Multi-institutional outcome study on the efficacy of closed-suction drainage of the scrotum in three-piece inflatable penile prosthesis surgery.

Infection is a devastating complication of penile prosthesis surgery that occurs in approximately 2-5% of all primary inflatable penile primary implants in most series. Prevention of hematoma and swelling with closed-suction drains has been shown not to increase infection rate and yield an earlier recovery time. Despite the intuitive advantages of short-term closed-suction drainage in reducing the incidence of postoperative scrotal swelling and associated adverse effects, many urologists are reluctant to drain the scrotum because of a theoretical risk of introducing an infection. In conclusion, this study was undertaken to evaluate the incidence of infection in three-piece penile prosthesis surgery with scrotal closed-suction drainage. A retrospective review of 425 consecutive primary three-piece penile prosthesis implantations was performed at three institutions in New Jersey, Ohio, and Arkansas from 1998 to 2002. Following the prosthesis insertion, 10 French Round Blake (Johnson & Johnson) or, in a few cases, 10 French Jackson Pratt, closed-suction drains were placed in each patient for less than 24 h. All subjects received standard perioperative antibiotic coverage. Average age at implant was 62 y (range 24-92 y). Operative time (incision to skin closure) was less than 60 min in the vast majority of cases. There were a total of 14 (3.3%) infections and three hematomas (0.7%) during an average 18-month follow-up period. In conclusion, this investigation revealed that closed-suction drainage of the scrotum for approximately 12-24 h following three-piece inflatable penile prosthesis surgery does not result in increased infection rate and is associated with a very low incidence of postoperative hematoma formation, swelling, and ecchymosis.

[Relevance of infectious diseases in a pediatric practice].

According to official statistics, infectious diseases only play a minor role in German children. To test this assumption, 1685/3405 charts of a private pediatrician were randomly chosen for further evaluation. 1112/1685 children had been seen in the office during the study period, 934 of them because of an infectious disease (83%). 700/934 charts were further reviewed in a standardized fashion. Pharyngo-tonsillitis was the most frequent diagnosis (18.6%), followed by (non-obstructive) bronchitis (18.5%), infectious diseases of the skin (10.2%) and Otitis media (9.9%). Typical "childhood diseases" (measles, mumps, rubella, varicella) only played a minor role. For some diseases age-specific as well as seasonal changes in incidence could be observed. These data suggest that infectious diseases largely contribute to the morbidity in German children. Prospective epidemiological trials are needed to obtain reliable data for appropriate public health decisions.

Thermotolerance kinetics and growth rate changes in the R1H tumour heated at 43 degrees C.

R1H rhabdomyosarcomas implanted into the foot of the right hind leg of female WAG/Rij rats were exposed to fractionated hyperthermia at 43 degrees C and the kinetics of thermotolerance and heat-induced growth rate changes were studied. Tumours of anaesthetized animals were exposed to heat by immersing the leg up to the thigh in a water bath. Tumour growth delay (TGD) and tumour volume doubling time were calculated from individual growth curves. After single heating, TGD increased with increasing heating time, the increase being linear for heating times exceeding 60 min. Thermotolerance was induced by a priming heat treatment at 43 degrees C for 60 min and the kinetics of development and decay was studied for fractionation intervals ranging from 4 to 144 h. After 4 h the thermal sensitivity of the tumours was enhanced by about 30 per cent, probably due to the sensitizing effect of heat-induced physiological alterations in the tumour tissue such as suboptimal environmental conditions caused by depressed blood flow. For longer time intervals thermotolerance developed and reached a maximum at 24 h where the thermotolerance ratio was 4.5 +/- 1.5. From 24 to 144 h thermotolerance decayed exponentially with a half-time of 28 +/- 8 h. Heat also affected the growth rate of the treated tumours. After single heat treatments at 43 degrees C for 15-60 min the tumours grew faster than untreated control tumours. This change was statistically significant. After prolonged single heating, growth rate was found to be reduced. Tumour volume doubling time was not detectably changed after fractionated heat treatments.

Effect of pH on development and decay of thermotolerance in CHO cells using fractionated heating at 43 degrees C.

The development and decay of thermotolerance at pH 6.7, 7.1 and 7.7 was studied after fractionated hyperthermia at 43 degrees C using exponentially growing CHO cells. The maximum of thermotolerance and the time interval to reach this maximum were found to correlate with the survival decrement after the priming heat treatment. Both parameters were only affected by pH in so far as the pH altered survival after the priming treatment. Decay of thermotolerance was exponential. For a given priming heat treatment for the time t1, the half-time of decay, tau 1/2, increased linearly with increasing cell doubling time, tau d, measured for non-heated cells growing at different pH. On the other hand, for a given cell doubling time, tau d, the half-time, tau 1/2, increased exponentially with increasing duration of the priming heat treatment, t1. For all measured data the half-time of thermotolerance decay could be described by the equation tau 1/2 = alpha. tau d.exp(k.t1), with k = 2.2 +/- 0.2 h-1 and alpha = 0.094 +/- 0.009 for all pretreatments applied and all pH conditions tested. This relationship might indicate that the decay of thermotolerance is governed by a single mechanism.

Step-down heating of CHO cells at 37.5-39 degrees C.

The thermosensitizing effect of step-down heating was studied using Chinese hamster ovary (CHO) cells. Exponentially growing cells were given priming heat treatments at 43 degrees C for 45 or 90 min, immediately followed by a second exposure to a temperature ranging from 37.5 to 39 degrees C. The measured rates of cell killing, 1/D0, increased exponentially with temperature; the slopes correspond to Arrhenius activation energies of Ea = 1200 +/- 150 kJ mol-1 and Ea = 1275 +/- 125 kJ mol-1 for cells preheated at 43 degrees C for 45 or 90 min, respectively. For the temperature range 39-43 degrees C an activation energy of Ea = 561 +/- 24 kJ mol-1 was obtained for step-down heated cells (43 degrees C, 45 min followed by T = 39-43 degrees C). These results indicate that there is a 'second inflection point' at 39 degrees C on the Arrhenius curve for step-down heating of CHO cells. Data evaluation using a mathematical model published previously (H. Jung, Radiation Research, 106, 56-72, 1986) showed that the rate constant c for the conversion of nonlethal lesions into lethal events increased with an activation energy of Ea = 1520 +/- 140 kJ mol-1 in the temperature range from 37.5-39 degrees C. For 39-45 degrees C the activation energy for c was Ea = 360 +/- 26 kJ mol-1, indicating that the temperature dependence of c shows a break at 39 degrees C similar to that observed on the 1/D0 Arrhenius plot.

Effect of chronically induced thermotolerance on thermosensitization in CHO cells.

Chronic thermotolerance was induced in Chinese hamster ovary (CHO) cells by pretreatment at 40 degrees C for various times ranging from 15 min to 16 h. The thermotolerant cells were either exposed to single heat treatments at 43 degrees C or subjected to step-down heating consisting of a priming treatment at 43 degrees C for 90 min immediately followed by a graded test treatment at 40 degrees C. The results showed that chronic thermotolerance affected the thermal sensitivity of step-down-heated CHO cells in two ways: by lowering the effectiveness of the priming treatment at 43 degrees C and by reducing the response to the test treatment at 40 degrees C. The effect on the priming treatment corresponds to a reduction in the effective heating time, i.e. the thermotolerant cells respond as if they were exposed to 43 degrees C for times shorter than 90 min. It was further shown that, for a given conditioning treatment, the effectiveness of both the priming and the test treatment was reduced by the same factor; the thermotolerance ratios determined for 43 degrees C and 40 degrees C showed an identical dependence on the duration of the thermotolerance-inducing conditioning treatment. Since thermotolerance development did not reverse heat sensitization by step-down heating, it is concluded that thermotolerance and thermosensitization are distinct phenomena which act independently.

Effect of chronic thermotolerance on thermosensitization in Chinese hamster ovary cells studied at various temperatures.

The effect of chronic thermotolerance on the thermal responses of Chinese hamster ovary (CHO) cells to single and step-down heating was studied. Thermotolerance was induced by pre-heating exponentially growing cells at 39 degrees C for 9 h, followed by test treatments for variable times at temperatures ranging from 39 to 43 degrees C. In the temperature range studied, the heat sensitivity of thermotolerant CHO cells was characterized by an Arrhenius activation energy of Ea = 1175 +/- 40 kJ/mol. This value agreed well with Ea = 1180 +/- 45 kJ/mol measured after single heating, indicating that the induction of chronic thermotolerance did not affect the activation energy for cell killing by heat. Thermosensitization was studied after a priming treatment at 43 degrees C for 50 min followed by step-down heating at temperatures ranging from 39 to 43 degrees C. The temperature dependence of the thermal response after step-down heating was characterized by an activation energy of Ea = 490 +/- 17 kJ/mol. When the cells were pre-treated for 1-16 h at 39 degrees C prior to step-down heating (43 degrees C, 50 min, followed by graded exposure to 39-43 degrees C), the activation energy was gradually enhanced and approached Ea = 825 +/- 42 kJ/mol for 39 degrees C, 16 h. This change in Ea reflects the effect of thermotolerance on the priming treatment at 43 degrees C for 50 min, whereas the effect on the final test treatment resulted in a parallel shift of the Arrhenius curve without changing the slope, indicating that the effect of thermotolerance on the priming and the test treatment is expressed in the Arrhenius diagram in different ways.

Surface roughness of polished amalgams.

The purpose of this project was to determine the amalgam surface roughness following finishing and polishing procedures. Nine high-copper amalgams were tested. They were hand-condensed in a 3 x 3 x 30 mm mold. Each sample was divided into four sections, and each section was finished by one of four methods: carving only; carving and then burnishing 15 minutes after carving; carving and then polishing at the prescribed times; or carving, burnishing 15 minutes after carving, and then polishing at the prescribed times. Each bar of amalgam had these surface treatments done at either 1, 3, 6, or 24 hours. The results indicated surface roughness may be more dependent on particle size, shape, and distribution of the individual high-copper amalgams than the polishing time or the surface treatment performed.

[A method to quantify diazepam with high resolution liquid chromatography in patients undergoing multiple therapy].

A rapid, sensitive and specific method useful in monitoring diazepam in patients receiving multiple therapy is presented. Plasma was extracted in the presence of borate buffer with hexane and the drug was quantified by HPLC in a Novapak C 18 column. The limit of detection in plasma was about 30 ng/mL. Analytical recovery of the drug added to plasma was about 70%. Within-day variation and between-day variation ranged from 5.9 to 8.3 and 5.9 to 10.7%, respectively. The method was applied in the determination of diazepam levels in patients treated with this drug in combination with other drugs commonly used in our hospital. The concentrations found in most of the patients were in the therapeutic range. It was not possible to detect the drug in one patient with concomitant administration of magnesium hydroxide, which can be attributed to a reduced absorption of diazepam in presence of the antiacid. Average steady-state concentrations of diazepam in psychiatric patients, submitted to 5 mg doses t.i.d. during four weeks, were in the range of 200 to 450 ng/mL.

Sterile versus non-sterile dialysis fluid in chronic hemodialysis treatment.

As the quality of water in the dialysis fluid varies considerably, and in view of the fact that endotoxin or active derivatives can cause acute side effects in patients, the dialysis fluid must be sterile. Therefore, we introduced ultrafiltration of dialysis fluid before entering the dialyzer. Fifteen patients, (ten women, five men) were treated for 4 weeks with nonsterile, and then with sterile dialysis fluid. The bacterial loading in the dialysis fluid before hemodialysis was 1.34 X 10(5)/ml, and after hemodialysis 2.9 X 10(3)/ml; the endotoxin concentration was high and varied between less than 1 EU/ml and greater than 10 EU/ml before and after hemodialysis. After ultrafiltration of the dialysis fluid by a polyamide hollow fiber membrane, all samples were free of bacteria and the concentration of endotoxin was lower than the detectable limit (less than 0.03 and less than 0.5 EU/ml). With ultrafiltration of dialysis fluid we can obtain sterile dialysate, which is endotoxin free. Interleukin-1 and tumor necrosis factor in the patients with ultrafiltration was significantly lower than without ultrafiltration.

[Sampling of the chorionic villi: transcervical fine-needle and endoscopic aspiration].

In 63 cases chorionic villi sampling has been performed under complete anesthesia just before legal abortion; nine of them were endoscopic transcervical and 54 transcervical by means of a catheter under direct ultrasonic control. 81% offered useful chorionic villi, 17.4% only decidual material. The last 21 aspirations by means of a catheter between the 8th and the 12th week of gestation (p.m.) caused no problem at all. From the first punction on useful chorionic material has been obtained.

[Effect of atypical pathogen colonization on cervical priming in cervix insufficiency].

In a prospective study the cervical bacterial flora of pregnant women with insufficiency of the cervix is compared with the flora of asymptomatic pregnant women. It could be demonstrated, that in case of insufficiency of the cervix a different bacterial flora is found: in addition to the incidence of pathological bacterial groups, a shift of the physiological flora with Doederlein's bacilli to a mixed flora is observed. New findings in the cervical priming lead to the idea, that an atypical cervical flora could influence the cervical priming. The changing of the cervical environment is able to induce an increased production of prostaglandins with cervical dilatation of its structure. Regarding the different cervical flora in case of cervical insufficiency, the importance of the circular suture in the prophylactic management of premature delivery will be discussed. The results lead to the necessity of precise vaginal check-up and therapy of genital infections during pregnancy.

[Initial studies of 3-dimensional imaging using ultrasound].

In the study reported here three-dimensional sonographic imaging of organs was achieved for the first time. To make this possible it was first necessary to ensure, by appropriate guidance of the probe, that the sequence of sonographic sections was coordinated in their spatial arrangement. This was accomplished by constructing a probe guide with which parallel sonographic sections could be demonstrated. The distance between these sections was known, so that with the aid of suitable computer programs three-dimensional reconstruction of a kidney examined in a water bath was possible. Since, however, due to the uneven surface of the body, it will hardly be possible to obtain parallel sonographic sections of an organ, a new solution had to be found to ensure the necessary coordinated sequence of sections. The solution lay in rotating the probe. A further device was constructed in which the probe could be rotated farther, by known angles, from section to section. The pivotal point was at the center of the probe tip. The computer knew the angular distance between these sections and reconstruction to a three-dimensional image was therefore possible. Prior the three-dimensional reconstruction the ultrasonographic sections had to be contoured, since only the surface of the organ was available for three-dimensional image construction. Three-dimensional imaging of an organ can be achieved on the one hand by binary representation and on the other with a continuous organ surface. The advantage of binary representation is that the original sonographic data are incorporated in the image-producing process without any computer manipulation; with a continuous surface the distance between the individual sections has to be interpolated.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical experiences with intracervical prostaglandin E2 administration for labor induction].

From January 1, 1986 to December 31, 1987, 119 patients among a total number of 2309 were recorded prospectively, who needed induction of labour for mixed indications (Group A), for a premature rupture of the membrane (Group B) or for medical indication (Group C). Independently of the ripe of cervix the results were significantly better in the multiparae than nulliparae respecting the success of induction, the duration of labour and the rate of Caesarean sections. 66.4% (n = 79) of all cases could be delivered within 24 hours after the first gel-application. Statistically significant differences in the partial collectives A, B, and C were seen only by unripe cervix (A = 60.0%, B = 71.4%, C = 30.0%). Repeated tries of inductions led to statistically significant more rates of Caesarean sections, fever during labour and prolonged course of labour. The rate of Caesarean sections and the fetal post-partum results comparing to the other remaining deliveries in 1986/87 were statistically not different. Side effects induced by Prostaglandins were only founded in two cases concerning nausea. Uterine hyperactivities with pathological CTG-patterns being resistant to treatment with beta-mimetics were found in one case. As severe complication the only case with particular rupture of uterus must be seen. Based on our experience the intracervical PGE2-application for induction of labour is not disadvantageous for mother and fetus, if there are certain indications and fixed criteria.

[Changes in erythrocyte P50 and its effect on O2 transport in intravenous tocolysis with fenoterol (Partusisten)].

Fenoterol (Partusisten), which has a considerable influence on glucose metabolism in addition to its well-known cardiac side-effects, does not appear to have any direct influence on the P50 of the maternal erythrocytes in in-vitro experiments. In incubation experiments with fresh erythrocytes, fluctuations in the P50 with an amplitude of 2-5 mmHg were measured at +37 degrees C; these fluctuations depend on the storage age of the erythrocytes and the glucose content of the incubation medium. Fenoterol has no effect on the amplitude, nor on the frequency of this P50 fluctuation. The glycolytic activity of the erythrocytes, relative to the 2,3-DPG and ATP content, also remains uninfluenced under Fenoterol. The addition of progesterone or cortisone to the incubation medium has no influence on the P50 of the maternal erythrocytes. In-vivo studies of the blood of pregnant women indicate changes of the P50 with maximal "shift to the left" after 6 hours and compensatory "shift to the right" after 20 hours under tokolysis. No fluctuations of the pH value were observed in our patients. The intraerythrocytic 2,3-DPG concentration changes simultaneously with the P50. Venous pO2 increases by approx. 12 mmHg in the first 3 hours following the start of tokolysis. However, the changes in the venous pO2 are not simultaneous with the changes in the P50. It can be stated that the progesterone level has no influence on the release of O2 from the maternal blood.(ABSTRACT TRUNCATED AT 250 WORDS)
