Since 1977, mass screening for breast cancer has been conducted in Miyagi Prefecture, Japan. The main activities consist of itinerant screening in the communities and group screening at the workplace. In addition, examinations were also carried out at a detection center. The total number of subjects examined was 90,076 in mass screening, with 4172 (4.6%) of them requiring a second examination. The overall breast cancer detection rate was 0.12% in the mass screening. In contrast, it was 3.1% at the center examination. Cytologic studies of nipple discharge were performed on 31,833 subjects. Positive findings were seen in 4 (0.004%). The incidence of smaller tumors was higher and that of nodal metastasis was lower in subsequent examinations than in the initial screening. In the high-risk group, who also underwent mammography at first screening, the detection rate was higher than that among general subjects examined.
We compared the in vitro antibacterial activities of ceftizoxime (CZX), cefotaxime (CTX), cefmenoxime (CMX), cefoperazone (CPZ), ceftazidime (CAZ), latamoxef (LMOX) and cefotetan (CTT) against 2,729 strains of 11 organisms freshly isolated from 10 medical institutions in Japan between June 1983 and January 1984 and obtained the following results: Against S. pyogenes, LMOX and CTT, which have the methoxy group at the 7 position, were less active than the other drugs. LMOX inhibited 80% of S. pyogenes at 0.78 micrograms/ml; CTT, at 1.56 micrograms/ml; but CZX and CTX inhibited 100% at 0.025 micrograms/ml or lower; CMX, at 0.05 micrograms/ml; and CPZ and CAZ, at 0.20 micrograms/ml. Against H. influenzae, E. coli, K. pneumoniae, P. mirabilis and indole-positive Proteus, these test antibiotics, especially CZX, CTX and CMX, which have the aminothiazolyl methoxyimino group, were potently active. Against S. marcescens CZX and CAZ were more active than the other drugs and against P. aeruginosa CAZ was more active than the other drugs. The test organisms did not tend to acquire resistance to these cephems when our results were compared with the results obtained at the development period.
Mass screening for breast cancer which was commenced by Miyagi Cancer Society in 1977 has been carried out on 94953 examinees. The main activities comprise itinerant screening in the communities and group screening at the workplaces. In addition to two types of mass screening, examination was also performed at the detection center. The overall breast cancer detection rate was 0.12% in mass screening. In contrast it was 3.1% at the center. Early breast cancer, however, was more frequently found in mass screening than at the center examination. The cytologic studies of nipple discharge were performed on 14314 subjects as first screening. Positive finding was seen in 5 (0.005%). In high risk group, which also underwent mammography at first screening, the detection rate was higher than that among general examinees. Method for effective procedures for detecting early breast cancer are now under study.

Two new glucose-6-phosphate dehydrogenase (G6PD) variants were discovered in Japan. The first, found in a 9-year-old male, was associated with chronic hemolysis and hemolytic crises after upper respiratory infections. The enzyme activity of the variant was 2.9% of normal. The patient's G6PD showed an increased utilization of substrate analogue, deamino-NADP, and thermal instability. The second variant occurred in a 7-year-old male with drug-induced hemolysis. The main enzymatic characteristics were reduced enzyme activity, being 6.4% of normal, faster-than-normal anodal electrophoretic mobility, slightly high Michaelis constant for glucose-6-phosphate, thermal instability, and biphasic pH optima. Enzymatic properties of these variants allowed each to be distinguished from previously reported variants. The first variant was designated Gd(-)Gifu and the other, Gd(-)Fukuoka.
A new glucose 6-phosphate dehydrogenase (G6PD) variant associated with chronic nonspherocytic hemolytic anemia was discovered in Japan. The patient showed hemolytic crises after upper respiratory infections. The enzyme activity was about 3.8% of the normal. The partially purified enzyme revealed slow anodal electrophoretic mobility, high Km NADP, marked thermal-instability, and increased affinity for a substrate analogue (deamino-NADP). A particular characteristic of this enzyme was a biphasic pH curve with a greatly increased activity at low pH values. From these results, this variant was clearly different from hitherto observed G6PD variants, and was designated G6PD Asahikawa.
The effects of antibiotic combinations against Pseudomonas aeruginosa infections frequently found in hospitalized patients were investigated. By means of an agar plate dilution checkerboard method, combinations of piperacillin-fosfomycin, cefoperazone-fosfomycin, and cefsulodin-fosfomycin were synergistic against 80.0, 85.0, and 82.6% of the strains tested. The mean fractional inhibitory concentration indices of piperacillin-fosfomycin, cefoperazone-fosfomycin, and cefsulodin-fosfomycin were 0.48, 0.42, and 0.46, respectively. The synergistic activities of these combinations were enhanced by the addition of a small amount of tobramycin, 0.25 micrograms/ml.

The mass screening for detection of breast cancer has been conducted in Miyagi Prefecture since 1977. The main activities comprise the itinerant screening in the residential districts and the group screening of female workers at the vocational sites. The routine examination at the Cancer Detection Center is also an integral part of the activities. In the present paper, the data from the above three procedures were compared. The examinees were 49,114 in the residential districts, 6,725 at the vocational sites and 3,055 at the Center. The breast cancer was detected in 51 cases (0.1%) by the itinerant residential screening, 14 (0.2%) at the vocational sites and 73 (2.4%) at the Center. The problems of the first-stage mass screening to detect breast cancer at the early stage were discussed.

The urine levels of immunosuppressive acidic protein (IAP) were measured in patients with and without cancer, using passive hemagglutination inhibition test and 24-h urine samples. In 23 control healthy adults, the urine levels of IAP were 0.13-1.3 mg/day. If an elevation of IAP levels were found in 46 (48%) of 96 cancer patients. Elevated urine IAP levels were found in 6 (18%) of 34 patients with non-cancerous gastrointestinal tract diseases. Increased urine IAP was demonstrated in 11 (58%) of 19 non-cancer patients with rheumatoid arthritis.
The levels of anticancer drugs in tissue were measured by bioassay method in 28 patients with gastrointestinal cancer, who were treated with intravenous administration of 500 mg of 5-FU combined with ANG-II during surgery. The levels of 5-FU in the cancerous tissue of the stomach and the regional lymph nodes were higher in the cases who received 5-FU and ANG-II than in the cases received 5-FU alone. Based on this result, we carried out cancer chemotherapy combined with ANG-II for 15 patients with advanced cancer. The good clinical response was observed in 4 out of 15 cases (26.7%) including 2 complete (CR) and 2 partial responses (PR), respectively. According to Karnofsky's criteria of response, more than I-A response was observed in 5 out of 15 cases (33.3%). The major side effects of ANG-II therapy were nausea, vomiting and breast pain. The incidence of the side effects was 8.7% in total.

A simple, rapid and highly specific method by selected ion monitoring (SIM), using 9 alpha, 11 alpha-[2H2]estrone, [2,4-2H2]estradiol-17 beta and 2,4-[2H2]estriol as internal standards, was developed for the determination of serum estrogens during pregnancy. Serum samples were submitted to a simple extraction procedure and were analysed after formation of the trifluoroacetic anhydride derivative. The inter-assay coefficients of variation for estrone, estradiol-17 beta and estriol were 3.73%, 3.42% and 3.49%, respectively. The results obtained by SIM were compared with analysis performed using radioimmunoassay.
