Laparoscopic repair of an abdominal hernia using an expanded polytetrafluoroethylene patch secured by a four-corner tacking technique.

An improved technique for performing laparoscopic repair of an abdominal hernia is described herein. To ensure a successful repair, it is most important that adequate tension of the expanded-polytetrafluoroethylene (e-PTFE) patch be achieved, and that the defect be completely covered and securely stapled. Our technique involves tacking the four corners of the patch to the abdominal wall with a 2-0 nylon suture using a straight needle, then stapling it to the anterior abdominal wall over the defect with a laparoscopic stapler. We believe that this technique is a safe and reliable method which will prove useful for laparoscopic surgery.

Laparoscopic transabdominal preperitoneal herniorrhaphy using abdominal wall-lifting method under regional anesthesia: a preliminary report.

This report describes a laparoscopic transabdominal preperitoneal herniorrhaphy of groin hernias using an abdominal wall-lifting method under regional anesthesia as compared with pneumoperitoneum under general anesthesia. The series of studies involved the repair of 20 groin hernias; 7 hernias were direct, 11 were indirect, and 2 were femoral. These included 7 recurrent, 1 incarcerated, and 4 bilateral hernias. There were no intraoperative complications, and both procedures required no conversion to open surgery. There are no significant differences between the two groups in operative time and postoperative hospital stay. The only postoperative complication of our procedure was temporary inguinal pain in 2 cases. The follow-up period ranged from 8 to 17 months. To date, no recurrence has developed. We conclude that our procedure is a safe, technically feasible, and useful method to perform laparoscopic herniorrhaphy for groin hernias.

[Biliary tract infection].

Biliary tract infection is a frequently encountered clinical problem and requires prompt diagnosis, proper understanding of the pathologic manifestations, and adequate treatment. If not managed properly, it can be the cause of disseminated intravascular coagulation and multiple organ failure and its prognosis can be fatal. This paper focuses on two of its representative diseases: acute cholecystitis and acute cholangitis.

[A multicenter comparative study in patients with liver metastasis to compare tegafur in new forms with existing drugs. SF-RC.SF-SP Comparative Study Group].

A multicenter comparative study was carried out in 101 patients with liver metastasis to compare tegafur in a new oral form (Sunfural S) and tegafur in a new soft-capsule suppository form (Sunfural Rectal Capsules) with existing drugs. Sunfural S showed a tendency toward high plasma and tissue levels as compared to existing drugs, and clinically, Sunfural S produced survival times that surpassed those of existing drugs. Liver metastasis at the present time has few effective treatments, but we expect that anticancer effects will be obtained through the administration of new drugs with improved bioavailability.

[Study on the concentrations of 5-fluorouracil (5-FU), tegafur (ET) and uracil in bile: comparison of UFT or FT].

The serum and bile tegafur (FT), 5-fluorouracil (5-FU) and uracil levels after administration of UFT were assessed in 13 cases of malignant biliary tumor accompanied by biliary obstruction in comparison with FT alone. The serum and bile FT and 5-FU levels showed almost the same transition pattern in both groups, reaching to the plateau in 1-2 weeks and revealing cumulative effect by continual administration. Correlation was obtained between serum and bile levels except for 5-FU level in UFT group (p less than 0.05). Correlation between 5-FU and uracil was obtained in the serum in both groups (p less than 0.05), but no effect of uracil was observed. In bile, correlation was seen only in UFT group (p less than 0.05), and the effect of uracil was observed in bile 5-FU level.

Effects of Dl- and l-propranolol, labetalol, and phentolamine on rabbit iris blood flow determined by hydrogen clearance method.

In rabbits anesthetized with urethane, blood flow in the iris and the eye lid was 104.0 to 120.0 and 96.6 to 112.4 ml/min/100 g tissue (determined by the hydrogen clearance method), respectively. Dl-propranolol (1.0 mg/kg, i. v.) or phentolamine (0. 5 mg/kg, i. v.) decreased blood flow in both iris and eye lid to a similar degree, and also decreased blood pressure. Neither saline solution (0.5 ml/kg, i. v.) nor d-propranolol (1.0 mg/kg, i. v.) changed blood flow in both iris and eye lid and blood pressure. Labetalol (1.0 mg/kg, i. v.) decreased blood flow in the iris markedly and that in the eye lid moderately with a moderate decrease in blood pressure ; blood pressure/iris blood flow ratio increased significantly without changes in blood pressure/eye lid blood flow ratio. None of the drugs tested changed pupil size of the anesthetized rabbits. It is suggested that labetalol decreases iris blood flow because of blood pressure decrease plus other mechanisms that might be different from alpha- and beta-adrenergic blockade.

Hypotensive effect of labetalol on intraocular pressure in rabbits: in relation to its alpha- and beta-adrenergic blocking action on the cardiovascular system.

Rabbits anesthetized with urethane were used. Labetalol was found to be about 3.7 times less potent than phentolamine in blocking phenylephrine-induced vasopressor response, and about 7.6 times less potent than propranolol in blocking isoproterenol-induced vasodepressor response. Labetalol (0.3, 1.0 and 2.5 mg/kg, i.v.) decreased both intraocular pressure (IOP) and mean arterial blood pressure (BP) significantly, and also decreased IOP/BP ratio. Propranolol (1.0 and 2.5 mg/kg, i.v.) reduced both IOP and BP slightly without affecting the IOP/BP ratio. Phentolamine (1.0 mg/kg, i.v.) reduced both IOP and BP significantly, but it did not alter the IOP/BP ratio. When both propranolol (1.0 mg/kg, i.v.) and phentolamine (1.0 mg/kg, i.v.) were injected at the same time, IOP and BP were reduced with a decrease in IOP/BP ratio. These findings suggest that labetalol is effective in decreasing the IOP, although the mechanism of the drug action cannot be explained simply by alpha- and beta-adrenergic blockade.

Development of 132 Ir sources for brachytherapy.

Techniques for the production of three kinds of 192Ir sources, i.e. hairpin, single-pin and seed, for brachytherapy were developed in Japan Atomic Energy Research Institute. Platinumiridium, alloy wire sheathed with platinum tube in advance were irradiated by the JRR-2 or JRR-3 reactor. The platinum tube cuts off beta-rays so as to minimize unnecessary radiation exposure to patients. Irradiation conditions were determined to obtain the radioactivity of 0.74 GBq (20 mCi) for a hairpin, 0.37 GBq (10 mCi) for a single-pin and 0.37 X 10(-1) GBq (1 mCi) for a seed, respectively, at the end of irradiation. Fluctuation in the linear distribution of the radioactivity along the hairpin source or the single-pin source was small enough on the viewpoint of its application. No significant impurity or surface contamination was observed in the source produced with pre-encapsulation. Seed targets were loaded in a polyethylene tube so as to be suitable for the new treatment technique of tumor. Irradiation of seed targets, packed alternately with nylon spacers in a polyethylene tube, was found to be satisfactory for preparing ribbon sources.

A case report of leiomyosarcoma originating in the ligamentum teres of the liver.

A case of leiomyosarcoma that appeared to have originated in the ligamentum teres hepatis of a 66-year-old female has been presented. The case was suspected of leiomyosarcoma originating in the ligamentum teres of the liver from the findings by preoperative examinations, and definitely diagnosed as such in a laparotomy and by histological examination of the resected tissue specimen.

[Test production of 60Co sealed sources of high specific activity (author's transl)].

To establish the production technique of 60Co sources with high specific activity, test production was started in 1967 by using JRR-3 and JMTR reactors. Two kinds of cobalt targets, a wafer type (diameter 8.0 mmxthickness 2.3 mm, 1.1g) and a pellet type (diameter 1.0 mmxlength 1.0mm, 6.9 mg) were used. The former was irradiated at a maximum nvt of 4.4x10(20) neutrons per cm2, yielding a maximum specific activity of 8.5 Ci per gram, and the latter at 2.2x10(21) neutrons per cm2, yielding a maximum specific activity of 68.1 Ci per gram, and the total activity of 60Co sources amounted to 4.1 kCi. After the 60Co sources were enclosed in stainless steel capsules, these sealed sources were submitted to wipe and immersion tests to examine surface contamination and leakage. Through the test production, could be obtained much important knowledge such as irradiation method, handling of highly active materials, sources assemblage and remote welding.
