Susac syndrome: a vasospastic disorder?

PURPOSE: The Susac syndrome is a microangiopathy that leads to visual symptoms, hearing loss and neurological symptoms. CASE REPORT: We report on a young woman suffering from this syndrome who also presented the following signs and symptoms typical of a vasospastic syndrome; 1) a history of cold hands, low blood pressure and migraine; 2) a typical alteration of conjunctival vessels; 3) prolonged flow arrest time after cooling in nailfold capillaromicroscopy; 4) increased resistivity in the orbital vessels measured by color Doppler imaging; and 5) an increased plasma level of endothelin-1. CONCLUSIONS: We postulate that the Susac syndrome is a manifestation of the vasospastic syndrome.

Interaction of disinfectant residues on cleanroom substrates.

This study will determine the levels of disinfectant residues on stainless steel surfaces after simulated manual cleaning activities. Additionally, this study will determine if chemical interactions between different chemical agents, representative of commonly used cleanroom disinfectant technologies, subsequently applied to the same surfaces exist, and to what degree these interactions impact sporicidal performance of an oxidizing biocide against Bacillus subtilis.

[Sleep disorders in old age].

Complaints about troubled and insufficient sleep increase with age. Sleep disorders are not a disease, but a symptom of different causes. The most important is the etiological treatment. For a symptomatic treatment, we dispose of psychological help, physiotherapy and phytotherapy. Hypnotic drugs should be used only if absolutely necessary and for a short time. One has to find the adequate drug for each case and the dose corresponding to the age of the patient. As up today there are no drugs inducing physiological sleep.

[Chlorambucil pulse therapy in progressive chronic polyarthritis. Initial results of an open multicenter study].

The first preliminary clinical results with chlorambucil pulse-therapy in patients with refractory rheumatoid arthritis are reported, with a dose of chlorambucil below that which might risk inducing leukaemia. The clinical response was good in 15 out of 19 patients; symptoms and objective findings of rheumatoid arthritis had significantly improved. However, multiple toxic reactions have been identified in our patients.

[Differential diagnosis of sleep disorders in old age].

Somnipathy represents neither an independent disease nor a nosological entity. The simple formulated complaint: "I can't sleep" requires a very detailed investigation of the surrounding conditions, habits and the physiological situation of the patient. Further one has to consider, resp. to rule out a great number of causes of diseases, i.e. those of the internal organs, the central nerve system and the psyche. The majority of cases of somnipathy in elderly patients reveals a treatable cause, of the prescription of hypnotics becomes unnecessary.

[Polymyalgia rheumatica].

Giant cell arteritis-mediated polymyalgia rheumatica is a typical disease of old age; though manifestation is rare in patients younger than 50 years, there is no upper age limit. Average disease-onset is at age 70. Typical symptoms and high serological inflammation are found. Laboratory data is nonspecific; temporal artery biopsy is imperative in unclear cases. Only glucocorticoid therapy initiates prompt pain relief, stops the inflammatory process and apprehends vascular complications. Monitoring of therapy requires careful guidance and experience.
