[A case of Behcet's disease with multiple intestinal ulcerations(author's transl)].

A case is reported of Behçet's disease in a 41-year-old man, in the course of which colitis with perforation developed over two years, with fatal results. This is a rare complication of Behcet's disease and must be differentiated from ulcerative colitis and Crohn's disease. Blood transfusions have a limited therapeutic value in the terminal phase of the disease, which among others is characterized by a generalized depression of response of peripheral lymphocytes to stimulation.

Haemoglobin G Norfolk: alpha 85 (F6) Asp leads to Asn.

The characterisation of haemoglobin G Norfolk (alpha2 85 Asp leads to Asn 2) is described. This variant has been identified in the heterozygous state in three apparently unrelated English families of which two are resident in England and the other in Canada.

Hormonal activity of testes in aged rats with leydig cell hyperplasia: histochemical investigation of some hydroxysteroid dehydrogenases.

The following enzymes were employed in order to investigate the hormonal activity of the testes in aged rats: 3-beta hydroxysteroid-DH, 11-beta-hydroxysteroid-DH, and 17-beta-hydroxysteroid-DH. Our investigations show that in the non-altered testis of the aged rat the same enzyme activities can be observed as in young and sexually mature animals. A proliferation of the Leydig cells (L.c.), occurring in the aging rat, brings about an increased activity of the above-mentioned steroid-DHs. An augmented production of testosterone is quite probable.

[Classification of mitral valve disease by clinical features and hemodynamic parameters (author's transl)].

Using the central venous oxygen saturation (SO2v) at rest and during exercise (50 Watt) a new classification of mitral valve disease is proposed, which corresponds with the recommendation of the NHA (1953). Contrary to the mostly subjective classification by clinical features, the new one results from objective hemodynamic parameters. The validity of the new proposed classification is shown using different clinical parameters.

Erythraemia due to haemoglobin San Diego.

A 63-year-old man with erythraemia was found to have a high affinity haemoglobin which was subsequently identified as Hb San Diego beta 109 (G11) Val leads to Met, recently described in a Filipino family (Nute et al, 1974). The oxygen affinity of the haemolysates was increased approximately two-fold at pH values between 6.04 and 7.90. The n value was nearly normal (2.5-2.9) in the upper range of saturation (70-95% oxygenation) but was somewhat diminished (1.9-2.1) below this range. In vitro biosynthesis of globin by reticulocytes showed that there was balanced synthesis of alpha and beta chains (alpha:beta ratio = 1).

A new cause of haemolytic anaemia in the newborn. A description of an unstable fetal haemoglobin: F Poole, alpha2-G-gamma2 130 trptophan yeilds glycine.

In a newborn twin with haemolytic anaemia an unstable fetal haemoglobin was found to be the cause. The anaemia improved spontaneously with the disappearance of the fetal haemoglobin. The new Hb F (alpha2gamma2) variant was shown to have a glycine at position 130 of the 146 residues of the gamma chain. This portion is inside the globin molecule and in all known normal globins it is occupied by a residue with a bulky hydrophobic side chain. Its replacement by glycine which has no side chain would be expected to cause instability. The human gamma-chains may either have a glycine or an alanine at position 136. Evidence is brought forward to suggest that in the abnormal chain position 136 is occupied by glycine.

Homozygous haemoglobin D Punjab.

A homozygote for the gene controlling Hb D Punjab is described. The diagnosis is supported by the peptide analysis of the haemoglobin and the examination of both parents. There was no anaemia or reticulocytosis and there was balanced synthesis of both the alphaA- and betaD-globin chains in reticulocytes. However, the oxygen affinity of the haemolysate had a small but significantly higher oxygen affinity than normal. The only other abnormalities were a somewhat decreased osmotic fragility and a slight anisocytosis of the red cells.

Haemoglobin A-2-NYU in a London population.

Electrophoresis of haemolysates from a population in London resulted in the detection of a variant of haemoglobin-A-2. Purification and characterisation of the variant globin revealed that this was a further example of haemoglobin A-2-NYU.

Haemoglobin G-Szuhu, beta80 Asn-Lys, in the homozygous state in a patient with abetalipoproteinaemia.

An 11-year-old Jewish girl of Turkish extraction with abetalipoproteinaemia was found to be homozygous for haemoglobin Szuhu (beta80 Asn leads to Lys). Except for the abnormal haemoglobin, no other haematological or biochemical abnormalities were found in her consanguineous parents and one sister. In the propositus, erythrocyte morphology showed the acanthocytosis known to be in association with abetalipoproteinaemia. Increased autohaemolysis was also found, which reverted to normal after treatment with vitamin E. This case represents the first reported association of abetalipoproteinaemia with an abnormal haemoglobin, and the first homozygous Hb G-Szuhu.

Futher investigations of the gamma-chaina in a ghanaian adult, homozygous for hereditary perisitence of fetal haemoglobin. Isolation of gamma CB-3 peptides and Ggamma:Agamma ratio determination in human Hb E.

The gamma-chain in a Ghanain homozygous for hereditary persistence of fetal haemoglobin was considered to be of the Ggamma type on the basis of the amino acid analysis of gammaTp XIV (gamma133-144) of the Hb F of this subject [1]. Recently, the sequence of residues gamma134-137 of the gamma-chain of this subject was determined and found to contain some alanine at position gamma136. It is therefore of the Ggamma + Agamma type. A rapid technique for the isolation of gammaCB-3 (gamma134-146) peptides in human fetal haemoglobin for Ggamma:Agamma ratio determination is described.

A new Hb variant: Hb F Sardinia gamma75(E19) isoleucine leads to threonine found in a family with Hb G Philadelphia, beta-chain deficiency and a Lepore-like haemoglobin indistinguishable from Hb A2.

A 59-year-old man with beta-thalassaemia major is unusually well. He has no beta-chains in his haemoglobin but is heterozygous for the genes responsible for alphaA and for alphaG Philadelphia. In addition he is also heterozygous for the genes responsible for gammaF and a new gamma-chain, gamma75(E19) Ile-Thr, named gammaF Sardinia. It was not possible to isolate the mutant gamma-chain, but from the overall ratios of Ggamma-:Agamma-Chain and gamma75 Ile : gamma75 Thr, it could be inferred that the mutation had presumably occured in the Ggamma-chain. Some of the propositus' siblings have high levels of Hb A2, and it is suggested that they possibly carry a gene for Lepore chain with the deltabeta-crossover after residue delta126.

[Immunglobulin levels and alterations of the lymphocyte function after extracorporal circulation and massive blood transfusion (author's transl)].

Cardiovascular operations are connected with an increased risk of infection. Postoperatively there happened a temporary fall of all immunglobulin levels. Concerning the cellular immunity, we found an impressive decrease of the transformation of the lymphocytes in the presence of polyclonal mitogens which normalises only partially after the eighth day. These results refer to a complex alteration of the immunological apparatus, imposing as a temporary immundeficiency, and could contribute to an elucidation of the increased risk of infections.
