The problem of participation in the Dutch National Dental Survey.

In 1986 a nationwide dental survey was carried out in the Netherlands. The aims of the study were to assess the prevalence of oral diseases, need and demand for dental care and oral selfcare in the Dutch population aged 15-74 yr. This paper describes the general sampling and analysis procedures to detect selectivity in the participation. Irregular dental attenders seemed to be underrepresented in the final sample. Poststratification was used to overcome this problem of selectivity.

Objective and subjective need for cosmetic dentistry in the Dutch adult population.

In order to assess the objective and subjective need for cosmetic dentistry in the Dutch adult population in 1986 a stratified sample of 2784 dentate persons, aged 15-74 yr, participated in the study. The aesthetics of anterior teeth and first premolars in the maxillary mandibular arch were determined by dental examiners as well as by the examinees. According to the dental examiners 16-63% of the people in the age range from 15 to 74 yr were in need of cosmetic dental care. Based on the patient's opinion (subjective need) these percentages varied between 18 and 40% in the different age groups. The discrepancy between the objective and subjective assessment in the percentage of people in need of cosmetic dental care increased with age.

Malocclusion and orthodontic treatment need of 15-74-year-old Dutch adults.

In 1986 a nationwide dental survey was performed in the Netherlands. This article describes the prevalence of dentofacial anomalies in the age group 15-74 yr, as well as the subjective and objective need for orthodontic treatment. The main findings were: severe crowding in the mandible was found in approximately 15%; an Angle Class II situation was seen in 28%; a maxillary overjet of greater than 5 mm was found in 23%; orthodontic treatment had or was being performed in approximately 25%; 45% of orthodontic treatment was carried out by a general practitioner; using professionally defined need, 39% of the population required treatment whereas only 14% of the people felt that this treatment was necessary.

Tooth loss and dental caries in Dutch adults.

As part of a nationwide dental survey in 1986 a caries study was executed in 3526 adults, aged 15-74 yr, living in 40 residential areas in The Netherlands. Marked differences were found between the social classes distinguished (more edentates in the lower classes) and between the different regions. It is supposed that these differences reflect the lack of dental manpower in the past.

[National dental health survey. 4. Dental visits. 1. Behavior and knowledge].

This paper is part of a series on the national oral health survey in the Netherlands (LEOT-project). Previous articles described the development of the questionnaires and the results concerning oral hygiene and nutrition. This paper presents the results as for the dental attendance behaviour and knowledge concerning it of the dentate Dutch population. As an average the Dutch dentate visited the dentist 2.4 times a year. 89% visited the dentist in the last year of this investigation. The largest part of the visits (68%) concerned 'check-up visits'. 14% did not visit the dentist for a check-up. Knowledge about dental visits is rather limited; especially concerning treatment possibilities.

Caries prevalence in Dutch elderly people.

In 1986 a nationwide oral survey was carried out in the Netherlands. One of the aims of this study was to assess the prevalence of oral diseases in adults. The present paper describes the prevalence of coronal and root caries in elderly Dutch subjects (45-74 years). The oldest age group (65-74 years) had, on average, the least number of teeth remaining; the posterior oral segment contained fewer teeth than the anterior segment. The anterior segment had the higher percentage of teeth with coronal caries and the posterior segment a higher percentage of filled teeth. The reverse was the case for root caries; a higher percentage of teeth had root caries in the posterior segment and the anterior segment a higher percentage of filled teeth.

Dental caries prevalence and the use of fluorides in different European countries.

Data in a WHO report relating caries prevalence and the use of fluorides in different European countries were compared with data obtained directly from investigators in the field of oral epidemiology. The reliability of DMFT indices of 12-year-old children, mentioned in the WHO report, appeared sufficient to make comparisons between countries on the basis of these data. The WHO data files on the different applications of fluoride were incomplete. When figures were present, they were not always in agreement with those from the investigators. A decrease of the DMFT indices could be shown in a large number of countries. In further comparative studies between European countries, cultural differences (in dietary habits, for instance) should be taken into account.

[The dental self care of the dentate Dutch population; nutrition].

This paper presents the results of a nationwide dental survey as far as the nutrition behavior and its determinants of the dentate Dutch population is concerned. It is found that the avoidance of sugar containing products for the sake of dental health is not very common; only 8% of the dentate population shows this behavior rather frequently. Despite the existence of a fair amount of knowledge about the relation of sugar consumption and dental health, the main part of the dentate population does not belief in the effectiveness of sugar avoidance in the preservation of the teeth. As an average the dentate Dutch takes, apart from the meals, nearly eight sugar containing products a day.

[Dental self care of the dentate Dutch population: oral hygiene].

This paper is part of a series on national oral health survey in The Netherlands (LEOT-project). It presents the results as for the oral hygiene behavior and its determinants of the dentate Dutch population. Nearly all dentate Dutch people (97.5%) reports to clean their teeth with an average of nearly two times a day. The use of other cleaning means is not very common. There is a fair amount of knowledge about dental hygiene. The great majority (about 85%) indicates that toothbrushing for them is an automated behavior, a habit.

[Revised recommendations about the use of fluoride].

At the end of 1989 the organisation for dental health education in The Netherlands 'Ivoren Kruis' presented new recommendations about the use of fluorides. The advice was based on a report of an expert committee and was put in a definite form in consultation with representatives of a number of dental and medical professional organisations. For children below the age of five a toothpaste with 250 ppm F- is recommended. Fluoride tablets (with 0.25 mg F-) are recommended for children below the age of 12. The advised number of fluoride tablets is reduced compared with the former recommendations.

[Dental fluorosis in relation to the use of fluoride tablets].

The prevalence of dental fluorosis on the buccal tooth surfaces was investigated in 15-year-old children in the towns Tiel and Culemborg, using the Tylstrup-Fejerskov scoring system. A study done in 1980 revealed that at the age of six, 53% of the children in Tiel and 30% of the children in Culemborg took one or more fluoride tablets (0.25 mg F-) daily. In Tiel the tablets were provided free of charge in a centre for child dental care. On the basis of the number of tablets supplied in the age period 1.5-6 years, most of the 15 year olds in Tiel could be classified as regular, irregular or non-tablet user. Only mild fluorosis (TF-score 1, 2 or 3) was found. The percentage of children with one or more buccal surfaces affected with fluorosis was 24 in Tiel and 22 in Culemborg. The percentages of buccal surfaces with fluorosis were 6 (Tiel) and 7 (Culemborg). In Tiel the percentages of surfaces with fluorosis in the groups regular, irregular and non-tablet users were 12%, 5% and 2% respectively.

Has the decline in dental caries been halted? Changes in caries prevalence amongst 6- and 12-year-old children in Friesland, 1973-1988.

In 1973, 1976, 1979, 1982 and 1988 caries investigations were carried out amongst 6- and 12-year-old primary schoolchildren to study the effectiveness of a dental health campaign begun in 1973. This campaign and the school dental health services were stopped in 1985. In all the years, except 1988, radiographs formed part of the caries examination. The percentage of 6-year olds with a caries-free dentition increased from 4 per cent in 1973 to 47 per cent in 1988, and for 12-year olds from 0 per cent in 1973 to 45 per cent in 1988. The mean dmfs score decreased steeply from 14.4 in 1973 to 3.8 in 1982 and rose slightly to 4.0 in 1988. This latter score was 4.6 if a correction for the X-ray effect were applied. The mean DMFS score in 12-year olds showed a constant decline, from 13.7 in 1973 to 2.0 in 1988. The half in the decline of caries in the deciduous dentition of these 6-year-old cohorts is regarded as a warning that caries may be on the increase again in this part of The Netherlands. Regular monitoring is therefore required. Explanations for this halt in the decline of caries are discussed.

[Dental survey (Leot-project): aims, sampling and participation].

In 1986 a nationwide dental survey was carried out in the Netherlands. Aims of the study were to assess the prevalence of dental diseases, needs and demands for care and dental behavior in the Dutch adult population aged 15 years and more. The paper describes sampling procedures, stratification factors and field organization of the study. As a result, representative (baseline) data of prevalence and need for care of dental diseases are now available. Those data will be published in a number of separate papers in this journal.

[Prevalence of TMJ signs and symptoms in the Dutch adult population].

In 1986 a nationwide dental survey was carried out in The Netherlands. One of the sub-objectives of the study was to assess the prevalence of TMJ signs and symptoms in the Dutch adult population. A stratified sample of 3526 persons, aged 15 to 74 years, participated in the study. Participants in the clinical examinations were asked by questionnaire about subjective signs and symptoms of TMJ. Clinical examination of joint sounds, deviation and pain during mandibular movements were also performed. With regard to the Helkimo classification it can be stated that 78.4% of the participants had no subjective signs and symptoms of TMJ, whereas clinically recorded 55.6% were symptomfree. 4.9% had severe subjective signs and symptoms, and 2.8% had clinically recorded severe signs and symptoms of TMJ. In general a higher percentage of women in comparison to men showed signs and symptoms of mandibular dysfunction. Only 1.4% of the people with severe subjective signs and symptoms had sought for professional help.

[Periodontal treatment need in the Dutch adult population].

In 1986 a nationwide dental survey was carried out in The Netherlands. One of the objectives of the study was to assess the periodontal treatment need in the Dutch adult population. A stratified sample of 3526 people aged 15 to 74 years, participated in the study. Based on the Community Periodontal Index of Treatment Needs (CPITN) it can be stated that more than 90% of the adult population of 25 years and over needs oral hygiene education and scaling in one of more sextants (TN2). Complex treatment, scaling and oral hygiene education (TN3) is needed in 19 to 28% of the population aged 45 year and over. The estimated time for performing the necessary periodontal treatments, based on four different time assessments, varied from 12.4 to 31.1 million hours.

[Oral health perspective in children and adolescents].

Members of a panel gave their opinions on future developments affecting the prevalence for caries and gingivitis among the Dutch youth (0-34). For the younger age groups a small increase of the dmf-t count was expected by the members of the panel, which was in line with the results of a simulation model. For the oldest group the model showed a smaller decrease for the DMF-T count then the panel members forecasted.

[Lesions of the oral mucosa].

Representative data were gathered about the prevalence of mucosal lesions in Dutch adults (15-74 years old). The reliability of the observation methods was moderate. Some data were not usable for this reason. Only a low percentage of people without a denture, had a mucosal lesion. About 60% of the people with a denture had hyperaemia of the palatine mucosa. A flabby ridge exists in 34% of the 35-54 year olds and in 51% of the 55-74 year olds with such a prosthesis.
