[A case of cardiomyoplasty with synchronous stimulated latissimus dorsi muscle].

In December, 1988, a 57-year-old man sustained an acute myocardial infarction. He suffered from progressive cardiac failure, despite bed rest and intensive medical treatment. Finally, he developed hepatic and renal failure. Subsequent angiographic studies revealed the total occluded LAD (Seg. 6) and a large LV aneurysm. In January 1989, a left ventricular aneurysmectomy and cardiomyoplasty using the latissimus dorsi muscle were performed successfully. Two years after the operation, he is alive and well.

[A case of the management of developed collateral vessels through left thoracotomy of recurrence of right bronchiectasia with hemoptysis].

A 40-year-old woman, with a previous history of right S1, S2 segmentectomy and right middle lobectomy for bronchiectasis with hemoptysis at the age of 24, was admitted for frequent hemoptysis. As a result of a chest roentgenogram, a CT scan and bronchographic examination, a recurrence of bronchiectasis in the right ventral segment of the upper lobe (S3) and the right superior segment of the lower lobe (S6) was diagnosed. The bronchial arteriogram and aortogram showed four vessels going into this region and a shunt lesion was located on located on the proximal side of the pulmonary artery. If a right pneumonectomy had been done, pulmonary function would have gotten worse. If a right thoracotomy had been performed, there would have been much bleeding from the entering vessels. Therefore, a left thoracotomy was preformed and those vessels ligated. The patient is doing well one year after the operation without hemoptysis.

Pituitary enlargement associated with Addison's disease.

We report a case of Addison's disease with pituitary enlargement which could easily have been misinterpreted as a pituitary adenoma or other pituitary disease. Serial computed tomography following replacement therapy was essential to confirm the diagnosis.

[Effects of preoperative administration of UFT in gastrointestinal cancer].

UFT, a combination antitumor drug consisting of 1 part Futraful and 4 parts Uracil, was administered preoperatively to 10 patients with gastric cancer, 9 patients with colo-rectal cancer and 1 patient with hepatocellular carcinoma. A pharmacokinetic study was then carried out after oral administration of 600 mg per day of UFT, measuring Uracil, Futraful and 5-FU levels in serum and tumor tissue. Preoperative total doses of UFT for gastric cancer were 3.0-11.4 g, for colo-rectal cancer 3.6-16.8 g and for hepatocellular carcinoma 8.4 g. Side effects, mainly gastrointestinal symptoms, were observed in 3 cases. Abnormalities of liver function test, depression of serum protein and bone marrow damage were observed in 4 cases. 5-FU concentration in the tumor tissue was higher than 0.05 mu/g in 15 of 19 patients (79%). This suggested that 5-FU was maintained in the tumor tissue for a longer period. However, it also suggested that the concentration of Uracil in the tumor tissue corresponded to the total dose of UFT as did the degree of side effects.

[Effect of CAF'-endocrine therapy on advanced and recurrent breast cancer].

Two cases of primary advanced breast cancer and 4 cases of recurrent breast cancer were treated with cyclophosphamide, Adriamycin and FT-207 combined with hormone therapy. Adriamycin (20 mg) was injected twice a week, and an intravenous drip infusion of cyclophosphamide 150 mg), and FT-207 (400 mg) was administered daily for 3 weeks. Oophorectomy was performed in 4 cases and combination administration of tamoxifen and calcitonin was continued. CR was achieved in one of the primary cases, PR was obtained in 4 cases but one patient with pulmonary metastasis died without showing any clinical response. All cases with bone metastasis responded well. Leucopenia, alopecia, nausea and masculinization were easily controlled. No cardiac toxicity was observed.

[Long-term follow-up of surgical patients with ischemic heart disease].

Long-term follow-up of a consecutive 333 patients who underwent coronary artery operations from 1974 through December of 1982 is reported. The surgical procedures in this series consisted of aortocoronary bypass grafting for angina pectoris in 269 patients (Group I), various operations for myocardial infarction and its mechanical complications in 44 patients (Group II), and combined operative procedures in 20 patients (Group III). The 5 year survival rate of Group I, Group II and Group III were 92.9% 72.7% and 80% respectively. Nine out of 14 late deaths in the whole group were not related cardiac deaths. Sixty eight patients with aorto coronary bypass grafting were asymptomatic and 27% were less symptomatic in the long term periods. The postoperative study indicates that aorto coronary bypass grafting increased coronary blood flow estimated by regional coronary venous flow associated with improvement of left ventricular function and myocardial metabolism resulting in so called "better quality of life". The successful aorto-coronary bypass grafting prevented the incidence of myocardial infarction at the time of subsequent combined vascular or malignant operation in the same individuals. Although the early death was somewhat high in the patients with acute myocardial infarction, the patients who survived the operation had the satisfactory long term results.

The studies of the mechanism of antiinflammatory action of 2-(5-ethylpyridin-2-yl)benzimidazole (KB-1043).

The mechanism of the antiinflammatory activity of 2-(5-ethylpyridin-2-ly)benzimidazole (KB-1043), a new benzimidazole derivative with antiinflammatory, analgesic and antipyretic activities, is described. KB-1043 inhibits the emigration of leucocytes and the exudation of protein into the site of injury. KB-1043 possesses also a potent membrane stabilizing activity. Although KB-1043, in vitro, did not inhibit the enzyme activity, the increase of enzyme activity at the site of injury was inhibited by the oral administration of KB-1043. The inhibitory effect of KB-1043 on prostaglandin synthesis was slightly weaker than that of acetylsalicylic acid (ASA). The antiinflammatory effect of DB-1043 was observed also in adrenalectomized rats and spinalectomized rats.

Pharmacological studies of a new non-steroidal antiinflammatory drug: 2-(5-ethylpyridin-2-yl)benzimidazole (KB-1043).

2-(5-Ethylpyridin-2-yl)benzimidazole (KB-1043) is a new benzimidazole derivative with marked antiinflammatory, analgesic and antipyretic properties. KB-1043 possesses potent inhibitory activities on the acute inflammatory edema induced by chemical and physical irritants in the hind paw of rats, but almost ineffective (similar to tiaramide) against adjuvant-induced polyarthritis in the rat. The antagonistic effects of KB-1043 on increased vascular permeability are more potent than those of phenylbutazone, and the analgesic effects of KB-1043 on the pain induced by chemical and mechanical stimulations were nearly equal to, or slightly less potent than, those of tiaramide and phenylbutazone. The fevers induced by brewer's yeast and polysaccharide from Pseudomonas fluorescens (T.T.G.) were lowered by KB-1043. THe ulcerogenic activity of KB-1043 was weaker than those of tiaramide and phenylbutazone. The therapeutic index of KB-1043 therefore is superior to those of tiaramide and phenylbutazone.

Synthesis and antiinflammatory activity of some 2-(substituted-pyridinyl)benzimidazoles.

A series of 2-(2-pyridinyl)benzimidazoles was synthesized and evaluated for antiinflammatory activity by the carrageenan-induced rat paw edema assay. Among several active derivatives, 2-(5-ethylpyridin-2-yl)benzimidazole (6) was selected for further study. A comparison of compound 6 with phenylbutazone and tiaramide revealed that 6 possesses stronger activity in acute inflammatory models possibly with slightly less gastrointestinal irritation than both phenylbutazone and tiaramide.

[Clinical evaluation of cefoxitin against intractable bronchopulmonary infections. Patients not responding to other conventional antibiotics (author's transl)].

Cefoxitin was administered to a total of 17 patients with intractable bronchopulmonary infections who had failed to respond to other conventional antibiotics, and the following results were obtained. (1) The clinical response, except 1 patient classified as evaluation impossible, was excellent in 3 patients, good in 10 and fair in 3 with an efficacy rate of 81.3%. (2) An improvement rate of more than 70% was observed in the findings of body temperature, dyspnea, colour of sputum, WBC and CRP. (3) There was no subjective nor objective side effects attributable to cefoxitin. In view of the results stated above, we have concluded that cefoxitin is a useful antibiotic for the treatment of intractable bronchopulmonary infections.
