[The effect of contrast medium injection into the left ventricle on systolic and diastolic ventricle function].

The influence of contrast medium on systolic and diastolic left ventricular function was studied in eight patients with coronary heart disease and in five dogs during and five minutes after ventriculography. Two biplane ventriculograms were performed in 5-minute intervals, in the dogs an additional one after 10 minutes. 0.66 cc/kg BW of sodium methyl-glucamin-iothalamate were injected during diastole into the left ventricel. 1. During ventriculography there are only slight changes of left ventricular volumes, pressures and diastolic compliance. The first four opacified heart cycles can be evaluated for the analysis of left ventricular function. 2. In patients with coronary heart disease there are much more pronounced alterations of systolic ventricular function 5 minutes after Ventriculography than in the normal dog. The ischemic ventricle has a reduced tolerance for volume load and negative inotropic effect of contrast media. In patients and dogs diastolic compliance is significantly increased 5 minutes after the injection of contrast medium. After 10 minutes however in the dogs compliance is significantly decreased. Analysing left ventricular function by repeated ventriculographies one has to take into account that the function is altered by the first angiogram. The degree of the changes depends on the initial hemodynamic conditions and on the time interval between the ventriculograms.

[Influence of nitroglycerin on hemodynamics, wall tension and oxygen consumption of the left ventricle].

The effects of nitroglycerin on left ventricular hemodynamics were determined in ten patients from biplane cineventriculograms and simultaneous pressure measurements. Before and after nitroglycerin (1.6 mg sublingually) the following parameters were measured: Systolic (PLV) and enddiastolic pressure (PLVED), enddiastolic (EDV) und endsystolic volume (ESV), cardiac output (SV), ejection fraction (EF) and ejection rate (ER), ejection time (tej), heart rate (HR), dp/dtmax, dp/dtmax Pi-Ped, maximal systolic wall stress (sigmamax), integrated systolic wall stress (sigma), outflow resistance (Rsys) and ventricular compliance (a). Nitroglycerin leads to a reduction of both, pre- and afterload, with a significant fall of PLV (10.8%), PLVED (21.8%), EDV (9.1%) ESV (13.3%) AND Rsys (14.4%). The changes of ventricular pressure and geometry result in a diminution of left ventricular wall stress (sigmamax- 11.8%, sigma - 26.4%), representing a decrease of calculated total oxygen-consumption of 11.5%. In six patients with reduced left ventricular diastolic compliance nitroglycerin leads to a remarkable increase of the compliance. This is probably due to the reversal of a latent ischemia by the decreased myocardial oxygen-consumption.

[Intra-aortic balloon pumping in cardiogenic shock after cardiac surgery (author's transl)].

The results of intra-aortic balloon pumping in 7 patients with cardiogenic shock after cardiac surgery are reported. 2 patients were unresponsive to IABP, 4 patients showed initial stabilization of the circulation, but became IABP-dependent and died. One patient could be weaned from IABP, but died of uncontrollable cardiac arrhythmias. These results are discussed.

[Mortality and preoperative left ventricular function in aortocoronary bypass surgery (author's transl)].

Left ventricular function in 8 patients with coronary heart disease, who died during or immediately after surgery, was compared with the corresponding data of 25 survivors. Both groups were similar in age and had about the same number of myocardial infarctions preoperatively. For LVEDP, EDV, EF and VCF critical values could be defined, which were found in none of the survivors. These values were: LVDEP approximately 20 mm Hg, EDV approximately 150 ml/m2, EF less than 30% and VCF less than 0,4 circ/s. The least prognostic significance was found for LVEDP; EDV and VCF correlated best with the surgical risk.

[Estimation of ventricular volume derived from three transverse diameters and the longitudinal axis, compared with an area-longitudinal axis method (author's transl)].

The difficulty of automatic volume estimations of the left ventricle from cine-ventriculography lies in the accurate definition of the ventricular contour during the whole of the cardiac cycle. On the other hand, automatic determination of the long axis of the ventricle, and of three transverse diameters, is feasible. An attempt was therefore made to see whether accurate ventricular volume estimations could be obtained from these measurements. Two different geometrical models were examined: a) A complete rotational ellipsoid b) A half rotational ellipsoid. Examination of ten cine-ventriculograms (totalling 546 frames) has shown that the ventricular volume can be determined with an adequate accuracy by this method. The half ellipsoid model produced better mean results than the full ellipsoid. Its mean deviation from the area-long axis method was only minus 1.2%, that of the complete ellipsoid was minus 3.3%.

[Bromhexine residues in calves, pigs, and in the milk of cows].

After several oral applications of N-cyclohexyl-N-methyl-(2-amino-3,5-dibromobenzyl)-aminehydrochloride (bromhexine, Bisolvon-R) to various calves and pigs the residue behaviour was examined. In animals slaughtered immediately after the final application significant amounts of Bisolvon were found. The substance showed a greater affinity to liver and fat tissue whilst, in general, in the muscle tissue, the lungs and kidneys only very small amounts were traceable. In the liver and lungs 3 different metabolites appeared in small quantities. They were partly in conjugated and partly in free form. Within 2 days the amounts greatly decreased and after 7 days at the latest, no residues were traceable; only in the fat of both animal species and in the kidneys of the calves were small quantities of Bisolvon still to be found. Excretion by the milk following oral and parenteral application was also studied. Only traces of Bisolvon were found, which rapidly decreased. Results of the studies show that, neither at a later date of slaughtering nor immediately after the final application, the residues in the animal tissues and in the milk examined present a danger to the health of the human consumer.

[Geometric changes of the left ventricle during the isovolumic contraction in healthy people and patients with coronary disease].

Biplane cineventriculograms of 5 normal and 12 patients with coronary artery disease were analysed in order to determine the dimensional changes of the left ventricle during the isovolumic phase of contraction. In postero-arterior projection the ventricular diameters in both groups decreased by an average of 3-6%, the wall thickness increased by 5-10%, In lateral projection there were no significant changes of geometry. The changes in regional diameters showed significantly greater maximal differences in diseased than in normal ventricles. Using ventriculographic data of enddiastole instead of aortic valve opening for determination of ejection phase parameters resulted in an overestimation of the mean velocity of circumferential fiber shortening by an average of 10-20%, of the stroke volume of 5-15% and of the ejection fraction of 5%. These results indicate that ejection phase indices can only be determined exactly from cineventriculograms when aortic valve opening and closure is considered.
