Characterization of biochemically atypical Vibrio cholerae strains and designation of a new pathogenic species, Vibrio mimicus.

Biochemically atypical strains classified as Vibrio cholerae were characterized by biochemical reactions, serology, antibiotic susceptibility testing, and deoxyribonucleic acid relatedness. Strains with the following atypical reactions were shown to be V. cholerae: mannose negative, mannitol negative, lysine decarboxylase negative, no growth in the presence of 5% NaCl, salicin and cellobiose positive. Sucrose-negative strains were shown to constitute a new species, Vibrio mimicus, whose type strain is 1721-77 (ATCC 33653). In addition to its negative sucrose reaction, V mimicus was differentiated from V. cholerae by its negative Voges-Proskauer, corn oil, and Jordan tartrate reactions and by its sensitivity to polymyxin. V. mimicus was isolated from shellfish and water, as well as from human diarrheal stools and ear infections. Most strains were typable with antisera against V. cholerae. Strains from three serogroups produced either a heat-labile or a heat-stable enterotoxin.

Energy management in long-term care facilities: a hot or cold issue?

Conservation of energy resources through total energy management programs is receiving considerable attention in the health services sector. Although the total energy management concept has been favorably implemented in hospitals, the record is not entirely clear for other health care institutions. Thirty-one Arizona and 37 Minnesota long-term care facilities were surveyed to examine the attitudes, knowledge and practice of energy management in the nursing home context. Specific questions were directed toward average monthly energy costs, energy consumption, energy conservation methods implemented, energy conservation methods planned for future implementation, and administrator attitudes on the energy management problem. The results of this study indicate that energy is not perceived to be a major problem in long-term care facilities. Administrators generally lack basic knowledge about energy consumption and energy-related characteristics of their facilities. Few long-range plans and programs have been established to address energy problems. These results suggest the need for new energy policies in the health care system, particularly for institutions other than hospitals.

Cost containment in health care: a model for management research.

Cost containment is a dominant problem in the health care field, but it has not been addressed from a comprehensive management perspective. To fill this gap, we have developed an inclusive model of the cost containment process. The model has implications for management research in several areas: cost containment baselines, incentive systems, organization structures, cost/quality trade-offs, and cost containment constraints.

Costs and cost containment in nursing homes.

The study examines the impact of structural and process variables on the cost of nursing home care and the utilization of various cost containment methods in 43 california nursing homes. Several predictors were statistically significant in their relation to cost per patient day. A diverse range of cost containment techniques was discovered along with strong predictors of the utilization of these techniques by nursing home administrators. The trade-off between quality of care and cost of care is discussed.

Gastro-oesophageal reflux and hiatal hernia in children.

One case of a para-oesophageal hernia and ten cases of gasto-oesophageal reflux with or without hiatal hernia are reviewed. Vomiting commenced from the first week of life and in three children progressed to stricture formation. Pulmonary manifestations of gastro-oesophagela reflux were intractable asthma and attacks of bronchiolitis. A surgical operation performed through the abdomen was successful in controlling the reflux in all cases. Follow-up barium studies in all cases showed no hernia recurrences. A conservative approach to fibrous reflux strictures has been adopted and it is found that these can be cured by dilatations once the reflux has been controlled.
