Pathology of spinal cord lesions caused by ossification of the posterior longitudinal ligament.

The pathologic changes in the spinal cord of three autopsied cases associated with ossification of the posterior longitudinal ligament ( OPLL ) are reported. Compression of the spinal cord was marked at the level of the intervertebral disc, and the spinal cord was strikingly flattened antero-posteriorly. Intensive damage was seen in the gray matter as compared to the white matter. The white matter showed demyelination and axon loss with status spongiosus, which was more marked in the postero-lateral than in the anterior columns. The most seriously damaged parts of the spinal cord showed tissue necrosis and cavity formation which extended from the central parts of the gray matter to the ventral parts of the posterior columns. Adventitial fibrous thickening of the vein, hyaline degeneration of the walls of the arterioles, and compression of the anterior spinal vein were observed in the damaged regions. It was evident that secondary circulatory disturbance due to the compression produced by the ossification was of significance in the pathogenesis of the spinal cord damage. A tangle of peripheral nerves, ectopic and reactive, with Schwann cell proliferation was present in the posterior median fissure and the postero-lateral columns in one case. The spinal nerve roots that showed marked demyelination and axon loss were damaged by ossification at the places where the anterior nerve roots emerged from the spinal cord and where the roots penetrate the dura. There was ossification of the dura mater in all cases examined.

Ultrastructure of cartilaginous tumors and S-100 protein in the tumors. With reference to the histogenesis of chondroblastoma, chondromyxoid fibroma and mesenchymal chondrosarcoma.

Twelve cartilaginous tumors were studied by electron microscopy and the presence of S-100 protein was studied immunohistochemically in order to clarify the cell origin of chondroblastoma, chondromyxoid fibroma, and mesenchymal chondrosarcoma. Three chondroblastomas were characterized by round or ovoid tumor cells with some cytoplasmic processes, well-developed organelles and thick fibrous laminae in the nuclear membrane, occasional multinucleated giant cells and scanty chondroid matrix. S-100 protein was demonstrated in the tumor cells and some multinucleated giant cells Two chondromyxoid fibromas revealed tumor cells of varied shapes with characteristic cartilaginous differentiation and abundant chondroid matrix. Spindle tumor cells showed the ultrastructural features of cartilage cells rather than of fibroblasts and S-100 protein was also demonstrated in their cytoplasm. Chondroblastoma and chondromyxoid fibroma were considered to arise from chondrocytes. Mesenchymal chondrosarcoma ultrastructurally exhibited round tumor cells with cartilaginous nature in cartilage islands. Poorly-differentiated portions were composed of primitive round or elongated cells with occasionally admixed round cells with ultrastructural features of cartilaginous differentiation. S-100 protein was demonstrated in the cells in cartilage islands and in single cells admixed in poorly-differentiated portions. These results support the hypothesis of primitive mesenchymal origin with a tendency to differentiate toward cartilage cells.

Synchronous bilateral double primary lung cancer associated with diffuse interstitial fibrosing pneumonitis (DIFP).

Double primary lung cancer with diffuse interstitial fibrosing pneumonitis (DIFP) was detected in a 61 year-old man at autopsy. Histologically, alveolar septa in whole lobes of right and left lungs were thickened diffusely with fibrosis, edema, and inflammatory cell infiltration. Alveolar cavities contained fibrinous exudate. A tumor mass measuring 8 X 5 X 5 cm occupying the right hilar portion was epidermoid carcinoma involving the right superior lobar bronchus and hilar lymph nodes. There were no metastatic foci in the right middle and lower lobes. In the lower lobe of the left lung, scattered foci of bronchiolo-alveolar carcinoma and those of epidermoid carcinoma coexisted. There were no metastatic foci in the left upper, lower lobes, and hilar lymph nodes. Both atypical bronchiolar epithelial proliferation associated with bronchiolo-alveolar carcinoma and squamous metaplasia associated with epidermoid carcinoma were detected at the same time. The clinical and pathological characteristics among 81 reported cases of lung cancer associated with DIFP are reviewed.

Malignant mediastinal myxoid tumor and renal cell carcinoma.

A 69-year-old man with a malignant mediastinal myxoid tumor and a right renal cell carcinoma died from bilateral compression atelectasis after a two months' clinical course. The mediastinal tumor simulated a myxoid malignant fibrous histiocytoma (MFH) because of great pleomorphism of tumor cells, although the histological differential diagnosis of myxoid MFH, other myxoid sarcomas, and metastatic renal cell carcinoma was very difficult. Three reported cases of primary mediastinal MFH are briefly reviewed.

Postoperative changes of pulmonary vascular resistance in patients with complete atrioventricular canal defect. Relation to the age at primary repair.

The postoperative changes of pulmonary vascular resistance (PVR) of two age groups of children with complete atrioventricular canal (CAVC) were compared. Patients were divided by age at the time of primary repair; Group-1 (G-1, n = 4) with age below 2 years (average 13.8 mo.) and Group-2 (G-2, n = 5) with age over 2 years (average 44.0 mo.). All except one were Down's syndrome. Preoperatively, pulmonary to systemic resistance ratio (Rp/Rs) were 0.87 +/- 0.50 in G-1 and 0.41 +/- 0.13 in G-2 (n.s.), and postoperative study (average 6.5 mo.) showed no significant falls in Rp/Rs in both groups. However, G-2 showed significantly lower Rp/Rs postoperatively (0.79 +/- 0.19 in G-1 vs 0.27 +/- 0.12 in G-2, p less than 0.05). Mean pulmonary artery pressure (mPA) showed significant fall after surgery in only G-2 (66.8 +/- 6.6 to 31.8 +/- 11.6 mmHg, p less than 0.005). Anatomically, type-A showed better postoperative change in mPA compared to type-C. This study summarized the early progression of pulmonary vascular obstructive disease with poor postoperative improvement even the primary repair was done before 2 years of age. The less advanced pulmonary vascular disease in G-2 might have some relations to the natural selection of the disease.

[Combination chemotherapy with neocarzinostatin(NCS), HCFU and picibanil(NHO therapy) for advanced carcinoma of the digestive system--a comparative study with NF, and NFO therapy].

We have previously reported the clinical effects of NF therapy (NCS + 5-FU) and NFO therapy (NCS + 5-FU + Picibanil) on patients with advanced carcinoma of the digestive organs. In the present study, (NHO therapy (NCS + HCFU + Picibanil) performed in 41 patients and 30 patients were evaluated for its clinical effects. In comparison with NHO, NF and NFO, partial regression (tumor regression exceeding 50%) was noted in 5 of 30 patients (16.7%) on NHO, which was superior to 7.4% on NF, but slightly inferior to 18.8% on NFO. However, six and twelve month survival rate and 50% survival month on NHO therapy were 31.6%, 10.5% and 4.6 months, respectively and they were superior to those of NF and NFO therapy. Though the incidence of the adverse effects by NHO was almost identical with that of NFO and not more frequent than that of NF therapy. Urinary frequency, hot sensation and urgency due to HCFU administration were observed approximately in 10% on NFO therapy. In the three modalities the advantageous clinical effects on patients with hepatic carcinoma irrespective of primary or metastatic were observed.

Occlusion of the basilar artery. A clinical and pathological study of thirteen autopsied cases.

The present report dealt with thirteen autopsied cases of basilar artery occlusion. The age of the patients ranged from fifty one to seventy six years with a mean age of fifty six years, and there were eleven males and two females. Basilar artery occlusion was found in one in every 160 autopsies. The average length of the clinical course of the disease was five months. Many patients had a history of hypertension, diabetes mellitus, and cerebrovascular attacks. The neurological signs and symptoms of basilar artery occlusion extremely varied and were complicated. In our series, occular bobbing, palatal myoclonus, Foville syndrome, and Millard - Gubler syndrome are significant. Arteriosclerotic thrombosis is the most important etiologic factor. The site of occlusion was most frequently encountered in the lower third of the basilar artery. Areas of softening were prominent in the midbrain and the pons. In the cerebellum, softenings were present particularly in the areas supplied by the superior cerebellar artery. Infarcts in the thalamus and the temporo-occipital lobes supplied by the posterior cerebral artery were observed very frequently. The distribution of softening was related to the site of occlusion of the basilar artery and the collateral circulation through the Willis ring.

Pencil-shaped softening of the spinal cord. Pathologic study in 12 autopsy cases.

This report describes pathologic findings of pencil-shaped softening in 12 autopsy cases. Nine cases were associated with extradural metastasis of malignant tumor and three were observed in brain death. The cavity of the pencil-shaped softening containing necrotic debris and abundant macrophages was located mainly in the ventral part of the posterior column or the dorsal horn. It extended longitudinally over several segments of the cord, predominantly in an upward direction, and had a very clear demarcated margin from the compressed surrounding tissue. Reactive change of the surrounding tissue was very rare. Pencil-shaped softening was found most often in the level of the thoracic cord. The cavity was always continuous with an area of transverse necrosis or and area of patchy necrosis. In previous reports, the factor of circulatory disturbance was considered important for the pathogenesis of pencil-shaped softening. In addition, we believe that mechanical compression is also an important factor.

Peliosis of the spleen.

Peliosis of the spleen was found in 10 out of 1,200 cases autopsied from 1977 through 1980 in our laboratory. Eight such cases had no peliosis of the liver. Histologically, it was noticed that the parafollicular areas were the most common sites of the lesions, and this feature seemed to be important for the histologic differentiation of peliosis from simple dilatation of splenic sinuses resulting from passive congestion. The clinical and anatomic features of these cases are described and previously reported cases of peliosis of the spleen are reviewed briefly.

Malignant lymphoepithelial lesion of the submandibular gland.

A case of malignant lymphoepithelial lesion affecting the submandibular gland of a 48-year-old Japanese man is described. Histologically, a well-encapsulated tumorous mass replacing almost the whole gland was composed of lymphoid and epithelial elements, and the latter revealed severe anaplasia with frequent mitotic figures indicating malignant changes. There is no sign of recurrence up to now, 32 months after the operation. Ultrastructural studies on a formalin-fixed sampling from the gland demonstrated the squamous nature of the epithelial component. This is the first case of malignant lymphoepithelial lesion of the submandibular gland detected in Japan.

Suppressive effect of elcatonin, an eel calcitonin analogue, on excessive urinary hydroxyproline excretion in polyostotic fibrous dysplasia (McCune-Albright's syndrome).

A 12-year-old Japanese girl with polyostotic fibrous dysplasia and endocrine concomitants, was treated with elcatonin, a synthetic eel calcitonin analogue, 10 MRC unit/twice a week given by intramuscular injection. Significant decreases in 24 hr urinary content of hydroxyproline and other amino acids from bone collagen were observed during the course of treatment over 5 months. This biochemical result suggests that the synthetic eel calcitonin analogue exhibits the therapeutic effect in patients with polyostotic fibrous dysplasia by inhibiting bone resorption.

Autopsy study on operative procedure and indication for transposition of the great arteries.

The surgical correction of the transposition of the great arteries (TGA) is divided into three procedures by the methods of switching i.e., (1) intra-atrial, (2) intra-ventricular, and (3) via the great arteries. Arterial correction is considered as an ideal technique physiologically as well as anatomically. With the above in mind, twenty formalin-preserved hearts of TGA were used to carry out the following studies: (1) feasibility of coronary artery relocation, (2) feasibility of switching of the aorta and pulmonary trunk, (3) feasibility of the conduit procedure, and (4) effect of left ventricular hypoplasia. The Jatene operation in which coronary artery relocation was needed proved feasible in 20% of the cases. The Damus-Kaye-Stansel operation seemed feasible in 45% of the cases. Hypoplasia of the left ventricle was suspected in 3 of the cases that belonged to Group 1 according to Kidd's classification.

[Clinical study on bone mineral mass in metabolic bone disorders--I-125 photon absorptiometry].

Changes in bone mineral mass were investigated by 125I-photon absorptiometry in 5 groups of patients; postmenopausal osteoporosis (n = 50), after partial or total gastrectomy (n = 54), thyroid disorders (n = 28), parathyroid disorders (n = 6) and healthy subjects (n = 214) as control. Radial bone mineral content (BMC) was measured at the distal 1/3 and 1/6 sites of the radius and the densitometric patterns were analized . The results obtained were as follows: Age variation in bone mineral mass and bone mineral change in pathological status were more sensitively detectable at the distal 1/6 site of the radius (predominantly trabecular) than the distal 1/3 site of the radius (predominantly cortical). In osteoporosis there was a diminution in radial 1/6 BMC and a significant decrease of radial trabecular bone index ( RTBI ) estimated from densitometric patterns, as compared with age-matched control. Sequential changes of bone mineral mass were evaluated during the treatment. There was no significant change in bone mineral mass during the first 3 months of 1 alpha-OH-D3 administration. Bone mineral mass then decreased about 20% in radial 1/6 BMC in the course of the treatment. An increase in bone mineral mass at the distal 1/6 site of the radius was revealed at 1 year treatment with 1 alpha-OH-D3. But at the distal 1/3 site of the radius only a little change in bone mineral mass was recognized during the treatment. Total gastrectomy caused a significant deficit of radial BMC as compared to partial gastrectomy. 1 alpha-OH-D3 was also effective on bone mineral loss after gastrectomy. Loss of radial 1/6 BMC occurred in hyperthyroidism (high turn over osteoporosis) but not in hypothyroidism. Striking bone mineral loss was observed in hyperparathyroidism, which recovered after surgical treatment.
