Production of rubratoxin B by Penicillium purpurogenum Stoll.

Culture filtrates and chloroform extracts from Penicillium purpurogenum isolated from foodstuffs were subjected to preliminary survey for toxic effects on HeLa cells and mice. The toxic metabolite was isolated in a crystalline form from P. purpurogenum and was identified as rubratoxin B. Pathological findings observed in HeLa cells and mice treated with the metabolite are briefly described.

The effect of macrophage colony-stimulating factor (M-CSF) on the progression of lipid-induced nephrotoxicity in diabetic nephropathy.

In order to elucidate the role of macrophage in lipid-induced nephrotoxicity in diabetic nephropathy, we examined the effect of macrophage colony-stimulating factor (M-CSF) on the progression of renal lesions in hypercholesterolemic steptozotocin (STZ)-diabetic rats fed with high cholesterol chow. Hypercholesterolemia aggravated albuminuria in diabetic rats accompanied by infiltration of macrophages in glomeruli. Treatment with M-CSF suppressed simultaneously infiltration of glomerular macrophages and urinary albumin excretion in hypercholesterolemic diabetic rats. These results suggest that infiltration of glomerular macrophage has a primary role in lipid-induced nephrotoxicity in diabetic nephropathy, and M-CSF is involved in this process as a preventive factor.

Docetaxel in combination with carboplatin for chemo-naive patients with epithelial ovarian cancer.

We conducted a study of docetaxel-carboplatin combination therapy to confirm the efficacy and toxicity in chemotherapy-naive patients with ovarian cancer. Docetaxel 70 mg/m2 and carboplatin (area under the concentration-vs.-time curve of 5) were administered consecutively on day 1 of a 21-day cycle for five planned cycles in chemo-naive patients with the International Federation of Gynecology and Obstetrics stage IC to IV ovarian cancer with or without successful cytoreductive surgery at staging laparotomy. Twenty-six patients (median age, 53 years; range, 34-76 years) were enrolled into this trial at Niigata University Hospital. The major toxicity with this regimen was neutropenia. The incidence of grade 3 and 4 neutropenia were 27% (7/26) and 69% (18/26), respectively. However, the neutropenia was brief and reversible with G-CSF support. Nausea/emesis, fatigue, arthralgia/myalgias, and alopecia were the most common nonhematologic toxicities, in which no grade 3 or 4 toxicity was observed. Neurotoxicity was infrequently observed. Nine of 11 assessable patients responded to the regimen. We conclude that the combination of carboplatin and docetaxel seems to be highly active in ovarian cancer with the major toxicity of neutropenia, and the extremely low incidence of clinically significant neurotoxicity. Randomized controlled clinical trials should be conducted to define a role for this regimen in ovarian cancer.

Combined effect of amphotericin B and flucytosine on hyphal growth of Candida albicans estimated at a single hypha level.

The in vitro efficacy of amphotericin B and flucytosine, separately and in combination, against the hyphal growth of Candida albicans was evaluated in situ using an automatic analysing system. A colony of C. albicans was in contact with a glucose-salt medium supplemented with biotin plus calf serum (GS medium) and GS medium containing the antifungal agent, in sequence. Minimum inhibitory concentrations at single hyphal level (S-MIC) were determined based on the response that was measured. Amphotericin B S-MICs ranged from 0.8 to 0.4 microgram ml-1, and S-MIC values > 102.4 micrograms ml-1 were obtained with flucytosine when the two agents were used independently. When the two agents were used in combination, however, a synergistic interaction between the two agents at concentrations below their individual S-MICs was observed.

Adult xanthogranulomatous intracranial lesion involving familial hypercholesterolemia.

A 35-year-old man was admitted because of loss of hearing in the left ear. The patient had been known to have familial hypercholesterolemia for at least 12 years. Computerized axial tomography of the brain showed a large tumor occupying in the left mastoid region. Surgical intervention revealed xanthogranuloma, histologically. Xanthogranuloma is classified as a kind of normocholesterolemic xanthomatoses. Hypercholesterolemia with adult xanthogranuloma (AXG) is extremely rare. Moreover intracranial involvement with AXG has been reported in only one previous case. We wish to report on the possibility of a new syndrome that has characteristics common to primary xanthomatoses, entities which have heretofore been considered etiologically distinct.

Amelioration of proteinuria with pravastatin in hypercholesterolemic patients with diabetes mellitus.

Pravastatin, a novel inhibitor of 3-hydroxy-3-methylglutaryl-coenzyme A reductase, was administered to nine hypercholesterolemic patients with diabetes mellitus to examine its effects on diabetic nephropathy. Pravastatin treatment resulted in lowering serum total cholesterol by 22.1% on the average (p less than 0.001), and led to a significant reduction in urinary excretion of albumin and beta 2-microglobulin in patients with an elevated urinary protein excretion rate at the baseline period. But glycemic control, blood pressure, urinary excretion of creatinine and that of N-acetyl-beta-D-glucosaminidase showed no significant change during the study. These results suggest that reduction of atherogenic lipids and lipoproteins with pravastatin could alleviate diabetic glomerular injury.
