[Randomized study of lentinan on patients with advanced gastric and colorectal cancer. Tohoku Lentinan Study Group].

A randomized study of mitomycin C (MMC)+5-FU [control group] vs MMC+5-FU+lentinan (LNT) [LNT group] was conducted in order to evaluate the effect of LNT against advanced gastric and colorectal cancers by the envelope method in 166 patients, comprising of 115 cases of gastric cancer and 51 cases of colorectal cancer. Significant increases were observed in the survival rates for the LNT group (p less than 0.05) for both patients with gastric and colorectal cancer. No significant difference was observed in response rates between the aforesaid two groups, though the response rate in the LNT group was slightly higher than the control group. These results suggest that LNT in combination with anticancer drugs prolong the survival time of patients with advanced gastric and colorectal cancer.

[Nutritional management of anastomotic leakage after gastrointestinal surgery--with special reference to the serious cases].

Effects of intravenous hyperalimentation (IVH) were studied in 38 patients with anastomotic leakage following gastrointestinal surgery. Spontaneous closure of the leakage were found in 23 patients (60%), and surgical closure were needed in 9 patients (24%). The duration required for spontaneous closure was 5 weeks on the average; and closure occurred readily in cases of tubular fistula. Death occurred in 6 patients (16%); among these, 5 were MOF cases. The following three points are emphasized concerning measures to increase IVH efficacy in serious cases such as MOF. GFX-solution is efficacious for reduced glucose tolerance. Administration of canrenoate potassium(Soldactone), linoleic acid and BCAA enriched amino-acid mixture is effective for accelerated albumin synthesis. Concomitant treatment by IVH and ED(elemental diet) is efficacious in cases of high caloric administration.

Seven-year follow-up study after the replacement of the esophagus with an artificial esophagus in the dog.

An artificial esophagus was made of silicone rubber (Phycon) tube covered with a Dacron mesh. A segment of thoracic esophagus of 16 dogs was replaced with this graft using three different types of anastomosis--overlayer end-to-end anastomosis with flanged tube, two-layer end-to-end anastomosis with flanged tube, and monolayer end-to-end anastomosis with no flange tube. Seven of 16 dogs (44%) survived more than 12 months without complications, four of them surviving more than 6 years. In six of seven of the prolonged survivors, extrusion of the graft was recognized in the 3 to 6 months after operation. Esophageal stricture increased slightly up to 6 months after extrusion of the graft, but it did not further advance until sacrifice. In these dogs, mucosal regeneration of the neoesophagus was complete with muscle layers and mucous glands in the submucosa recognized microscopically. Proximal esophagus from the replaced portion was apparently dilatated more than that of the distal portion. There was no definite difference between the anastomotic techniques with regard to complication or prognosis. These results suggest that this artificial esophagus should be considered as a possibility for clinical trials in the future.

[Immunotherapy of postoperative cancer patients with immune RNA-sensitized lymphocytes and tumor antigen].

Tumor cell suspension was prepared from resected tumor tissue of various cancer patients. 1-RNA was extracted from lymphoid tissues of rabbits immunized with each tumor cells and CFA. When the postoperative condition of patient recovered stable, autologous lymphocytes were prepared with blood cell separator (IBM 2997 type) and incubated with I-RNA, then, return to himself. Total number of sensitized lymphocytes was 4 X 10(9) - 1.2 X 10(10). After this passive immunization was finished, active immunization with autologous 3M KCl tumor extract was added. Three of gastrointestinal cancer, 3 of lung cancer, 2 of miscellaneous malignant tumor were treated with this schedule. T-cell ratio, blastogenesis of T-cell with PHA, skin reaction with PPD and PHA, CEA, IAP and Ig in serum was not definitely changed before and after the immunotherapy. But 3 of them, LAI AND LMI became to positive after the treatment and NK cell activity also increased in 3 cases. In these 3 cases, tumor regression was recognized after the treatment. In a case of synovial sarcoma, skin reaction for autologous tumor antigen became to positive and the metastatic lesion in the lung was gradually regressed and regression is still continuing 6 months after the treatment (reduction ratio 90%).
