Correction of a chest wall deformity utilizing latissimus dorsi with a turnover procedure.

Infraclavicular hollowing and the abnormal anterior axillary fold are often seen in patients with Poland's syndrome and related conditions. The operative procedure described in this article corrects these deformities. The procedure uses the latissimus dorsi island flap. The inserted muscle is moved anteriorly and sutured between the deltoid and the biceps. Then the muscle flap is twisted between its origin and where it is inserted. The muscle fibers act like a pivot and the thickness of the flap is increased, which enables us to achieve a round anterior axillary fold. The muscle flap fills the infraclavicular region and softens the deep hollow area.

Re-endothelialization stages at the microvascular anastomosis.

After microsurgical suturing, the wound healing process at the rat femoral artery anastomosis was studied with scanning electron microscopy. The re-endothelialization was divided into four successive stages, based on the development of a fibrin network and the rate of endothelial regrowth. Within 5 minutes of blood flow re-establishment the fibrin network formation started to cover the wound surface; it was completed by the 3rd or 4th day. The endothelial recovery started on the 2nd day and continued to the base of the suture by the 6th day. Re-endothelialization from the wound edge, which was formed between intact and injured endothelium toward the suture-restrained site, was a two-fold process: initially, a single endothelial film covered the anastomosis site, then, the singled endothelial sheet proliferated and thickened to complete the endothelialization. A better understanding of the stages involved in microvascular repair is beneficial in evaluating vascular recovery and its relationship to patency rates in microvascular surgery.

[Fundamental and clinical studies on aztreonam in the field of obstetrics and gynecology].

The following results were obtained through the fundamental and clinical studies of aztreonam (AZT), a new monobactam antibiotic, in obstetrics and gynecology. Satisfactory tissue penetration was not recognized in 3-5 hours after intravenous injection of 1 g. However, the concentration was more than MIC for the majority of aerobic Gram-negative bacteria in intravenous drip infusion of AZT 1 g and in intravenous injection and intravenous drip infusion of 2 g. There was no difference of serum concentration of AZT between uterine arterial blood and cubital venous blood. AZT was administered by intravenous drip infusion to 2 cases of puerperal endometritis, 2 cases (the same patient) of vaginal wall abscess, 1 case of postoperative lymphocele infection and 1 case of infected ovarian cyst suspected, and it was effective for all of them. Neither subjective/objective side effects nor abnormal laboratory findings were noted in any case.

Nasal chondroma: a case report.

Chondromata of the nose are rare. Since it was first described by Morgan in 1842 (cited by Kilby and Ambegaokar, 1977), 131 cases have been recorded in the world literature. We present an unusual case of histologically proven benign chondroma arising in the nasal vestibule, treated by excision and repair of the defect with a composite free graft from the ear.

Aesthetic surgery of the trunk and extremities in the Japanese.

The number of patients seeking dermolipectomy of the trunk and thighs is increasing among the Japanese. The bodily proportions of the Japanese are different from those of Caucasians. Furthermore, the Japanese tend to hypertrophic scarring. We report our thigh lift technique, including preoperative and postoperative management.

Evaluation of facial osteotomy with the aid of Moiré contourography.

The Moiré method, a new technique for recording contour lines on the body surface, was applied to the evaluation of facial osteotomy in patients with oro-facial disharmony. The advantages of the methods are three-dimensional, with no contact. Small changes of the facial profile after osteotomies were measured three-dimensionally and the prediction of probable facial contour changes by osteotomies is discussed.

Correction of myopathic face associated with myotonic muscular dystrophy. A case report.

Two cases of myotonic muscular dystrophy manifested oro-facial disharmony and dento-labial dysfunction. Le Fort I osteotomy, sagittal osteotomy of the mandible, genioplasty and wedge excisions of the mucosa of the lower lip were performed on both patients. The desired results with regard to oro-facial appearance and dento-labial function were achieved in both patients after follow-up periods of six and twelve months.
