[Origin and development of treatment of Guzhang syndrome (ascitis and abdominal distention) in TCM].

The theory and treatment of Guzhang syndrome was continuously developing as time went on. From Spring and Autumn, Warring States to the Han-Jin period, Neijin and Shang Han Lun laid down the foundation for the development of later ages. During the Tang-Song-Jin-Yuan periods, there were breakthrough and inventions in the recognition theory and treatment, particularly the contention between Gong (purgation) and Bu (reinforce) schools, thus further promoting the development of research on Guzhang syndrome. During Ming-Qing dynasties, based on summing up the experiences of forerunners, new implication or the causes, pathomechanism, diagnosis and treatment appeared with even greater progress. After inheriting and developing the available experiences, modern and contemporary doctors paid attention to combining TCM with modern medicine so that the theory and practice can be more perfected.

[The changing vasculo-cardiology in the midst of revolution of modern science and technology].

Modern revolution of science and technology exerts profound and overall influences on the basic research, diagnosis and treatment of cardiovascular diseases. The application of molecular biology has been revealing the nature of cause, pathogenic mechanism, physiology and pathology of cardiovascular diseases, while gene therapy is expected to yield a radical cure of these diseases. The products of modern technology including computerized tomography, computer enhanced digital angiography, magnetic resonance imaging, ultrasonic-cardiography, positron emission tomography, single photon emission computerized tomography, percutaneous transluminal coronary angioplasty and other techniques, laser, artificial pacemaker, artificial heart etc. have greatly improved the capacity of diagnosis and therapy of cardiovascular diseases. Now the studies probe not only the levels of organ and cell, but also the levels of protein, nucleic acid and gene. The diagnosis of cardiovascular diseases is focussed on, besides the traditional "four diagnostic methods", i.e. inspection, palpation, percussion and auscultation, precise diagnosis with the help of various advanced medical equipments. As to the treatment, in addition to traditional drugs and surgical operations, new therapeutic methods such as gene therapy and interventional measures are also applied.

[Reversal of drug resistance in multidrug resistant tumor cells by an oligomer complementary to the MDR1 gene].

OBJECTIVE: To overcome the multidrug resistance (MDR) in tumor cells. METHODS: Human drug resistant cell line KB-8-5 was transfected with a synthetic oligodeoxynucleotide complementary to the 5' end region of MDR1 gene (ODN). In vitro drug sensitivity was measured by MTT assay. Intracellular drug concentration was assessed by flow cytometric analysis and expression of P-glycoprotein (Pgp) was determined by immunohistochemistry method. RESULTS: The administration of ODN for 72 hours increased daunorubicin (DNR) accumulation and decreased Pgp expression in KB-8-5 cells. Incomplete reversal of MDR in the KB-8-5 cells was observed when lipofectin was used to deliver ODN to the cells. Lipofectin enhanced the reversing activity, and approximately 74.43% of the cells lost their resistance to DNR. CONCLUSION: ODN circumvents resistance in the KB-8-5 cells may be ascribed to the reduced expression of Pgp in the cells.

[Synthesis of N-salicylidene leucine 3d metal complexes and studies on their thin-layer chromatography and ultraviolet spectrum].

The Schiff base derived from salicylaldehyde and leucine and its Mn(II), Co(II), Ni(II), Cu(II), Zn(II) complexes were synthesized. The purity of the products obtained was confirmed by elemental analysis. The results of elemental analysis and molar conductance show that coordination ratio with M(II) is 1:1 and its complexes are non-electrolyte in DMF. The thin-layer chromatography behavior of complexes was studied on silica gel G plate with ethyl acetate-methanol-acetone-water-acetic acid (4.44:3.33:2.22:0.25:0.12, V/V) as mobile phase. The results show that the Rf value of complexes increases as follows Mn<Zn<Ni<Cu<Co. The lambda(max) values of ultraviolet spectra of ligand and complexes are determined in methanol and DMSO. The lambda(max) values of complexes increase as follows: Mn(II)<Zn(II)<Ni(II)<Cu(II)<Co(II)<ligand. The Rf values and R band lambda(max) values of complexes change in the same direction as the stability constants of the complexes. This proves that the Rf values of complexes on silica gel G and R band lambda(max) values of complexes depend on coordinaton of M(II) with ligand.

[A high performance liquid chromatographic method for the determination of teniposide in brain tissue using electro-chemical detection].

A reversed-phase HPLC method for the determination of teniposide in brain tissue is described. Teniposide can be separated on a Hypersil ODS column with a mobile phase of methyl alcohol-water-acetic acid (56:41:3, V/V) and flow rate of 0.8 mL/min (10 MPa). Column temperature was 35 degrees C and operating potentials for electrochemical detection was 0.70 V. To 50 mg brain tissue were 1 mL 50% ammonium sulfate and 4 mL ethyl acetate. The sample was vortexed for 5 min and ultrasonically agitated for 15 min, then centrifuged at 3000 r/min for 15 min. The upper (organic) layer was collected and the lower layer reextracted with 4 mL of ethyl acetate by vigorously vortexing, then centrifuged at 3000 r/min for 15 min. The organic layer was combined with the previously collected ethyl acetate layer. This organic extract was dried under a gentle nitrogen stream, and the residue was reconstituted with 1 mL internal standard of guaifenesinum before HPLC analysis. The linear range was 0.1-10.0 mg/L, and the detection limit 0.1 mg/L. Intra-day and inter-day RSD for assaying brain tissue sample were 0.87% and 1.41%, respectively. The average recovery was 92.87% with coefficient of variation of 2.35%.

[Analysis of ECG on the staffs exposed to microwave in the radio calling signal station].

321 electrocardiographies (ECG) from the staffs exposed to microwave in a radio calling signal station were analysed. The subjects exposed to microwave were female computer operators, who worked over six months in the station. 107 female non-exposers were served as control. The intensities of microwave in work site were below the standard (20-50 mW/m2). The abnormal prevalence of ECG in the exposed group was 27.73%, which was significantly higher than that in the control group (11.21%, P<0.01). The prevalence of sinus arrhythmia in exposed group (19.63%) was significantly higher than that in the control group (5.60%, P<0.01). The P-R intervals were also lengthened in exposed group [(0.1554+/-0.021) ms] than that in control group [(0.1487+/-0.0227) ms, P<0.01]. No significant differences were found in the heart rate, QRS intervals, Q-T intervals, sinus bradycardia, sinus tachycardia and left axis deviation between exposed and control groups (P>0.05). It suggested that computer operators exposed to microwave of lower intensity in the radio calling signal station might lead to sinus arrhythmia, due to the disorder of vegetative nerve system.

[Flame atomic absorption spectrometry determination of zinc and ferrum in enamel].

The amount of zinc and-ferrum in enanmel was determined by flame AAS. For twenty six healthy men, the average of zinc and ferrum were 325.6 +/- 84.8 and 34.7 +/- 9.1 microg/g, respectively. For fifty four patients with dental caries disease, the average of zinc and ferrum were 269.9 +/- 63.5 and 38.7 +/- 10.0 microg/g, respectively. The difference between healthy men and patients of zinc and ferrum rate was insignificant. As to the determination method, the rate of recovery varied from 97.0 to 101.5% for zinc and from 97.2 to 102.4% for ferrum. The RSDs fo Zn and Fe are less than 2.60 and 2.10% respectively.

[X-ray photoelectron spectroscopy (XPS) and mass spectrum (MS) study of organic tin compounds II].

Seven new synthetical (Z)-1-[2-(Triarylstannyl) vinyl]-1-cycloheptanols organic tin compounds were studied by X-ray photoelectron spectroscopy and mass spectrum method. The influence of different replacement radical in compounds on the d orbit of tin atom was discussed. The coordinate substance had an effect on the chemical displacement of Ols binding energy. The O-->Sn bond was affected by the different replacement radical. The results of X-ray photoelectron spectroscopy and mass spectrum show that Cl, Br and I had the important action.

RNA apatamers for yeast ribosomal protein L32 have a conserved purine-rich internal loop.

Two in vitro selection experiments were conducted to determine the RNA sequence requirements for binding ribosomal protein L32 (RPL32) from Saccharomyces cerevisiae. To preserve the wild-type stem-internal loop-stem fold, only a limited portion of the RNA comprising the internal loop region was randomized. Most of the selected RNAs have secondary structures similar to that of the wild-type, and four purines on both sides of the internal loop are highly conserved. Indeed, a pair of 5'-GA-3' dinucleotides is found in all but one of the stem-loop-stem L32 aptamers and these conserved purines may contact the protein directly or form a necessary RNA secondary or tertiary structure. These aptamers have a potential G:U pair bordering the loop adjacent to the conserved GAs, but a cytidine replaces a phylogenetically conserved adenosine at one loop position in many of the selected RNAs. In model RNAs, the cytidine-bearing variant binds protein slightly more strongly than does the wild-type RNA. That the seven-member, 2 + 5 internal loop is important for protein binding is reinforced by the finding that the position, but not the size, of the loop is variable. A minority of the RNA aptamers has three consecutive uridines and may fold into a similar structure, but with the internal loop inverted.

Safety and efficacy of electroconvulsive therapy in patients over age 85.

The authors report that electroconvulsive therapy (ECT) can be used safely and effectively in patients over age 85 with concurrent psychiatric and medical problems. Affective and psychotic illnesses are common and debilitating in geriatric patients. This same population has a high frequency of medical comorbidity, which makes it difficult for patients to tolerate adequate doses of standard pharmacotherapies to treat psychiatric problems. Also, many of these individuals are nonresponsive to psychopharmacologic therapies tried. Consequently, ECT becomes an important treatment option in elderly patients.

Semiparametric estimation of major gene and family-specific random effects for age of onset.

Analysis of familial diseases with variable age of onset is a common problem in human genetics. Most existing methods make some parametric distributional assumption on age of onset, and few methods have been designed with the goal of testing the hypothesis of a Mendelian gene against other hypotheses of familial dependence. We introduce the Cox model with major genetic and random familial effects to model age-of-onset dependence patterns among family members and to incorporate family heterogeneity. This model allows testing for and estimating major gene effects in the presence of residual correlations. Generalized maximum likelihood estimation using a Monte Carlo EM algorithm is used for parameter estimation. The methods are illustrated by a simulated data set and a data set from a case-control family study of breast cancer.

Surgical treatment for carcinoma of the oesophagus in Chinese language publications.

INTRODUCTION: This review is a meta-analysis of Chinese language publications on surgical treatment for oesophageal cancer. METHODS: Between 1980 and 1994, 17,815 patients had surgery for carcinoma of the oesophagus. The male:female ratio was 3.9:1 and mean age was 51.9 (range 16-80) years. RESULTS: Mean resectability rate was 86.7 per cent and hospital mortality rate was 3.8 per cent. The crude 5-year and 10-year survival rates for all patients were 29.6 and 16.4 per cent respectively. For patients with stage 0 and stage I tumours, the 5-year survival rate was 95 and 89 per cent respectively, significantly greater than that for those with stage II, III and IV tumours (19.4 per cent, P < 0.01). The 5-year survival rate of all 2226 patients with tumour resection after radiotherapy was 38.0 per cent. There was no significant difference in survival following combination therapy compared with surgery alone. When compared with a report for the interval 1940-1979, there was no significant difference in the resectability, hospital mortality or 5-year survival rates. CONCLUSION: Oesophageal carcinoma remains a disease with dismal prognosis but surgery still offers the best chance of long-term survival. Adjuvant therapy has not yet proved an advantage in terms of survival but can be useful for palliation. Mass screening might improve survival but only in a population with a high incidence of oesophageal carcinoma.

Histopathological changes in the testes of prepubertal male rats after chronic administration of cocaine.

PURPOSE: This study was conducted to evaluate histopathologic findings in the testes of prepubertal male rats after long-term cocaine exposure. METHODS: At 25 days of age, male Sprague-Dawley rats were administered cocaine hydrochloride daily (15 mg/kg body weight corresponding to an average single dose for a heavy cocaine user). The treatment was continued for 100 days when all the rats were sacrificed. Morphological analysis of the testes were assessed by qualitative and quantitative histological means. RESULTS: In all the groups, a minimum of 5 to 10 representative seminiferous tubules were examined. The mean diameter of the seminiferous tubules was less in the treated group than in their respective controls (p < 0.05). The thickness of the germinal epithelium was much reduced in the cocaine-treated groups when compared with their controls (p < 0.05). The number of degenerating germ cells was greater in the treated group than in the controls. There was evidence of failure to release the mature spermatids in the treated groups. There was no evidence of sloughed Sertoli cells or germ cells in the tubular lumen or the epididymis. CONCLUSION: There were distinct histopathological changes noted after chronic administration of cocaine. These changes are characteristic of toxic effects on the testes, but the exact mechanism is not clear. Further studies are underway in our laboratory to delineate the exact mechanism of action by cocaine on the testes.
