Body image changes in adolescents II comparison among patients with eating disorders and controls with thin, normal and obese body shapes.

The Self-Rating Body Image (SRBI) test was used to determine whether the patients with eating disorders such as anorexia nervosa or bulimia showed their body image disturbance or not. The SRBI was completed by 120 subjects who consisted of 30 low weight (LW) controls, 30 normal weight (NW) controls, 30 high weight (HW) controls, 18 anorexic patients (AN) and 12 bulimic patients (BN). The AN group had a significantly greater dissatisfaction with the scales of the body shape, visceral organ and face image of the SRBI than the weight-matched LW group. The BN group had a significantly greater dissatisfaction with the visceral organ image shape than the weight-matched NW group. However, no significant difference in the body shape and face images between the BN and NW groups was found. Our results suggest that the anorexic patients may disturb more parts of the body image than the bulimic patients though both the anorexic and bulimic patients showed the disturbance of body image.

Combined treatment with cyclophosphamide and prednisolone can induce remission of nephrotic syndrome in a patient with renal amyloidosis, associated with rheumatoid arthritis.

A 67-year-old woman, who had been diagnosed with classical rheumatoid arthritis (RA), was admitted to our hospital because of massive proteinuria. Biopsy of the kidney revealed deposition of amyloid fibrils in the subepithelial and subendothelial spaces of the glomerular capillary walls. Though the treatment with prednisolone and dipyridamole against nephrotic syndrome and amyloidosis due to RA was not effective, cyclophosphamide, which was added after tapering of prednisolone, was able to induce remission of nephrotic syndrome after two years. The levels of CRP and serum amyloid A protein (SAA) returned to within the normal limits. As the impairment of renal function is thought to be due to deposition of amyloid supplied from the precursors of amyloid fibrils filtered from the general circulation in RA patients, remission of nephrotic syndrome might result from the suppression of production of SAA or removal of amyloid fibrils. Cyclophosphamide, which has the potential both to suppress disease activity in RA and to produce degradation of amyloid fibrils in glomeruli, may be useful against renal or systemic amyloidosis complicated by RA.

A new method for measuring small local vibrations in the heart using ultrasound.

In order to diagnose ventricular dysfunction based on the acoustic characteristics of the heart muscle of the ventricle, it is necessary to detect vibration signals from various parts of the ventricular wall. This is, however, difficult using previously proposed ultrasonic diagnostic methods or systems. The reason is that the amplitude of the cardiac motion is large during one beat period which produces large fluctuations in the transit time required for ultrasonic waves to travel from the transducer to the heart and back. This paper proposes a new method for overcoming this problem and accurately measuring small vibrations of the ventricle wall using ultrasound. In this method, the demodulated ultrasound signal reflected at the heart wall is converted from analogue to digital (A/D) signal at a high sampling frequency; from the resultant digital signal, the velocity of the wall is accurately obtained over a wide dynamic range based on the Doppler effect. The proposed method is preliminarily applied to the detection of small vibrations on the aortic wall and the interventricular septum. The new method offers potential for research in acoustical diagnosis of heart and artery dysfunction.

Effects of endothelin and angiotensin II on renal hemodynamics in experimental mesangial proliferative nephritis.

The renal hemodynamic response to pressor substances in the diseased kidney has been suggested to be different from that in the normal kidney. The aim of this study was to investigate the effects of endothelin and angiotensin II on renal hemodynamics in experimental nephritis induced by the administration of antithymocyte serum in Wistar rats. This model showed mesangiolytic lesions in the glomeruli on day 2 and hypercellular lesions on day 8. Prior to the injection of either endothelin or angiotensin II, the glomerular filtration rate and renal plasma flow were significantly lower in model rats on day 2 or day 8 than in the control rats. The basal glomerular filtration rate and renal plasma flow on day 8 were negatively correlated with the mesangium cell number. The injection of endothelin (0.5 ng/kg BW) led to a decrease in both renal plasma flow and glomerular filtration rate in rats on day 8 which was significantly greater than that in the control rats. Similarly, angiotensin II infusion (0.2 micrograms/kg BW) reduced both renal plasma flow and glomerular filtration rate in the rats on day 8 and the reductions were significantly greater than those in the control rats. In conclusion, renal hemodynamics in rats with mesangial proliferation of the kidney were more sensitive to both endothelin and angiotensin II than those in the normal kidney.

Histological abnormalities in a biopsy specimen obtained from the left ventricle without reduced function in a patient with right ventricular dysplasia.

Left ventricular histological examination was performed in a patient with right ventricular (RV) dysplasia. Although the left ventricle (LV) appeared to have a normal shape and function, the LV biopsy specimen revealed apparent myocyte hypertrophy and substantial fibrotic changes without fatty infiltration. It was not clear whether these considerable histological changes were part of right ventricular dysplasia or not. The present case indicates that even with normal LV shape and function in patients with right ventricular dysplasia, histological examination of LV may provide additional information on its pathogenesis and prognosis.

Evaluation of intraplatelet and urinary 5-hydroxytryptamine (5-HT), and urinary 5-hydroxyindoleacetic acid (5-HIAA) levels in patients with toxemia of pregnancy.

Toxemia of pregnancy is considered to represent a disorder of activated blood coagulation. To identify possible indicators for evaluating the clinical course of toxemia, we measured the intraplatelet and urinary levels of 5-HT and its urinary metabolite, 5-HIAA in 25 patients with toxemia vs. 29 non-toxemic pregnant women. 5-HT and 5-HIAA were assayed by high performance liquid chromatography (HPLC). During the 24-34 weeks of gestation, the toxemic patients and the non-toxemic women each had low intraplatelet levels of 5-HT which didn't differ significantly. A significant (p < 0.05) decrease in intraplatelet 5-HT was observed in the toxemic patients vs. the non-toxemic women after the 35th week of gestation until delivery. The untake of exogenous 5-HT by platelets was increased in the toxemic patients, and their urinary (5-HT+5-HIAA) value increased significantly (p < 0.05) as compared with the non-toxemic women after the 35th week of gestation until delivery. A decrease in the intraplatelet level of 5-HT and an increase in the urinary level of (5-HT+5-HIAA) were observed in the toxemic patients, findings that paralleled the progression of this disorder. Serial measurements of intraplatelet and urinary levels of 5-HT and urinary levels of 5-HIAA in the toxemic patients revealed a significant correlation between disease activity and those levels. These observations support the concept that the measurement of intraplatelet and urinary levels of 5-HT and of urinary levels of 5-HIAA may be useful in following the clinical course of the toxemic patients.

Solution structures of Myb DNA-binding domain and its complex with DNA.

The DNA-binding domain of Myb consists of three imperfect tandem repeats, each of which contains three conserved tryptophans. The solution structure of the third repeat was established by NMR; it includes three helices maintained by a hydrophobic core formed by three tryptophans, together with two histidines, and the second and third helices form a helix-turn-helix-related motif. Here, we report the whole structure of the DNA-binding domain, as determined by NMR. Each of the first and second repeats resembles the third one in structure, retaining three helices. As a whole, the DNA-binding domain contains an entirely unique structure, a triple helix-turn-helix-related motif. To determine the role of each repeat in DNA-binding, a 16-mer DNA duplex that is specifically recognized by Myb was synthesized and then its complex with the DNA-binding domain has been investigated by 3D-NMR. The interaction mode between the whole DNA-binding domain and the duplex has shown that the third helix in each repeat likely locates in the majour groove of DNA. It is suggested that the Myb DNA-binding domain wraps around the DNA duplex by fitting the triply repeated helix-turn-helix-related motifs in the majour groove.

[Dynamic analysis in the knees with chronic anterior cruciate ligament insufficiency--an evaluation of antero-posterior instability, leg rotation and ground reaction force].

A dynamic analysis was made on the knees with chronic anterior cruciate ligament (ACL) insufficiency for antero-posterior instability and abnormal rotation, also evaluating them for ground reaction force and muscle strength of knee extension. Studies were carried out on 51 patients with chronic unilateral ACL insufficiency and 80 knees of 40 healthy male and female young adults as controls. Using a knee motion analyser, an apparatus designed to analyse three dimensional knee motion, the gait was studied on a force plate. At the same time, the muscle strength of knee extension was measured with a Kinetic-Communicator (KIN-COM). In the dynamic analysis of the knee motion anterior instability was notable at a small angle of flexion. Qualitative evaluation of the knee motion revealed three patterns of rotation. The evaluation of ground reaction force showed that the rise from the heel strike was slow, its slope was gentle and the effect of weight removal was unclear. The evaluation of the muscle strength of knee extension revealed a decrease in torque of muscular contraction at 20 degrees of knee flexion.

Combined histochemical and biochemical demonstration of nigral vulnerability to lipid peroxidation induced by dopa and iron.

Using a newly developed lipid peroxidation-inducing system composed of DOPA and iron, we examined the vulnerability of substantia nigra to peroxidation in comparison with that of caudate-putamen obtained from normal or vitamin E-deficient animals. Histochemical detection of lipid peroxidation revealed that substantia nigra was far more susceptible than caudate putamen to DOPA and iron treatment, which was biochemically supported by measurements of thiobarbituric acid-reactive substances. Vitamin E deficiency accelerated such susceptibility of substantia nigra but had no influence on the histochemical findings observed in caudate-putamen.

An analogue of Joro spider toxin selectively suppresses hippocampal epileptic discharges induced by quisqualate.

The anticonvulsant effect of 1-naphthylacetyl spermine, an analogue of Joro spider toxin (JSTX), was studied against seizures induced by quisqualate (QUIS), a non-NMDA agonist, as assessed electrophysiologically and behaviorally in freely moving rats. Electrodes were implanted into right dorsal hippocampus and an injection cannula for drugs into right ventricle. The pretreatment with JSTX analogue significantly inhibited both of QUIS-induced hippocampal discharges (80-11%) and generalized tonic clonic seizures (100-33%) in a dose-dependent manner, whereas JSTX had no effect on seizures induced by quinolinate, a NMDA agonist. The paper provides the first direct evidence that the JSTX analogue exerts a potent and selective suppression of hippocampal epileptic discharges mediated by non-N-methyl-D-aspartate (non-NMDA) receptors.

Atypical Moyamoya disease associated with brain tumor.

A 4-year-old boy with right retinal hemorrhage, mental retardation, and multiple minor anomalies was referred to our hospital. Computed tomography scanning revealed a cystic brain tumor at the vermis. Angiography showed stenosis of both internal carotid arteries at the supraclinoid portion and the Moyamoya vessels. The right ophthalmic artery was dilated as wide as the internal carotid artery. Stenosis of the basilar artery was also observed. Collateral circulation via the posterior inferior cerebellar artery and Moyamoya vessels in the area of the posterior cerebral artery was observed.
