CT findings in congenital adrenal hyperplasia due to 11 beta hydroxylase deficiency at puberty age.

The CT findings of a case of congenital adrenal hyperplasia due to 11 beta hydroxylase deficiency at the age of 17 is presented. CT showed marked bilateral symmetric adrenal enlargement with smooth borders. In upper parts of the enlarged adrenals the contours were preserved, but the lower parts of the adrenals appeared ball-like. The adrenals were homogeneous in density before contrast, after administration of iodinated contrast agent the glands were markedly enhanced with obvious vasculature observed. These CT findings were pathognomonic of congenital adrenal hyperplasia, though not specific for the type of 11 beta hydroxylase deficiency.

Studies on plasma endothelin changes in varicocele patients.

AIM: To investigate the possible role endothelin may play in the pathogenesis of varicocele and varicocele-induced infertility. METHODS: In varicocele patients, radioimmunoassay is used to determine the blood endothelin concentration in the spermatic vein and the antecubital vein separately. RESULTS: The blood endothelin concentration of the spermatic vein (66.8 +/- 23.5 pg/mL) was significantly higher (P < 0.05) than that of the antecubital vein (55.3 +/- 25.3 pg/mL). The endothelin concentration in the spermatic vein in Grade II varicocele patients was not significantly different from that in Grade III patients (P = 0.87). CONCLUSION: The higher blood endothelin level in the spermatic vein may indicate the involvement of endothelin in the pathophysiology of varicocele and varicocele-induced infertility.

[An introductory clinical trial of three-monthly injectable contraceptive-depot medroxyprogesterone acetate].

OBJECTIVE: To evaluate contraceptive efficacy, continuative rate of use, side effects and acceptability of depot medroxyprogesterone acetate (DMPA) injectable contraception preparation in Chinese women. METHOD: This was an open study. One thousand nine hundred and eighty-five women acquiring contraception received DMPA injection after obtaining informed consent and were followed-up every 3-month up to one year. RESULTS: The total experience accumulated was 1,731.7 women-year with DMPA for 1,985 users. The one year continuation rate was 77.4%. Three subjects became pregnant, the one year accumulative life table failure rate was 0.2%. The accumulative discontinuation rates at one year for other reasons were amenorrhoea (4.8%), bleeding related (13.4%), other medical reasons (2.6%), other personal reasons (2.1%) and lost to follow-up (1.6%). The main complaints were irregular bleeding, amenorrhoea and prolonged bleeding. Lactating women had significantly lower rate of complaint. CONCLUSION: DMPA is a highly effective long-acting contraceptive, it could have good acceptability, especially for lactating women, under the well consultation.

[Diagnostic value of human cytomegalovirus late-mRNA detection for intrauterine active human cytomegalovirus infection].

OBJECTIVE: To study diagnostic value of late-mRNA detection for intrauterine active human cytomegalovirus (HCMV) infection. METHODS: The HCMV late-mRNA in peripheral blood of the pregnant women with HCMV-IgM positive and their fetal appendages (such as chorionic villi, amniotic fluid, umbilical blood and placenta) were detected by reverse transcription polymerase chain reaction(RT-PCR). RESULTS: The late-mRNA was detected in 23 out of 42 HCMV-IgM positive pregnant cases, with the 54.3% of corresponding rate between the results of HCMV-IgM and HCMV late-mRNA. 7 samples out of 13 fetal appendages from m-RNA positive mothers were late-mRNA positive, while in 12 mothers with late-mRNA negative group only one fetal sample was late-mRNA positive, Between two groups there was significant difference in mother-fetus transmission rate. CONCLUSIONS: The positive result of HCMV-IgM can not accurately reflect HCMV activity at the time being detected, which is closely related to the mother-fetus transmission rate. As a symbol of active HCMV infection, late-mRNA can not only reflect the mother-fetus transmission rate during active HCMV infection correctly, but also provide some information about activity of the HCMV in fetal tissue, It also has a great value in prediction for prognosis of infectious fetuses.

[The application of external skin expansion in the soft tissue donor site].

OBJECTIVE: To overcome the insufficiency of donor tissue for repairing a large postburn defect. METHODS: The donor site was pre-expanded with an external expansion device in 54 clinical cases. RESULTS: The expansion process was uneventful. The external expansion had delay effect that enabled the flap to be transferred with greater length to width ratio. After external expansion, the donor site of a flap or skin graft could be closed directly. The recipient and donor sites healed simultaneously. CONCLUSION: External expansion is a practical method to provide 'extra' skin for reconstruction of a large skin defect.

[Quantitative study of vascular endothelial growth factor expression in primary lung cancer].

OBJECTIVE: To study the expression levels of vascular endothelial growth factor (VEGF) and its relation with intratumoral microvessel density (MD), cellular proliferation activity (PI), and lymph node metastasis of primary lung cancer. METHODS: Surgical samples of 53 patients with primary lung cancer were measured. The VEGF was immunofluorescence stained with VEGF polyclonal antibody. The propidium iodide was used to stain cellular DNA. VEGF expression levels and DNA content were analyzed quantitatively using flow cytometry. Intratumoral microvessel density was detected by factor VIII polyclonal antibody using immunohistochemistry. RESULTS: VEGF expression levels (FI) were high in lung cancer (1.3 +/- 0.6, positive ratio 73.58%). The cellular proliferation activity (PI) in VEGF-positive tumors was significantly higher than that in VEGF-negative tumors (29.8% +/- 9.2% vs 18.7% +/- 6.5%, P < 0.01). The intratumoral microvessel density (MD) in VEGF-positive tumors was higher than that in VEGF-negative tumors (19.4 +/- 11.3 vs 12.3 +/- 8.0, P < 0.05). No association was found between the MD and PI (r = 0.169, P > 0.05). The VEGF expression levels in lung cancer tissues with lymph node metastasis (n = 33) were significantly higher than that in those without lymph node metastasis (n = 20) (1.5 +/- 0.6 vs 1.0 +/- 0.5, P < 0.01). CONCLUSION: VEGF plays an important role in the growth and metastasis of lung cancer.

[Comparative pharmacokinetic study of two different ways of using L-asparaginase as the main drug in combination chemotherapy].

OBJECTIVE: To search for a safer and more effective way of using L-asparaginase as the main drug in combination protocol. METHOD: Twenty children with lymphoid malignancies were randomly divided into two groups: Group A and Group B, for L-asparaginase therapy. Group A: Leunase 6,000 KU/m2, continuous intravenous injection every day for 8 times; Group B: Leunase 6,000 KU/m2, continuous intravenous injection every other day for 8 times. Blood samples were collected every day for group A and every other day for group B and serum L-asparaginase and L-asparagine levels were tested. RESULTS: 1. The pre-treatment mean value of serum L-asparaginase in group A was significantly higher than that in group B (P < 0.01). The levels of L-asparaginase in group A tended to increase day by day during the therapeutic course while those in group B without increasing tendency. 2. The L-asparagine levels and their dynamic changes were not significantly different between the two groups (P > 0.05). CONCLUSION: The administration of Leunase by continuous intravenous injection every other day is a much safer and effective protocol and worthy of recommendation.

[Expression of ICAM-1/LFA-1 in the pocket area of adult periodontitis].

OBJECTIVE: To study the expression of ICAM-1/LFA-1 in the pocket area of adult periodontitis. METHODS: Expressions of ICAM-1/LFA-1 in adult gingival of periodontitis and healthy subjects were studied by alkaline phosphorylase-antialkaline phosphorylase technique. RESULTS: Junctional epithelium and apical part of sulcus epithelium expressed ICAM-1 in both adult periodontitis and healthy gingiva, showing an ICAM-1 gradient change with maximal staining at tooth aspect and weaker staining in the basal layer of keratinocytes. Significantly more LFA-1 positive leukocytes were observed in connective tissues and within pocket epithelium in adult periodontitis than those in healthy gingiva. CONCLUSION: ICAM-1/LFA-1 may provide important adhesion pathway for leukocytes migration into gingiva sulcus in adult periodontitis lesions.

[The effects of transforming growth factor alpha and beta 1 on the proliferation of alveolar type II cells in vitro].

OBJECTIVE: To investigate the effect of transforming growth factor alpha and beta(1) (TGFalpha and beta(1)) on the proliferation of alveolar type II cells in vitro and the molecular mechanism related. METHODS: (3)H-TdR incorporation and cell counting for the assay of cell growth; dot blot, in situ hybridization, and immunohistochemical methods for analysing the expression of cyclin D1 and cycle-dependent kinase 4 (CDK4) genes. RESULTS: With increasing of the concentration of TGFalpha and beta(1) (0.01 ng-100 ng/ml), TGFalpha enabled to increase (3)H-TdR incorporation and the number of alveolar type II cells, while the effect of TGFbeta(1) was opposite and both effects were dose-dependent (P < 0.01). TGFalpha-treated cells decreased the expression of CDK4 mRNA, and the relevant in comparing with the control (P < 0.01). On the contrary, TGFbeta(1)-treated cells decreased the expression of CDK4 mRNA), CDK4 as well as the cyclin D1 proteins (P < 0.05). CONCLUSION: Both TGFalpha and beta(1) were considered implicated in the regulation of proliferation of cultured alveolar type II cells isolated from the adult rats and CDK4 might be a common target against TGFalpha and beta(1) signals.

[Experimental study on apoptosis and Apo-1 expression of buccal carcinoma cell(BCC) induced by cisplatin].

OBJECTIVE: To study apoptotic characteristics and expression of apoptosis molecule Apo-1 (Fas, CD95) on buccal carcinoma cell (BCC) induced by cytotoxic drug cisplatin and exploit the apoptotic mechanisms of oral cancer chemotherapy. METHODS: Human BCC line BcaCD885 was used as in vitro study object. Morphological features of apoptotic BCC induced by cisplatin were observed by use of lightmicroscopy and electronmicroscopy, meanwhile, apoptotic rates (AR) were analysed by the flow cytometry. RESULTS: 1. Condensation and crack of nucleus and apoptotic bodies appeared in apoptotic BCC; 2. There existed time and dosage dependent AR, and AR were respectively 5.4%, 7.4%, 9.4%, 20.3% and 22.6% after BCC had been acted by 15 micrograms/ml cisplatin for 2, 6, 12 and 24 hours, moreover, there was significantly difference (P < 0.01) among the 12-hour group, the 24-hour group and the control group; 3. Cell cycle of BCC was apparently arrested at G0/G1 by cisplatin (P < 0.05); 4. Apo-1 expression of BCC were enhanced 58% and 62% respectively after it was induced by 25 micrograms/ml cisplatin for 18 and 42 hours (P < 0.05). CONCLUSION: Apoptosis may be an important mechanism of BCC killed by cisplatin, and Apo-1 may play a role in the process of apoptosis induced by chemotherapy drugs.

[Changes of CGRP activity and distribution in facial nucleus after facial nerve injury of guinea pig].

OBJECTIVE: In order to study the changes of CGRP activity in facial nucleus of guinea pig after facial nerve injury. METHOD: The activity of CGRP was detected by the technique of immunohistochemical and image analysis methods were used. RESULT: It was showed the there were CGRP-immunoreactivity (IR) in normal facial nucleus and its' subnucleus. An initial increase of CGRP-IR was noted and the content of CGRP under image analysis were significantly different when compared with control group (P < 0.01) at day 1 after injury of facial nerve. CGRP continued to rise to a maximal level at day 7 (P < 0.001). And after which it gradually decreased. We noticed distinct bundle-shaped CGRP fibers elongated to the peripheral part from ventral facial nucleus in the period of 14 d-35 d. CONCLUSION: It suggests that during the course of facial nerve regeneration following injury, CGRP changed characteristically. CGRP may play the regulational role in facial nerve regeneration.

[Clinic and immunophenotypic studies on midline malignant histiocytosis].

OBJECTIVE: To understand the clinic opatholgic changes of midline malignant histiocytosis (MMH), and to explain the source of atypical lymphoid cells (ALC) of MMH, so as to search after the relation between MMH and malignant lymphoma. METHOD: Histopathologic section of 39 cases of MMH were taken HE staining and the histopathologic changes were obsered under microscope. The immunohistochemical staining was also used to examine the immunophenotype of ALC. RESULT: 1. Pathologic changes of MMH: coaglation necrosis and mixed inflammatory cells infiltration, with epithelial infiltration which distributed around the center of blood vessel were their characteristics. 2. Immunohistochemical staining result: 28 in 39 cases of MMH (71.8%) expressed the immunophenvtype of T-cell; among them 13 (33.3%) were finded both positive reaction to CD3 and CD57 antigens with ALCs at the same time. CONCLUSION: According to immunophenotypic studies, most ALCs came from proliferation and infiltration of atypical T cell. ALCs of MMH expressed not only T-cell markers (CD3), but also NK-associated antigen (CD57). MMH was a mucosa related peripheral T/NK cell lymphoma originally.

[Immunoregulation of IL-2 in Trichinella-infected mice].

AIM: To study the immunoregulation of IL-2 in Trichinella-infected mice. METHODS: Mice infected respectively with HL strain and AM strain larvae of Trichinella spiralis were treated i.p. with IL-2 from the second day postinfection for 3 days. Serum IgG antibody levels were determined by ELISA and the infection capacity was determined using reproductive capacity index (RCI). RESULTS: In HL, higher dosage of IL-2 injection induced lower RCI and showed apparent anti-Trichinella effect. In AM, both low and high dose of IL-2 had no measurable effect on RCI, however, high dose of IL-2 reduced the infectivity of newborn larvae. CONCLUSION: IL-2 exhibits apparent suppressive effect on the infectivity of T. spiralis of HL strain.

[Effect of Callicarpa herbs on antioxidation in vitro].

The concentration of half inhibition (IC50) of Callicarpa herbs, including Callicarpa kochiana(Ck), C. bodinieri (Cb) and its fruit (Cbf), C. japonica, C. macrophrlla, C. cathaarana, C. giraldi(Cg) on lipid peroxidation (LPO) of tissus homogenate incubated in 37 +/- 0.5 degrees C were tested. The result showed that all but Cg could inhibit lipid peroxidation. The IC50 of Cbf was minimum. IC50 of Cb, Cbf and Ck inhibiting red blood cell (RBC) LPO and hemolysis caused by hydroxyl free radical (.OH), inhibiting brain homogenate LPO induced by .OH and inhibiting or clean .OH formation by system of H2O2 and FeSO4 were also tested. It was found the IC50 were Cbf > Cb > Ck. These results suggested that the most strong inhibitor to automatic LPO reaction was Cbf, but to LPO caused by .OH was Ck.

[The use of ultrasound biomicroscope in the diagnosis of anterior segment intraocular foreign bodies].

PURPOSE: To evaluate the use of Ultrasound Biomicroscope(UBM) in the diagnosis of anterior segment intraocular foreign bodies(IOFBs). METHODS: Besides the rountine x-ray photography and B-scan ultrasound examination, 9 cases with difinite IOFBs injury history were examined by UBM. RESULTS: Nine cases of IOFBs were shown in UBM image, among them 4 cases were invisible in x-ray photography and B-scan ultrasound examination. The IOFBs located in anterior chamber(1 case), in posterior chamber (2 cases), in pars plana (5 cases) and in iris root(1 case). The size of IOFBs varied from 0.1 mm x 0.25 mm to 1 mm x 3 mm. The IOFBs were extracted by combined intraocular surgery or direct magnetize extraction. CONCLUSION: Although most IOFBs could be localized by X-ray and B-scan ultrasound, some located in the posterior chamber or ciliary body are hard to detect, especially when they are nonmetal and extremely small. The high resolution image of the anterior segment of the eye created by UBM can help to localize the IOFBs and show the relation between IOFBs and surrounding tissue, which is important in the diagnosis and the choice of surgery method.

[The development of an intelligent vision testing instrument].

The paper introduces an intelligent vision testing instrument, which works on automatic method or manual method. The examinee inputs his or her judgement about the direction of the letter "E" with remote control. The system examines and judges the examinee's judgement, and then prints the testing report. The vision testing range is from logarithm vision 3.0 to 5.3. The distance between examinee and optometer can be measured automatically by the system with infrared and ultrasonic testing functions and a good accuracy.

[Expression and characterization of two outer capsid proteins VP2 and VP5 of bluetongue virus in insect cells].

OBJECTIVE: To study the biological characteristics of the outer capsid proteins VP2 and VP5 of bluetongue virus (BTV) expressed in insect cells and their potential use in the assembly of BTV and genetic engineering vaccine. METHODS: The genes which encode the two outer capsid proteins VP2 and VP5 of bluetongue virus (BTV) 10 were separately cloned into pFastBac1 vector and the corresponding recombinant baculoviruses were obtained. RESULTS: BTV VP2 could be expressed in Sf-9 cells better than VP5. Further works indicated that VP2 could elicit neutralizing antibodies to BTV10(1:64), and also could partially neutralize BTV1(1:16), but could not neutralize BTV13. The co-immunizing of VP2 and VP5 could induce higher neutralizing antibodies to BTV10 and BTV1. VP2 also showed a hemagglutination activity. CONCLUSION: VP2 expressed in insect cells could induce neutralizing antibodies to BTV and had the biological activity of hemagglutination, VP5 could enhance the ability of neutralizing antibody induction of VP2, they can be used for the assembly of virus-like particles and for the development of genetic engineering vaccine.
