Increased serum levels of squamous cell carcinoma-related antigen in pemphigus.

Serum levels of squamous cell carcinoma-related antigen (SCC-RAG) were measured in five cases of pemphigus, five cases of bullous pemphigoid and 18 cases of benign and malignant dermatoses other than SCC. The SCC-RAG titres were significantly raised in four of five patients with pemphigus, while they remained within the normal range in the other dermatoses except in one case. In three pemphigus cases in whom serial measurements were made, SCC-RAG levels seemed to be related to disease activity. The SCC-RAG levels in blister fluids were much higher than those in serum, suggesting that the skin is a major source of serum SCC-RAG. These results show that SCC-RAG is increased not only in SCC, but also in some cases of pemphigus, and suggest that pemphigus antibodies may cause the production or release of SCC-RAG.

Contact dermatitis from a compound mixture of sugar and povidone-iodine.

2 patients with leg ulcers got worse after the application of a compound mixture of sugar and povidone-iodine (sugar/PI compound). Because they had been suffering from stasis dermatitis, symptoms of contact dermatitis were ambiguous. Patch tests showed positive reactions to 10% povidone-iodine in water and 5% potassium iodide in water, with no response to sugar. They were also tested with sugar/PI compound, containing 3% povidone-iodine, resulting in another positive reaction. They improved after the application of sugar/PI compound was discontinued. Contact dermatitis from topical agents should be considered as more probable than angry back syndrome in cases of leg ulcer.

Effects of sera from patients with obstructive jaundice on the generation of oxygen intermediates by normal polymorphonuclear leukocytes.

Recently it has been suggested that oxygen intermediates play an important role in the pathogenesis of tissue damage. The effect of sera from patients with obstructive jaundice on the generation of oxygen intermediates by normal polymorphonuclear leukocytes (PMNs) was investigated. Sera from patients with obstructive jaundice increased superoxide anion (O2-), hydrogen peroxide (H2O2) and hydroxol radical (OH.) generation compared with sera from healthy individuals or patients with biliary tract stones and/or tumors of the biliary tract or pancreas (without obstructive jaundice). In particular, the hydroxyl radical, which is one of the most potent oxidants capable of causing tissue damage, was produced in large quantities. Sera from patients with obstructive jaundice have a strong capacity to induce production of oxygen intermediates from PMNs, and oxygen intermediates may play a role in the pathogenesis of hepatic and other organ injury in obstructive jaundice.

Distribution of vitamin B12 R-binder in carcinomas of the digestive tract.

To evaluate whether the increase in serum transcobalamin I, seen in patients with carcinoma, is caused by synthesis of R-binder to tumour cells, the distribution of vitamin B12 R-binder in 125 malignant growths of the digestive tract was studied. Positive staining for R-binder with immunoperoxidase was observed in 85 (70%) carcinomas. Positive staining for R-binder was observed in all four cholangiocarcinomas studied, but was absent in nine hepatocellular carcinomas. These findings suggest that determination of R-binder in liver tumours may be of some value in differentiating hepatocellular carcinomas and cholangiocarcinoma, and that synthesis of R-binder by tumour cells causes an increase in serum transcobalamin I.

Effect of ba-wei-di-huang-wan (hachimi-jio-gan) on thymidine kinase and its isozyme activities in the prostate glands in rats.

Ba-wei-di-huang-wan (Hachimi-jio-gan, TJ-7) has been frequently used for the treatment of sterilitas virilis, prostatic disorders, etc. The hormonal effects of TJ-7 were studied using thymidine kinase (TK), an enzyme that catalyzes the phosphorylation of thymidine via the pyrimidine salvage pathway. TK isozyme activity increases in rapidly proliferating tissues. TJ-7 may act as a weak androgen and enhance prostatic TK and in particular, a specific TK isozyme activity, and also as an anti-androgen and reduce prostatic TK activity in the presence of androgen in immature rats.

Metabolic fate of indeloxazine hydrochloride: alpha-glucoside formation in rats.

1. After oral administration of indeloxazine hydrochloride ((+/-)-2-[(inden-7-yloxy)methyl]morpholine hydrochloride) to rats, two conjugates, which were labile to alpha-glucosidase hydrolysis but refractory to beta-glucosidase, were isolated from the urine. 2. Mass spectral and n.m.r. analyses confirmed that these conjugates were alpha-D-glucopyranosides of M-2 (trans-4-(2-morpholinylmethoxy)-1,2-indandiol) and M-3 (trans-6-[[(1,2-dihydroxy-4-indanyl)oxy]-methyl]-3-morpholinone). 3. These are probably the first examples of foreign compounds conjugated with glucose in the alpha-configuration.

Sequelae of the osmotic blood-brain barrier opening in rats.

Histopathological sequelae of the osmotic blood-brain barrier opening were studied in 69 adult Wistar rats sacrificed between 2 minutes and 6 days after infusion of 1.6 M mannitol into the unilateral internal carotid artery. The results were correlated with immunohistochemical localization of autologous albumin in the brain parenchyma on paraffin sections. Extravasation of serum albumin was evident in all rats, and the albumin immunoreactivity, commonly localized to the territories of the ipsilateral anterior, middle, and posterior cerebral arteries and contralateral anterior cerebral artery, showed maximum intensity in the rats sacrificed 30 minutes after infusion. The albumin immunoreactivity remained macroscopically visible in the brain parenchyma for 24 to 48 hours, and then gradually faded out. Serum extravasation was accompanied by widening of the perivascular space and focal edema, which largely subsided within 48 hours as the albumin immunoreactivity of the tissue diminished. Although no overt neurological sequelae were seen in the present experiment, minute but definite foci of infarction with focal accumulation of albumin were found in 23 (38%) of 61 rats surviving more than 30 minutes. In addition, ischemic neuronal death of delayed onset was encountered among neurons in the CA-1 region of the hippocampus, in the cerebellum, and in the thalamus in five (25%) of 20 rats sacrificed between Days 4 and 6. Thus, care should be exercised in the practice of this procedure.

Expression of vitamin B12 R-binder in breast tumors. An immunohistochemical study.

Vitamin B12 R-binder, a vitamin B12-specific binding protein, was demonstrated by immunohistochemistry in 103 cases of breast tumors of various types and grades. In normal breast tissues, R-binder was found on the luminal surface of interlobular, intralobular, and small terminal ducts. R-binder localization was observed in all fibroadenomas and phyllode tumors, in 90% of the benign epithelial tumors, and in 33% of the carcinomas. Among malignant tumors, 43% of the ductal carcinomas expressed the R-binder, whereas none of the lobular or mucinous carcinomas were positive. In invasive ductal carcinomas, R-binder expression was demonstrated more often in well-differentiated lesions.

[Multicentric papillary and follicular carcinomas of the thyroid].

A 63-year-old female, complaining of a swelling in the neck, was diagnosed as having a thyroid carcinoma. Thus, a subtotal thyroidectomy was performed, leaving a part of the right lobe. Two thyroid carcinomas were found in the left lobe, and one carcinoma in the right lobe with a right cervical lymph nodal involvement. Each carcinoma was small and encapsulated by thick fibrous tissue, but there was no continuity among them. The three carcinomas were regarded as being multicentric in origin, and not an intraglandular metastasis.

Plasma levels of secretory IgA in patients with gastric cancer.

The levels of plasma secretory IgA were measured in patients with gastric cancer and found to be slightly higher (9.6 +/- 6.2 micrograms/ml) than those in healthy controls (7.0 +/- 2.6 micrograms/ml, 0.05 less than P less than 0.1). Secretory IgA levels in those with hepatic metastases (19.7 +/- 12.2 micrograms/ml) were significantly higher than those in patients without hepatic metastases (P less than 0.001). In the latter, there was no significant relationship between plasma secretory IgA levels and the deepest layer of cancerous invasion or lymph node metastases. The secretory IgA levels in cases of well differentiated tubular adenocarcinoma were significantly higher than those with poorly differentiated adenocarcinoma (P less than 0.05). Although there is small diagnostic value in the detection of gastric cancer by measuring the levels of secretory IgA, high levels of secretory IgA in gastric cancer patients may be indicative of the presence of hepatic metastases.

Immunohistochemical distribution of vitamin B12 R-binder in renal cell carcinoma.

Renal cell carcinomas and normal kidney tissues were examined for the expression of vitamin B12 R-binder by the indirect immunoperoxidase method. In normal kidney tissue, the presence of the vitamin B12 R-binder was shown to be confined to the straight portion (pars recta) of proximal tubules. Seven of the 38 cases of renal cell carcinoma (18%) expressed the vitamin B12 R-binder antigen. This provides a further evidence of the proximal tubular nature of renal cell carcinoma, and suggests that a small proportion of renal cell carcinomas originate from the straight portion of the renal proximal tubules.

A variant Burkitt-type translocation (2p-;8q+) in a patient with diffuse large cell lymphoma.

A case of diffuse large cell lymphoma with t(2p-;8q+) is reported. Immunologically the lymphoma cells were shown to be of B-cell origin and positive for surface gamma and kappa chains, B4, CALLA, and Ia1 markers. Karyotypically three major clones were detected: 47,XX, + 12,t(2;8)(p11-13;q24) (52%); 47,XX, + 12 (26%); and 46,XX,t(2;8)(p11-13;q24) (15%). A t(2p-;8q +) has been exclusively reported in cases of Burkitt's lymphoma or Burkitt-type acute lymphocytic leukemia. The present case is the first one with t(2p-;8q +) observed in non-Burkitt-type lymphoid malignancy of the B-cell lineage. The t(2p-;8q +) may play a primary role in the early stage of transformation of B cells, and trisomy 12 may provide them secondarily with an advantage for tumor progression. The phenotypic pictures provided by 8q24 rearrangements seem to be heterogeneous, as previously suggested.

High levels of IgA-containing circulating immune complex and secretory IgA in Kawasaki disease.

Sera of patients with Kawasaki disease were studied for the levels of IgA-containing (C3-fixing) circulating immune complexes (IgA-CIC), IgG-containing (IgG-)CIC, total IgA, secretory IgA, and complement component (C3) by means of enzyme-linked immunosorbent assays or single radial immunodiffusion methods. There was significantly high level of IgA-CIC, but not IgG-CIC. The levels of total IgA, secretory IgA, and C3 were significantly elevated. Significantly high levels of secretory IgA were found in 22 (51%) of 43 patients. The proportion of secretory IgA to total IgA also increased. These abnormalities in the IgA system may play a role in Kawasaki disease.
