Primary aldosteronism treated by trilostane (3 beta-hydroxysteroid dehydrogenase inhibitor).

The practicability and tolerability of trilostane, a competitive inhibitor of 3 beta-hydroxysteroid-delta 5-dehydrogenase, for the therapy of primary aldosteronism was assessed in 1 patient with aldosterone-producing adenoma (APA) and 3 subjects with idiopathic adrenal hyperplasia (IHA). Trilostane afforded reduction of plasma levels of aldosterone, progesterone, deoxycorticosterone, 17-OH progesterone, cortisol, delta 4-androstenedione, and urinary excretion of 17-hydroxycorticosteroid. Conversely, circulating levels of dehydroepiandrosterone, dehydroepiandrosterone sulfate, and urinary excretion of 17-ketosteroids were increased following this drug therapy. Suppression of mineralo- or glucocorticoid biosynthesis was accompanied by an increase in plasma renin activity. One patient with APA or 3 subjects with IHA showed slight or remarkable improvement of hypertension and hypokalemia. Based on these findings, efficacy and tolerability of trilostane appear to aid in the treatment of IHA.

Fast axonal transport of membrane protein and intra-axonal diffusion of free leucine in a neuron of Aplysia.

We injected radioactive leucine into a neuron soma of Aplysia, and found that the leucine and synthesized membrane proteins moved in the axon, but soluble proteins did not. The movement of membrane proteins showed typical characteristics of fast axonal transport, whereas that of leucine could be explained totally due to intra-axonal diffusion by the following observations: the migration profiles of the leucine closely coincided with the theoretical diffusion; the migration continued even when the fast axonal transport of membrane proteins was stopped by local cooling of the axon; and a damming phenomenon, an indication of axonal transport, seen in membrane proteins was not observed when the axon was occluded or when the axonal transport was blocked by the local cooling.

Intravesical instillation of doxorubicin hydrochloride and its incorporation into bladder tumors.

We studied single dose intravesical doxorubicin instillation (50 mg. dissolved in 30 ml. saline) in 29 patients with bladder tumor and 2 with dysplasia of the bladder. The results demonstrated that 1) the levels of doxorubicin hydrochloride in extracts of tumors were significantly higher than those of histologically normal bladder tissues in all regions examined except the dome, 2) the incorporation concentration of the drug into the smallest tumor was 3-fold greater than that of larger tumors, 3) tissue concentration of the drug in histologically normal bladder tissues was 2.4-fold greater in patients with recurrent than with primary bladder tumors, and 4) a high concentration of the agent was noted in unifocal, multifocal and primary tumors classified as papillary noninvasive or invasive transitional cell carcinoma. These findings indicate that intravesical installation of doxorubicin hydrochloride can be incorporated to a high degree by relatively small papillary noninvasive or invasive transitional cell carcinoma located in almost all regions except the bladder dome.

[Immunohistochemical demonstration of carcinoembryonic antigen (CEA) and ABO (H) blood group antigens on tissue sections of urinary bladder tumors].

The carcinoembryonic antigen (CEA) and ABO (H) blood group antigens of 47 bladder tumors were investigated by immunohistochemistry (CEA by peroxidase-antiperoxidase method and blood group antigens by Avidin-biotin-peroxidase complex method) and the results were compared with the histopathological diagnosis. Twenty five percent of the bladder tumors had CEA-positive tumor cells. They were found in 11.1% of pT1 cases, in 12.5% of pT2 cases, 70.0% of pT3 cases and 50.0% of pT4 cases and were also found in 13.3% of grade I cases, in 50.0% of grade II cases and in 40.0% of grade III cases. On the other hand, positive blood group antigen cells were found in 85.2% of pT1 cases, 50.0% of pT2 cases, 20.0% of pT3 cases and 50.0% of the pT4 cases and also in 80.0% of the grade I cases, 41.1% of the grade II cases, and 20.0% of the grade III cases. A statistical difference was seen between the low stage (pT1) and high stage cases (pT2-pT4) and between the low grade (G-I) and high grade cases (G-II, III) (P less than 0.01) for both CEA and the blood group antigens which were statistically related (P less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[Pathophysiology and treatment of idiopathic chronic constipation with megarectum in children].

Anorectal manometry was carried out in 65 children with idiopathic chronic constipation. Forty-five had no megarectum, and 20 had megarectum. Patients without megarectum showed normal anorectal pressures, recto-anal reflex, rectal sensation and rectal compliance, but motility of the colon was disturbed in these patients. Patients with megarectum also had normal anorectal pressures, recto-anal reflex and rectal sensation, but they showed remarkably high rectal compliance, or excessive rectal reservoir. Patients without megarectum were treated with laxatives and all of them were cured within 6 months after institution of therapy. Patients with megarectum were treated with rectal washout and suppository and 14 out of 20 were cured within 10 months. Six out of 20 patients with megarectum, however, were resistant to conservative therapy and necessity of surgical treatment to reduce the excessive rectal reservoir was proposed for these patients.

Nephrogenic adenoma of the ureter.

We report a case of nephrogenic adenoma in the left ureter. The patient presented with lumbago on the left side. A small calculus was incarcerated in the left ureter 4 cm. from the ureteropelvic junction and an adenoma, which resembled histologically the renal tubules, was observed in the ureter. A search of the world medical literature revealed 58 cases of nephrogenic adenoma, although only 1 case in the ureter has been reported previously.

Solitary multilocular cyst of the kidney with embryonic tissue in a child.

A case of multilocular cyst of the kidney with embryonic tissue has been reported. The patient was a five-month-old girl. A large multicystic space-occupying lesion was found in the right kidney. Some embryonic renal tissue was observed in the cystic mass by microscopy. The significance of the presence of the embryonic renal tissue in a multilocular cyst of children was discussed referring to other cases reported in the literature.
