Cor triatriatum dextrum is a rare malformation of the right atrium usually without typical signs or symptoms. Today the intra vitam diagnosis of C.t.d. is done by echocardiography and angiocardiography. Once the diagnosis is established, surgical correction of this anomaly is possible in symptomatic patients. When the presence of an abnormal membrane in the right atrium is not recognized before operation, the outcome may be unsatisfactory.

CT was preoperatively performed in a patient with gall-bladder carcinosarcoma. This very rare mixed tumour consists of epithelial and mesenchymal elements that can also contain cartilage and bone tissue. This appeared in the computer tomogram as an atypical calcification. Infiltration of the right lobe of the liver was also seen.
Changes of various cardiovascular functions and intra-abdominal pressure ratios occur during pregnancy as a result of adaptation processes. Besides the arterial and venous vascular systems, the portal vascular system is also affected. Bearing this in mind, the lienoportal and mesenterioportal vascular system were studied in 87 women during the puerperium after childbirth, and compared with a group of healthy women. Sonographically, the portal vascular system was found to be dilated after pregnancy. These changes were reversible within the first three weeks after delivery. Transient portal hypertension is discussed as a possible cause of this phenomenon. This should be differentiated against acute diseases with pathological and/or already pre-existing portal hypertension.

This study was designed to investigate the impact of postoperative coronary sinus drainage pressure on coronary flow reserve (CFR) assessed by Doppler guidewire in patients long term after Fontan operation. Twenty-nine patients (median age, 17.4 years female, 11) at a median of 10.6 years after Fontan operation were examined with intracoronary Doppler guidewire during cardiac catheterization. Fourteen patients had coronary sinus (CS) drainage to the systemic venous atrium and 15 patients had CS drainage to the pulmonary venous atrium after Fontan operation. Median CS drainage pressure was significantly higher in systemic venous CS drainage compared to pulmonary venous CS drainage (11 vs 5 mmHg, p < 0.0001). Median CFR values for the right and left coronary artery did not differ significantly with respect to CS drainage. There was a positive correlation between coronary flow reserve and pulmonary arteriolar resistance (p < 0.05) in multivariate regression analysis. The site of coronary drainage into the systemic atrium or the pulmonary venous atrium did not significantly affect CFR. Our data do not support a surgical strategy of elective redirection of coronary sinus blood to a low-pressure compartment but support an early staged approach. The positive correlation between CFR and pulmonary resistance demands further evaluation.
30 amino acids and their derivatives could be found in the ejaculates of humans by ion-exchange chromatography. There were significant differences between diabetics and controls in the levels of threonine, serine, glutamine, glycine, iso-leucine, leucine, tyrosine, phenylalanine, histidine and in 3-methyl-histidine. There was a positive correlation between levels of certain amino acids and the number of pathologically motile spermatozoa, particularly microforms and head alterations. An impaired Krebs cycle function may explain an elevation of amino acid levels as they are utilized mainly by this cycle, which function depends on intact substrate flow from glucose catabolism.
