As total-body hyperthermia (TBHT) therapy may lead to a reduction in the immune response of cancer patients because of the immediate effect of heat on lymphocytes, the authors studied the immunity of advanced cancer patients receiving combined TBHT and anticancer chemotherapy. A decrease was found in their lymphocyte blastogenesis, skin reactions to PPD and PHA, lymphocyte rosette formation, IgG, and C3c component. These parameters returned to their pretreatment levels at 1 week after completion of TBHT therapy. This result indicates that there is no necessity for giving special consideration to a reduction of cell-mediated immunity in TBHT therapy.
Immunosuppressive acidic protein (IAP) in the serum from peripheral, splenic and regional venous blood of gastric cancer patients was assayed. Peripheral venous blood was collected before, and for several months after the surgery. The IAP values of peripheral venous blood, increased with advancing cancer stage; in patients at the same cancer stage, the IAP values of venous blood draining from the cancer lesion were the highest. In curatively operated patients, the IAP values increased during the first postoperative month and decreased thereafter. In patients with non-resectable cancer and in patients subjected to palliative surgery, these values remained high; they increased sharply just before death. Determination of the serum IAP values in gastric cancer patients may be useful in estimating the immunological state of gastric cancer patients and for monitoring the postoperative immunological status.

This study was designed to investigate the relationships of myocardial concentrations of norepinephrine (NE) and cyclic AMP (c-AMP) to the development of ventricular fibrillation (VF) with reference to the effects of a premedication of dibutyryl cyclic AMP (DBC-AMP) and propranolol in dogs with experimental myocardial infarction. Myocardial specimens were obtained serially from the ischemic and the non-ischemic zones by mini-drill biopsy, and NE and c-AMP levels were determined by high-performance liquid chromatography and radioimmunoassay, respectively. Before the occurrence of VF, myocardial NE increased in both the ischemic and the non-ischemic zones, and c-AMP increased significantly in the ischemic zone but did not in the non-ischemic zone. In dogs premedicated with DBC-AMP an increase of c-AMP was observed in both the ischemic and the non-ischemic zones in association with an increased incidence of VF. On the other hand, no significant increase of myocardial c-AMP was observed in both the ischemic and the non-ischemic zones of propranolol-premedicated dogs which were free from VF. A significant increase of myocardial c-AMP in the ischemic zone was observed in dogs which suffered from VF in spite of the premedication of propranolol. The incidence of VF was significantly reduced by 26.5% in dogs pretreated with propranolol. No significant changes in myocardial norepinephrine and c-AMP were observed in dogs which were free from VF throughout the experiments.
A short-term intensive chemotherapy (STIC) consisting of large dose of mitomycin C (MMC), tegafur (FT-207), streptococcus preparation picibanil (OK-432) and Krestin (PSK), combined with IVH was performed in 72 patients with advanced gastric cancer. Thirty-three patients were unresectable and 39 patients were recurrent. In this modality, an intensive chemotherapy was able to carry out, maintaining, or increasing in host immunologic parameters and body weight in spite of their disturbed condition in oral food take. Partial response was obtained in 10 of 51 evaluable cases (19.6%) and the survival period on STIC was superior to that on controls. However, no significant difference was observed between the two groups. Four patients had become to be able to take full oral food again by STIC and resulted in the removal of IVH. Hyperglycemia and septicaemia induced by IVH were observed in 9 of 72 patients (12.5%), one died of septicaemia. Full attention should be paid in the management of IVH for patients with advanced cancer.
We studied immunological significance of splenectomy in gastric cancer surgery with special reference to serum suppressor factor. The results showed that the sera from splenic venous blood in advanced gastric cancer was more suppressive to PHA blastogenesis of normal lymphocytes and also higher in the values of circulating immune complexes, as compared to the sera from peripheral blood. Furthermore, the media from cultures of spleen cells of patients with advanced gastric cancer indicated to suppress PHA blastogenesis of normal lymphocytes, as compared to the media from cultures of peripheral lymphocytes of the same patients. The suppressive activity was more increased in the media from cultures of nonadherent cell population obtained from the spleen cells. From these data, it was suggested that the spleen of patients with advanced gastric cancer was capable of producing serum suppressor factor. Therefore, gastrectomy combined with splenectomy may be reasonable in patients with advanced gastric cancer.
To study the role of regional (RLN) and distant (DLN) lymph nodes and of the spleen in the regulation of tumor immunity, we monitored tumor growth in mice subjected to lymph node removal or splenectomy. We found that tumor growth was facilitated when RLN were removed before or soon after tumor inoculation and that RLN removal after extirpation of the immunizing tumor decreased the host resistance to a subsequent challenge. When splenectomy preceded tumor inoculation, tumor growth was not affected, however, marked growth inhibition was observed when splenectomy was performed 5 days after tumor inoculation. Furthermore, splenectomy combined with extirpation of the immunizing tumor increased the host resistance to a subsequent challenge. We conclude from these studies that RLN or the spleen may be of immunologic importance in the host's tumor resistance.

The antianginal efficacy of niludipine (Bay a 7167), a new calcium antagonistic drug, was investigated in 51 patients with ischemic heart disease. 16 patients were diagnosed as effort angina and 31 patients had both angina at rest and on effort. Six anginal patients had myocardial infarction. One patient was diagnosed as a variant form of angina and 3 others as unstable angina. 11 patients had essential hypertension. 49 completed the study and 2 dropped out. Niludipine (60 mg to 80 mg every 24 h) significantly reduced the mean weekly rate of angina attacks from 6.5 to 2.2 (p less than 0.001). Marked reductions of nitroglycerin requirement were also noted (p less than 0.001). In 71% of the patients complete control of anginal attacks was achieved, and in over 77% the frequency of angina was reduced by at least 50%. Niludipine was at 93.8% effective in patients with ischemic heart disease. It decreased significantly both systolic and diastolic blood pressure in angina patients with essential hypertension, but there were no significant changes of blood pressure in normotensive anginal patients. The agent was tolerated very well and there were no side effects. These findings suggest that niludipine is a highly effective drug for the treatment of both ischemic heart disease and essential hypertension.

To clarify the relationship between the concentrations of norepinephrine (NE) and cyclic AMP (c-AMP) in the myocardium and the incidence of ventricular fibrillation (VF), we studied 48 dogs in which VF was induced by the ligation of the left anterior descending coronary artery. The animals were divided into two groups of equal size: The first received no drugs, the second received d,l-propranolol (0.5 mg/kg) 15 min before coronary ligation. Myocardial samples were taken at regular intervals from normal and ischemic areas to determine NE and c-AMP concentrations. In the untreated group, no significant change in the concentrations of NE and c-AMP was observed in 12 dogs that did not develop VF. In the other 12 dogs that developed VF, there was a significant increase in c-AMP in the ischemic area immediately after the onset of the dysrhythmia. A significant increase in NE concentration both in normal and ischemic myocardium was observed in only 8 of these latter 12 dogs. Premedication with propranolol significantly reduced the incidence of VF, but it still occurred in 5 out of 24 dogs. Increase in c-AMP without an increase in NE in ischemic area was observed in these five dogs. These results suggest that c-AMP may play a role in the development of VF during acute ischemia.
The CT appearance of bronchiectasis on inspiration and expiration in a patient with congenital bronchiectasis due to cartilage deficiency (Williams-Campbell syndrome) is reported. Computed tomography demonstrated characteristic findings of this syndrome, ballooning of bronchi on inspiration and collapse on expiration.
