Cubitus varus deformity following supracondylar fractures of the humerus in children.

A retrospective study of 63 patients with cubitus varus deformity following supracondylar fracture of the humerus was carried out in order to identify the causes of cubitus varus, to define the best method for prevention, and to evaluate the indications for and results of treatment of the established deformity. There was no evidence of growth disturbance in any of our patients after a mean follow-up of 5 years postfracture. Cubitus varus was found to be secondary to medial tilting of the distal fragment. Failure to recognize this initial or subsequent medial tilt during early treatment of the fracture was the major factor in the development of cubitus varus deformity, thus explaining the average delay in diagnosis of 6 to 10 weeks postfracture. Since all patients regained essentially normal use of the upper extremity after the fracture, cosmesis was the primary indication for corrective surgery. Fifteen supracondylar osteotomies are reported with 33% unsatisfactory results. Prevention of this deformity by careful clinical and radiological evaluation during initial treatment of the supracondylar fracture is emphasized.

The abnormal lateral patellofemoral angle: a diagnostic roentgenographic sign of recurrent patellar subluxation.

On roentgenograms made with the patient supine, the knees flexed 20 to 30 degrees, the x-ray tube between the ankles, and the cassette held proximal to the knees and perpendicular to the x-ray beam, it was found that a line between the femoral condyles and a line between the margins of the lateral facet of the patella formed the lateral patellofemoral angle, an angle that was of diagnostic value in patients with subluxation of the patella. In 100 clinically normal patients, these lines were parallel in three and formed an angle open laterally in ninety-seven. In thirty patients with subluxation of the patella, the lines were parallel in twenty-four and formed an angle open medially in six. In 100 patients with chondromalacia of the patella, however, the roentgenographic study was of no diagnostic value since the lines were parallel in ten and formed an angle open laterally in ninety.

Efficacy of diclofenac in lateral epicondylitis of the elbow also treated with immobilization. The University of Montreal Orthopaedic Research Group.

OBJECTIVE: To evaluate the efficacy of an oral nonsteroidal anti-inflammatory drug in the treatment of lateral epicondylitis. DESIGN: Multicenter double-blind randomized controlled trial in which the following hypothesis was tested: whether diclofenac sodium provided a 20% or greater improvement over rest and cast immobilization in the response rate to treatment of lateral epicondylitis beyond and over rest in an experimental group compared with a control group after 4 weeks of treatment. SETTING: Recruitment from urban general practices and referrals to 4 university hospitals. SUBJECTS AND METHODS: During a 1-year period, 206 subjects aged 18 to 60 years with lateral epicondylitis were recruited from the clientele treated by family physicians. Thirty subjects refused to participate and 47 presented with exclusion criteria, leaving 129 subjects who entered the study. One subject withdrew after 21 days. INTERVENTIONS: The experimental group was treated with a daily dose of diclofenac sodium (150 mg) for 28 days, while the control group received a placebo during the same period. In addition, both groups were immobilized in a cast for 14 days and were told not to perform repetitive movements of the involved limb for 21 days. MAIN OUTCOME MEASURES: Measuring instruments consisted of grip strength measurements with a squeeze dynamometer, a visual analog pain scale, a visual analog function scale, and an 8-item pain-free function index. RESULTS: A statistically and clinically significant reduction of pain was associated with treatment with diclofenac, but no clinically significant difference in grip strength or functional improvement could be detected between the 2 groups. Secondary effects (diarrhea and abdominal pain) were significantly more frequent in the diclofenac-treated group. CONCLUSION: Taking into account the limited improvement noted over rest and cast immobilization and the number of associated adverse events, it is difficult to recommend the use of diclofenac in the treatment of lateral epicondylitis at the dosage used in this study.

Relationship between morphologic somatotypes and standing posture equilibrium.

Previous studies have identified height and weight as important factors affecting quiet standing stability but studies have not addressed body morphology as a global factor. Using anthropometric measurements, the morphologic somatotypes were defined in terms of body composition and structure. The aim of this study was to test the hypothesis that morphologic somatotypes were related to standing posture equilibrium in able-bodied girls. A total of 43 able-bodied girls having a mean age of 13.8 +/- 2.2 years participated in this study. Somatotype measurements were taken to determine their endomorphic, mesomorphic or ectomorphic components. Then, subjects were asked to stand still on a force platform for 64 s with their eyes opened, feet about 23 cm apart and arms aligned with the trunk. Afterwards, subjects were grouped based on the highest value of their somatotype component. There was no statistical difference in age, height and weight among the groups. The surface area of an ellipse delineated by the displacement of the centre of pressure (COP) was statistically larger (236.9 +/- 134.3 mm2) for the ectomorphs than for the endomorphs 137.7 +/- 71.4 mm2). The minor axis was longer (8.1 +/- 2.9 mm) for the ectomorphs than for the endomorphs (5.7 +/- 2.2 mm). The decrease in standing posture stability of the ectomorphic group was attributed to a relatively low muscle component, a high height weight ratio and an elevated position of the body centre of mass in this population of girls. Somatotypes should be considered when assessing standing posture in both able-bodied subjects and patients.

Peracute streptococcal pyomyositis: report of two cases and review of the literature.

Pyomyositis occurs infrequently in temperate climates. The fulminating peracute form caused by group A Streptococcus is exceedingly rare. We present two children with streptococcal pyomyositis. One child was admitted in septic shock and required intensive supportive care during the acute stage of his illness. Persistent swelling and tenderness of his left thigh presented a diagnostic problem, which was eventually resolved with the aid of computerized tomography. Despite appropriate antibiotic therapy from the onset of illness, surgical debridement of the affected muscle was necessary. The second child presented with pyomyositis of the left paravertebral muscles and signs of incipient shock but did well on antibiotic therapy alone.
