Genetic epidemiology of the Mitsuda reaction in leprosy.

This study comprised 544 nuclear families with 2,925 individuals tested for the Mitsuda reaction, from the Campinas region in Brazil. Segregation analyses suggest the segregation of a major gene (chi 2(1) = 0.07 - 0.07 = 0, p = 1), by failing to reject the hypothesis of Mendelian transmission and by rejecting the hypothesis of nontransmission of a major gene (chi 2(3) = 0.07 - 0.0 = 0.07, p > 0.99; chi 2(2) = 198.28 - 0.0 = 198.28, p < 0.0001).

Serum uric acid levels in Chagas' disease.

Uricemia was studied in a sample of 192 individuals from a highly endemic site for Chagas' disease (Bambuí, State of Minas Gerais, Brazil). The sample had 50 serologically negative individuals (controls) and the positive ones were classified on the basis of the presence of electrocardiographic alterations (63), altered esophageal emptying (16), or without any sign on symptom of the disease (76). Only the individuals with the digestive form of chronic Chagas' disease showed hyperuricemia, when compared with the appropriate controls. Family data suggest that hyperuricemia is an effect of the digestive pathology, rather than a cause, since the non-infected sibs of the megaesophagous patients did not show elevated levels of serum uric acid. Possible mechanisms responsible for these findings are postulated.

On the association between HLA-A1 and B5 and clinical forms of schistosomiasis mansoni.

The association between both HLA-A1 and B5 antigens and chronic forms of human schistosomiasis was studied in 64 patients and 26 normal controls from a southern Brazilian hospital. No apparent correlation between the chronic forms of the disease and the expression of those antigens was detected. However, the analysis of these data together with those observed on an Egyptian sample suggests that the presence of either of the antigens and the hepatomegalic forms of schistosomiasis is significant, without heterogeneity. Conversely, the association of histocompatibility antigens with splenomegaly is consistent and significant only for HLA-B5, but not HLA-A1.

An appraisal of the epidemiology of Trypanosoma cruzi serology in Brazil.

A large bibliographic survey provided data on Trypanosoma cruzi serology covering the period 1948-1984. Epidemiological-demographic methods provided an estimate of 11% for the prevalence of positive serology in Brazil, by 1984. Significant temporal trends were observed for most of the Brazilian geographical regions as well as for Brazil, as a whole. The parabolic curve that fit best for the entire country, indicates that by 1991, the incidence of new positive serology would be close to zero. This conclusion needs further fine-adjustment, since the forecast point is somewhat distant from the measured period.

A model for intra-familial distribution of an infectious disease (Chagas' disease).

A probabilistic model for intra-familial distribution of infectious disease is proposed and applied to the prevalence of positive serology for Trypanosoma cruzi infection in a Northeastern Brazilian sample. This double binomial with one tail excess model fits satisfactorily to the data and its interpretation says that around 51% of these 982 families are free of infection risk; among those that are at risk, 3% have a high risk (0.66), probably due to high domestic infestation of the vector bug; while 97% show a small risk (0.11), probably due to accidental, non-domestic transmission.

Malaria control in an agro-industrial settlement of Rondônia (Western Amazon region, Brazil).

A malaria control pilot project was developed in the Urupá agro-industrial farm that is situated in the State of Rondônia (Western Amazon Region, Brazil). Around 180 inhabitants had been surveyed for the past five years. The control measures were based on (1) training a community agent to perform on the spot microscopical diagnosis of malaria and to treat the uncomplicated cases of malaria; (2) limiting the use of insecticides to a short period before the high transmission season. This resulted in a significant reduction in the time between the onset of clinical symptoms and specific chemotherapy which fell from 3.5 to 1.3 days. In relation to the previous three reference years the total number of malaria cases was reduced to 50% in the first year and to 25% in the second year. The introduction of these measures coincided with pronounced reduction in the frequency of Plasmodium falciparum infections but this was less marked for P. vivax infections. In the second year of the pilot experiment there was no P. falciparum transmission on the farm. During the last decade there was a general decrease in the endemicity of malaria in the State of Rondônia. The linear regression coefficient values indicate that the decline was more pronounced in Urupá than in the general municipality and that the falciparum malaria API in Urupá farm is significantly lower than in the general municipality of Candeias were the farm is situated.
