[A case of interstitial nephritis induced by a super antigen produced by methicillin-resistant Staphylococcus aureus (MRSA) presenting as acute renal failure].

We report the case of a 21-year-old man who had been developing acute renal failure with Methicillin-resistant Staphylococcus aureus (MRSA) colitis and sepsis. He was admitted for consciousness disturbance, nausea, vomiting, and diarrhea. Oliguria was also observed and his serum creatinine level was elevated to 10 mg/dl. Urinary protein was positive and an abundance of hyaline cast were seen in urinary sedimentation. Diarrhea and pyrexia were prolonged and serum C-reactive proteins were elevated, but lymphocyte and leukocyte counts temporarily decreased from the 3rd to the 6th hospital day and remained low until normalizing after the 14th day. His clinical symptoms improved with hemodialysis (HD) and effective antibiotic therapies. An MRSA strain producing toxic shock syndrome toxin-1 (TSST-1), a super antigen which specifically stimulates human V beta 2-positive T cells, was separated from his feces and blood. To ascertain the cause of his renal dysfunction, a renal biopsy was performed on the 8th day. His renal histology revealed acute interstitial nephritis with severe inflammatory cell infiltration around the medullary areas without glomerular changes. Most of the infiltrated cells were small monocytes, and lymphoid cells were rich in the interstitium. With immunohistochemical staining, over 70% of T-cells were V beta 2-positive. TSST-1-producing MRSA was detected in his blood specimen. Furthermore, V beta 2-positive T cells were accumulated in the renal intersititium, and transient lymphocytopenia was observed. These data suggested the following possible pathogenesis for interstitial nephritis: TSST-1 acts as a super antigen in the renal interstitium where major histocompatibility complex (MHC) is class-2-positive, thereby resulting in interstitial nephritis with T cell migration.

[Anesthetic management of a patient with tracheal bronchus with one lung ventilation].

A 56-year-old-male with malignant pleural mesothelioma of the left lung underwent pneumonectomy and pleurectomy. Fiberoptic bronchoscopy was not done preoperatively. Anesthesia was induced rapidly and a double-lumen endobronchial tube was inserted. When we checked the position of the tube with a fiberoptic bronchoscope, we found that the normal right upper lobe bronchus was absent and that the inflated tracheal cuff had obstructed the right upper lobe bronchus originating above the carina. Then we changed the double-lumen endobronchial tube to a endotracheal tube with the blocker. Thereafter, the surgery was completed safely and his postoperative course was uneventful. Routine bronchoscopy is essential just after intubation and before extubation of the endobronchial tube in safe airway management. How to use a fiberoptic bronchoscope to check the position of a double-lumen endobronchial tube is also discussed.

A pharmacotherapy algorithm for the treatment of dysthymia in Japan.

Based on randomized controlled trials conducted in western countries, we proposed a pharmacotherapy algorithm for DSM-IV dysthymia in Japan. The main strategy of the algorithm are: firstly, one of the antidepressants is selected for an initial agent, and secondly, efficacy of the selected antidepressant is evaluated at 8 to 12 weeks. The agent with sufficient efficacy is continued for at least 6 months, while ineffective agent is replaced to another antidepressant. The algorithm is preliminary mainly because of the lack of data from Japanese samples and the unavailability of some useful agents in Japan, for instance, selective serotonin reuptake inhibitors and monoamine oxidase inhibitors.

Natriuretic peptides regulate the expression of tissue factor and PAI-1 in endothelial cells.

In the present study, we demonstrate that brain natriuretic peptide (BNP) and C-type natriuretic peptide (CNP) interact with angiotensin II (Ang II) in regulative blood coagulation and fibrinolysis by suppressing the expressions of both tissue factor (TF) and plasminogen activator inhibitor-1 (PAI-1) induced by Ang II. The expressions of TF and PAI-1 mRNA were analyzed by northern blotting methods, and the activities of TF on the surface of rat aortic endothelial cells (RAECs) and PAI-1 in the culture media were respectively measured by chromogenic assay. Both BNP and CNP suppressed the expressions of TF and PAI-1 mRNA induced by Ang II in a time- and concentration-dependent manner via cGMP cascade, which suppressions were accompanied by respective decrease in activities of TF and PAI-1. However, neither the expression of tissue factor pathway inhibitor (TFPI) nor tissue-type plasminogen activator (TPA) mRNA was affected by the treatment of BNP and CNP.

The effects of angiotensin metabolites on the regulation of coagulation and fibrinolysis in cultured rat aortic endothelial cells.

Not only angiotensin II (Ang II) but also other angiotensin metabolites such as angiotensin I (Ang I), angiotensin III (Ang III), angiotensin IV, or angiotensin 1-7 have recently been reported to have various activities. Few data, however, are available on the regulation of thrombus formation. In this study, we investigated the effects of angiotensin metabolites on the mRNA expression of tissue factor (TF), tissue factor pathway inhibitor (TFPI), plasminogen activator inhibitor-1 (PAI-1), and tissue type plasminogen activator (TPA) in cultured rat aortic endothelial cells. None of the used angiotensin metabolites altered TFPI or TPA mRNA expression levels. Ang I, Ang II, and Ang III made TF and PAI-1 mRNA inductions which were inhibited by an selective antagonist of angiotensin II type 1 receptors. These metabolites made TF predominant to TFPI or PAI-1 to TPA, and could render endothelial cells thrombogenic.

MICA gene polymorphism in Takayasu's arteritis and Buerger's disease.

To further clarify the HLA-linked genes susceptible to arterio-vasculitis of unknown etiology, Takayasu's arteritis and Buerger's disease, polymorphism in the MICA gene, a newly identified gene near the HLA-B gene and expressed in epithelial cell lineage, was investigated. Polymerase chain reaction (PCR)-DNA conformation polymorphism (DCP) analysis and subsequent sequencing of the MICA gene have revealed that there are 5 MICA alleles which are different in the number of a GCT repeat in exon 5: MICA alleles MICA-1.1, -1.2, -1.3 and -1.4 have 9, 6, 5 and 4 GCT repeats, respectively, and MICA-1.5 has 5 GCT repeats with a 1 bp frameshift insertion in the repeat. MICA genotyping data in 81 Japanese patients with Takayasu's arteritis, 38 Japanese patients with Buerger's disease, and 160 healthy Japanese controls showed that MICA-1.2 and -1.4 were significantly associated with Takayasu's arteritis and Buerger's disease, respectively. Because MICA-1.2 and -1.4 were in strong linkage disequilibria with HLA-B52 and -B54 in the Japanese populations, respectively, we have compared the odds ratio (OR) of the risk to the diseases for individuals having both or each of the disease-associated MICA and HLA-B alleles. It was found that MICA-1.2 gave a significantly high OR of risk to Takayasu's arteritis in the absence of HLA-B52, suggesting that the HLA-linked gene susceptible to Takayasu's arteritis is mapped near the MICA gene. In contrast, MICA-1.4 gave a significantly high OR of risk to Buerger's disease only in the presence of HLA-B54, suggesting that the HLA-linked gene susceptible to Buerger's disease is linked to the HLA-B54-MICA-1.4 haplotype, and may be differently mapped from that to Takayasu's arteritis.

Association of clinical manifestations with HLA-B alleles in Takayasu arteritis.

HLA-DNA typing using PCR-SSOP and PCR-DCP methods was performed in 85 patients with Takayasu arteritis and 492 healthy controls who had been typed for HLA by serological method. Frequencies of HLA-B52 (B*5201) and B39 (B*3901 and B*3902) were significantly increased in the patients. Frequency of HLA-DRB1*1502 was also increased but it was suggested to be a reflection of its linkage disequilibrium with B52. Association of HLA-B52 and B39 with seven clinical manifestations--pulmonary infarction, ischemic heart disease, aortic regurgitation, systemic hypertension, renal artery stenosis, cerebrovascular disease, and visual disturbance--in 132 HLA-typed patients with Takayasu arteritis was studied. In HLA-B52 positive TA patients, aortic regurgitation (vs B52(-)-B39(+), OR=3.8, P<0.05, vs B52(-)-B39(-), OR=5.49, P<0.001), ischemic heart disease (vs B52(-)-B39(+), OR=12.05, P<0.05, vs B52(-)-B39(-), OR=2.85, P<0.05), and pulmonary infarction (vs B52(-)-B39(+), OR=5.74, P<0.03) were found to be significantly prevalent. On the other hand, in HLA-B39 positive TA patients, frequency of renal artery stenosis was significantly increased (vs B52(+)-B39(-), OR=12.14, P<0.001, vs B52(-)-B39(-), OR=5.21, P<0.03). These observations have suggested that HLA-B52 molecule and B39 molecule would contribute to different clinical manifestations by binding different antigenic peptides to cause inflammations. Thus HLA-B molecule may play an important role in pathogenesis or determining clinical manifestations of Takayasu arteritis.

Detection of telomerase activity in prostate needle-biopsy samples.

BACKGROUND: Telomerase in an enzyme ribonucleoprotein responsible for cell immortality and oncogenesis. Telomerase activity has been detected in most cancers, including prostate cancer. To verify whether the detection of telomerase in prostate needle biopsy samples could have diagnostic value, we blindly assayed telomerase activity in samples from patients who were clinically suspected of having prostate cancer. METHODS: A total of 183 prostate biopsy samples was obtained from 63 patients who were suspected of having prostate cancer. Telomerase activity was determined by polymerase chain reaction (PCR)-based telomeric repeat amplification protocol (TRAP) assay. An internal telomerase assay standard (ITAS) was used in this study to distinguish false negatives. RESULTS: Telomerase activity was detected in 17 of 19 (89.5%) patients with prostate cancer (chi(2) test, P < 0.005). In 39 of 42 (92.9%) biopsy samples from these 19 prostate cancer patients, confirmed histologically as prostate cancer, telomerase activity (chi(2) test, P < 0.005) was detected. Meanwhile, we also found one false-positive sample. In contrast, all of 44 normal or benign prostate disease patients (124 biopsy specimens) were telomerase-negative. We also revealed that poorly differentiated prostate cancer often expresses a high level of telomerase activity. The area of cancer tissue in biopsy specimens was not associated with relative telomerase activity. CONCLUSIONS: This study suggests that determination of telomerase activity in prostate needle-biopsy samples might be a useful tool for detecting prostate cancer.

Design of compact absorbers for high-heat-load X-ray undulator beamlines at SPring-8.

A compact and high-heat-load absorber for the SPring-8 X-ray undulator beamline has been developed and installed. It consists of an upper heat-absorber part and a lower photon duct part, which are configured together in a water-cooled GlidCop body. The absorber part has a horizontal notch shape and the photon duct part forms a rectangular open channel under the absorber part. Two types of absorber are designed: one, with wire mesh channels, is 486 mm long, 70 mm high and 64 mm wide; the other, with smooth-bore channels, is 610 mm long, 75 mm high and 70 mm wide. Thermal and stress analyses show that they withstand the 12.3 kW heat load and the maximum heat flux of 940 W mm(-2) at normal incidence.

Present status and performance of SPring-8 front ends.

SPring-8 front ends have a novel structure which makes it easy to rearrange them and exchange the components. The structure has a common support for all the components except X-ray beam-position monitors and lead collimators. The alignment of the common support as well as the components was carried out with an accuracy of 0.25 mm in the vertical and horizontal directions. Replaceable pumping systems have also been placed on the common support and have achieved a vacuum of 2 x 10(-8) Pa at the upstream part of the front ends without synchrotron radiation. During the commissioning with synchrotron radiation, the pumping systems displayed good pumping-down characteristics. Commissioning has been successfully performed for four standard in-vacuum X-ray undulators and three bending-magnet-beamline front ends up to July 1997. Measurements of temperature rise show that absorber, pre-slits and XY slits can handle the anticipated maximum heat load at a ring current of 100 mA.

Initial results from an in-vacuum undulator in the NSLS X-ray ring.

A short-period in-vacuum undulator for the NSLS X-ray Ring has been developed in a collaboration between SPring-8 and the NSLS, and has achieved its project design goals during commissioning studies. The device is called IVUN (in-vacuum undulator) and employs magnet arrays (31 periods, with an 11 mm period) developed at SPring-8, while the requisite vacuum chamber and mechanical systems were developed at the NSLS. At a magnet gap of 3.3 mm, IVUN produces 4.6 keV radiation in the fundamental, with useful photon fluxes in both the second and third harmonics. The magnet gap is adjustable between 2 and 10 mm. A brief overview of IVUN is presented, together with initial commissioning results: the dependence of electron-beam lifetime and bremsstrahlung on magnet gap, and the output radiation spectrum.

Performance of a helical undulator of the UVSOR.

A helical undulator was installed in the 0.75 GeV storage ring of the UVSOR facility of the Institute for Molecular Science. The undulator was designed to produce the fundamental of the circularly polarized undulator radiation in the energy range 2-43 eV, and the higher harmonics with elliptical polarization in the energy range up to 300 eV. Recently, the first spectrum from the undulator was observed. The performance of the undulator and the obtained spectrum are reported.

Rotating/Helmholtz coil system using a lock-in amplifier method.

Measurement of integrated magnetic induction is an important part of the construction of insertion devices. The so-called 'flipping coil system' is normally utilized to characterize the integrated multipole components of the device, a requirement which varies from one storage ring to another. A Helmholtz coil system is used to determine a magnetization vector of each magnet piece in the device. Both systems are designed to measure the integrated magnetic flux to deduce the necessary quantities using the known relationship. We have developed an unconventional system which has a continuously rotating mechanism that allows the use of a lock-in amplifier instead of an integrator or a voltmeter. Detailed descriptions of the equipment are given.

In-vacuum figure-8 undulator for hard X-rays with both horizontal and vertical polarization.

A figure-8 undulator of the in-vacuum type has been adopted as an insertion device for BL24XU, the Hyogo Beamline at SPring-8, to provide hard X-rays with both horizontal and vertical polarization instead of a tandem undulator consisting of horizontal and vertical undulators. The undulator will be operated with the gap almost fixed at 11.6 mm to provide the fundamental radiation with horizontal polarization at 9.5 keV and the 1.5th harmonic with vertical polarization at 14 keV.

An insertion device for the soft X-ray photochemistry beamline at SPring-8.

A figure-8 undulator is an insertion device (ID) proposed at SPring-8 to reduce the heat-load problems encountered with ordinary linear undulators. This device is useful when the K value is high in order to obtain low-energy photons as the fundamental of the undulator radiation. It has therefore been adopted as the ID for the soft X-ray photochemistry beamline (BL27SU) at SPring-8 because linearly polarized photons between 100 and 2000 eV are necessary for the experiments on this beamline. The figure-8 undulator for BL27SU is now under construction and, in the first phase of operation, is expected to cover the energy range down to 500 eV. In the second and third phases, the lowest energy may be reduced to 170 and 100 eV, respectively.

Construction of a vertical undulator at SPring-8.

A new-type insertion device (ID) has been constructed for the structural-biology beamline (BL45XU) at SPring-8. The ID consists of two undulators which can provide vertical polarized radiation (vertical undulators). By changing the individual gap of each undulator independently, photons with two different energies can be obtained. Magnetic field measurements show that the maximum horizontal field is 0.49 T and the magnetic performance is as good as expected.

SPring-8 in-vacuum undulator beam test at the ESRF.

Before the commissioning of SPring-8, the in-vacuum hybrid undulator developed at SPring-8 had been brought to the ESRF for the first beam test in the summer of 1996. The purpose of this test was to investigate the influence of the in-vacuum undulator on the beam and check its vacuum system. However, heating by the resistive wall impedance turned out to be a critical issue for the in-vacuum undulators.
