Histological evaluation of the nodules resected in the treatment of pulmonary metastatic disease.

During an 18 month period, 41 patients who had previously had a primary tumor removed, underwent 60 operations for resection of newly detected radiographic pulmonary nodules. Primary tumors included 16 osteogenic sarcomas, 9 malignant melanomas, 12 soft tissue sarcomas, 1 Ewing's sarcoma, 2 adenocarcinomas of the rectum, and 1 testicular teratocarcinoma. One hundred and eight-one pulmonary specimens were submitted for pathologic examination and 149 discrete nodules were found in these specimens by the pathologist (83%). Metastases represented 76-100% of nodules removed in 68% of the operations, 51-75% of nodules in 12%, 26-50% of nodules in 14%, and less than 25% of nodules in 6% of the cases. Metastatic disease was found in 90% of cases where nodules were greater than or equal of 0.5 cm in diameter, while metastases were found in on 33% of cases where the largest nodules removed were less than 0.5 cm. In only two cases did the histology of the pulmonary metastases differ from that of the primary tumor. We conclude that although benign lesions cannot be distinguished from metastases except by resection, nodules less than 0.5 cm in diameter probably should be observed for a further increase in size prior to operation.

Rapid adrenocortical recovery after short-term glucocorticoid therapy.

The recovery time of adrenocortical function was studied in 13 children receiving intermittent high-dosage glucocorticoid therapy (prednisone, 2 mg/kg/day) for one month out of every four months as treatment for acute leukemia. The data indicate that the adrenal gland's ability to respond to stress recovers rapidly from such treatment. The results of our study clearly show that routine supplementation with glucocorticoid therapy in these patients during periods of clinical stress occurring later than nine days after glucocorticoid therapy is not warranted.

Leiomyosarcoma of the stomach: results of surgery and chemotherapy in an eleven-year-old girl with liver metastases.

This report describes an 11-7/12-year-old girl with leiomyosarcoma of the stomach metastatic to liver who was treated with surgical resection and combination chemotherapy, which included adriamycin. She remains well 52 months after diagnosis and 27 months after cessation of chemotherapy. The favorable course of this patient suggests that aggressive adjuvant chemotherapy should be considered for patients with leiomyosarcoma who have poor prognostic features.

Serum beta 2 microglobulin levels in patients with testicular cancer.

To determine whether serum beta 2 microglobulin could be useful as a tumor marker in patients with testicular cancers, sera stored in our serum bank from 23 patients were assayed by radioimmunoassay for beta 2 microglobulin levels. Sera were selected from 12 preorchiectomy patients, 8 patients who had undergone orchiectomy, and 3 patients who had undergone orchiectomy and retroperitoneal lymphadenectomy. Patients with other genitourinary tract disorders and normal age and sex-matched controls were assayed for comparison. The laboratory range for normal serum beta 2 microglobulin values was 1.1 to 2.4 mg/L. Patients without documented testicular tumors had values ranging from 1.4 to 9.7 mg/L. Values for normal controls ranged from 1.7 to 2.4 mg/L. Only 3 patients (12.5 per cent) had elevated serum beta 2 microglobulin values. Serum beta 2 microglobulin seems to have no value as a tumor marker in patients with testicular tumors.

The effectiveness of pediatric nurse associates as clinical instructors of medical students.

Pediatric nurse associates (PNAs) have provided health care services for children, taught physical diagnosis to first-year medical students, and acted as role models for students and residents. Their role as clinical instructors has not been measured objectively. This study evaluated the effect of PNA interaction with medical students in a pediatric clerkship. Students taught by PNAs had higher gains in factual knowledge and greater skills in physical examination as measured by a videotape of a patient encounter. The PNAs demonstrated that they can be effective instructors of medical students.

4-Aminoimidazole-5-carboxamide excretion in acute leukemia.

The urinary excretion of 4-aminimidazole-5-carboxamide (AIC) as been reported to be increased in children with acute leukemia and has been correlated with disease status. Using a modification of the method of Skibba et al [5], determinations were made on urine from 26 children with acute leukemia. The urine from ten normal children served as controls. The effect of chemotherapy on urinary AIC was studied comparing patients on vincristine and prednisone (V+P) with those on 6-mercaptopurine, methotrexate, and cyclophosphamide (Triple Rx). Patients in remission on Triple Rx had lower levels of urinary AIC than did patients on Triple Rx in relapse or patients on V+P in either remission or relapse. Twenty patients had sequential measurements. Values for individual patients were not predictive of disease status. One such patient is described. This study demonstrates that chemotherapy, as well as disease status, affects the urinary excretion of AIC in children with acute leukemia.
