Use of urinary hydroxyproline excretion as a tumor marker in diagnosis and follow-up of prostatic carcinoma.

Urinary hydroxyproline/creatinine ratio was measured in 49 patients of prostatic carcinoma and 13 patients with benign prostatic hypertrophy, the latter group serving as controls. The results show that it is a very sensitive indicator of osseous metastases in prostatic carcinoma. The ratio was also measured in 18 patients of prostatic carcinoma with bony metastases before commencement of treatment and during treatment. The results show that the elevated pretreatment values were significantly reduced in those who responded to therapy whereas in nonresponders, the values remained high. Changes in urinary hydroxyproline appeared to reflect the nature of response to treatment better than other parameters.

Tension-torsion characteristics of the canine anterior cruciate ligament--Part I: Theoretical framework.

It has been observed that the fascicles in the canine anterior cruciate ligament do not follow a straight line path between the tibial and femoral attachment sites, but rather their path is approximately helical about the ligament axis. Thus, a mathematical model for the mechanical response of a ligament was developed from the observed geometrical configuration of the fascicle. The principal result of the model indicated that a torque must always accompany the force during elongation of a ligament. The relationships between the ligament and fascicle variables are also examined in detail.

Rotational mobility associated with the protein moiety of human serum lipoproteins from tryptophan phosphorescence anisotropy measurements.

The steady-state phosphorescence anisotropy of human low density (LDL) and very low density (VLDL) serum lipoproteins in a glycerol--buffer solvent has been investigated. By virtue of the long lifetime of the triplet state, depolarization of the emission resulting from slow rotational motions is observed between -90 and -50 degrees C. Despite the number of emitting residues within these complexes, the rotational behavior can be represented within experimental error by a single correlation time. The magnitude and temperature dependence of the rotational correlation times observed for LDL are consistent with overall rotation of essentially spherical rigid particles as the source of the phosphorescence depolarization. Preliminary data for VLDL indicate the presence of internal peptide motions which are more rapid than those of the overall complex.

Suburethral diverticulum treatment by partial ablation.

The surgical technique of partial ablation was used to treat 34 patients with a symptomatic suburethral diverticulum. Results were excellent: Symptoms disappeared and the complications of urinary fistula and stress incontinence did not occur. There were no recurrences.

Malignant lymphoma, mixed cell type, diffuse.

This retrospective study of diffuse mixed (DM) cell lymphoma was undertaken as a collaborative study between the Repository Center for Lymphoma Clinical Studies and four cooperative oncology groups (CALGB, ECOG, SECSG, SWOG), and was based on 62 patients from the files of the Repository Center. We wanted to ascertain whether there were any significant clinical differences among the various morphological subtypes of this lymphoma. All patients were treated according to different protocols of the Cooperative Oncology Groups sponsored by the National Cancer Institute. In 16 patients (26%), the malignant lymphoma (ML) had morphological features consistent with follicular center cell origin (FCC); in 34 patients (55%), the ML did not have features of follicular center cell type (non-FCC), but had morphology described for peripheral T-cell-derived ML. In 8 of the patients (13%), no agreement could be reached by the 7 histopathologists who participated in the study, and these were classified as unresolved; the remaining 4 (6%) were unclassifiable. We compared the survival times of the 16 patients having the morphological features of the FCC subtype with the survival times of the 34 patients with the non-FCC subtype and found that patients with FCC lived longer (p = 0.07 Cox's regression). In the FCC group, all patients who had complete remissions (CR) were alive; however, their survival times were similar to those who had a partial or no response (p = 0.32). In contrast, in the non-FCC group, the median survival was 20 mo, and patients with a CR had a significantly longer survival than did incomplete responders (p = 0.003). According to these results the non-FCC diffuse mixed cell lymphoma appears to be a high-grade malignant lymphoma, whereas the FCC type is not.

Mantle-zone lymphoma: a follicular variant of intermediate lymphocytic lymphoma.

A clinicopathologic analysis of 12 cases of a morphologically distinctive non-Hodgkin's malignant lymphoma which appeared to arise from the mantle zones of secondary lymphoid follicles is presented. In lymph node and spleen sections, atypical small lymphoid cells with slightly irregular and indented nuclear contours proliferated as wide mantles around non-neoplastic-appearing germinal centers. Surface marker analysis performed on lymph nodes revealed a monoclonal B-cell proliferation in two cases. Clinically, the patients usually had advanced disease (Stage III or IV) associated with splenomegaly. This lymphoma appears to be relatively low-grade, since nine of 12 patients are alive with a median follow-up period of 28.5 months. We believe that thie "mantle-zone lymphoma" represents a nodular or follicular variant of intermediate lymphocytic lymphoma.

High-pressure liquid chromatographic method for determination of Sch 29482 in human serum.

A high-pressure liquid chromatographic method was developed for quantitative determination in human serum of a new penem antibiotic known as Sch 29482 (5R,6S,8R-2-ethylthio-6(1-hydroxyethyl)penem-3-carboxylic acid). The method involves serum extraction at an acid pH with ether, followed by separation on a reverse-phase column and UV light detection at 254 nm. This technique produced a good linear relationship between the peak height ratio and the Sch 29482 concentration, which ranged from 0.09 to 35.64 micrograms/ml. In addition, this procedure proved to be quite specific for Sch 29482, since all beta-lactam antibiotics tested did not interfere with the assay. For high concentrations (greater than 0.5 micrograms/ml), the mean values obtained from the high-pressure liquid chromatographic method correlated very well (r = 0.997) with those obtained from a microbiological assay. This method is accurate and reproducible, with a sensitivity of about 0.09 micrograms per ml of serum. It is useful for monitoring serum drug levels in animals and humans and can also be used for drug pharmacokinetic studies in humans.

The clinical significance of the morphological subdivision of diffuse "histiocytic" lymphoma: a study of 162 patients treated by the Southwest Oncology Group.

We grouped 162 patients wtih advanced, diffuse histiocytic lymphoma (DHL) into various morphological subtypes to ascertain whether there were any significant differences in survival among them. These patients were staged and treated from 1972 to 1977 according to the protocols of the Southwest Oncology Group. Of the 159 patients on whom a consensus on the diagnosis was reached, 115 were classified morphologically as having large non-cleaved, 26 as B-immunoblastic, 9 as large cleaved, and 6 as T-immunoblastic. The 3 remaining patients did not fit any of these subtypes, but each had a single prominent nucleolus in most tumor cells ("prominent nucleolus" type). Morphological subdivision of DHL did not identify any subgroup of patients with a significantly longer survival, but clinical parameters such as stage, symptoms, and type of treatment significantly influenced survival times.

Ultrastructural identification of neoplastic histiocytes--monocytes: an application of a newly developed cytochemical technique.

We applied a newly developed ultrastructural cytochemical technique utilizing 2-naphthyl thiol acetate (NTA) as a substrate in 3 cases of true histiocytic-monocytic malignancy and 3 controls. The enzyme NTA esterase was identified in both neoplastic and benign histiocytes-monocytes in the form of electron-dense material within the cytoplasm. The NTA esterase varied in size and shape and showed no clear relationship with cytoplasmic organelles. The distribution and pattern of staining were very similar to those of the light-microscopically demonstrated alpha-naphthyl acetate esterase reaction. The main advantages of this method are: 1) histiocytes-monocytes can be identified in the absence of lysosomes or phagocytosis; 2) unequivocal, simultaneous documentation of both histiocytic esterase and subcellular structure is possible; and 3) both blood and solid tissue specimens can be utilized. Our findings demonstrate the usefulness and applicability of ultrastructurally demonstrated NTA esterase in the study of histiocytic-monocytic malignancies.
