[The role of nitric oxide on the functional change of interleukin-1 beta and interleukin-6 induced rat pancreatic islets of Langerhans].

OBJECTIVE: To observe the effects of interleukin (IL)-1 beta and IL-6 on nitric oxide (NO) production, islet cell DNA and insulin content, and MTT assay of rat pancreatic islets of Langerhans. METHODS: Isolated pancreatic islet cells from SD rats were cultured in monolayer in vitro. Nitrite production, islet cell DNA and insulin content, and MTT assay in rat pancreatic islet cells incubated with IL-1 beta and IL-6, singly or in combination, were measured. The levels of these parameters of the cytokines in the absence and presence of N(G)-monomethyl-L-arginine (L-NMMA), an inhibitor of nitric oxide synthetase, were compared. RESULTS: IL-1 beta induced a significant increase in nitrite production, a decrease of islet cell DNA and insulin content and reduction of cell activity (MTT, P < 0.001); these changes were blocked by L-NMMA (P < 0.001). IL-6 produced no significant change in those parameters (P > 0.05). The combination of IL-1 beta and IL-6 increased NO production, decreased islet cell DNA and insulin content and reduced cell activity (P < 0.001); the changes were similar to those with IL-1 beta alone (P > 0.05). CONCLUSION: The cytotoxic effects of IL-1 beta on pancreatic islets might be mediated by NO. IL-6 may not be directly cytotoxic to pancreatic islets. It may be related with the fact that IL-6 does not induce NO production.

[Low fever of unknown reason in patients undergoing autologous stem cell transplantation after hematopoietic reconstitution: a clinical analysis of 13 cases].

OBJECTIVE: To investigate the reason of low fever in patients undergoing autologous stem cell transplantation after hematopoietic reconstitution. METHODS: The reasons for low fever in 13 patients undergoing autologous stem cell transplantation after hematopoietic reconstitution were comprehensively analyzed. RESULTS: After analysis of clinical features, laboratory tests, imaging and microbiological data, no reason was found for the low fever. There was no effect on the fever with anti-microbial therapy. However the low fever was controlled by low dose prednisone 5-10 mg/d or dexamethasone 2-4 mg. CONCLUSION: The reason for the low fever is not clear; it is probably nonspecific low fever induced by high-dose radiotherapy or high-dose chemotherapy.

[A study on the endotoxin and bacterial contamination in reverse osmosis water and dialysis fluids].

OBJECTIVE: To evaluate the endotoxin (ET) and the bacterial contamination in reverse osmosis (RO) water and dialysis fluids for hemodialysis treatment. METHODS: Specimens of RO water and dialysis fluids were obtained at random from 18 hospitals in Beijing. ET was measured with chromogenic limulus amoeboeyte lysate (LAL) assay. Bacteriologic samples were processed on heterotrophic plate count agar with pour plate technique. The plasma IL-1, IL-6 and TNFalpha were tested with enzyme-linked immunosorbent assay (ELISA). RESULTS: The level of ET in RO water was 0.116 +/- 0.063 EU/ml, and the culture of bacteria had 2 positive results, both counting 100 CFU/ml. The level of ET was much higher in acid concentrate (Protocol B) than in basic concentrate (Protocol A). The culture of bacteria had 11 positive results in Protocol B concentration with 8 samples equal or higher than 2,000 CFU/ml. The culture of bacteria had only 3 positive results in Protocol A concentration, all being lower than 2,000 CFU/ml. Another investigation showed that the level of ET in dialysis inlet was higher than 0.5 EU/ml in 5 cases of a total of 16. Their plasma IL-1 and TNFalpha levels were significantly increased. CONCLUSION: Both ET and bacterial contamination exist in RO water and dialysate. It is essential to examine the level of ET and perform bacterial culture, as well as to disinfect RO and dialysate tank at regular intervals in order to reduce pyrogenic reactions.

[The association between plasma homocysteine and the number of coronary artery with stenosis].

OBJECTIVE: Previous studies have shown that elevated plasma homocysteine is a risk factor for arterosclerotic disease and is associated with increased severity of carotid-artery stenosis and peripheral arterial disease. In this study, we examine the precise relation of homocysteine to the number of coronary artery with stenosis. METHODS: One hundred and seventeen subjects who underwent coronary angiography from June to October 1998 in our hospital were recruited in this study. Cases (n = 101) with >or= 50% stenosis in at least one of the three coronary vessels were angiographically defined as having coronary heart disease (CHD). While subjects (n = 16) had 0% stenosis in all the three coronary vessels were considered as controls. CHD cases were divided into single-vessel (n = 29), double-vessel (n = 35) and triple-vessel group (n = 37). Plasma homocysteine was measured within 1 week before angiography in all subjects. RESULTS: Mean plasma homocysteine level was significantly higher in coronary patients than in controls [(17.56 +/- 1.14) micromol/L versus (10.59 +/- 0.43) micromol/L, P = 0.02]. Homocysteine level in the triple-vessel group or double-vessel group was significantly higher compared with that in the controls [(19.49 +/- 1.98) micromol/L versus (10.59 +/- 0.43) micromol/L, P = 0.006 and (17.41 +/- 2.03) micromol/L versus (10.59 +/- 0.43) micromol/L, P = 0.036, respectively]. The level of homocysteine in the stingle-vessel group tended to be slightly higher as compared with that in the controls [(15.2 +/- 61.18) micromol/L versus (10.59 +/- 0.43) micromol/L, P = 0.16]. CONCLUSION: Homocysteine level is elevated in patients with coronary heart disease. Higher level of homocysteine is associated with increased number of coronary arteries with stenosis.

[Open lung biopsy in diagnosis of interstitial lung disease].

OBJECTIVE: To evaluate the role of open lung biopsy in diagnosis of interstitial lung disease. METHODS: 24 patients with interstitial lung disease who underwent open lung biopsy from 1993 to June 1998 were reviewed. RESULTS: The documented pathologic diagnosis in 24 patients with interstitial lung disease who underwent open lung biopsies were: 7 IPF (UIP), 1 acute interstitial pneumonia (AIP), 3 BOOP, 2 DPB, 3 sarcoidosis, 2 tuberculosis, 1 histiocytosis-X and 1 lung cancer. Most of these patients did not definitely diagnosed through their clinical evaluation including history, chest X-ray, pulmonary function tests and TBLB. CONCLUSIONS: Open lung biopsy, is a useful method, as it is sensitive and specific in diagnosis of interstitial lung disease. It is helpful in defining the site and extent of interstitial lung disease and diagnosing rare and atypical interstitial lung disease.

[Detection of lipoarabinomannan-IgG in cerebrospinal fluid with enzyme-linked immunosorbent assay for diagnosis of tuberculous meningitis].

OBJECTIVE: To ascertain the value of lipoarabinomannan-IgG (LAM-IgG) determination in the diagnosis of tuberculous meningitis. METHODS: LAM-IgG in cerebrospinal fluid was detected with enzyme-linked immunosorbent assay. RESULTS: The positive rate of LAM-IgG in 73 patients with tuberculous meningitis was 51%, 36 control cases were all negative. The specificity was 100%. CONCLUSIONS: Detection of LAM-IgG in cerebrospinal fluid is a useful tool for diagnosis of tuberculous meningitis.

[The expression of Mdm2 protein and p53 mutation in lung squamous cell carcinomas and their clinicopathological significance].

OBJECTIVE: In order to study the interrelation between p53 mutation, Mdm2 overexpression and pulmonary squamous cell carcinomas. METHODS: Forty-five pulmonary squamous cell carcinomas were investigated for expression of Mdm2 protein and p53 mutation using immunohistochemical techniques and PCR-SSCP. The findings were correlated with the clinicopathological features of carcinomas. RESULTS: (1) Mdm2 protein overexpression (62%, 28/45) and p53 mutation (51%, 23/45) are a common event in pulmonary squamous cell carcinomas (P < 0.05). (2) Statistical analysis revealed a strong correlation between Mdm2/p53 coexpression and lymph metastases (66%, 10/15) was observed (P < 0.05). CONCLUSIONS: The coexistence of Mdm2 protein and p53 aberration in a subset of pulmonary carcinomas may be indicative of a gain of function phenotype with more aggressive characteristic.

[Early use of Chinese drug rhodiola compound for patients with post-trauma and inflammation in prevention of ALI/ARDS].

OBJECTIVE: To evaluate the effects of the Chinese drug rhodiola on the preventive treatment of acute lung injury (ALI) caused by post-traumatic/inflammatory and thoracic-cardiovascular operations. METHODS: 104 patients who coincide with their diagnosis and standard scale were divided randomly in to single number groups A(1), B(1), C(1) and D(1) (early preventive treatment group EG) and double number groups A(2), B(2), C(2) and D(2) (control group CG). The treatment of primary disease both in EG and CG was similar, but in EG four grams of rhodiola compound (RCO) were used every day for a period of 3-15 days since admission and 5-7 days after operation. The indicators related to ALI in each groups were measured. RESULTS: PaO(2), AaDO(2) and hemorheology were significant in EG than in CG (P < 0.05) or (P < 0.01). The complication rate of ARDS was decreased about of 50.0% in EG than in CG. CONCLUSIONS: Early use of RCO may protect against major risk factors of ALI/ARDS caused by massive trauma/infection and thoracic-cardio-operations.

[Transformation of human bronchial epithelium by EB virus gene LMP1 and EBNA2].

OBJECTIVE: To observe Epstein-Barr virus (EBV)-dependent transformation of human bronchial epithelium. METHODS: Eukaryotic expression plasmide pBS-LMP-Hyg and pSG5-EBNA2-Hyg were transfected into immortalized human bronchial epithelium cell line TR. RESULTS: In situ hybridization and Western blot confirmed EBNA2 and LMP1 expression in one hygromycine B resistant cell clone TR/LMP1-EBNA2. Growth curve and MTT value showed that transformed cells grew faster. Soft agar plating rate of TR/LMP1-EBNA2 was 50.1per thousand, 68.2per thousand, empty vector PBs-Hyg transfected cell strain TR/Hyg 2.0per thousand, 3.0per thousand, TR 11.0per thousand, 0.7per thousand. CONCLUSION: Expression of both LMP1 and EBNA2 led to decreased anchorage dependence and increased proliferation rate of TR, thereby further transformed immortalized human bronchial epithelium. Our results suggest that EBV may play a role in genesis of lung cancer.

[Efficacy of TACE in treatment of intraportal tumor thrombi with double helical CT].

The present study observed alterations of the intraportal tumor thrombi before and after transcatheter hepatic arterial chemo-embolization(TACE) in 36 patients with massive hepatocellular carcinoma(HCC). Elscint double helical CT was used to scan liver in the portal phase before and after TACE. The results showed that the scan technique could be used to evaluate the distribution of intraportal tumor thrombi. After treating with TACE, retention of iodized oil was found in the intraportal tumor thrombi(14/36), and the intraportal tumor thrombi were reduced in 28 out of 36 cases. The survival rate was more than 1 year in 20 patients. These data indicate that the blood supply of the intraportal tumor thrombi comes from hepatic arterial system. TACE seems to be valuable for the treatment of intraportal tumor thrombi with massive HCC.

[Effects of Squama manitis on cardiac systolic function and platelet aggregation].

OBJECTIVE: To study the effect of Squama Manitis(S. M.) on cardiac function and platelet aggregation in vitro. METHOD: Isolated working rat hearts were prepared, and cardiac performance was measured before and after adding S. M. in the Krebs-Hensleit(K-H) perfusion buffer. Controls with K-H buffer were also investigated. Rich platelet plasma and Poor platelet plasma were centrifuged from the blood of 15 patients with coronary heart disease. Each sample was divided into three parts for use by three groups: control, group of S. M. and group of aspirin. Platelet aggregation was stimulated by adenosine diphosphate. RESULT: LVSP, +dP/dtmax and CO of isolated working rat hearts were significantly increased after treating with S. M. liquor. S. M. had no obvious effect on -dP/dtmax and LVEDP. These parameters did not change significantly in the control. Percentages of platelet aggregation were significantly lower in the group of S. M. and the group of aspirin as compared with the control. CONCLUSION: S. M. increases cardiac systolic function, has positive inotropic effect on myocardium and inhibits platelet aggregation.

[Effect of benazepril on renal interstitial inflammatory cell infiltration in rats with unilateral ureteral obstruction].

Effect of benazepril on renal interstitial inflammatory cell infiltration was observed in rats with unilateral ureteral obstruction(UUO). The results showed that the relative volume of interstitium and number of infiltrating inflammatory cell on the fifth day were significantly less in rats with benazepril than the control group, but the difference between two groups lose significance on the tenth day. These data indicate that benazepril can ameliorate early phase of renal interstitial inflammation in UUO.

[Determination of isofraxidin in Sarcandra glabra (Thunb.) Nakai by HPLC].

OBJECTIVE: To establish a method for the determination of isofraxidin in Sarcandra glabra. METHOD: Using HPLC with mu-Bondapak C18 column, acetonitrile -0.1% phosphoric acid solution(20:80) as mobile phase and detection wavelength at 344 nm. RESULT: The recovery and RSD were 98.1% and 1.9% (n = 5) respectively. CONCLUSION: This method is rapid, simple and accurate, thus may be used for the quality control of this drug.

[Dose-related response of adenosine in hypoxic pulmonary hypertension in canines].

The vasodilator test is of critical importance in determining the reversibility of pulmonary hypertension and hence the indication of surgical treatments for cases of marginal pulmonary hypertension associated with left to right shunt. This study aimed at the dose-related vasodilation effect of adenosine (AD) on hypoxic pulmonary hypertension. The hemodynamic indexes were examined in 8 canines with acute hypoxic pulmonary hypertension after administration of intravenous adenosine. The results showed that AD decreased both systemic and pulmonary pressures in a dose-dependent way, with very close linear correlation; so did the systemic and pulmonary resistance (r = -0.970 and -0.994, respectively; P < 0.001). The PASP reductions were significantly greater than SASP reductions at AD doses of 100 to 200 micrograms/(kg.min) (from -11.0% +/- 4.2% to -27.7% +/- 14.8% vs from -2.6% +/- 7.4% to -14.9% +/- 7.0%, respectively), while PVR reductions were significantly greater than SVR reductions (-18.8% +/- 8.05% vs -10.2% +/- 6.86% and -29.8% +/- 14.46% vs -21.1% +/- 9.53%, respectively, P < 0.05). AD increased cardiac output and cardiac index to the significant degree only when the dosage increased to 300 micrograms/(kg.min) or above. These results suggest that AD seems to be the choice of drug in vasodilator test of left to right shunt pulmonary hypertension, and the proper dosage ranges from 100 to 200 micrograms/(kg.min).

[A treatment to serious esophageal cicatrices stenosi by metal and silica gel dilator].

OBJECTIVE: To find an effective method of treating the esophageal cicatricial stenosis. METHOD: Six cases with esophageal cicatricial stenosis were treated by mental and silica gel dilator. RESULT: The effects in all six cases were satisfactory and no any complications were finded. CONCLUSION: The method is safe, effective and of no complications, the treatment time is shorter also.

[The prospects of research on VR rehabilitation engineering].

In this paper, we present the recent development of Vitual Reality (VR) technology and rehabilitation engineering and discuss the rationality and feasibility of the application of VR to rehabilitation. With VR technology, psychological induction can be better introduced in rehabilitating exercises and patients can be tremendously released from fret during traditional physical training for rehabilitation. The VR is expected to bring about a technological revolution in rehabilitation engineering.

[Study and application on the device of the drop volume of amniotic fluid in estimating fetal lung maturity].

A novel device of drop volume of amniotic fluid has been made to estimate fetal lung maturity. The design of the device is based on the principle that the surface tension of amniotic fluid is inversely proportional to the quantity of surfactant in the solution. The device measures the falling time of fluid drops from drop-tube and estimates the fetal lung maturity by using the method of comparison and electric-optic timing-counting. 148 samples of amniotic fluid were measured by the device and the data were compared with those obtained by biochemical method. The results demonstrate that this device has high stability and accuracy, and the rate of agreement between the two methods is as high as 93%.

[Some basic techniques of virtual reality body healthiness vehicles for rehabilitation].

Some know-how techniques of virtual reality rehabilitation vehicles are discussed in this paper after a brief introduction of the application background and the structure and functions of the vehicles. This kind of vehicles can be used for psychological treatment during rehabilitation. Pateints can select different modes of physical exercises according to their individual situations, and therefore they can get rehabilitation much faster and more easily than those under the traditional treatments.
