Transcriptional induction of prostaglandin G/H synthase-2 by basic fibroblast growth factor.

In serum-free mouse osteoblastic MC3T3-E1 cells, basic fibroblastic growth factor (bFGF) induced mRNA and protein for prostaglandin G/H synthase-2 (PGHS-2), the major enzyme in arachidonic acid (AA) conversion to prostaglandins. mRNA accumulation peaked at 1 h with bFGF 1 nM. In cells stably transfected with a 371-bp PGHS-2 promoter-luciferase reporter, bFGF stimulated luciferase activity, which peaked at 2-3 h with bFGF 1-10 nM. In the presence of exogenous AA, bFGF stimulated PGE2 production, which paralleled luciferase activity. In serum-free neonatal mouse calvarial cultures, bFGF stimulated PGE2 production in the absence of exogenous AA. bFGF stimulated PGHS-2 mRNA accumulation, which peaked at 2-4 h and then decreased; there were later mRNA elevations at 48 and 96 h that were inhibited by indomethacin. In both MC3T3-E1 cells and neonatal calvariae, bFGF produced smaller and slower increases in PGHS-1 mRNA levels than for PGHS-2. bFGF stimulated bone resorption in mouse calvariae with a maximal increase of 80% at 1 nM. Stimulation was partially inhibited by nonsteroidal anti-inflammatory drugs. We conclude that bFGF rapidly stimulates PGE2 production in osteoblasts, largely through transcriptional regulation of PGHS-2, and that prostaglandins mediate some of bFGF's effects on bone resorption.

Acute myocardial infarction in a patient with primary coronary dissection and severe coronary vasospasm. A case report.

A case of myocardial infarction associated with coronary artery dissection in a thirty-five-year-old woman is reported. An emergent coronary angiogram revealed extensive dissection and thrombosis in the right coronary artery; in addition, severe vasospasm was observed in the left coronary artery on the next day. She was successfully treated with intracoronary thrombolysis and intra-aortic balloon pumping. The patient is alive and well two years after infarction.

Effect of transforming growth factor beta-1 on the intracellular calcium in cardiac fibroblasts.

We attempted to determine the effects of transforming growth factor beta-1 on intracellular Ca2+ concentration changes in the presence of isoproterenol in cardiac fibroblasts. Transforming growth factor beta-1 inhibited the increase of intracellular Ca2+ concentration in the presence of isoproterenol in fibroblasts. It also inhibited the production of cyclic-AMP in fibroblasts in the presence of isoproterenol. Islet-activating protein did not block these reactions of transforming growth factor beta-1. Forskolin did not affect the intracellular calcium concentration change resulting from treatment with transforming growth factor beta-1. Binding of [3H]CGP-12177 was decreased to 47% of control preincubated for 24 hours with transforming growth factor beta-1 in fibroblasts. Scatchard plots suggested a decrease in beta-adrenergic receptor number without specific change in receptor affinity. These results suggested that transforming growth factor beta-1 modulates the signal transduction through beta-adrenergic receptor and intracellular Ca2+ concentration by regulating the number of receptors in fibroblasts.

Helicobacter pylori infection, serum gastrin, and gastric acid secretion in teen-age subjects with duodenal ulcer, gastritis, or normal mucosa.

BACKGROUND: Many studies have confirmed the close association of Helicobacter pylori with duodenal ulcer (DU) in adults. However, in the subtype of DU known as 'childhood' or 'early onset DU' genetic factors seem to play a prominent role in the pathogenesis. The aim of this study was to investigate the prevalence of H. pylori in teen-age subjects with DU, gastritis, and normal mucosa and to examine the relationship of H. pylori to serum gastrin levels and gastric acid secretion. METHODS: Sixty-one teen-age subjects (24 with DU, 14 with gastritis, and 23 normal subjects) were investigated for the presence of H. pylori, antral histology, gastrin levels, basal acid output (BAO), and maximal acid output (MAO). RESULTS: All 24 patients with DU and 8 of 14 with gastritis were infected with H. pylori; none of the normal subjects were infected. Mean gastritis scores and fasting serum gastrin levels were significantly higher in patients with DU or H. pylori-positive gastritis than in subjects with H. pylori-negative gastritis or normal mucosa (p < 0.05). The difference in serum gastrin levels was also significant when patients with DU were compared with those with H. pylori-positive gastritis (p < 0.05). BAO and MAO were significantly higher in patients with DU than in subjects with H. pylori-positive gastritis or normal mucosa (p < 0.05), but there was no difference between subjects with H. pylori-positive gastritis and those with normal mucosa. CONCLUSION: H. pylori infection is associated closely with teen-age DU and gastritis and with hypergastrinemia but does not affect BAO and MAO in most infected teen-age subjects.

[Electric gustatory threshold in diabetics and its clinical significance].

Electrogustometry was performed in diabetics and the correlation between the electric gustatory threshold and clinical factors of the patients (age, disease period and diabetic complications) were evaluated (n = 50). The results obtained were as follows: 1. The electric gustatory threshold rose with aging. The patient age range for further analysis was confined to 50 approximately 69 years old (n = 34) to exclude age dependency. 2. The patients with longer disease histories tended to show a higher electric gustatory threshold, indicating that taste abnormalities in diabetics progress with disease duration. 3. Elevation of the electric gustatory threshold in diabetics was observed before onset or at the earliest stage of the three major complications (diabetic neuropathy, retinopathy and nephropatho), and the threshold rose further with the progression of diabetic complications. Thus, the electric gustatory threshold is a sensitive indicator of diabetic nephropathy and was demonstrated to be a remarkably useful indicator for the prevention of diabetic complications, since it allows detection of the three major diabetic complications at their earliest stages.

[Serum levels of soluble receptors for tumor necrosis factor in sarcoidosis].

Tumor necrosis factor alpha (TNF-alpha) has been shown to play an important role in granulomatous diseases, including sarcoidosis. TNF-alpha starts affecting cell function by binding to specific, high-affinity receptors on the cell surface, and two types of TNF-alpha receptors have been identified. Recently, soluble forms derived these cell surface receptors (sTNF-R type I and type II) have been shown to exist and have been investigated in several diseases. The levels of sTNF-R type I and type II in serum from patients with sarcoidosis were measured, and the clinical significance of sTNF-R was evaluated. The levels of both sTNF-R type I and type II were significantly higher in serum from patients with sarcoidosis (n = 36) than in serum from control subjects (n = 15): type I, 1.93 +/- 1.28 ng/ml vs 1.31 +/- 0.40 ng/ml, p < 0.01; type II, 3.48 +/- 2.60 ng/ml vs 1.56 +/- 0.35 ng/ml, p < 0.001. The levels of these receptors were significantly higher in patients with active sarcoidosis than in those with inactive sarcoidosis: type I, 2.43 +/- 1.83 ng/ml vs 1.57 +/- 0.45 ng/ml, p < 0.05; type 4.71 +/- 2.24 ng/ml vs 2.25 +/- 0.77 ng/ml, p < 0.01. The levels of sTNF-R type I and type II correlated significantly with the level of ACE, r = 0.70, p < 0.01; and r = 0.55, p < 0.05, respectively. We conclude that measurement of the levels of both types of sTNF-R may be useful in the evaluation of disease activity in sarcoidosis.

[A case of acute eosinophilic pneumonia: bronchoalveolar lavage findings before and after steroid treatment].

An 18-year-old woman presented with coughing, fever, progressive dyspnea, and diffuse infiltrates on the chest X-ray film. Analysis of bronchoalveolar lavage fluid showed 73% eosinophils. Acute eosinophilic pneumonia was diagnosed. Methylprenisolone, 1 g per day was given for three days and her condition improved dramatically. No relapse was observed. Analysis of bronchoalveolar lavage fluid also showed lymphocytosis, abnormally high concentrations of ECP, GM-CSF, IL-5 and sICAM-1, and hypersegmentation of eosinophil nuclei. After steroid treatment almost all these findings returned to normal; only lymphocytosis remained. Precipitating antibodies against four kinds of fungi, including Trichoderma viridae, were noted in the serum, but the environmental provocation test was negative and those fungi were not detected in the environmental culture growth. Comparison of bronchoalveolar lavage findings obtained before and after steroid treatment can provide information on the mechanism of eosinophil accumulation in the lung. This case also draws attention to the relationship between acute and chronic eosinophilic pneumonia.

Pernicious anemia and Helicobacter pylori infection in Japan: evaluation in a country with a high prevalence of infection.

OBJECTIVES: To evaluate the degree of gastritis and the prevalence of Helicobacter pylori in Japanese patients with pernicious anemia (PA). METHODS: Histological assessment for mucosal atrophy and inflammation was performed in gastric biopsy specimens taken from 24 Japanese patients with PA and from 24 age- and sex-matched controls. The prevalence of H. pylori was evaluated by Giemsa staining and serum IgG antibodies. Serum gastrin and pepsinogens were determined by radioimmunoassay. RESULTS: All patients with PA had severe fundic atrophic gastritis, and 17 (71%) also had antral atrophic gastritis. Thirteen (54%) of 24 age- and sex-matched controls had fundic atrophic gastritis, and 15 (62%) also had antral atrophic gastritis. Mucosal inflammation was identified in the fundus of all 24 patients and in 15 (62%) controls and in the antrum of 22 (92%) patients and 16 (67%) controls. H. pylori was not detected by Giemsa staining or serum IgG antibodies to H. pylori in any patient with PA but was present in 16 (67%) controls. Serum gastrin levels were significantly higher, and serum pepsinogen I, II, and the I/II ratio were significantly lower in patients than in controls (p < 0.001). CONCLUSIONS: Our results confirm that H. pylori infection is infrequent in PA and is unlikely to be a factor in producing type A gastritis in PA.

The role of prostaglandins in the regulation of bone metabolism.

Prostaglandins are likely to play an important role in the physiologic and pathologic responses of skeletal tissue. They are potent agonists that can stimulate and inhibit bone resorption and formation. In vivo, the major effect of exogenous prostaglandins, particularly prostaglandin E2, is to stimulate resorption and formation. These effects appear to involve replication and differentiation of osteoclast and osteoblast precursors, and to be mediated at least in part by cyclic 3' 5' adenosine monophosphate. Prostaglandins can inhibit the activity of isolated osteoclasts, probably also by a cyclic 3' 5' adenosine monophosphate-mediated mechanism. Inhibition of collagen synthesis can be seen in cell and organ cultures and appears to be caused by a receptor selective for prostaglandins of the F series and to involve activation of protein kinase C. Prostaglandin production by bone cells is regulated highly by mechanical forces, cytokines, growth factors, and systemic hormones. Prostaglandins also can amplify their own production. Regulation is associated with marked changes in the newly described "inducible" prostaglandin G/H synthase with less effect on the constitutive enzyme. Prostaglandins also may play a role in postmenopausal bone loss because estrogen deficiency, which increases bone turnover, can increase prostaglandin production in bone.

[A case of acute eosinophilic pneumonia with acute onset of dyspnea].

A 16-year-Old boy was admitted with acute onset of fever, cough, and dyspnea. Chest radiography showed diffuse infiltrated shadow and right pleural effusion. BALF revealed a high level of eosinophils (56.8%), and TBLB specimen showed infiltration of inflammatory cells, chiefly eosinophils. Symptoms and radiographic findings resolved spontaneously within a few days after admission. Acute eosinophilic pneumonia was diagnosed as in the case reported by Allen et al. However, the present patient had a high IgE level and history of atopic allergic rhinitis. In more than one third of previously reported cases in Japan, there was a history of atopy, unlike in Allen's report.

[The effects of recombinant human growth hormone (r-hGH, SM-9500, genotropin) on growth failure in children with uremia. Japanese Multi-Center Study (Genotropin) Group on Children with Renal Disease].

We treated 90 pediatric patients with chronic renal failure with recombinant growth hormone (r-hGH) for 12 months to improve their growth retardation due to uremia. They were divided into two groups, non-dialyzed and dialyzed children. The dose of r-hGH was 0.5 or 1.0 IU/kg/week in dialyzed children. After 12 months of the treatment using r-hGH, growth velocity was significantly increased in any group of children. Growth velocity was stimulated to about twice as much as before treatment (that were: in non-dialyzed group, 4.2 +/- 2.6cm/year vs. 6.2 +/- 2.0cm/year, P < 0.05, in dialyzed children treated with 0.51U of r-hGH: 2.7 +/- 1.8cm/year vs. 5.2 +/- 2.6cm/year, P < 0.001, and in dialyzed children treated with 1.01U of r-hGH: 3.0 +/- 1.5cm/year vs. 6.3 +/- 2.2cm/year, P < 0.001). No severe side effects was noted and no disturbance of renal function. Our results were consistent with those reported from Europe and USA. We conclude that r-hGH treatment is very effective in improving retarded growth in children with renal disease.

[The effects of recombinant human growth hormone (r-hGH, SM-9500, genotropin) on growth disturbance in children after renal transplantation. Japanese Multi-Center Study (Genotropin) Group on Children with Renal Disease].

We treated 27 children with renal transplantation who showed growth failure due to deteriorated graft function and/or corticosteroids therapy with recombinant growth hormone (r-hGH) to improve their growth disturbance. The dose of rhGH was either 0.5 or 1.0 IU/kg/week. After 12 months treatment of r-hGH, growth velocity was significantly increased in both groups. Growth velocity was improved from 5.0 +/- 2.9 cm/year to 7.7 +/- 2.3 cm/year, P < 0.05, in 0.51U group and 3.7 +/- 2.4 cm/year to 6.3 +/- 3.3 cm/year, P < 0.05, in 1.01U group. The most important side effect of r-hGH was relevant to graft function. 7 out of all 27 children showed deterioration of graft function. However only 2 children showed significant decreases in their graft function during r-hGH therapy. Thus we conclude that r-hGH therapy was effective to improve growth failure in uremic children even after renal transplantation due to poor graft function and/or corticosteroids therapy.

[Analysis of percutaneous hepatic abscess drainage with an emphasis on factors influencing clinical outcome].

A retrospective review of 52 patients who underwent percutaneous abscess drainage (PAD) of liver abscesses was carried out. Several kinds of bacteria were isolated from the pus in 31 of 52 cases. There were four cases of amebic liver abscess. Correlations with the shape, number, size, ascites, biliary communication, and underlying status of patients were investigated. Rates of cure, palliation, and failure were 59.6% (31 of 52 patients), 32.7% (17 of 52 patients), and 7.7% (4 of 52 patients), respectively. The cure/palliation rates in patients with malignancy or biliary diseases were significantly lower than those in patients without malignancy or biliary diseases (n = 48, p < .01). There was no statistically significant difference in the cure/palliation rate according to the shape or size of the liver abscess. Twelve patients of 52 (23.0%) died during drainage, because they were critically ill. Massive bleeding (s> 500 ml) as the major complication was seen in three (5.8%). Recurrence was noted in five (9.6%), four of whom were associated with biliary diseases. The author stresses that PAD is an effective therapeutic adjunct for any kind of liver abscess.

[Ultrasonography of pediatric right lower abdominal pain: correlation with clinical and pathological results].

Right lower abdominal pain is a common problem in the pediatric population. Ultrasonography (US) is a well-accepted imaging modality in the clinical management of this problem. One hundred and six consecutive pediatric patients with right lower abdominal pain were prospectively evaluated. There were 54 boys and 52 girls, with ages ranging from 3 to 15 years (mean: 9.9 years). Final diagnoses were established based on pathological findings or clinical follow-up. US had sensitivity of 90%, specificity of 98%, and accuracy of 94% in the diagnosis of acute appendicitis. Among all true negative cases (n = 55), there was a variety of US findings other than appendiceal abnormality in 21 cases including intestinal and/or mesenteric (n = 19), gallbladder (n = 1), and ovarian (n = 1) abnormalities. All 21 cases were managed medically without complication. We conclude that US of pediatric right lower abdominal pain is a reliable imaging modality not only for the diagnosis of surgical cases but for eliminating the unexpected surgical intervention.

The relationship between gastric secretion and type of early gastric carcinoma.

To determine the relationship between gastric secretion and gastric carcinoma, we investigated gastric acid secretion and the fasting serum levels of pepsinogen I and gastrin in 50 Japanese patients with early gastric carcinoma. After the histological and macroscopic type of carcinoma had been determined, results were compared with findings in 50 Japanese control subjects whose gastric mucosa was endoscopically normal. The maximum gastric acid secretion and fasting levels of serum pepsinogen I were significantly lower in intestinal type gastric carcinoma than in diffuse type carcinoma and in the controls. They were also significantly lower in the non-ulcerative (elevated or flat) type than in the ulcerative (depressed) type of carcinoma. The serum gastrin levels in patients with early gastric carcinoma of either the intestinal or diffuse type were higher than those in the control subjects, though the difference was not significant. Gastric acid secretion and serum pepsinogen I levels were related with both the histological and macroscopic types of gastric carcinoma. These findings suggest that the serum pepsinogen I level might be useful as a maker for early gastric carcinoma of the intestinal type.

Role of endotoxin in wound healing impairment.

We studied the effect of endotoxin on cutaneous wound healing and the mechanism involved. Rats underwent dorsal skin incision and subcutaneous placement of plastic nylon wound chambers. Immediately after this procedure, lipopolysaccharide (LPS) was administered intraperitoneally at doses of 1 to 10 mg/kg. In the rats given LPS at doses of 3 mg/kg or higher, the wound breaking strength and the production of hydroxyproline in the wound chambers were significantly reduced in a dose-dependent manner. The wound fluid serially collected from the wound chambers after surgery was examined for the effect on the growth of fibroblast. While the wound fluid from control rats showed a marked fibroblast growth-accelerating activity on postoperative day 7, this activity in the wound fluid from LPS-treated rats was significantly decreased. In the wound fluid from LPS-treated rats, the fibroblast growth-accelerating activity recovered to the control level by the addition of anti-TNF-alpha neutralizing antibody. These results suggested the following: wound healing is accelerated as a result of the production of fibroblast growth factor(s) in the site of the wound. The production of tumor necrosis factor-alpha (TNF-alpha) induced by endotoxin inhibits the effect of the growth factor(s) in the wound area, which results in decreased collagen production. Subsequently the wound healing process is impaired.

Expression and regulation of basic fibroblast growth factor mRNA levels in mouse osteoblastic MC3T3-E1 cells.

Basic fibroblast growth factor (bFGF) is a potent mitogen for bone cells and is a constituent of the bone matrix. We have found that osteoblastic MC3T3-E1 cells expressed bFGF mRNA transcript of 4.5 kilobases (kb). We examined factors that regulate the expression of bFGF mRNA and protein in MC3T3-E1 cells. Treatment of MC3T3-E1 cells with bFGF (10 nM) for 4-48 h induced another 7-kb bFGF transcript at 4 h. Treatment of MC3T3-E1 cells with TGF beta (10 ng/ml) also induced the 7-kb transcript of bFGF mRNA. In contrast, heparin, parathyroid hormone, and interleukin-1 had no effect on bFGF mRNA. Western blot analyses revealed that MC3T3-E1 cells produced a 24-kDa bFGF protein, which was increased by TGF beta. Immunofluorescence showed that bFGF protein was localized to the cytoplasm in serum-deprived MC3T3-E1 cells. Treatment of these cultures with medium containing fetal calf serum or TGF beta caused increased cytoplasmic staining for bFGF and marked shape change. Furthermore, in the cells treated with TGF beta there was both nuclear and cytoplasmic staining for the protein. These data demonstrate that bFGF mRNA and protein are expressed in osteoblastic cells and are regulated by treatment with TGF beta and bFGF. Production of bFGF may be important as an autocrine and paracrine mediator of bone cell replication, differentiation, and function.
