Delayed neurotoxicity produced by an organophosphorus compound (Sumithion). A case report.

A case of late-onset manifestation of organophosphorous insecticide poisoning is described. The patient was a 70-year-old female who ingested 40 ml Sumithion emulsion (50% fenitrothion). At first, no toxic symptoms were apparent. However, 48 h after ingestion certain signs became apparent. An impediment in consciousness was observed. Fasciculation and muscular weakness were noted, while plasma and urinary 4-nitro-3-methylphenol (NMC), the degradation product of Sumithion, reached a maximum. Neither atropine sulfate nor PAM (pyridine 2-aldoxime methiodine) were effective. For 3 weeks the patient required ventilatory support, and consequently her muscle strength and neurological status gradually recovered with falling NMC levels.

Diagnostic microbiology laboratory susceptibility test results discriminate distinctive antibiotic resistance plasmids.

The sizes of the zones of inhibition around routinely tested antibiotic disks classified gentamicin-resistant isolates of Klebsiella pneumoniae from one hospital into four major antibiotype classes. From each isolate of the prevalent class (A1), two plasmids could be transferred conjugally. One carried genes for resistance to tetracycline, sulfonamides, and chloramphenicol, and for the SHV beta lactamase. The other carried genes for two aminoglycoside-inactivating enzymes, APH (3')-I and AAC (3)-III, for the TEM 1 beta lactamase, and for resistance to sulfonamides. Transconjugants of either plasmid from any A1 isolate yielded the same DNA fragments after restriction endonuclease digestion, but the two plasmids had no fragments in common. Fragments or genes from either plasmid were variously combined or lacking in plasmids from variant isolates (A2, A3, and A4). Plasmids transferable from isolates of classes B and C shared no common DNA restriction fragments with each other or with either plasmid from Class A. Fragments and genes of the plasmids from C isolates, however, were identical with those of a plasmid endemic in a nearby hospital. Routine monitoring by diagnostic microbiology laboratories of distinctive antibiotypes and of the plasmids that produce them would aid infection control and antibiotic usage policy.

[Comparative study on intravenous drip infusion of dibekacin once daily and twice daily in treatment of complicated urinary tract infections].

Dibekacin (DKB) was administered to patients with complicated urinary tract infections without any indwelling catheter to evaluate objectively and comparatively the efficacy, safety and usefulness of intravenous drip infusion once daily and twice daily in a well-controlled study. A 50 mg dose of DKB was administered twice a day to group A, and a 100 mg dose was given once a day to group B. In both groups the drug was given by 1-hr i.v. infusion for 5 consecutive days. Drug efficacy was evaluated in 72 (group A: 36, group B: 36) of the 83 patients treated, and the safety was evaluated on 81 patients (group A: 41, group B: 40). There were no significant differences in the background characteristics between the two groups. The overall clinical efficacy judged by the Committee for Evaluation of Clinical Efficacy was "excellent" in 14% and "moderate" in 50% of group A, and "excellent" in 17% and "moderate" in 64% of group B, the efficacy being higher for group B than group A, but the difference was not statistically significant. The overall drug efficacy rate for each type of infection excluding group 2, was slightly higher in group B, but this difference was not significant either. The overall clinical efficacy for each site of infection, was higher for group B but the differences were not significant. The overall clinical efficacy as judged by the attending physicians was "excellent" in 17% and "moderate" in 58% of group A, and "excellent" in 25% and "moderate" in 61% of group B. The intergroup difference was thus smaller than that judged by the Committee. The elimination rates against bacteriuria were 58% for both groups A and B, and the decrease rates including "cleared" were 42% against pyuria for both groups A and B. Bacteriological evaluation, showed that there was no significant difference in the eradication rates, between group A (65%) and group B (70%). But the eradication rate for gram-positive bacteria was 40% in group A and 81% in group B, there being a significant difference (P less than 0.05) between them. The evaluation of usefulness gave 44% and 53% "satisfactory" rates, respectively, for groups A and B. The results for the "average score" were also the same in both groups. There were no side effects in any of the 81 patients examined. Abnormal laboratory test values attributed to the drug were seen only in 3 and 2 patients in groups A and B, respectively, there being no difference between the groups.(ABSTRACT TRUNCATED AT 400 WORDS)

[Comparative studies of DL-8280 and pipemidic acid in complicated urinary tract infections by double-blind method].

The clinical efficacy, safety and usefulness of DL-8280 for the treatment of complicated urinary tract infections were compared with those of pipemidic acid (PPA) by a double-blind method. DL-8280 and PPA were orally administered at a daily dose of 600 mg (t.i.d.) and 2.0 g (q.i.d.) for 5 days, respectively. Of the 311 patients who received DL-8280 or PPA, clinical efficacy, safety and usefulness were evaluated in 228 patients (DL-8280, 115; PPA, 113), 306 patients (DL-8280, 153; PPA, 153) and 250 patients (DL-8280, 124; PPA, 126), respectively. There was no significant difference in the background characteristics between the two groups. In the DL-8280 group the overall clinical efficacy was excellent in 39.1% and moderate in 41.7%, the effectiveness rate being 80.9%, whereas in the PPA group it was excellent in 23.9% and moderate in 33.6%, the effectiveness rate being 57.5%. The efficacy in the DL-8280 group was significantly higher than that in the PPA group (P less than 0.001). According to classification by the type of infection, the overall clinical efficacy of DL-8280 in groups except group 2 (monomicrobial infection, post prostatectomy) and group 3 (monomicrobial infection, upper urinary tract infection) was superior to that of PPA, the difference being significant. Pyuria was cleared or improved in 59.1% of the patients treated with DL-8280 and in 46.0% of the patients with PPA. The difference was not significant. Bacteriuria was eliminated in 76.5% in the DL-8280 group and in 50.4% in the PPA group. DL-8280 demonstrated a significantly higher response than PPA (P less than 0.001). Of the bacteria isolated from the DL-8280 group and PPA group 89.0% and 72.1%, respectively, were eradicated after the treatment, a significant difference being observed between the two groups (P less than 0.001). The clinical efficacy evaluated by the doctor in charge was excellent in 46.1% and good in 37.4% of the patients treated with DL-8280 and excellent in 26.5% and good in 34.5% of the patients treated with PPA, the intergroup difference in the efficacy being significant (P less than 0.001). The evaluation of usefulness of DL-8280 and PPA was "satisfactory" for 71.8% and 47.6%, respectively, the difference being significant (P less than 0.001). Side effects were observed in 11 patients (7.2%) in the DL-8280 group and in 12 patients (7.8%) in the PPA group, but none were serious.(ABSTRACT TRUNCATED AT 400 WORDS)

Sequential induction of phospholipid methylation and serine esterase activation in a B cell differentiation factor (BCDF)-stimulated human B cell line.

The incubation of CESS cells with BCDF induced phospholipid methylation, i.e., 3H-methyl incorporation into phosphatidylcholine and phosphatidyl-N-monomethyl-ethanolamine, within 15 to 30 min. A threefold increase of 3H-DFP-binding was also observed in BCDF-stimulated CESS cells 2 hr after stimulation, showing the activation of serine esterase after phospholipid methylation. The inhibitors of methyltransferase, SIBA, adenosine, or HEH showed a dose-dependent inhibitory effect not only on phospholipid methylation but also on serine esterase activation and on BCDF-induced IgG production; DFP inhibited IgG production but not phospholipid methylation. The treatment of cells with DFP in the presence of SIBA abrogated the irreversible inhibitory effect of DFP on IgG production. These results showed that BCDF stimulation induced phospholipid methylation that set the stage for serine esterase activation and IgG induction.

Toxicological studies on bestatin. I. Acute toxicity test in mice, rats and dogs.

Studies on acute toxicities of bestatin (NK421) were carried out in both sexes of mice and rats, and male dogs. NK421 was administered subcutaneously, intraperitoneally and orally in mice and rats, and orally in dogs respectively. Mice and rats were observed for 14 days after treatment and LD50 values were calculated by the probit method. NK421 showed very low toxicity and no death occurred in any species following the oral administration of the maximum dose capable of dosing such as 4 g/kg for mice, 2 g/kg for rats and 1.2 g/kg for dog. General toxic signs seen in mice and rats following subcutaneous and intraperitonial injections were as follows; depression, suppressed movement, piloerection, inhibition of spontaneous movement, anorexia and emaciation. Death occurred within 5 days after administration. The toxic target organs of NK421 were found to be kidney, lymphoid tissue and liver based on histopathological examination of dead animals.

Toxicological studies on bestatin. II. Subacute toxicity test and recovery study in beagle dogs.

Subacute toxicity and its recovery of bestatin (NK421) was studied on both sexes of 34 Beagle dogs. At dose levels of 600, 240, 96 and 38.4 mg/kg, NK421 was administered orally to dogs for 90 successive days. The control group was treated orally with 2 g/dog of corn starch. Each group was constituted of 3 males and 3 females, and 2 males and 2 females were added to the 240 mg/kg group for the recovery test for 35 days. As general symptoms, loss of appetite, vomiting, abnormal feces (loose stool, diarrhea, mucous stool), eye mucus, decoloration of the visible mucous membrance and unkempt fur were observed slightly and almost dose-dependently in the group dosed with more than 96 mg/kg. Body weight decreased with the passage of time in the 600 and 240 mg/kg groups, but no death appeared in any group. In correlation with general signs, slight anemia was seen hematologically, and the increased alkaline phosphatase activity and the decreased albumin ratio in serum protein fraction were observed biochemically. The slight abnormal findings of bone marrow, spleen and liver were also demonstrated histopathologically. All the above findings disappeared during the recovery period. The maximum non-toxic dose of NK421 in this study is estimated to be 38.4 mg/kg in dogs.

[Effect of major histocompatibility complex disparity on reproductive performance].

From the viewpoint of transplantation immunology, the fetus is a allograft which bears antigens incompatible with the mother. So immunological rejection reaction has been considered as one possible mechanism of abortion. To investigate this possibility, we applied some immunological procedures to rat and mouse. The results were as follows: 1) Prior active immunization with allogeneic placental cells or spleen cells emulsified with Freund's complete adjuvant increased the rate of abortion in the rat. But this effect was not manifested in the cases to which these cells were administered without the adjuvant. 2) Prior active immunization with major histocompatibility complexes had no deleterious effect on gestation in either the rat or the mouse. 3) Compared with inbred mating, decrease in the incidence of abortion, increase in fetal weight and increase in litter size were noticed in outbred mating in the rat. The latter two were statistically significant but the former was not significant. These results suggest that the major histocompatibility complex disparity between the fetus and mother does not have a deleterious but a beneficial effect on reproductive performance. Some possible immunological mechanisms of abortion are discussed.

Permanent right bundle-branch block heralded by intermittent block and ventricular extrasystoles. A 13-year observation.

A patient is described in whom the occurrence of frequent ventricular extrasystoles of right ventricular origin and intermittent right bundle-branch block caused by interpolation of these extrasystoles heralded the slow development of permanent right bundle-branch block over a period of observation of 13 years. Such a clinical course suggested gradual progression of an organic lesion in the intraventricular conducting system. The differential diagnosis of intermittent bundle-branch block and aberrant intraventricular conduction, and possible mechanisms of ectopic impulse formation in the presence of bundle-branch block are discussed.

The requirement for esterase activation in T cell replacing factor (TRF)-induced IgG production in a human B blastoid cell line.

Involvement of stimulus-activatable serine esterase in the process of TRF-induced IgG production in a human B blastoid cell line (CESS) was demonstrated. Addition of DFP or PMSF (1 X 10(-3) M to 1 X 10(-5) M) showed a dose-dependent inhibitory effect on TRF-induced IgG induction in CESS cells. Other organophosphorous inhibitors, such as p-NPEPP and PEPP, were also effective, but a nonphosphorylating analogue of DFP, DMP, was not inhibitory. Simultaneous addition of DFP with TRF showed the inhibitory effect, and the inhibition was irreversible. However, pretreatment of the cells with DFP did not abrogate TRF-induced IgG production. In synchronized cells, signals provided by TRF could be transduced only when cells were in the G1-phase, and DFP showed an inhibitory effect only when it was added to cells in the G1-phase with TRF. One amino acid derivative (AAME) partially inhibited TRF-induced IgG production.

IgG induction in a human B cell line by red cell-mediated microinjection of the cytoplasm from T cell factor-stimulated B cells.

Generation of a cytoplasmic factor(s) that induced IgG secretion in nonstimulated cells was demonstrated in TRF-stimulated cells by using red cell-mediated microinjection. Injection of the cytoplasm from a TRF-stimulated B lymphoblastoid cell line (CESS) into nonstimulated cells induced an increase of IgG-producing cells. Injection of TRF itself did not induce an increase of IgG-producing cells. Active substance(s) in the cytoplasm were generated at 2 hr after TRF stimulation, and IgG-producing cells reached their maximum level at 40 hr after injection of the factor(s). DFP, but not actinomycin D, inhibited the generation of the cytoplasmic factor(s). The activity of the cytoplasmic factor(s) was not destroyed by RNase and not absorbed with anti-IgG. These results suggested that binding of TRF with its acceptors induced the generation of the cytoplasmic factor(s) involved in the transmission of TRF-mediated signals from membrane to nuclei.

Induction of IgG production in a human monoclonal B lymphoblastoid cell line by a B cell-specific monoclonal antibody.

IgG production was induced in a human B blastoid cell line, CESS, by stimulation with a human B cell-specific monoclonal antibody (IIIA4). The number of IgG-producing cells reached its maximal level within 48 hr after incubation with IIIA4. IgG-producing cells induced by an optimal concentration of the antibody was about one-half as many as those induced with TRF, but higher concentrations of the antibody were rather inhibitory. Simultaneous addition of an optimal concentration of the antibody with TRF partially inhibited TRF-induced IgG production in CESS cells, but higher concentrations of the antibody did not block TRF-induced IgG production. TRF activity was completely absorbed with IIIA4-coated CESS cells as well as nontreated CESS cells. These results suggested that the membrane antigen recognized by IIIA4 was not the acceptor molecule for TRF. IIIA4 did not show any inhibitory effect on PWM-induced proliferation of normal B cells, but completely inhibited PWM-induced Ig production of normal B cells.

[Thin-section CT scan of the pituitary gland (author's transl)].

Topographic anatomy of the pituitary fossa was studied by 2 mm thin-section CT scan (Somatom II). Nineteen with normal pituitary (control group) and 20 with suspected pituitary abnormality were selected. Plain and contrast CT were performed in all cases. Contrast CT was carried out immediately after the rapid infusion of 220 ml of 30% iodinated contrast medium. In all of control group but two, pituitary gland was detected as homogenous density and its density was as same as density of normal brain tissues, and was enhanced in degree of about 25 CT number. In 2 cases, small low density was visualized in the pituitary gland. Pituitary gland was differentiated from cavernous sinus was usually higher than the pituitary gland. In abnormal group, the microadenoma of the pituitary gland was diagnosed in 5 cases and 3 out of 5 cases was proved by surgery. All 3 microadenomas proved slightly dense by plain CT and enhanced higher than normal pituitary gland by contrast CT. Polytomograms showed no abnormality of the sella turcica in one of these 3 cases. Although 3 microadenomas were detected by the abnormal enhancement, we are not sure whether all of microadenoma can be detected by CT alone. Arachnoid herniation into the pituitary fossa was diagnosed in 7 of control group and 2 of abnormal group. Four out of these 9 cases were verified by using Metrizamide CT. By plain thin-section CT, the diagnosis of arachnoid herniation seems to be possible without Metrizamide CT.
