[Determination of right ventricular volume by subxiphoid two-dimensional echocardiography (author's transl)].

To establish the measurement method of right ventricular (RV) volume by two-dimensional (2-D) echocardiography, a comparative study was performed in respect to the quantitative evaluations of the RV areas or volumes obtained by biplane 2-D echographic and angiographic methods. At first, a preliminary investigation was performed to define the optimum tracing of the endocardial surface on the 2-D echogram by using a fresh mongrel dog heart. It was revealed that the 2-D echographic views of the ventricular surface contained some artifact echoes inside the true echo of the endocardium. As a result, the optimum line for tracing was considered to be in external margin of the echo which seemed to represent the endocardium. This technique was then applied to 10 patients with congenital heart disease who were followed by cardiac catheterization. Ages ranged from 9 months to 5 years with an average of 2 years and 4 months. The RV was imaged by a subxiphoid approach. The RV apex, pulmonary valve (PV) and mid-portion of the tricuspid valve were showed in a frontal view, which allowed to measure the frontal RV area (f). In a sagittal plane, the RV wall and the PV were imaged, and then the lateral area (l) and the RV long axis (d) were measured. From the biplane cineangiography, the frontal RV area (f') lateral area (l'), RV long axis (d') and RV end-diastolic volume (EDV) by arealength method were calculated. The f and l were correlated with the f' (r = 0.90) and with the l' (r = 0.86), respectively. However, the values by the echocardiographic measurement were smaller than those by the cineangiographic ones. The f was also well correlated with the RVEDV (r = 0.94). The f.l/d, considered to represent a variable of echography measurement of the RV volume by area-length method, was well correlated with the RVEDV (r = 0.84). Although there are still some problems related to the imaging technique and sharpness of the echogram, further improvement to establish a echocardiographic measurement of RV volume is expected with the advance of the techniques.

Results of total gastrectomy for gastric cancer.

To compare the results of total gastrectomy performed during the last 30 years, we classified 256 patients into four chronologic groups. Despite advances facilitating early detection of gastric cancer, the percentage of patients with stage IV cancer was not significantly different among the four groups. However, operative mortality decreased and curability increased in the two most recent groups. The 5 year survival rate was significantly increased in patients operated on after 1960. Our results show that in Japan, operative mortality and 5 year survival compare favorably with reports from Western countries.

Therapeutic significance of noncurative gastrectomy for gastric cancer with liver metastasis.

The therapeutic significance of noncurative gastrectomy for gastric cancer with liver metastasis was evaluated and the histologic characteristics of such cancer examined. The mean postoperative survival time of patients with liver metastasis limited to one lobe or a few scattered metastases to both lobes without peritoneal metastasis or direct cancer invasion to other organs was significantly prolonged by noncurative gastrectomy. Differentiated adenocarcinoma, including papillary adenocarcinoma, and intravenous cancer invasion were found to be the histologic characteristics of gastric cancer with liver metastasis.

Erythremia with special reference to sideroblastic anemia.

An autopsy case of erythremia with sideroblastic tumor cell proliferation is described. A 60-year-old man was admitted to the hospital due to general fatigue and anorexia. Bone marrow aspiration revealed abnormalities in erythropoiesis (megaloblasts, 4%; sideroblasts, 84%; ring-formed, 39%, and PAS-positive, 5%). Therapy was directed to pulmonary tuberculosis. Anemia was not improved despite repeated whole blood and platelet transfusions. Serum iron and percentage saturation of the total iron-binding capacity rose during the course. Administration of vitamin B12, B6 or folic acid was inefffective. INAH was replaced by its derivative, IHMS, during the course, but the population of sideroblasts especially of ring-sideroblasts was invariably large (78%-100% and 39%-65% for total sideroblasts and ring-sideroblasts, respectively). He died with increasing abdominal pain and jaundice after three months' hospitalization. Main autopsy findings were: diffuse proliferation of atypical erythroblasts in the bone marrow, systemic lymph nodes, liver, spleen and kidneys. Most of the cells positively stained with iron. Tuberculosis of lungs with cavity formation. Discussion is focussed on the relationship between erythremia and sideroblastic anemia.

Evaluation of gastrectomy combined with the resection of other organs in the treatment of gastric cancer.

We evaluated the prognostic significance of gastrectomy combined with resection of adjacent organs in 154 cases. Patients who had undergone gastrectomy with or without combined splenectomy showed similar results and cases with combined colectomy showed a poor survival rate. However, relatively favorable results were obtained in gastrectomy cases in which combined pancreas or liver resection was performed due to direct cancer infiltration.

Chronologic comparison of postoperative results in gastric cancer surgery.

Patients who underwent gastric cancer surgery in our clinic during a 12-year period from 1960--1971 were divided into two groups. Group A is comprised of patients who were treated in the first, and group B of patients treated in the second half of this 12-year period and the short- and long-term results were evaluated and compared for both groups. We discovered that both short- and long-term results were better in group B than in group A. Group B contained more cases of histologically early cancer stages and extensive lymph node resection was performed more frequently in group B than in group A. These factors may account for the better results obtained for group B. In addition, performance of gastrectomy combined with the resection of other neighboring organs may have effected the better long-term results observed in group B.

Clinical and pathologic evaluation of patients with recurrence of gastric cancer more than five years postoperatively.

From the clinical and pathologic findings, the characteristics of recurrence of gastric cancer more than five years postoperatively were evaluated as compared with recurrence of less than five years postoperatively. The late (more than 5 years postoperative) recurrence rate was 2.0 per cent, or 17 of 864 patients who had undergone the so-called radical operation. The duration of disease prior to the first operation was comparatively long in the patients with late recurrence. Late recurrence is characteristic of localized cancer, such as Borrmann's type II, and differentiated adenocarcinoma with nonserosal invasion (pathologic findings of the resected stomach at the first operation) and by a recurrence pattern of hematogenous metastasis, especially liver metastasis.

Establishment and characterization of two hamster macrophage cell lines.

Two hamster macrophage cell lines (HM-1 and HM-2) were established in vitro from lymphoid tumors produced in hamsters by direct implantation of normal human umbilical cord leukocytes. Despite long-term culture, both cell lines maintained the morphological, functional and surface characteristics of normal macrophages. It is considered that these cell lines were derived from host macrophages infiltrating the heterotransplants.

Mechanism of the formation of megamitochondria by copper-chelating agents. IV. Role of fusion phenomenon in the cuprizone-induced megamitochondrial formation.

Megamitochondria were induced within 36-40 hours in mouse hepatocytes by injecting cuprizone into the peritoneal cavity. Induction of megamitochondria was dependent upon the amount and the time intervals of the injection of cuprizone: 200 mg of cuprizone/kg of body weight-injected every 12 hours or 400 mg of cuprizone/kg of body weight-injected every 24 hours. When the latter amount of the noxious reagent was administered to the animal every 12 hours, fatty changes of the liver was observed. Involvement of the fusion phenomenon in the mechanism of megamitochondrial formation is discussed in the light of turnover rates for various components of the mitochondrion.

Mechanism of the formation of megamitochondria by copper-chelating agents. V. Further studies on isolated megamitochondria.

Effect of cuprizone has been studied on some biochemical properties of megamitochondria obtained from the mouse liver. (1) Contents of Ca2+, Mg2+ and Cu2+ in the blood or the liver homogenates were not altered by cuprizone intoxication, whereas those in liver mitochondria were significantly altered: after 3-4 days' intoxication, content of Ca2+ was decreased and was remarkably increased after 14-15 days' intoxication. Content of Mg2+ behaved contrarily. (2) Both cytochrome oxidase and ATPase activities were unchanged in the liver megamitochondria, but monoamine oxidase (MAO) activity was significantly decreased. Value of I50 (50% inhibition) for MAO was determined to be 0.33 mM using the control liver mitochondria. Cuprizone had almost no effect on MAO activity of kidney or heart mitochondria both in vivo and in vitro. (3) The amount of lysolecithin was increased in the liver megamitochondria. These results were discussed in the light of membrane fusion phenomenon which plays a key role in the mechanism of megamitochondrial formation.
