[Autotransfusion in urological surgery--experience with transurethral resection].

With the purpose to avoid the risk of homologous transfusion, autologous blood transfusion was performed in 26 patients who underwent transurethral resection of the prostate (TUR-P). Autologous blood (200-400 ml/time, 1-3 times, total 200-800 ml, mean 381 ml) was predeposited 4 days to 3 weeks prior to elected TUR-P and was transfused during or just after the operation. Since the start of this program in February 1988, homologous transfusion rate was decreased to 13.1%, whereas it has been 22.3% in 203 cases between January 1984 and January 1988. The mean hemoglobin level fell to 81.6% of the predeposition level. However, the hemoglobin level recovered to 90.1% one month after operation. The circulatory condition of the patients with autologous transfusions was stabler than that of the patients with homologous transfusion or no transfusion. No clinical hemostatic problems occurred. In cases with mild to moderate volume resection of the prostate, this autologous blood transfusion is recommended.

[Spontaneous resolution of reflux in children with primary VUR].

Although the adverse effects of vesicoureteral reflux on the renal function and renal growth have been well documented, there is still controversy regarding the factors for spontaneous cure of reflux and the time required for the resolution. We performed a retrospective analysis of 829 ureters in 541 children who have been followed for more than 3 years since 1973. Reflux was judged as spontaneously cured only after 2 micturition cystourethrograms (MCU) had shown no reflux. In total reflux resolved in 220 ureters (26.5%) of 182 children (33.6%). Resolution rate was 39.5% in boys and 28.0% in girls, and there was statistically significant difference between both sexes. In children who had underwent urethrotomy on congenital urethral stenosis, e.g. bulbous urethral stenosis in the boy and distal urethral stenosis in the girl, resolution rate was 43% in boys and 23% in girls. Resolution rate according to VUR-grade was 85.5% in grade I and II reflux, 45.8% in grade III, and 6.7% in grade IV and V. Of 92 children who underwent urodynamic evaluations, 37% showed findings of unstable bladder. Renal scarring was observed in 7.3% of renal units which was significantly less frequent than the incidence among total renal units with reflux. Reflux was resolved in 30% of renal units without scarring and 10% of renal units with scarring. The incidence of small kidney in the spontaneously resolved group was 3.6%, which was significantly lower compared to the total renal units with reflux. Influence of surrounding media on preservation of cell wall ultrastructure of Candida albicans revealed by low temperature scanning electron microscopy.

Influence of surrounding media on the surface structures of the cell wall of Candida albicans was discussed with respect to the preservation of ultrastructure during the specimen preparation for scanning electron microscopy. The fibrillar structure of the cell surface was distinctly identified by the rapid-freezing technique. It was difficult, however, to observe this structure by the conventional specimen preparation technique. The reason for the difference between these two preparation techniques was studied using a low temperature SEM. Through investigating the influence of each step of the conventional technique on the fibrillar structure, it was found that the fibrils were drastically deformed and disappeared during the dehydration step in ethanol above 80% in concentration. In order to study which physicochemical properties participated in this disappearance phenomenon, yeast cells were treated with various media: solutions in different pH ranges and at different salt concentrations, ionic solutions, surfactants, formamide, dimethyl sulfoxide, acetone and Fehling's solution. As a result, the fibrillar structure was found well preserved when the medium had an affinity for the constituent molecules of the fibrils. When without affinity, the fibrils suffered a remarkable deformation. The mechanism of this deformation is discussed in terms of molecular interaction of solute and solvent.

[A case of spontaneous disappearance of pulmonary metastasis of renal cell carcinoma following nephrectomy].

A 76-year-old man was admitted to our hospital on October 3, 1988 complaining of general fatigue and left flank pain. A large movable mass was palpable in his left flank. Intravenous pyelography and computerized tomography confirmed left renal tumor. Chest X-ray showed a coin lesion in the left lung. Left nephrectomy was performed on October 14, 1988. Histopathological diagnosis was adenocarcinoma of clear cell type. Chest X-rays, on the postoperative eleventh day, showed disappearance of the pulmonary coin lesion which was seen preoperatively. Recurrence of pulmonary metastasis has not been seen for 14 months after operation.

[Antitumor activity of UFT against murine renal cell carcinoma: a study on the suppression tumor metastases].

Experimental chemotherapy with UFT was performed against murine renal cell carcinoma (Renca) of spontaneous origin in BALB/c mice and the antitumor activity of UFT was compared with that of 5-FU. By oral administration started from the day after inoculation of Renca cells under the capsule of a kidney, the growth of tumor and formation of the spontaneous metastases to the lymph nodes, lung, spleen, and abdominal wall were inhibited significantly. The UFT or 5-FU treatment started on the 8th day after tumor inoculation also extended the survival time of the tumor-bearing mice and the anti-tumor effect of UFT was more marked than 5-FU. However, UFT treatment started on the 15th day did not prolong the survival time of Renca-bearing mice. From these results, UFT therapy seems to be beneficial for prevention of metastases after nephrectomy in the patients with renal cell carcinoma.

[Consideration on transfusion in urologic surgery--a retrospective study on homologous and autologous transfusions].

To obtain data for promoting autologous transfusion in urologic surgery, the cases of perioperative homologous transfusions in the past 5 years are reviewed. Although the mean perioperative homologous blood transfusion rate was not so high (10.2%), the rate in common urological surgery was comparatively high (eg. radical nephrectomy 39.4%, simple or palliative nephrectomy 21.6%, total nephroureterectomy 33.3%, renal allotransplantation 82.8%, total cystectomy 96.2%). Mean homologous transfusion volume in these surveys was 982 ml. Two successful cases of autologous blood transfusion by the predeposit method or intraoperative collecting method are presented. Transfusion-associated adverse effects, especially viral infection and immunosuppression, were discussed and the value of autotransfusion was stressed.

[A histological evaluation of the therapeutic effects and the clinical findings in prostatic carcinomas].

A histological evaluation of the effects of therapy has been conducted on 22 prostatic carcinoma cases, according to the criterion proposed by the committee to establish the general rule for clinical and pathological studies judging the effects of therapy in cases of prostatic cancer. Studied in particular was the relation between the histological effects of therapy and the clinical course. Marked therapeutic effects were observed in cases of low grade tumors. Further, the patients' prognosis tended to be affected by the histological grade rather than by the histological effect of the treatment. The course of the serum total acid phosphatase levels tended to correlate with the histological effect of the treatment.

[Polyesthesia--report of two cases].

Polyesthesia is an abnormal sensation in which a single sensory stimulation seems to be felt as several ones. We report two cases showing this rare phenomenon. The first case was a 34-year-old man having a history of SLE for 20 years. When he was 22 years old, he suffered from transverse myelopathy at the tenth thoracic level. Two years after this episode, he noticed a curious sensory experience. Namely, when he rubbed his hand or lateral trunk, similar sensation occurred on the foot of the same side simultaneously. This phenomenon has continued for ten years. The characteristics of this double sensation were as follows; effective modes of stimulation were rubbing, tapping and vibration. Pinprick or cold/warm stimuli could not elicit any sensation on the foot. The sensation observed on the foot was always ipsilateral and simultaneous in almost symmetrical distribution. Rubbing the foot could not elicit any sensation on the hand or trunk. Somatosensory evoked potentials (SEPs) following tibial and peroneal nerve stimulation could not be recorded on the scalp. Dermatomal SEPs below the tenth thoracic level were also silent over the scalp. SEPs obtained by median nerve stimulation were normal bilaterally. Various neuroradiological examinations including CT and MRI were noncontributory. This phenomenon, diagnosed as synesthesia, may be related to denervation supersensitivity after spinal cord lesion and sprouting from the fibers conveying hand and thoracic information to those denervated neurons at the posterior column nuclei. The second case was a 70-year-old woman having a history of syphilis. She noticed a characteristic sensation for ten years. Namely, when she rubbed her face, especially her forehead and cheeks, another sensation occurred 1 second after primary stimulation at the same place.(ABSTRACT TRUNCATED AT 250 WORDS)

Protective effect of the plasticizer di(2-ethylhexyl) phthalate against damage of the mitochondrial membrane induced by calcium: possible participation of the adenine nucleotide translocator.

The effect of di(2-ethylhexyl) phthalate (DEHP) on the response of isolated rat liver mitochondria to Ca2+ was investigated. DEHP was found to inhibit more than 60% of the auto-accelerating release of respiration induced by 100 microM Ca2+, being maximally inhibitory at 40 microM. Prior addition of DEHP also partially inhibited Ca2+-induced swelling of the mitochondrial matrix. However, DEHP did not change the net rate of Ca2+ uptake measured by the steady-state infusion method. DEHP also reduced the rate of adenine nucleotide exchange across the mitochondrial membrane. Another alkyl phthalate and alkyl citrates had similar effects on Ca2+-induced membrane damage, but their potencies depended on the lengths of their alkyl chains. These results suggest that the effects of DEHP and other alkyl esters on mitochondrial functions are mainly based on their actions on membrane lipids surrounding adenine nucleotide translocator (AdNT), resulting in alteration of the interaction between these phospholipids and AdNT, and consequent modification of the state of the protein.

Purification of a 51 kDa endo-beta-N-acetylglucosaminidase from Staphylococcus aureus.

A bacteriolytic enzyme obtained from the culture fluid of Staphylococcus aureus FDA 209P was purified to homogeneity utilizing dye-ligand affinity column chromatography, hydrophobic interaction high pressure liquid chromatography (HPLC) and hydroxyapatite HPLC. Subsequent characterizations indicated that the purified enzyme acted as endo-beta-N-acetylglucosaminidase. The molecular weight determined by sodium dodecyl sulphate polyacrylamide gel electrophoresis (SDS-PAGE) was 51,000 and the isoelectric point was higher than 10. The optimum pH for the enzyme activity on whole cells of Micrococcus luteus as a substrate was 8.0. Some heavy metal cations (Cu2+ and Zn2+) inhibited the enzyme activity at a concentration of 0.1 mM and others (Ba2+, Mg2+ and Co2+) showed a stimulating effect at a concentration of 1 mM.

N-terminal sequencing of low-molecular-mass components in cyanobacterial photosystem II core complex. Two components correspond to unidentified open reading frames of plant chloroplast DNA.

We recently reported the presence of several low-molecular-mass protein components in the PS II O2-evolving core complex from the thermophilic cyanobacterium, Synechococcus vulcanus [(1989) FEBS Lett. 244, 391-396]. Here we have characterized the three components (4.1, 4.7, 5 kDa) of the same cyanobacterial core complex by N-terminal sequencing. There were two components in the 4.7 kDa region, both having a blocked N-terminus. One has a sequence highly homologous to open reading frame 34 of plant chloroplast DNA (tentatively designated psbM), while the other has a sequence partially homologous to open reading frame 43 of chloroplast DNA (designated psbN), although neither of the two gene products has yet been confirmed in chloroplasts. The cyanobacterial 4.1 kDa protein partially corresponds to the 4.1 kDa nuclear-encoded core component of higher plant PS II. The cyanobacterial 5 kDa component, however, shows a sequence that is unrelated to any other known proteins.

Identification of photosystem I components from the cyanobacterium, Synechococcus vulcanus by N-terminal sequencing.

The photosystem I core complex isolated from a thermophilic cyanobacterium, Synechococcus vulcanus, is composed of eight low-molecular-mass proteins of 18, 14, 12, 9.5, 9, 6.5, 5 and 4.1 kDa in addition to the PS I chlorophyll protein. N-terminal amino acid sequences of all these components were determined and compared with those of higher plants. Clearly, the 9.5 kDa component corresponds to the protein which carries the non-heme iron-sulfur centers A and B. This protein is so poorly visualized by staining that it has probably been overlooked in gel electrophoresis analyses. The 18, 14, 12 and 9 kDa components show appreciable homology with respective subunits of higher plant PS I. In contrast, the 6.5, 5 and 4.1 kDa components do not correspond to any known proteins except that the sequence of the 4.1 kDa component matches an unidentified open reading frame (ORF) 42 (liverwort) or ORF44 (tobacco) of chloroplast DNA.

[Combined effect of intraperitoneally administered OK-432 and recombinant interleukin (rIL-2) against mouse tumors].

Combined effect of the streptococcal preparation, OK-432 and rIL-2, both administered intraperitoneally, was examined against Meth-A tumor, a syngeneic tumor of inbred BALB/c mice and an analysis of the effector cells also was performed. The combined treatment resulted in significant in vitro inhibition of tumor growth and increase in the length of survival of the Meth-A bearing mice. The spleen cells obtained from Meth-A bearing mice treated with both OK-432 and rIL-2 on the 12th day after the tumor transplantation were capable of lysing syngeneic Meth-A tumor cells in addition to NK-sensitive allogenic YAC-1 cells and LAK-sensitive EL-4 cells in vitro in a 4 hour 51Cr-release assay. Neither spleen cells obtained from the tumor bearing mice treated with OK-432 or rIL-2 alone, nor those obtained from normal mice treated with both agents, showed cytotoxic activity against Meth-A cells. The in vitro growth of NK-resistant allogenic P-388 was not inhibited by the spleen cells obtained from any treated mouse, with or without Meth-A tumor. In order to determine the effector subpopulation against Meth-A cells, the spleen cells obtained from the tumor-bearing mice treated with OK-432 and rIL-2 on the 12th day after tumor transplantation, were treated with either anti-Thy-1, 2 antibody plus complement, anti-asialo GM1 serum plus complement, or by adherence to a plastic plate or nylon wool column.(ABSTRACT TRUNCATED AT 250 WORDS)

Low-molecular-mass proteins in cyanobacterial photosystem II: identification of psbH and psbK gene products by N-terminal sequencing.

The O2-evolving photosystem II core complex was isolated from a thermophilic cyanobacterium, Synechococcus vulcanus Copeland. Analysis by SDS-polyacrylamide gel electrophoresis revealed that the complex contained at least seven low-molecular-mass proteins in addition to the well characterized CP47 apoprotein, CP43 apoprotein, 33 kDa extrinsic protein, D1 protein, D2 protein and large subunit of cytochrome b-559. The separation of these low-molecular-mass proteins were very similar between cyanobacterial and higher plant PS II. N-terminal sequences of the 6.5 kDa and 3.9 kDa proteins of cyanobacterial core complex were determined after blotting to a polyvinylidene difluoride membrane. The sequence of the 6.5 kDa protein showed high homology with an internal sequence of plant psbH gene product, so-called 10 kDa phosphoprotein, but did not conserve the Thr residue which is specifically phosphorylated in plants. The sequence of the 3.9 kDa protein corresponded to the K protein of higher plants (mature form of psbK gene product). These results indicate that the products of both psbH and psbK genes are present in cyanobacterial PS II as well as being associated with the O2-evolving core complex.

Is extensive lymphadenectomy necessary for surgical treatment of intramucosal carcinoma of the stomach?

This study was undertaken to elucidate those histological and gross features associated with gastric carcinoma that can be adequately treated by gastrectomy with less aggressive lymphadenectomy. The frequency of metastasis to the lymph nodes was analyzed in 514 cases of resected, solitary, gastric carcinomas. The frequency of metastasis to the lymph nodes increased in proportion to the increase in the extent of penetration by the cancer into the gastric wall. Lymph nodes were not involved in cases of intramucosal carcinoma of the intestinal type, by Laurén's histological classification. By contrast, metastasis to the lymph nodes was observed in cases of intramucosal carcinoma of the diffuse type, by Laurén's classification. We conclude that extensive lymphadenectomy is not mandatory for patients with intramucosal carcinoma of the stomach of the protruded type, since the lymph nodes do not become involved in this type of gastric carcinoma.

Effect of hyperbaric oxygen therapy on essential haematuria.

Forty-three patients with essential haematuria were studied for 35 days to determine the efficacy of hyperbaric oxygen (OHP) exposure on haemostatic ability. Out of 15 untreated patients, haematuria persisted for the study period in all but one patient (6.7%). Out of 11 patients who received 90 mg/day of carbazochrome sodium sulfate and 750 mg/day of tranexanic acid excellent haemostatic results had been obtained in 2 (18.2%). Results of exposure of 17 patients to 2 atm. pressure of OHP for 90 min daily were excellent in 10 (58.8%) and good in one (5.9%). No serious adverse effects had been noted following OHP treatment. These results indicate that OHP can be applied for the treatment of essential haematuria if routine haemostatic drugs fail to improve the condition.

Impaired urine concentrating ability in Itai-itai (ouch-ouch) disease.

A case of Itai-itai (ouch-ouch) disease with reduced urinary kallikrein excretion and slightly enhanced renin-angiotensin-aldosterone system is described. Although it is well known that cadmium toxicity frequently affects the renal tubular lesions, this report is the first to demonstrate the impaired urine concentrating ability in this disease.
