[Effective tracing of osteoporosis at a fracture and osteoporosis clinic in Groningen; an analysis of the first 100 patients].

OBJECTIVE: To analyse the results from a fracture and osteoporosis (FO) outpatient clinic in order to achieve efficient case-finding for osteoporosis in patients of 50 years and older with a fracture due to low-energy trauma. DESIGN: Descriptive. METHOD: Following the publication of new professional guidelines for case-finding and treatment of osteoporosis, an FO outpatient clinic was opened at the University Hospital of Groningen, The Netherlands, to which patients of 50 years and older with a fracture due to low-energy trauma could be referred for further diagnosis and treatment after initial treatment for trauma. Bone-mineral density of the lumbar spine, hip and distal radius was assessed with dual-energy X-ray absorptiometry (DEXA). Patients with manifest osteoporosis, defined as having a fracture and a T-score < or = -2 SD at one of the measured sites, were put on medication. The results from the first 100 patients were analysed. RESULTS: In the first five months 74% (116/156) of the patients were seen in the FO clinic. In January 2004 the first 100 patients completed the diagnostic process. A total of 67 patients had manifest osteoporosis, 20 osteopenia and 13 had normal bone density. Furthermore, 48% of the patients between 50 and 60 years old had manifest osteoporosis. Unrecognised vertebral fractures were found in 21 patients. Forty-three percent of patients with manifest osteoporosis had low 25-OH-vitamine D levels (< 30 nmol/l). Eleven patients were sent to the Department of Internal Medicine on indication of secondary osteoporosis. CONCLUSION: The FO outpatient clinic proved to be effective and useful for identifying and treating a population at risk of osteoporosis.

Functional outcome in patients with thoracolumbar burst fractures treated with dorsal instrumentation and transpedicular cancellous bone grafting.

The aim of the study was to develop an insight into the impairments in spinal fracture patients, operatively treated with an internal fixator, and also into their ability to participate in daily living, return to work and quality of life as defined by the World Health Organization. Nineteen patients operated for a type A fracture of the thoracolumbar spine (T9-L4) between 1993 and 1998 in the University Hospital Groningen, the Netherlands, aged between 18 and 60 years, without neurological deficit were included in the study. Operative treatment consisted of fracture reduction and internal fixation using the Universal Spine System, combined with transpedicular cancellous bone grafting and dorsal spondylodesis. No ventral fusion operations, laminectomies or discectomies were done. Restrictions in body function and structure were measured on radiographs and in functional capacity tests, such as lifting tests and ergometry. Restrictions in activities were studied with the Visual Analogue Scale (VAS) Spine Score and the Roland Morris Disability Questionnaire (RMDQ). Restrictions in participation/quality of life were analysed with the Short Form 36 (SF36) and described in the return to work status. The radiological results are comparable to the literature. The reduction of the anterior wedge angle was followed by a gradual partial loss of intervertebral angle and regional angle. The maximum oxygen uptake (VO2-max) was reduced in only 8.3% of the patients. Arm and trunk lift was within the normal range in 87% and 80% of the patients respectively, but only 53% of the patients were able to perform a leg lift within the normal range. A mean RMDQ score of 4.0 positive items (SD 6.0) was found, and the mean VAS Spinal Score was 79.4 (SD 25.0), both better than in other series. No significant differences compared to the values of a comparable (healthy) age group could be identified in any variable of the SF36. A high correlation was seen between RMDQ, VAS Spine Score and the SF36 categories. No correlation was found between the anterior wedge angle and the regional angle on the one hand, and functional capacity tests or questionnaire scores on the other. Of the patients in paid employment before the trauma, 87% had returned to work at follow-up. About 50% of the patients had been obliged to change the intensity of their work or the kind of work they performed after the injury and treatment. In this matter, leg (muscle) performance seems a more important factor than overall condition (VO2-max). The results of the study indicate that patients with thoracolumbar spinal fractures without neurological deficit, treated with dorsal instrumentation, perform like healthy people 3-8 years after injury, according to the RMDQ, VAS Spine Score and SF36 results. Physical capacity tests reveal that leg (muscle) performance seems a more important factor in impairment than arm lift or overall condition.

[Clinical treatment of equestrian injuries in Groningen, 1990-1998: serious long-term effects].

OBJECTIVE: To determine whether equestrian injuries result in long-term residual symptoms and to establish the risk factors for these outcomes. DESIGN: Retrospective, descriptive. METHOD: A total of 88 patients older than 17 years of age who had been admitted to the Department of Surgery, University Hospital Groningen during the period 1990-1998 because of an equestrian injury were included in the study. After an average period of five years an inventory was made using the Sickness Impact Profile 68 (SIP68), if absenteeism from work or sport and disabilities or handicaps were experienced. Following this the risk factors for the occurrence of long-term effects were investigated. RESULTS: The response was 65/88 (74%). Equestrian injuries often resulted in long-term effects: 35% of the patients had a SIP68 score > 0.11% were permanently unfit for work, 25% no longer participated in sport as an effect of the injury and 43% still experienced some form of disability or handicap; on average this was all 5 years after the injury. The type of injury (notably intracranial, fracture), body region (notably trunk), and the injury severity score (ISS) were significantly correlated with the occurrence of long-term effects. CONCLUSION: Equestrian injuries which required clinical treatment were serious in nature and often resulted in disabilities and handicaps.

Sprain of the neck: quality of life and psychological functioning. A 4-year retrospective study.

Aim of the study was to analyse quality of life and psychological functioning in patients with sprain of the neck, to analyse the relationship between complaints, quality of life, psychological functioning and personality factors, and to analyse the profile of patients with whiplash associated disorders (WAD), 4 years after trauma. From the University Hospital Groningen 193 patients with the diagnose sprain of the neck filled out a questionnaire. Of this group 100 subjects did not have complaints before the accident and were therefore at risk for the development of complaints as a result of sprain of the neck. Quality of life and psychological functioning were assessed using the RAND-36 and the SCL-90, respectively. Personality was assessed by means of the Dutch Personality Questionnaire. Of the group at risk (56% women and 44% men, mean age: 33.9, SD: 14.6) quality of life was significantly worse in subjects with complaints (mean: 78.4, SD: 15.5) compared to subjects without complaints (mean: 87.5, SD: 8.7). Psychological functioning did not differ significantly between the group with complaints compared to group without complaints. Personality did not differ between the groups. Personality and complaints together were significantly related to quality of life (r: 0.77) and psychological functioning (r: 0.85). No specific profile of WAD patients was found. In conclusion, personality and complaints influence quality of life and psychological functioning to a considerable extent.

Long-term outcome of sports injuries: results after inpatient treatment.

OBJECTIVE: To investigate whether sports injuries result in long-term disabilities and handicaps and to establish variables with a prognostic value for the occurrence of these long-term consequences. MATERIALS AND METHODS: All patients older than 17 years of age and admitted to the University Hospital Groningen because of a sports injury were entered in the study. By filling in a questionnaire 1-4 years after the injury an inventory was made of the long-term consequences. MAIN OUTCOME MEASURES: Absenteeism from work and sports, experienced disabilities or handicaps and the Sickness Impact Profile 68 (SIP68). RESULTS: Out of 306 patients 229 (75%) returned a completed questionnaire. Sixty-seven per cent of the working population had been unfit for work up to one year, whereas 4% still had not resumed work. Absenteeism from sports was also considerable; nearly half of the population did not participate in sports for more than a year. Furthermore, 32% of the patients still experienced disability or handicap following the injury. This finding is in agreement with the results of the SIP68 (odds ratio 6.8; confidence interval (95% CI): 3.51-13.08). Two prognostic variables could be distinguished: 'gender' and 'type of sport'. Long-term consequences occur more often in women (p < 0.03) and with playing outdoor soccer, horse riding or skiing (p < 0.01). CONCLUSIONS: Sports injuries can lead to long-term disabilities and handicaps. The variables 'gender' and 'type of sport' were of prognostic significance.

Burst fractures of the thoracolumbar spine: changes of the spinal canal during operative treatment and follow-up.

Although multiple studies have concluded operative decompression of a traumatically narrowed spinal canal is not indicated because of spontaneous remodeling, instrumental decompression is frequently used as part of the operative treatment of spinal fractures. To investigate the process of remodeling, we studied the diameter of the spinal canal in 95 patients with burst fractures at the thoracolumbar junction (T9-L2). To measure and compare the spinal canal's diameter we used either computed tomography (CT) scans or radiographs, made preoperatively, postoperatively, after 9 months and after 24 months. In lateral plain radiographs we found that the initial percentage of cases with bony canal narrowing preoperatively of 76.5 was reduced to 18.4% postoperatively, to 8.2% at 9 months, and to 2.4% at 24 months. In CT scans in a selection of patients, the mean residual diameter of the spinal canal was 53% preoperatively and 78% at 24 months. The posterior segmental height increases during operation and decreases in the respective periods after operation. So ligamentotaxis can only play a role in the perioperative period. We conclude that a significant spontaneous remodeling of the spinal canal follows the initial surgical reduction. Two years after operation, bony narrowing of the spinal canal is only recognizable in 2.4% of the patients on plain lateral radiographs. The remodeling of the spinal canal can be seen on plain radiographs, although not as accurately as on CT scans.

[Bicycle spoke injuries in children: accident details and consequences].

OBJECTIVE: To describe the accident details and the effects of bicycle-spoke accidents on the physical and psychosocial functioning of children. DESIGN: Retrospective, descriptive. METHODS: The parents of 87 children aged between 1-12 who came to the central casualty department at the Groningen University Hospital with bicycle-spoke injuries during the period 1 January 1998 to 31 October 1999, were asked to complete a questionnaire on the accident details, the quality of life and the functional health status (behaviour) of their child in January 2000. RESULTS: Eighty-seven children fulfilled the inclusion criteria: 44 boys and 43 girls, with a mean age of 4.4 years (SD: 1.6, range: 1.4-10.2). Fifty-nine parents filled out the questionnaire (68%). A quarter of the children had been transported in a bicycle-seat (25%) and half of the children (51%) were seated on the carrier without any foot supports. Twenty-four percent of the bicycles were equipped with unbroken coat guards. The younger children (1-5 years of age) had significantly lower scores regarding motor functioning compared with the reference group (p < 0.001). The study population did not have significantly lower scores for the other quality of life domains compared with the reference groups. Eight parents (14%) attributed behavioural problems to the bicycle-spoke accident. CONCLUSION: Not all children were fully recovered one year after the bicycle-spoke accident. The sequelae included physical as well as behavioural aspects of functioning. The bicycles lacked adequate protective measures.

Classificational problems in ligamentary distraction type vertebral fractures: 30% of all B-type fractures are initially unrecognised.

The clinical records, operation records, X-rays and CT-scans of 160 operatively treated patients with A-type and B-type spinal fractures were evaluated in a retrospective study. The preoperative diagnosis was compared with the postoperative diagnosis. Analysis of characteristics of patients with A-type fractures (without the unrecognised B-type fractures), initially unrecognised B-type (uB) fractures, and B-type fractures (without the unrecognised B-type fractures) was performed. We analysed the age of the patients, the respective fracture levels, neurologic deficit, anterior wedge angles (AWA), anterior corporal height (ACH), posterior corporal height (PCH), and the percentage of frontal corporal collapse (FCC). The t-test was used for statistical analysis. The mean age of patients in each group did not show a significant difference. The group of unrecognised B-fractures had a more caudal fracture level than the recognised B-type fractures. The fracture levels of the A-group and the uB-group patients showed no difference using the t-test. The percentage of patients with spinal fractures with neurologic deficit is 16% in the A-type fracture group, 12% in the uB-fracture group and 50% in the B-type group. The preoperative classification of patients in the A-group and in the uB-group showed that patients in the uB-group have more than proportional relatively simple preoperative A-fractures. The AWA and ACH did not show significant differences between the groups. The mean PCH of the uB-group was higher than the PCH of the A-group. No differences were measured between the uB-group and the B-group. The mean percentages of frontal corporal collapse (FCC) did not show a significant difference. Thirty percent of B-type fractures are misdiagnosed when plain X-rays and CT scans with 2D reconstructions are used as the only preoperative diagnostic tools. A large PCH with a normal interspinous distance should raise the suspicion of a B-type lesion. A large AWA does not point to a ligamentary B-type fracture.

Immunohistopathological appearance of three different types of injury in human skin.

OBJECTIVE AND DESIGN: To determine whether underlying mechanisms of inflammation, like cellular infiltrates, expression of adhesion molecule and cytokine patterns are similar under different conditions of injury. Skin biopsies were taken of three different groups of patients in which local inflammation of the skin might occur. MATERIAL, PATIENTS: Skin activation was studied as a result of incision during surgery under aseptic conditions, as a result of a local bacterial infection and as a result of blunt trauma, resulting in a femoral fracture, without disruption of the epithelial barrier. METHODS: Skin biopsies were snap frozen for immunohistochemical analysis. RESULTS: Incision of the skin resulted in a granulocyte infiltrate, paralleled by E-selectin expression. As a result of infection granulocytes were observed and monocyte/macrophage and T cell numbers were increased. Furthermore, E-selectin, VCAM-1 and ICAM-1 expression increased and cytokine expression markedly changed compared to normal skin. Skin taken at the site of the femoral fracture showed no signs of inflammation. CONCLUSION: Different stimuli lead to different local inflammatory responses in human skin.

Tumor necrosis factor alpha (TNFalpha) in human skin: a comparison of different antibodies for immunohistochemistry.

Conflicting results have been reported regarding the localization and presence of TNFalpha in normal human skin. To study TNFalpha expression, we tested a panel of antibodies directed against human TNFalpha. First, antibodies were tested for immunoreactivity on cytospots of isolated LPS-stimulated peripheral blood mononuclear cells. Second, antibodies were tested to detect recombinant TNFalpha in Western blots. Some antibodies were found to be unable to detect recombinant TNFalpha in the blots. However, most antibodies were able to bind TNFalpha protein, but they did not bind to other irrelevant proteins that were also present in the blots. Finally, antibodies were tested on cryosections of normal human skin. Antibodies that did not react with TNFalpha in the blots were incubated with TNFalpha before the staining procedure to see whether these antibodies specifically bound TNFalpha. We found that, although all the antibodies bound TNFalpha, there were clear differences in staining patterns. This indicates that these antibodies may recognize distinct epitopes or different forms of TNFalpha. The differences found in this study and those reported previously could be the result of differences in the concentration of antibody used, the staining procedure or specificity of the antibody itself. So, for unambiguous interpretation of data, it is important to know the characteristics of the antibodies used.

Thoracolumbar spinal fractures: radiological results of transpedicular fixation combined with transpedicular cancellous bone graft and posterior fusion in 183 patients.

In internal posterior fixation of thoracolumbar fractures combined with transpedicular cancellous bone graft and posterior fusion of the intervertebral facet joints at the level of the destroyed end plate it is still uncertain as to whether significant vertebral body collapse and loss of correction of the regional angle (RA) and the intervertebral angle (IVA) occur (after removal of the implants). These questions were investigated in a retrospective study of 183 consecutive patients, 18-65 years old, with a spinal fracture between the 9th thoracic and the 5th lumbar vertebral body (inclusive), treated operatively between 1988 and 1996 (27% had objective neurological deficit, 37% had multiple injuries). According to the Comprehensive Classification, 128 type A, 32 type B and 21 type C fractures were identified preoperatively. Changes in the anterior wedge angle (AWA), the IVA and the RA were measured preoperatively, and within 1 month, 9 months and 24 months postoperatively. The effect of implant failure was also evaluated. The normality of the distribution was tested using the Kolmogorov-Smirnov (K-S) test. The one-sample runs test and the t-test were used to evaluate angle changes. Angles in patients with and without implant failure were compared using the unpaired t-test. Almost complete restoration of the AWA could be achieved during operation. Postoperative changes in AWA were either very small or not significant. The reduced vertebral body did not collapse after 9 months, when most of the patients (170) underwent removal of the implants, but significant changes in IVA were found after implant removal. Correction of the RA was statistically significant before implant removal, but the RA 2 years after surgery had become almost the same as the preoperative values. Changes at the level of the intervertebral space, occurring after implant removal, contributed to the loss in the RA. Broken pedicle screws (10.9% of the patients) resulted in significant changes in the AWA and RA before implant removal, but did not influence the IVA.

Applying the Quebec Task Force criteria as a frame of reference for studies of whiplash injuries.

Research prior to 1995 showed a diversity of either inclusion or exclusion criteria (or both) for diagnosing whiplash injury. As a consequence, the Quebec Task Force (QTF) developed expert-based criteria, which may be considered as a the 'new' gold standard. Here, we examined the inclusion criteria and exclusion criteria used in research populations from the major 82 research studies performed during the period 1980-1998, comparing their similarities and dissimilarities to the QTF standard. None of the articles satisfied the QTF definitions completely, either before or after their introduction in 1995. Nevertheless, the QTF still seems to have had some impact on either the published inclusion or exclusion criteria. We observed that both sets of criteria showed a qualitative shift following the QTF publication in 1995. For the inclusion criteria, we found both a statistically significant increase in use of the QTF definition (acceleration-deceleration mechanism, rear-end collision, motor vehicle collision or other mishaps) and in the criterion 'neck pain'. We also observed some smaller changes in both inclusion and exclusion criteria but none of these was significant statistically.

Perfusion of burn wounds assessed by laser doppler imaging is related to burn depth and healing time.

Average perfusion in various burn wounds was assessed using Laser Doppler Imaging (LDI). The time necessary for a complete healing of the wound was compared to the results of the LDI measurements. A certain depth of burn was associated with a typical pattern of perfusion in the course of time. There was a significant difference between average perfusion in a full thickness burn requiring surgical treatment and burns with regenerative capacities.

Local cellular inflammation as a result of elective standardized vascular surgery.

During surgery, incision of the skin under aseptic conditions is performed. Despite the absence of noxious agents, an inflammatory response may be induced. We studied the local inflammatory response in human skin as a result of surgical intervention, under aseptic conditions. Elective standardized vascular surgery served as a model. A series of skin biopsies was taken from the wound edge at different time points after first incision. Biopsies, directly taken at first incision were considered to represent normal skin. Additional biopsies were taken at 30 min after the start of surgery and just before closure of the wound, maximally 270 min after surgery. Kinetics of recruitment of cells, expression of adhesion molecules and the presence of pro-inflammatory cytokines was studied. Granulocytes were observed at first at 30 min after incision of the skin and their number increased in time. This granulocyte infiltration is paralleled by E-selectin expression on endothelial cells, which also was observed at first at 30 min after surgery with a further increase in number in time. Incision of the skin did not change P-selectin, ICAM-1, VCAM-1, TNFalpha, IL1alpha, IL1beta, IL6 and IL8 expression. These results show that incision of the skin under aseptic conditions during elective standardized vascular surgery induces local nonspecific cellular inflammation.

Sprain of the neck in clinically treated patients in The Netherlands: an inventory of different categories of car accidents.

Different categories of car accidents of victims with sprain of the neck were investigated for both drivers and passengers. The predominant category of the car crash was a collision with another car for drivers as well as for passengers. The second cause was unknown. The distribution of the accidents was statistically significantly different for drivers and passengers.

Neck sprain after motor vehicle accidents in drivers and passengers.

Neck sprain is a general term denoting a soft tissue injury of the neck, which seldom causes major disability but is considered a modern epidemic. The purpose of the present study was to determine the prevalence of sprain of the neck injury due to motor vehicle accidents (MVAs) in both drivers and passengers. In addition, the degree of seat belt wearing in both driver and passenger was analysed. A second aim was to identify groups at risk by analysing the age and gender distribution of patients with neck sprain. The results of this population-based study revealed a sharp increase in neck sprain from 1989 through 1995, whereas a more or less stable pattern was found for seat belt use. The sharp increase was found to be attributable to outpatients. Finally, we found a driver predominance as well as a female predominance; groups at risk were the 20- to 24-year-olds for drivers and 15- to 19-year-olds for passengers.

Procalcitonin behaves as a fast responding acute phase protein in vivo and in vitro.

OBJECTIVES: Procalcitonin (PCT) is a 13 kD protein of which plasma concentrations are strongly increased in inflammatory states. PCT concentrations are claimed to have a more powerful discriminatory value for bacterial infection than the acute phase proteins serum amyloid A (SAA) or C-reactive protein (CRP). The source of production and its mechanism of induction are unknown. We investigated the inducibility of PCT both in vivo and in vitro and compared the behavior of PCT with those of SAA and CRP. DESIGN: A prospective descriptive patient sample study and a controlled liver tissue culture study. SETTING: A university hospital. PATIENTS: Cancer patients who were treated with human tumor necrosis factor-alpha (rhTNF-alpha; 5 patients) or interleukin-6 (rhIL-6; 7 patients). MEASUREMENTS AND MAIN RESULTS: Serial serum samples were collected for analysis of concentrations of PCT, SAA, and CRP. In the TNF-alpha group, frequent sampling was performed on the first day to allow analysis of initial responses. In a human liver slice model, the release of PCT, SAA, and CRP was measured on induction with rhTNF-alpha and rhIL-6 for 24 hrs. We found that PCT displayed acute phase reactant behavior in vivo after administration of both rhTNF-alpha and rhIL-6. After rhTNF-alpha-administration, PCT reached half-maximal concentrations within 8 hrs, 12 hrs earlier than either SAA or CRP did. PCT, SAA, and CRP were produced in detectable quantities by liver tissue in vitro. PCT production by liver slices was enhanced after stimulation with rhTNF-alpha or rhIL-6; SAA and CRP concentrations were elevated after stimulation with rhTNF-alpha. CONCLUSIONS: We found that PCT and acute phase proteins such as CRP are induced by similar pathways. The liver appears to be a major source of PCT production. Thus, PCT may be considered an acute phase protein. The different kinetics of PCT, rather than a fundamentally different afferent pathway, may explain its putative diagnostic potential to discriminate bacterial infection from other causes of inflammation.

Blunted rise in platelet count in critically ill patients is associated with worse outcome.

OBJECTIVE: To test the hypothesis that a low rate of change of platelet counts (PCs) after admission to the intensive care unit (ICU) is associated with mortality. Low PCs are known to be associated with disease severity in critically ill patients, but the relevance of time-dependent changes of PCs has not been investigated. DESIGN: Retrospective study. SETTING: A 12-bed surgical ICU of a university hospital. PATIENTS: All adult patients admitted to the ICU for at least 4 days during a 7-yr period. INTERVENTIONS: At admission, Acute Physiology and Chronic Health Evaluation scores were calculated. PCs and leukocyte counts were analyzed from admission to day 10. The daily rise of the PCs (deltaPC/deltat from day 2 to day 10 was calculated. Rates for 30-day mortality as well as hospital mortality were determined. MEASUREMENTS AND MAIN RESULTS: A total of 1,415 admissions were studied. Median PCs (interquartile range) initially decreased and subsequently increased, with a higher PC in 1,203 survivors than in 212 nonsurvivors from day 2 onward (302 [range,181-438] x 10(3)/mm3/day vs. 129 [range, 62-228] x 10(3)/mm3 at day 10; p < 0.001). After stratification of patients per type of surgery, within each group PC was also higher in survivors. Mean deltaPC/deltat was more than five times higher in survivors compared with nonsurvivors: 30 +/- 46 x 10(3)/mm3/day vs. 6 +/- 28 x 10(3)/mm3/day (p < 0.001). The area under the receiving operating characteristic curve of deltaPC/deltat for 30-day survival was 0.743 compared with 0.728 for the Acute Physiology and Chronic Health Evaluation. Leukocyte counts showed marginal differences between nonsurvivors and survivors. CONCLUSION: A blunted or absent rise in PCs in critically ill patients is associated with increased mortality. deltaPC/deltat is a readily available and simple parameter to improve assessment of critically ill patients.
