

Four crystalline substances were isolated from rhizome of Alpinia officinarum Hance. They were identified as beta-Sitosterol (I), Galangin (II), Emodin (III) and Quercetin (IV), I and III were separated from this plant for the first time.
