[Analysis of 24-hour ambulatory EEG monitoring in 315 children with paroxysmal diseases].

The 24-hour ambulatory electroencephalography monitoring (AEEG) of 315 children with various paroxysmal diseases were monitored. Among them 224 cases (71.11%) were lightly abnormal including 142 cases (45.08%) recorded as epilepsy burst; the absence seizures and convulsions were the highest positive rates in these patients who have been recorded epilepsy burst. We found that AEEG was more accurate to get abnormal burst wave or epilepsy burst wave than EEG. These data may indicate that AEEG is more accurate in children with paroxysmal diseases.

[Complications of osteotomy of vertebral arch and body in kyphosis].

Among total 67 postoperative complications resulted from osteotomy of vertebral arch and body in 439 cases of kyphosis, massive bleeding accounted for 2 cases (0.46%); spondylolisthesis, 6 (1.37%); paraplegia caused by compressing of the spinal cord, 5(1.14%); root pain, 6(1.37%); loss of correction, 26(29.55%); leaking and subcutaneous accumulation of CST, 5(1.14%); incision necrosis, 16(3.64%); lifting of luque rod with peneatration through the skin, 1(0.23%). Causes, treatment and prevention of these complications have been discussed in this study. We emphasize that the operation should be done after the stable of ankylosing spondylcitis. The degree of correction by osteotomy at one-level should be limited.

[The mutual benefit role of pancreas and liver in combined transplantation].

We study observed the mutual benefits of pancreas and liver in combined hepaticopancreatic transplantation in rats. The result indicated that pancreas, when transplanted with liver, could survive for a significantly longer time (13.4 +/- 1.01 days) than it transplanted alone (9.2 +/- 1.14 days) (P < 0.05). The interstitial rejection was mild and its rejection grade was significantly different from that of pancreas transplanted alone (P < 0.05, chi 2 test). The liver, when transplanted with pancreas, regenerated with strong competence and contact structure morphologically compared with liver transplanted alone. We concluded that pancreas could be immunologically protected against rejection and liver can be nutritionalized by pancreas in combined pancreas and liver transplantation.

[Effect of white noise on activity and gene expression of nitric oxide synthase in cochlear nuclei of guinea pigs].

NADPH-diaphorase (NADPH-d) histochemistry method and semiquantitative polymerase chain reaction (PCR) technology were adopted to study nitric oxide synthase (NOS) neurons and to investigate the content change of NOS mRNA and the relationship between it and auditory threshold and to explore the possible role of NOS neurons in cochlear nuclei of guinea pigs after exposed to white noise. The results showed that the quantity and staining intensity of positive NOS neurons in cochlear nuclei increased obviously after exposed to noise. It reached the peak 2 week later and remained 5 week than normal. The regulation of NOS mRNA was consistent with that of morphological observation. NOS mRNA content in 2 w group was the highest and was 4.02 times higher than normal. After exposed to noise, ABR threshold shift was in positive correlation with NOS mRNA content in cochlear nuclei (r = 0.9655, P < 0.01). It is suggested that positive NOS neurons in cochlear nuclei might be involved in controlling injury and repair of cochlear nerves. The high expression of NOS gene might be an important factor in pathogenic mechanism of noise deafness.

[Experimental study on the effect of nitric oxide on cochleas of guinea pigs].

The effects of nitric oxide (NO), 1-monoarginine (L-Arg), sodium nitroprusside (SNP) and N-methyl-1-monoarginine (NMLA) (which is an antagonist of nitric oxide synthase) on endocochlear potential, compound action potential(CAP) and cochlear microphonic(CM) of cochleas were studied by means of perilymphatic perfusion of the agents. The results showed that NMLA reduced EP, CAP and CM, L-Arg perilymphatic perfusion could reverse the change caused by NMLA. Continuous NO perilymphatic release restored the changes of EP, CAP and CM back to normal, sometimes above normal, followed by rapid reduction. After SNP perilymphatic perfusion, EP, CAP and CM went up transiently and then declined gradually and remained at a lower level. Addition of NMLA did not reverse the changes. The change of the latent period of CAP N1 and drift of CM was consistent with their amplitude changes. It was suggested that NO might help maintaining inner ear function under physiological condition and participate in controlling the micromechanical character and sensitivity of cochlear hair cells, undue expression of NO may cause toxicity to cochleas.

En bloc transplation of kidney and whole pancreas with a segment of duodenum in rats.

For meeting the clinic needs in simultaneous pancreas and kidney transplantation (SPK), we successfully establish a syngeneic SPK transplatation model in Lewis rats. The results indicate that this model is feasible with a 82.6% successful rate of operation and a 69.6% survival rate in the first postoperative week. In long-term survived rats, the blood supplies are well established, function of the grafts (pancreas and kidney) maintains normal. This model is suitable for theoretical research in SPK transplantation for its reasonable physiology with pancreatic juice drained into intestine and reduced postoperative complications in urinary tract and carbohydrate metabolism.

[The experimental observation on characteristics of soft tissues infection in maxillofacial region wounded by high velocity missile].

In this experimental study, maxillofacial regions of dogs were wounded by steel spheres, which weight 1.03 g at a mean primary velocity of 1500 m/s. Aerobe and anaerobe bacteria in the muscles of different interval from wound track edge were cultured at different time after wound. The results showed that there was bacteria infection in the soft tissues between 0 and 0.5 cm from wound track edge in 6 hours after wound. The quantity of bacteria obviously increased following time prolonging, on the contrary, it was much lower than threshold value of bacteria infection within 24 hours. The experimental results suggested that there was obvious bacteria infection in the soft tissues within 0.5 cm distance from wound track edge in maxillofacial firearm wound region. During the debridement removing soft tissues within 0.5 cm distance from wound track edge, the infection of wound region could be controlled. Using effective antibiotics is an important measure of controlling infection of wound after debridement.

[Studies on the antilipid peroxidation of nine sorts of Chinese herbal medicines with the function of protecting liver].

The antilipid peroxidation of nine sorts of Chinese herbal medicines with the function of protecting liver, including Salivia miltiorrhiza, Hypericum japonicum, Scutellaria baicalensis, Callicarpa cathayana, Chrysanthemum indicum, Paeonia latiflora, Lysimachia christinae, Ligustrum lucidum (L1), Patrinia villosa (Pv) on hepatic homogenate of rat were tested. It was found that all tested medicines showed inhibition with dose-effect relationship, the inhibitory rate of L1 and Pv were lower than the other's. All results showed that these Chinese herbal medicines have strong antilipid peroxidation and are natural oxidation inhibitor. It was suggested that their function of protecting liver and others have relationship with the antioxidation.

[Study on the distribution and structure of polyhalides by gas chromatography-mass spectrometry].

A series of unexpected products were obtained when 1,4-dichloro-2-butyne reacted with allyl bromide with the catalyst PdCl2 (PhCN)2. It is difficult to separate these products from each other and to identify the structure of each one due to their similar properties. In order to solve this problem, gas chromatography-mass spectrometry (GC-MS) was chosen to analyse the above products because of its good sensitivity and selectivity. The analytical results showed that (A) the 8 compounds were 5,6-dichloro-4-chloromethyl-1, (E) 4-hexadiene (III), 5,6-dichloro-4-chloromethyl-1, (Z) 4-hexadiene (IV), 5-bromo-6-chloro-4-chloromethyl-1, (E) 4-hexadiene (V), 5-bromo-6-chloro-4-chloromethyl-1, (Z) 4-hexadiene (VI), 5-bromo-6-chloro-4-bromomethyl-1, (E) 4-hexadiene (VII), 5-bromo-6-chloro-4-bromomethyl-1, (Z) 4-hexadiene (VIII), 5,6-dibromo-4-bromomethyl-1, (E) 4-hexadiene (IX) and 5,6-dibromo-4-bromomethyl-1, (E) 4-hexadiene (X). Most compounds, except (V) and (VI), were the results of exchange reaction between chloride and bromide anion; each pair of the products are geometrical isomers. (B) in the four pairs of geometrical isomers, the ratio of Z-isomer/E-isomer decreased from 95:5 to 53:47 as molecular weight increased from compounds (III) and (IV) to compounds (IX) and (X).

Heterozygous P0 knockout mice develop a peripheral neuropathy that resembles chronic inflammatory demyelinating polyneuropathy (CIDP).

Demyelinating peripheral neuropathies are clinically divided into inherited and acquired types. Inherited demyelinating neuropathies are caused by mutations in genes expressed by myelinating Schwann cells, whereas acquired ones, including chronic inflammatory demyelinating polyneuropathy (CIDP), are probably caused by autoimmune mechanisms. We find that heterozygous P0 knockout (P0+/-) mice develop a neuropathy that resembles CIDP. By one year of age, P0+/- mice develop severe, asymmetric slowing of motor nerves, with temporal dispersion or conduction block, which are features of acquired demyelinating neuropathies including CIDP. Moreover, morphological analysis of affected nerves reveals severe and selective demyelination of motor fibers, focal regions of demyelination, and inflammatory cells. These data suggest that immune-mediated mechanisms may contribute to the pathogenesis of the neuropathy in P0+/- mice.

Identifying and recruiting healthy control subjects from a managed care organization: a methodology for molecular epidemiological case-control studies of cancer.

Case-control studies with stringent matching criteria require large pools of healthy subjects from which to select matched controls. This paper describes a successful method of identifying a large pool of potential control subjects to participate in two molecular epidemiological case-control studies of lung cancer, each enrolling 400 case subjects and 400 control subjects. These studies are not population based, and the study base is not well-defined. Therefore, potential control subjects are being identified and recruited through 20 area clinic sites of a large multispecialty health maintenance organization. Because the research focus is driven by genetic hypotheses and we are controlling for multiple smoking-related variables, representativeness is of lesser concern. To identify potential control subjects, a one-page questionnaire is distributed to patients in the waiting room to assess contact information as well as data relevant to the case-control matching process. An average of 2,228 questionnaires are returned monthly toward a target pool of 40,000; of these, 59% of the respondents fulfill eligibility criteria as a control subject for one of the studies and are not averse to being contacted in the future for the purpose of research. When compared to former smokers and never smokers, current smokers in the control population were least likely to refuse further contact. A collaborative arrangement with a managed care organization offers a feasible mechanism through which researchers can access a large, ethnically diverse population of potential control subjects.

Destruction of hematopoietic microenvironment by cytotoxic T cells.

Coculture of cytotoxic T cells (STIL-3 C5) derived from L8313 leukemic mice with hematopoietic supportive stromal cells (MS-5) resulted in the detachment of MS-5 cells from the culture dish, whereas helper T cells (STIL-3 DF) did not induce this detachment. The response of bone marrow (BM) adherent cells to the same treatment was similar to that of MS-5 cells. The detached cells were unable to proliferate further, and genomic DNA of these cells showed fragmentation, suggesting that hematopoietic stromal cells died of apoptosis. Reverse transcriptase-polymerase chain reaction (RT-PCR) analysis revealed that STIL-3 C5 cells, but not STIL-3 DF cells expressed perforin, granzyme A & B, and Fas ligand. Fas was expressed in MS-5, BM adherent cells, MS-K and NIH/3T3 cells, which do not support hematopoiesis. These data suggest that the aforementioned factors mediate induction of apoptosis in MS-5 cells induced by direct cell-to-cell interaction with STIL-3 C5. This may explain the mechanism responsible for the destruction of the hematopoietic microenvironment by cytotoxic T cells in L8313 leukemia, from which STIL-3 cells are derived; it also suggests that destruction of hematopoietic tissue may be caused by leukemic cytotoxic T cells in some cases of leukemia.

Location of language in the cortex: a comparison between functional MR imaging and electrocortical stimulation.

PURPOSE: To determine the accuracy of functional MR imaging in locating language areas for planning surgical resection. METHODS: Intraoperative photographs were digitized and overlaid on functional MR language maps. The sensitivity and specificity of functional MR imaging for identifying language areas were determined for five different language tasks by comparing functional MR areas of language activation with results of electrocortical stimulation. A match was considered to occur if an activated area contacted overlapped, or surrounded a language tag. The borders of the activation areas were extended by 1 and 2 cm to determine whether the number of matches changed. Language and nonlanguage tag matches were tabulated separately. RESULTS: Sensitivity/specificity for all patients and all language tasks ranged from 81%/53% for areas that touched to 92%/0% for areas separated by 2 cm. Individual language tasks were not as sensitive as a battery of language tasks combined. Location of language areas varied among subjects for a given task and among tasks for a given subject. CONCLUSION: Functional MR imaging should be considered a useful presurgical planning tool for mapping cortical language areas, because it is sensitive, it provides increased time for planning before surgery, and it is noninvasive.

Correlates of mutagen sensitivity in patients with upper aerodigestive tract cancer.

Although tobacco and alcohol use are the major determinants of upper aerodigestive tract carcinogenesis, not all smokers develop cancer. This phenomenon is due to individual variation in genetic susceptibility to carcinogens. One explanation may be differences in mutagen sensitivity (as measured by the in vitro bleomycin-induced mutagen sensitivity assay) in patients with squamous cell carcinoma of the upper aerodigestive tract. Antioxidant supplementation has also been shown to decrease DNA damage and thus may also inhibit carcinogenesis. In this study, we examined whether smoking, alcohol intake, and dietary antioxidant intake were correlated with mutagen sensitivity. The 612 patients evaluated are part of an ongoing multicenter Phase III trial of 13-cis retinoic acid for the prevention of second primary tumors. We found that patients with pharyngeal cancers were more likely than patients with oral cavity or larynx cancers to be mutagen sensitive. There were no significant differences in the distribution of mutagen sensitivity by sex or alcohol use. Never smokers were significantly more likely (61.1%) to be mutagen sensitive than current smokers (35.6%). Dietary consumption of the micronutrients alpha-carotene, beta-carotene, lutein, lycopene, and vitamin C was not correlated with mutagen sensitivity. Therefore, we suggest that mutagen sensitivity is an independent marker of cancer risk not affected by other known risk factors.

Velocity of translation of single actin filaments (AF) by myosin heads from antigen-sensitized airway smooth muscle.

We have previously reported increased velocity of shortening (Vo) in the sensitized airway (0.36 1o/s, +/- SE) smooth muscle compared to the control (0.26 1o/s, +/- 0.017 SE) and subsequent experiments indicated this was due to increased phosphorylation of the 20 kDa myosin light chain resulting from increased total myosin light chain kinase activity. The motility assay technique described by Kron and Spudich was employed to determine whether additionally the molecular motor (actomyosin crossbridge) itself was altered in airway smooth muscle by ragweed pollen sensitization. The motility assay measures the velocity of actin filament translation by myosin molecules. The negative results of the motility assay were valuable in determining that the pathogenesis of allergic bronchospasm is not at contractile protein level but at regulatory enzyme level.

Clinical significance of the immunohistochemical detection of p53 protein in endoscopic biopsy specimens.

OBJECTIVE: To evaluate the utility of immunohistochemical staining of p53 expression in endoscopic biopsy specimens in clinical practice. METHODS: Using a streptavidin-peroxidase (S-P) conjugate method with monoclonal p53 antibody Do-7, the expression of p53 was investigated immunohistochemically in 150 endoscopic biopsies and paired brushings from patients with benign and malignant gastric diseases. Among them, 22 cases with gastric carcinoma underwent the gastrectomy and the immunohistochemistry was repeated in the resected cancerous tissues of the same patient. RESULTS: p53 expression in endoscopic specimens was detectable in 35.7% cases of gastric carcinoma (n = 70), 5.9% of gastric polyps (n = 17), 16.7% of gastric mucosal dysplasia (n = 18) and 4.0% of chronic atrophic gastritis (n = 25), but no protein expression was observed in cases of chronic active gastritis (n = 20). In 22 patients with tumor resection, 8 (36.4%) were identified with p53 expression in endoscopic specimens before surgery, while 10 (45.5%) were identified with the same expression in surgical specimens, the real positive rate was 80.0%, no false positives were observed in endoscopic specimens. Furthermore, p53 expression in gastric carcinoma was closely correlated with clinical stage and lymph nodes involvement, but did not correlate with histologic type. CONCLUSIONS: Assay of p53 by immunohistochemistry in endoscopic biopsy specimens and brushings is an easy and reliable technique to assess p53 status in gastric carcinoma, and thus could serve as a marker to foresee the prognosis in these patients and assist in the diagnosis of malignancy before surgery. It may also be a valuable marker in screening patients with high risk of gastric carcinoma.
