[Visits to emergency rooms and hospital ownership categories].

Trends in emergency room (ER) visits in Israeli general hospitals were analyzed and differences were found according to category of hospital ownership, Data for 1986-90 were gathered from 23 hospitals which included 90% of the country's acute care beds. An upward trend in ER visits in the 1970's and early 1980's ended in 1987. Despite a 7% increase in population during 1987-90, total ER visits decreased by 4%. In Jerusalem's public hospitals ER visits decreased by 13%, in government and government-municipal hospitals by 7%, while in Kupat Holim Clalit hospitals there was a 2% increase. The proportion of ER visits resulting in hospital admissions also varied by hospital ownership and also among individual hospitals within any given ownership category. Factors which might influence ER visits include size and composition of populations, development of alternative sources of care in the communities, price of services, changes in hospital admission policies, nature of the competition between hospitals, supply of physicians, and the use of ER's for primary care.

Sleep and dreaming in Holocaust survivors. Dramatic decrease in dream recall in well-adjusted survivors.

Sleep data were obtained on 12 well-adjusted and 11 less-adjusted Holocaust survivors and on 10 control subjects. Each was also awakened from rapid eye movement sleep for dream recall. The less-adjusted survivors had more prolonged sleep latency than the well-adjusted and the control groups and lower sleep efficiency than the control subjects. The well-adjusted group had a significantly lower dream recall rate (33.7%) than the less-adjusted (50.5%) and control groups (80%). There were also significant between-groups differences in dream structure and dream content, in the direction of less complex and less salient dreams in the well-adjusted survivors. It is suggested that the decrease in dream recall is one of the forms of long-term adjustment to severe traumatic events.

Geographical variations in unnecessary hospitalization of the elderly: the effects of local conditions.

Three hundred and fifty-three elderly patients hospitalized in acute-care general wards in Tel Aviv and Jerusalem were studied to check whether their continued hospitalization was medically justified. Of these, 53 (15%) were found not to be in need of acute hospital care, the proportion being significantly higher in Jerusalem (20%) than in Tel Aviv (12%). The major contribution to the overstays in Jerusalem was prolonged hospitalizations of patients in need of custodial care. There was also a significant association between physical and behavioral impairment and patient overstay in both cities. Our findings suggest that hospital overstays could be reduced by the creation of an intermediate level of care for patients with placement problems following acute hospitalization.

Inhibitory effect of a second stimulus on reaction time to a primary stimulus: a partial successful replication.

A second visual stimulus, to which no response was required, following a primary one lengthened the reaction time to the first stimulus for 25 college students. Reaction time rose as a function of the increase in the interstimulus interval. Duration time of the second stimulus did not affect this response. These results were found under a condition of stimulus-response certainty.
