Neuronavigation in interventional MR imaging. Prospective stereotaxy.

A practical MR imaging-based guidance/control methodology has been developed successfully and validated for improving the performance of stereotactic neurosurgerical procedures such as brain lesion biopsy. The use of the device and method in 40 routine MR-guided procedures has revealed its potential as an alternative approach. Superior to the traditional stereotactic systems, which rely on the old images, the new method, based on prospective guidance, can provide good and acceptable targeting accuracy in the presence of brain shift. Furthermore, the use of MR monitoring of the overall neurosurgical procedure provides another independent assurance for the success of a complicated surgery. The advantages of the new surgical guidance system and method are simple and compatible with the existing capabilities of conventional MR scanners. More importantly, it allows a more effective surgical guidance in the presence of brain shift during the typical neurosurgery. Another important advantage of the guidance method and device is the performance of a truly MR-guided neurosurgical procedure in a conventional, short bore high field MR scanner. Surgical procedures using the guidance system and method have been accepted by radiologists and neurosurgeons as an attractive MR-based stereotactic approach. It can be expected that this guidance scheme will be a useful addition to the MR-based stereotactic system for neurosurgery, and animal research, in which cumbersome stereotactic frames have been used.

Renal resources on-line. Interventional nephrology resources on the Internet.

Interventional nephrology is an emerging field in which the nephrologists themselves place and maintain accesses for dialysis. The number of patients on dialysis is increasing in the U.S., as are the number of accesses and related problems. The Internet is fast becoming a major source of information on this emerging field. Numerous teaching institutions offer information over the Internet and the volume of information is steadily increasing. Even though the Internet offers a great deal of information, the consumer must be cautious in using it as the information is not peer-reviewed and validated by regulatory agencies.

[Genetic studies of 13 loci in Guangdong Han population by two multiplex PCR systems and its forensic application].

OBJECTIVE: The genetic studies of 13 short tandem repeats(STRs) loci in two multiplex amplification systems were carried out on Chinese Han population in Guangdong. METHODS: DNA samples from 328 unrelated individuals were screened. The 13 loci were D3S1358, vWA, FGA, D8S1179, D21S11, D18S51, D5S818, D13S317, D7S820, D16S539, TH01, TPOX and CSF1PO. The PCR products were analyzed and genotyped by ABI 377-96 Sequencer. RESULTS: The combined power of discrimination (DP) was 0.999999999999993 and the combined paternity of exclusion(PE) was 99.999%. These 13 loci met the Hardy-Weinberg expectations. CONCLUSION: The two multiplex amplification systems were very useful in forensic case investigation.

[A retrospective clinical study of 6539 cases of malignant oral-maxillofacial tumors].

OBJECTIVE: The aim of this study is to obtain the general information of post-operation malignant oral-maxillofacial tumor patients in forty-seven years. METHODS: A total of 6539 patients with malignant oral-maxillofacial tumors in the hospital of stomatology, west china university of medical sciences, from 1953 to 2000 were retrospected. RESULTS: The average age of patients with malignant oral-maxilloficial tumor is 48.7 years old. The ratio of male to female is 2.31:1. The most common original malignant tumors in this oral-maxillofacial region are malignant oral tumors, malignant maxillary tumors, malignant mandibular tumors, and malignant sialoma successively. As to the original tissues of tumors, there were 5869 cases of epithelial tumors, and made up 89.75% of all the investigated patients. Tumors came from lymphatic and hematopoietic systems and mesoderm were much less those came from epithelia. There were 4177 cases of squamous cell carcinomas and the percentage was 63.88%. CONCLUSION: The incidence rate of malignant tumors in oral-maxillofacial region tends to increase, and the average age of patients also rose. The ratio of male to female decreases gradually. Epithelial tumors, especially squamous cell carcinomas are very common tumors happening in this region.

[Expression of rice dwarf virus outer coat protein gene(S8) in insect cells].

Outer coat protein gene(S8) of RDV was cloned into the transfer vector pVL 1393 to construct a recombinant vector pVL1393-S8. The recombinant vector pVL1393-S8 and the linear baculovirus RP23. LacZ were cotransfected into sf9 cells to produce the recombinant virus RP23-S8. RP23-S8 infected sf9 cells were collected and analysed by SDS-PAGE and Western-blot. The results showed that the S8 gene of RDV was expressed in sf9 cells and the expression level of sf9 cells was higher between 72-96 h after infected.

Construction of the enhanced yellow fluorescent protein expression vector carrying IFN-gamma gene.

PURPOSE: To construct the enhanced yellow fluorescent protein (EYFP) vector carrying interferon-gamma gene (ifn-gamma) in order to provide an ideal reporter in the expression of ifn-gamma and location of protein in vitro and in vivo. METHOD: According to the nucleotide sequence of ifn-gamma gene, a pair of oligonucleotides was designed as primer whose two end contained nucleotide sequence of EcoR V and Not I restriction endonuclease respectively. The gene encoding for inf-gamma was amplified using PCR technqiue. After the PCR product was retrieved and purified, it was digested with EcoR V and Not I restriction endonuclease, and then cloned into the plasmid pIRES-EYFP. The recombinant plasmid pIRES-EYFPIFN-gamma was identified by restriction endonuclease enzyme analysis and DNA sequence analysis. RESULTS: The ifn-gamma was successfully amplified and verified by partial DNA sequence analysis. The recombinant plasmid was correctly screened. CONCLUSION: The EYFP expression vector carrying ifn-gamma gene was successfully established. This research work has formed a base for monitoring the ifn-gamma gene expression and protein position in living cells.

[The role of Anopheles anthropophagus in malaria transmission in in Xinyang City of Henan Province].

OBJECTIVE: To study the role of Anopheles anthropophagus in malaria transmission and transmission threshold so as to provide basis for vector surveillance and malaria control strategy. METHODS: Parasitological and entomological methods were used in the investigation at 5 villages of Xinyang City, Henan Province. RESULTS: From July to August, 1999, 74 febrile cases (10.9% of the total population) were examined. Among them 50 were infected, the incidence in the population of surveyed spots was 7.4%. Active detection was made in another randomly selected two villages and found that the parasite rate in the inhabitants was 2.0%, and the positive rate of IFA was 8.4%. Only vivax malaria was detected. An. anthropophagus and An. sinensis were collected, with An. anthropophagus as the predominant one in human dwellings. The estimated man-biting rate and the human blood index were 4.9388 and 0.7858 respectively. The vectorial capacity of An. anthropophagus was 5.5296. The critical man-biting rate of An. anthropophagus was 0.2407 as calculated by the formula (ma = -rlnP/abPh) according to Macdonald's model. The local man-biting rate was 20 times higher than that of the critical man-biting rate. CONCLUSION: The results demonstrated that An. anthropophagus is the principal vector in malaria transmission in the area. The findings imply that the critical man-biting rate is of practicable importance in vector surveillance.

[Gene point mutation in the dihydrofolate reductase-thymidylate synthase gene of Plasmodium falciparum].

OBJECTIVE: To investigate the gene point mutation in the dihydrofolate reductase-thymidylate synthase(dhfr) gene of Plasmodium falciparum isolate from Yunnan Province strongly associated with pyrimethamine and cycloguanil resistance. METHODS: Nested PCR and restriction endonuclease digestion were applied to detect the gene mutation using dried blood filter paper collected from the fields in Yunnan Province. RESULTS: Different mutations were found in 4 amino acids at positions 16, 51, 108 and 164 of dhfr gene, particularly, Asn-108 and Ile-51, the mutation frequency being 94.1% and 90.1%, respectively. The frequency of the wild-type genotype (3D7-type) Ser-108 appeared lower (9.1%), while the frequency of the Ala-16 was high(61.8%); the mutation-type was very high, the ratio of HB3-type, 7G8-type/FCR3-type and Cambodian-type was 1:21:7.5. CONCLUSION: The investigation first demonstrated that Plasmodium falciparum Yunnan isolate dihydrofolate reductase-thymidylate synthase gene (dhfr) at positions 16, 51, 108 and 164 exhibited different degrees of point mutation. The frequency of mutation of the 7D8-type involved in pyrimethamine resistance was higher, while that of the FCR3-type involved in cycloquanil resistance was lower.

[Investigation on genotype constitution of different Plasmodium vivax isolates and its geographical distribution in China].

OBJECTIVE: To investigate the population constitution and geographical distribution of Plasmodium vivax in China using molecular technique. METHODS: Blood-spot filter paper samples with related epidemiological data were collected from vivax malaria patients living in malarious area of 10 provinces (autonomous region) in China. Semi-nested- or nested-allelic-specific PCR genotyping method was used to identify CSP genotypes, families and types of Plasmodium vivax of each isolate from these patients. RESULTS: Of 384 field isolates of Plasmodium vivax, 258 temperate zone family strains were identified, including 14 allelic variant genotypes spreading among 10 sampling provinces; allelic variants sized less than 731 bp were only seen in 5 provinces in southern China; 79 tropical zone family strains including 5 genotypes were also distributed in 5 provinces of southern China south to 25 degrees N. lat; and 14 PV Type-2 strains including 2 genotypes were found in some areas of Hainan and Yunnan Provinces. In addition, 33 isolates from genotype-mixed infections were revealed. CONCLUSION: At present, area north to 25 degrees N. lat. of the country is the sole area prevalent for Plasmodium vivax family strains of temperate zone; there is overlapping distribution of P. v. of temperate zone family and tropical zone family of this parasite in the southern China south to 25 degrees N. lat; where the most complex isolate constitution is in Yunnan and Hainan Provinces, and PV Type-2 strains have been found in some areas of the two provinces. Besides, there were 2 groups of genotype with distinct geographic distribution feature within the temperate zone family.

[The development of cerebral circulation analyzer].

In this paper a new cerebral circulation analyzer is introduced, inducing the main structure, the operating principle, the software program and its clinical applications. We can get the cerebrovascular hemodynamics indexes such as resistance, compensatorg blood flow etc., from the blood velocity, pressure waveform, and arterial diameter detected in carotid and vertebral arteries.

[Study on DNA oxidative damage of O3 aging model in mice].

OBJECTIVE: This study was aimed to detect the animals' DNA oxidative damage. METHODS: A free radical mouse aging model was established by putting the mouse in a device for inhaling quantities of ozone (O3). Single cell gel electrophoresis (SCGE) was used for assessing the splenolymphocytes' DNA damage. RESULTS: The appearance rate of comet and the length grade of DNA migration for the young control group (P < 0.05). No significant difference in the degree of DNA damage was observed between the O3 model group and the natural aging group(P > 0.05). CONCLUSION: The DNA damage of O3 animal model is consistent with that of natural aging animal; this animal model DNA damage of O3 animal model is consistent with that of natural aging animal; this animal model and the method of SCGE are useful for future researches on aging and antiaging medicine.

[Effects of retinoic acid on secretion of apolipoproteins A I, A II, B100, C III and E by cultured HepG2 cells].

OBJECTIVE: This study inquired into the role of retinoic acid in lipoprotein metabolism. METHODS: We observed the effect of retinoic acid on the secretion of apolipoproteins A I, A II, C III, B100 and E by cultured HepG2 cells. The apolipoprotiens contents in culture media were measured by radioimmunodiffusion assay (RID) kits developed by our research laboratory. 20-fold lyophilizely condensed culture media were used for the assays. RESULTS: Retinoic acid increased the secretion of apoA I, B100, C III and A II, and it inhibited the secretion of apoE. The effect of retinoic acid was strengthened in a dose-dependent manner. When the concentration of retinoic acid in cultured media was 2 x 10(-4) mol/l, the secretion of apoA I, A II, B100 and C III increased by 14.3% (P < 0.01), 23.8% (P < 0.05), 16.1% (P < 0.01) and 47.6% (P < 0.01) respectively, and the secretion of apoE decreased by 37.2% (P < 0.01). CONCLUSION: These findings indicate that retinoic acid does not have a general effect on apolipoprotein secretion in HepG2 cells.

[Investigation on the value of applying interventional radiologic treatment in the management of advanced gynecological malignancies].

OBJECTIVE: To investigate the value of applying interventional radiologic treatment (IRT) in the management of advanced gynecological malignancy. METHODS: Twenty-four historically confirmed advanced gynecological malignancy patients from September 1997 to June 1998 were included. After receiving internal iliac arterial infusion chemotherapy with Seldinger, the patients' tumor tissues were pathologically studied at different times. The tumor size was measured by ultrasonography two weeks after IRT. RESULTS: Before internal iliac arterial infusion chemotherapy, internal iliac arteriography showed that uterus artery was thickened and tortuous, the new vessels appeared and the tumors were stained. In some cases, the image of blood vessel-lake and arteriovenous shunt were seen in tumor tissues. Twenty-four hours after internal iliac arterial infusion chemotherapy, focal necrosis was observed in tumor cells, seventy-two hours after, the necrosis became more thorough. In regard to the tumor size, two patients were CR, nineteen patients were PR, and two patients were NC. Only one patient failed to respond to IRT. Chemotherapy side effects were slight. Patients' WBC returned normal within two weeks after chemotherapy. No complication happened. CONCLUSION: 1. Internal iliac arteriography has practical significance in assessing the tumor size and evaluating the therapeutic effects; 2. the internal iliac arterial infusion chemotherapy can reduce the tumor size, which prepares the patient in a better condition for operation. The most appropriate time for operation is two weeks after internal iliac arterial infusion chemotherapy. Even for the patient who is not suitable for operation, the reduction of the tumor size still can enhance the therapeutic effects of radiotherapy. 3. IRT is easy to perform, effective and with few slight side effects and complication. It is an effective way to save the patient's life.

[Analysis of the clinical salience for sudden hearing loss in acoustic neuroma].

OBJECTIVE: To avoid misdiagnosis and misapplied therapy of acoustic neuroma. METHODS: 92 cases of acoustic neuroma treated between 1983 and 1997 were retrospectively reviewed. Among them, 20 patients (21 ears, 21.7%) presented with sudden hearing loss as the starting symptoms. The diagnosis was based on audiological and radiologic examinations. RESULTS: The clinical and audiological analysis demonstrated that 57.1% of the ears showed hearing loss over 71 dBHL, and all ears had deteriorated ABRs. Acoustic reflex was nonreactive in all 10 ears tested. Positive findings in CT scan were 88.89%. CT pneumoencephalography or MRI provided useful information of diagnosis in those with negative findings of place CT scan. CONCLUSION: ABR should be used as a routine test for patients with sudden deafness. When ABR was abnormal, CT scan around the internal auditory meatus was needed.

[Important prognostic factors in patients with skull base erosion from nasopharyngeal carcinoma after radiotherapy].

OBJECTIVE: To evaluate the long-term outcome and prognostic factors in patients with skull base erosion from nasopharyngeal carcinoma (NPC) after initial radiation therapy. METHODS: From January 1985 to December 1986, 100 patients (71 male, 29 female) with the diagnosis of NPC were found to have skull base erosion from computed tomography (CT). The mean age was 41 (16-66) years. Ninety-six patients had World Health Organization (WHO) type III undifferentiated carcinoma while four had type I carcinomas. Metastatic workup including chest radiography, liver ultrasound and liver function test were negative. All patients underwent external beam radiotherapy alone to 66-80 Gy over 6-8 weeks. Daily fraction size of 2 Gy was delivered utilizing cobalt-60 or linear accelerator. No patient received chemotherapy. All patients were followed up at regular intervals after irradiation. Median follow-up was 22.3 months (2 months-174 months). Overall survival of the cohort was computed by the Kaplan-Meier method. Potential prognostic factors on survival were examined. Multivariate analyses was performed using Cox regression model. RESULTS: One, three, five, and ten year overall survival rates for the cohort were 79%, 38%, 27% and 13%, respectively. However, in the sub-group of patients with both anterior cranial nerves (I-VIII) and posterior cranial nerve (IX-XII) involvement had a five-year survival of only 7.7%. Causes of death included local recurrence (59 patients), distant metastases (21 patients), both local recurrence and distant metastases (1 patient) and unrelated causes (5 patients). After multivariate analysis, complete recovery of cranial nerve involvement, cranial nerves palsy and recovery of headache after irradiation were found to be independent prognostic factors in this cohort. CONCLUSION: This report presents one of the longest follow-ups of patients with nasopharyngeal carcinoma invading skill base. It demonstrates the importance of cranial nerves involvement as well as recovery of headache and cranial nerve palsy. These factors should be evaluated carefully from history and physical examination as well as imaging studies. Currently available magnetic resonance imaging (MRI) is highly recommended as an additional test. A subgroup of patients with skull base involvement had long-term survival after radiotherapy alone. More aggressive strategy such as combined chemo-radiotherapy and altered fractionation radiotherapy may improve outcome in patients with poor prognostic factors.
