Japanese clinical trials with Takeda acellular pertussis vaccine.

Japanese clinical trials with Takeda acellular pertussis vaccine revealed that infants 3 to 8 months of age reacted sufficiently to the vaccine: anti-PT ant-FHA levels after vaccination were as high as those observed with 2 year old children. No substantial difference in general reactions was noted between infants and 2 year old children. Less local reactions were noted in infants.

The non immunogenicity of CALLA in leukemic children.

The sera from 55 children with acute lymphoblastic leukemia (ALL) treated with active immunotherapy were examined for the presence of antibodies against common ALL antigen (CALLA). A solid phase indirect radioimmunometric assay (IRA) was developed which enabled the detection of anti-CALLA antibody in patients' sera, utilizing the ability of affinity-purified CALLA to bind Ricinus communis agglutinin and anti-CALLA antibody simultaneously. Using IRA, anti-CALLA antibody activity could not be detected in a majority of patients. We concluded that the patients did not raise comparable antibodies against CALLA, indicating this antigen is not immunogenic for ALL patients.

New therapeutic approach to aortic dissection complicated by cardiac tamponade.

The management of aortic dissection with cardiac tamponade may result in increased blood pressure and thereby itself make the aortic dissection worse. Nevertheless, it is important to prevent cardiac failure caused by cardiac tamponade. We describe a case of aortic dissection with cardiac tamponade. Echocardiography and aortography showed DeBakey IIIb-type aortic dissection with retrograde dissection, complicated by cardiac tamponade and aortic insufficiency. To treat this condition, a new therapeutic approach was undertaken. A vasodilator was administered, then pericardiocentesis guided by echocardiography was performed. To prevent abrupt elevation of blood pressure in response to the relief of cardiac tamponade, the pericardial aspiration was carried out slowly--it took four hours for the complete drainage of 415 ml of blood--and a vasodilator, sodium nitroprusside, was administered. After drainage, cardiac function was reversed fully, and the systolic pressure was controlled under 140 mmHg. Then, using extra-corporeal circulation, the surgical procedure was performed successfully. We conclude that it is useful to treat cardiac tamponade by controlling blood pressure with slow drainage and use of a vasodilator in preparation for performing the surgical procedure.

Discontinuing chemoimmunotherapy in childhood acute lymphoblastic leukemia.

We examined the results of discontinuing therapy in Japanese children with acute lymphoblastic leukemia. Of the 209 patients in chemoimmunotherapy study, 120 (57.4%) had all chemotherapy stopped after 3 years of complete remission, and 72 (34.4%) reached the point of discontinuing immunotherapy after 5 years of complete remission. Of the 120 children removed from chemotherapy, 14 (11.7%) have relapsed, mainly in the extramedullary sites (5 testis, 5 bone marrow, 3 central nervous system, 1 bone); relapses occurred 1-23 months after cessation of chemotherapy (median 11 months). Boys had a higher post-chemotherapy relapse rate than girls (0.21 vs. 0.08, P less than 0.05). None of the 72 children removed from immunotherapy have yet relapsed. Long-term remission and possibly cure can be expected in approximately one half of newly-diagnosed Japanese patients. Moreover, the active immunotherapy could be of benefit to elimination of bone marrow relapses after cessation of chemotherapy in children with acute lymphoblastic leukemia.

[Experiences with sultamicillin granules in pediatric patients].

Sultamicillin (SBTPC) is a combination drug of sulbactam (SBT) and ampicillin (ABPC) in an ester bonding at 1:1 ratio. SBT, a new semisynthetic beta-lactamase inhibitor restores and extends the spectrum of ABPC against resistant strains of bacteria. SBTPC granules were administered to 26 pediatric patients with infections, and clinical efficacies were studied in 23 patients. The clinical efficacy rate was 95.7% and the drug was evaluated to be highly effective for the treatment of infectious diseases in the pediatric field. SBTPC was safe and well tolerated.

Overview of currently available Japanese acellular pertussis vaccines and future problems.

Acellular pertussis diphtheria, tetanus vaccine (APDT) was licensed in 1981 in Japan. This vaccine contains pertussis toxin (PT), filamentous hemagglutinin (FHA) and agglutinogen (AGG) as the main protective antigens. The new APDT vaccine produced by each company differs slightly in composition. There are two representative types of vaccine. One vaccine (B type) contains PT and FHA in a ratio of 1 to 1 and the other one (T type) contains PT and FHA in a ratio of 4 to 1 or 9 to 1 and also contains different amounts of AGG. We have been comparing the effectiveness of these two types of vaccine. The adverse reactions of APDT were local reactions such as redness and swelling, with a few febrile cases. No central nervous system adverse reactions were observed. The antibody protective level of this vaccine is also being investigated. After we changed from conventional vaccine to APDT, the frequency of serious adverse reactions was reduced and the number of pertussis infections also gradually decreased. This vaccine should be used for the children world-wide.

Purification and characterization of aplysianin E, an antitumor factor from sea hare eggs.

An antitumor factor, aplysianin E, inducing tumor lysis was purified to apparent homogeneity from the supernatant of a homogenate of eggs of the sea hare Aplysia kurodai. Purified aplysianin E was a 250-kDa glycoprotein containing three different subunits. This factor was half-maximally active at 2-114 ng protein/ml and lysed all the tumor cells tested but did not lyse normal white or red blood cells. Aplysianin E was labile on treatments with heat, low pH, urea, guanidine, sodium lauryl sulfate, and periodate, but not with proteases or organic solvents, Aplysianin E completely inhibited the syntheses of DNA, RNA, and protein by tumor cells within 2 h and caused their complete cytolysis within 15 h. Tumor lysis by aplysianin E was inhibited by N-acetylneuraminic acid, suggesting that recognition of the sugar moiety is a key step in cytolysis induced by aplysianin E. Aplysianin E also prolonged the survival of mice bearing syngeneic MM46 ascites or solid tumors. These results suggest that aplysianin E, found in an invertebrate, the sea hare, is a new antitumor factor.

Changes of neurotensinlike immunoreactivity in the striatum of the cat after intrastriatal injection of kainic acid.

In the course of our study on the effects of kainic acid (KA) on neuropeptidelike immunoreactivity in striatal neurons, it was found that neurotensinlike immunoreactivity (NT-LI) became visible immunohistochemically in cell bodies of some striatal neurons after injecting KA stereotaxically into the head of the caudate nucleus (Cd) of the cat. The adult cats injected with KA were allowed to survive for 1-56 days, and their brains were examined by the indirect immunoperoxidase method. In the intact Cd, NT-LI was observed in axonal elements that accumulated to form irregularly shaped patches and displayed a discrete island-and-matrix pattern. At the site of KA injection, where neuronal cell loss and gliosis were produced. NT-LI almost disappeared by the fourth day after KA injection. In parallel with these changes, many neuronal cell bodies with NT-LI became visible in striatal regions surrounding the site of KA injection. Most of these neuronal cell bodies that showed NT-LI were medium-sized and extended thick dendrites with spines; spiny processes were rarely seen on the cell bodies. On the other hand, when a lesion was placed in the head of the Cd by thermocoagulation, no NT-LI could be seen in cell bodies of striatal neurons. Thus it was presumed that NT-LI in cell bodies of medium-sized, spiny neurons of the cat striatum might be at a low level under normal conditions but enhanced in cell bodies of the neurons that survived the KA injections.

Evidence that neurokinin A (substance K) neurons project from the striatum to the substantia nigra in rats.

The striatonigral neurokinin A (NKA) pathway in rats was examined with an antiserum specific to the N-terminal region of NKA. Thermal lesions of the striatum decreased both substance P-like immunoreactivity (SP-LI) and NKA-like immunoreactivity (NKA-LI) in the substantia nigra (SN). The releases of SP-LI and NKA-LI in the SN were measured by a push pull superfusion technique. Addition of 50 mM K+ to the superfusion medium increased the releases of both SP-LI and NKA-LI to 1.5-2.0 times the basal values. These results suggest that NKA neurons as well as SP neurons in the striatum project to the SN in rats.

Analysis of reducing sugars as their chromophoric hydrazones by high-performance liquid chromatography.

A chromophoric hydrazide, 4'-N,N-dimethylamino-4-azobenzene sulfonyl hydrazide (DABS-hydrazide), was prepared from 4'-N,N-dimethylamino-4-azobenzene sulfonyl chloride by reaction with hydrazine. Reducing sugars were derivatized with DABS-hydrazide at 50 degrees C for 120 min. The chromophoric hydrazones were separated by reversed-phase HPLC isocratically using a short column (4.6 X 50 mm) and 0.08 M acetic acid-acetone as an eluant with no sample pretreatment and were quantitated at the picomole level. This method was applied to the sugar analysis of 5 micrograms of glycoproteins. Dansyl hydrazide derivatives of sugars were also separated by this HPLC system.

Direct projections from the dorsal column nuclei and the spinal trigeminal nuclei to the cochlear nuclei in the cat.

A retrograde and anterograde wheat germ agglutinated horseradish peroxidase WGA-HRP study in the cat indicated that some neurons in the dorsal column nuclei and the interpolar and caudal spinal trigeminal nuclei send fibers to the dorsal and ventral cochlear nuclei; to the pyramidal cell layer of the dorsal cochlear nucleus and to the cochlear granule cell domain, bilaterally with an ipsilateral dominance.

Oncostatic and immunomodulatory effects of a glycoprotein fraction from water extract of abalone, Haliotis discus hannai.

The liquid from heat-treatment of an abalone, Haliotis discus hannai, which is normally discarded as waste, was found to contain a new glycoprotein antineoplastic agent. A fraction of the liquid obtained from chromatography that was 22% carbohydrate and 44% protein was injected locally or systemically into ICR mice or BALB/c mice inoculated s.c. with allogeneic sarcoma 180 or syngeneic Meth-A fibrosarcoma, and growth of the tumors was strongly inhibited. There was an optimum dose range for the inhibition of the growth of sarcoma 180, and optimum timing. The fraction did not have antitumor activity in T cell-deficient nude mice (CD-1 nu/nu or BALB/c nu/nu mice), and administration of carrageenan in vivo decreased its activity in ICR mice. This fraction activated the cytostatic activity of peritoneal and alveolar macrophages in vivo. These results suggest that the antitumor activity is not due to a direct toxic effect but to stimulation of a host-mediated response.

The importance of primary cytomegalovirus infection in childhood cancer.

Sixty-eight paediatric patients with malignant tumours or leukaemia were followed for signs of infection with human cytomegalovirus (HCMV) over 1 year. HCMV was isolated from 24 out of 68 patients at some point during the observation period; from urine in 14, from both urine and throat in 9 patients, and from throat alone in 1 patient. Previous antibody analysis indicated the presence of HCMV antibodies in 10 of the 24 virus-shedding patients, while 7 patients were seronegative and 7 undefined. Thus the incidence of reactivation appears to be higher than that of primary infection in these immunocompromised patients. The mean duration of virus shedding was 4.2 months in the primary infection group, 1.7 months in the reactivation group and 1.1 months in the undefined group. No difference in the incidence of HCMV-associated illness was observed between patients with leukaemia and those with malignant tumours. Clinical symptoms associated with HCMV infection (pneumonia (2), fever (6) and hepatitis (1)) were observed in all patients with primary infections and in only five patients with reactivated infection.

Schistosoma mansoni: migration potential of normal and radiation attenuated parasites in naive guinea pigs.

Compressed tissue autoradiography using [75Se]selenomethionine labelled parasites has been used to investigate the migration potential of normal and radiation attenuated cercariae of Schistosoma mansoni in naive guinea pigs. By Day 14 after infection. 44% of normal parasites were detected as reduced silver foci in the liver; this value corresponded well with the number of liver parasites recovered by retrograde perfusion of the hepatic portal system on Day 42 (42% of the challenge). In contrast, cercariae subjected to 50 krad of gamma irradiation failed to migrate out of the skin. The migration capacity of 20 krad irradiated parasites was less severely affected in that about half of the challenge parasites reached the lungs, but virtually none moved to the liver. These data are discussed in relation to the kinetics of immunity induced in guinea pigs by infection or vaccination with normal or radiation attenuated parasites.

Neurophysiological analysis of ataxia in capsular ataxic hemiparesis.

A quantitative analysis of ataxia and the readiness potential were studied in four cases of ataxic hemiparesis resulting from a small infarct in the posterior limb of the internal capsule. The ataxia appeared to be the result of involvement of the corticopontine tract originating from the precentral region (areas 4 and 6) at this level. The voluntary movements of the affected limbs were characterized by slowness and irregularity similar to those seen in cerebellar ataxia. The weakness per se was not such that it could account for the ataxia. The dentato-rubro-thalamo-cortical system did not appear to be significantly involved on the basis of normal readiness potentials.

Amino acid sequence of two sea anemone toxins from Anthopleura fuscoviridis.

The amino acid sequences of two toxins, AFT-I and AFT-II, from the sea anemone Anthopleura fuscoviridis were determined. AFT-I and -II consist of 47 and 48 amino acid residues, respectively, and are cross-linked with three disulfide bridges. The sequences have high homology to those of toxins I and II from Anemonia sulcata and anthopleurin-A and -B from Anthopleura xanthogrammica.
