[Reactions to vaccinations against tetanus and tick-borne encephalitis caused by merthiolate (thiomersal)].

Thirty patients with suspected adverse reactions to tetanus- or tick-borne encephalitis-vaccines were subjected to allergy tests. In 8 of 30 patients epicutaneous and/or intracutaneous tests with merthiolate were positive. Testing anorganic mercury, formaldehyde, aluminium hydroxide, gentamycin and egg white (i.c. and RAST), no positive reactions were found. After vaccination - prior to testing - merthiolate - positive patients had suffered from local inflammatory reactions at the injection site, fever and lymphadenopathy (four patients), urticarial (three patients) or lichenoid exanthemas (one patient). Reviewing the literature it is suggested that alternatively merthiolate-free vaccines be provided for sensitized individuals.

[Repetitive skin testing in patients with lower leg eczema and leg ulcer].

16 patients with lower leg dermatitis and venous ulcers had been subject to patch-testing during 1974 to 1977. When retested in 1982, the number of positive reactions per patient increased from 3.82 to 5.87 on an average. 54% of the positive epicutaneous test reactions could be reproduced in the repeated patch test. Meanwhile, 9 of 16 of patients were free of lower leg dermatitis and 8 of them were free of venous ulcers. This paper deals with the problematic nature as well as the likely causes of the apparent inconsistency of epicutaneous test reactions in patients with lower leg dermatitis and venous ulcers.

[Local treatment of ulcus cruris venosum (author's transl)].

In this survey, the principles of local ulcer treatment and the most common therapeutic techniques are discussed. The data concerning the risk of sensitisation by local therapeutics are presented in the tables 1, 2, 3, 4.

[Disappointing results with dinitrochlorobenzene in alopecia areata (author's transl)].

22 patients suffering from long-standing alopecia areata totalis or fere totalis were treated with dinitrochlorobenzene (DNCB) locally. No hair growth was induced in patients with total alopecia, but 3 out of 11 patients with moderate forms showed cosmetically acceptable regrowth. The possible causes of these comparatively poor results are discussed.

[Is chronic recurrent urticaria a psychosomatic disease?].

A random group of 37 patients with chronic recurrent urticaria, 26 female and 11 male, was subjected to multidimensional personality tests (Freiburg personality inventory, FPI, Freiburg aggression test, FAF) whereas, deviating from establishing standards, introversion, nervousness, psychosomatic disturbance were more pronounced in the urticaria group, the following traits in deviation from normal standards were not noted: depression, irritability, sociability, emotional instability, composure. Nevertheless, this group appeared to be less communicative, more inhibited, compliant, and less dominant and aggressive. Neither did they show signs of socially accepted expression of negative and annoyed emotions nor socially desirable signs of assertiveness. A high rate of coincidence with other psychosomatic disorders such as frequent headache (18/37), chronic gastritis (19 of 37) and ulcus duodeni (5 of 37) and migraineous headache (6 of 37) was also found in this group. In all cases explorable latent conflictive situations (frequent ambivalence) and negative childhood experience are further indicative of psychosomatic diseases.

Challenge of penicillin-allergic volunteers with penicillin-contaminated meat.

In order to obtain information about the clinical course and immunological background of possible allergic reactions due to penicillin-containing food, nine volunteers with penicillin allergy proved by case history, positive skin tests and/or positive penicilloyl-specific RAST were exposed to penicillin-contaminated pork. For this purpose a farm pig from an inbred strain received "therapeutic" doses of benzyl procain penicillin before being slaughtered. Meat samples were taken from muscle tissues, kidney and liver and analyzed by diffusion and extraction methods. Very small amounts of penicillin were detectable in muscle tissue, whereas kidney material contained up to five times as much (0.12 mcg/g). Except two cases which reacted with itchy sensations after ingestion of penicillin contaminated meat "á la tartare", all probands were clinically free of symptoms. Assaying penicilloyl-specific IgE antibodies by RAST during the trial, a tendency to increased levels in the serum after 2 and 24 h was noted, whereas the titers of BPO-specific IgG antibodies measured by the red cell-linked antigen-antiglobulin reaction (RCLAAR) remained unchanged. It is concluded from this study that small and hidden amounts of penicillin and its derivatives in food seldom induce anaphylactic reactions in penicillin allergic patients.

Contact sensitization to benzoyl peroxide.

The incidence of contact allergy to a 5% benzoyl peroxide (BP) gel has been studied by patch testing in 222 patients. Positive allergic reactions to BP were found in 3.1% of cases in a general clinical population (94 patients), whereas 5.8% of 69 patients with eczematous dermatoses were found to be BP-sensitized. In a group of acne vulgaris patients having used a commercial 5% or 10% BP gel for a period of 12 days to 3 1/2 years (mean: 10.7 months), positive allergic reactions were found in three cases (5.1%).

[Compression therapy of lymphedema (author's transl)].

In spite of the development of micro-surgical techniques which enable functioning lymphovenous anastomoses to be formed, surgical interventions are not usually indicated in extremities with lymphedema which are for the most part already irreparably damaged. For want of an effective medicinal alternative compression therapy is still the method of choice. Its mode of action, indication and performance are dealt with. The "manual lymph drainage" which has received much propaganda in recent years shows good results only in the initial stages which are still reversible. In pronounced indurative scleredermas a considerable reduction in volume can be achieved by wrapping the extremity in non-elastic or tubular bandages which must be consistently preserved by compression stockings of class 3 and 4.

[Occupational immediate type allergy to hemp pollen and hashish (author's transl)].

A 26-year aged non-atopic criminological technician reacted to hemp (Cannabis sativa) and hashish with urticarial eruptions on hands and lower arms, rhinitis and conjunctivitis. Rubbing tests with pollenrich hashish were strongly positive. As hemp antigens were unavailable a hyposensitization therapy with hop (Humulus lupulus)--a plant belonging to the Cannabacea family--was begun.

[Intolerance to acetylsalicylacid and food additives in patients suffering from recurrent urticaria (author's transl)].

Ninety patients suffering from recurrent urticaria were orally exposed to 500 mg acetyl salicylic acid (ASA). 36.6% proved to be ASA intolerant in this group. 26 ASA-positive and 18 ASA-negative probands were selected to take part in an additional provocation test with 8 different food additives (preservative and colouring matters). Altogether 31 tests sheets were exaluated. Urticarial reactions were seen after administration of p-hydroxybenzoic acid methylester (5), p-hydroxybenzoic acid propylester (6), benzoic acid (9), sodium benzoate (6), tartrazine (6). Ponceau rouge (5) and indigo carmine (3). Detailed research was carried out on the occurrence of the tested substances. With a diet avoiding salicylates, benzoates and colouring matter 20% of these patients recovered spontaneously and became symptom-free, whilst a further 55% of cases showed marked improvement.
