[Cimetidine and ranitidine in intensive care patients. Double-blind randomized cross-over study on intravenous administration: hemodynamics, plasma coagulation, blood gases and acid-base status].

Intravenous cimetidine 200 mg, and ranitidine 50 mg were administered as 2 minute infusions to 50 intensive-care patients. Cimetidine and ranitidine differed significantly in their effect on systolic arterial blood pressure measured during the second minute (alpha = 0.01). In the case of cimetidine the haemodynamic parameters measured over 10 minutes revealed a clearly defined fall in systolic, diastolic and mean arterial blood pressure as well as a rise in heart rate. The initial values were regained 5 minutes after drug application. Adverse effects of ranitidine on haemodynamics were much less than those of cimetidine. Neither drug produced any essential change in clotting parameters (partial thromboplastin time, plasma thrombin time, thromboplastin time and fibrinogen) measured before and 5 minutes after application, nor had they any effect on blood-gases or acid-base status.

[Prognostic significance of programmed ventricular stimulation in the noninvasive registration of ventricular late potentials in the postinfarct period].

The prognostic significance of programmed ventricular stimulation for assessing ventricular vulnerability and of signal averaging for detection of ventricular late potentials was assessed prospectively in 132 survivors of acute myocardial infarction. The median day of these studies was the 22nd. Programmed ventricular stimulation (PVS) included single and double premature stimuli during sinus rhythm and paced ventricular rhythms at rates of 120, 140, 160 and 180 bpm. The endpoint for stimulation was the induction of 4 or more consecutive echo beats. 59 of 132 patients (45%) had late potentials in their body surface signal-averaged ECG. The prevalence was 35% in those with anterior wall infarction and 54% in those with inferior wall infarction (chi 2 = 3.9; p less than 0.05). In 61 patients (46%), 4 or more consecutive echo beats were induced. In 28 of these patients, sustained ventricular tachycardia or fibrillation were initiated. During follow-up (15 +/- 11 months; mean +/- S.D.), there were 4 sudden deaths. Neither PVS nor signal averaging were able to predict these events. Spontaneous symptomatic sustained ventricular tachycardia occurred in a total of 9 patients, all of whom had an abnormal PVS result (sensitivity 100%); 7 of these patients also had late potentials (sensitivity 78%). Stepwise analysis revealed that the combination of late potentials (duration greater than or equal to 40 ms), and of induction of sustained ventricular tachycardia at rates less than 270 bpm had the highest predictive value (50%). In contrast, the prevalence of sustained ventricular tachycardia was as low as 3% in those without late potentials.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effect of ticlopidine on peripheral obliterating arteriopathy].

The efficacy of and tolerance to ticlopidine, a platelet inhibitor, in preventing progression of obliterative arterial disease was compared with that of a placebo in a double-blind and randomized trial in 43 patients (22 on placebo, 21 on ticlopidine) over a period of one year. The course of the disease in both groups was assessed by serial angiography of the lower limbs, both at the beginning and at the end of the study. There was a significantly reduced progression in the ticlopidine group (P less than 0.01). The results support the hypothesis that platelets are of great importance in the progression of arteriosclerosis and that inhibition of platelet function is an effective principle of prevention. Ticlopidine in this respect is an alternative to the use of acetylsalicylic acid.

[Acute respiratory failure in a medical intensive care unit. Incidence and prognosis].

Acute respiratory failure had occurred in 89 of 1594 patients in a medical intensive care unit (5.6%), 26.8% of all patients (332) on long-term mechanical ventilation. Compared with the other chronically ventilated patients those with acute respiratory failure averaged a lower age, the proportion of women was higher and the duration of ventilation longer. The death rate was significantly higher (78.7% compared with 58.3%). The important prognostic factors included the underlying disease, additional abnormal organ function, severity of pulmonary gas exchange abnormality, and advanced age. If there was septicaemia, peritonitis, liver cirrhosis with bleeding oesophageal varices or polytrauma with acute renal failure the death rate was over 80%; after hypovolaemic shock, pancreatitis or postoperative pulmonary failure it was less than 65%. Patients who had abnormal function of at most one other organ in addition and an inspiratory arterial pO2 difference below 250 mm Hg, measured 12 hours after onset of mechanical ventilation, had a relatively favourable prognosis with a death rate of 33%, while in the other groups of patients it was 86-100%.

Exclusion of chromosomal mosaicism in prenatal diagnosis.

For use in prenatal diagnosis, tables were prepared giving the number of metaphases or clones, respectively, which must be analysed in order to detect fetal mosaicism of a given degree (= percentage of the aberrant cell population) or higher with at least 95% or 99% probability. Different tables are provided for the two techniques of chromosomal preparation: the colony method and the flask method.

[Qualitative and quantitative determination of bacterial populations in aquatic habitats. 5. Comparison of 2 rapid sand filters].

The bacterial examinations of raw water and the effluent of two rapid sand filters showed differences with respect to the colony count, the identified species and the activity of the isolated bacteria. The raw waters of the treatment plants Si and Me are originated from an oligotrophic artificial lake and from ground-water respectively. The two raw waters contained only low numbers of bacteria, whereas the effluents of the filters showed a considerable increase of the bacterial counts. The results indicate a development of microbial populations in the filters. Furthermore there were also differences between the two filters examined. The first one led to the growth of Pseudomonas- and Flavobacterium species, the second one to Flavobacterium, Chromobacterium and Acinetobacter species. Comparing the raw water and the effluent, a reduction of special and total physiological activities of the isolated bacteria could be observed in filter I, whereas there wasn't any change in filter II.

[Effect of prosthetic heart valve replacement on the natural course of isolated mitral and aortic as well as multivalvular diseases. Clinical results in 783 patients up to 8 years following implantation of the Björk-Shiley tilting disc prosthesis].

In order to establish the influence of prosthetic valve replacement on the natural course of mitral, aortic, and double-valve disease of NYHA class III and IV, the progress of 359 patients with Björk-Shiley mitral-valve prostheses (BSM), 317 with aortic valve prostheses, and 107 with double valve prostheses was compared with that of patients who had been treated medically. In this last group of patients, valve replacement had been recommended in the period 1968-1976, but for various reasons had not been carried out. Cumulative survival rates after 8 years were 77% for the BSM group, but only 32% for the medically treated group (p less than 0.0001). One year after operation, 70% of the BSM patients showed an improvement equivalent to one NYHA class, and in the majority this was still apparent after 5 years. In patients with aortic-valve incompetence, cumulative survival rates were 86% for the surgically treated group and 32% for the medically treated group (p less than 0.00001). In aortic-valve stenosis, these differences of cumulative survival rates were even more pronounced (p less than 0.000001), and were calculated to be 85% in surgically treated and 10% in medically treated patients. Clinical improvement in the BSA group averaged 1.5 NYHA classes. The 5-year survival rate for the patients with mitral and aortic-valve disease was 32%, while following doublevalve replacement it was 67% (p less than 0.005). Clinical improvement after double-valve replacement was similar to that following mitral-valve replacement. Prosthetic heart-valve replacement significantly prolongs life in patients with isolated mitral- or aortic-valve lesions as well as in patients with double-valve disease of NYHA classes III and IV. Even in the early years of the study, when the operative mortality was relatively high, the surgically treated groups had a significantly higher survival rate than the conservatively treated groups, and this was already apparent at an early stage in the follow-up period. Improvement of functional capacity and relief of symptoms amount to a difference of approximately one NYHA class in mitral and double-valve disease and of approximately 1.5 NYHA classes in aortic-valve disease.

Coronavirus-associated antibodies in nasopharyngeal carcinoma and infectious mononucleosis.

Coronavirus-like particles are found within the cytoplasm of NPC tumor cells, within the cytoplasm of the tumor cells of the regional metastases, and within tumor cells grown on nude mice. For the immunologic identification of the coronaviruses, the cultures of human tracheal epithelium (MRC-C) were used and inoculated with a known coronavirus strain. Whereas blood sera from NPC patients (n = 73) contain significantly elevated antibody titers against corona viruses, unselected sera from patients without NPC showed a low antibody titer (n = 83). Only patients suffering from infectious mononucleosis (n = 40) showed a titer pattern similar to that of NPC patients. For demonstration of antigen-antibody reaction within the NPC tumor cell cytoplasm, sera with a high antibody content against coronaviruses deriving from other than NPC patients or anticoronavirus sera from rabbits were used. By indirect immunofluorescence, the NPC tumor cells showed a bright cytoplasmic fluorescence. No fluorescence was seen when tumor cells were exposed to human sera with known low or absent corona antibody titer or to normal rabbit sera. The results indicate that next to a DNA virus infection (EBV), an RNA virus infection (coronavirus) may play a role in NPC as well as in infectious mononucleosis.

[Qualitative and quantitative examination of bacteria found in aquatic habitats. 3. Communications: Recording the data for identification and analysing by numerical methods (author's transl)].

A sheet for optical page mark was developed to transfer the large amount of identification criteria, necessary for quantitative and qualitative determination of bacterial populations, to the computer-assisted analysis. Empirical and numerical methods are used for the taxonomic evaluation. Different metrics are applied to the different groups of criteria. The clusters were formed by the single-linkage method. An example is given to explain how cluster analysis and formal descriptions are an useful aid for the classification of bacteria.

[Plasma level of etidocaine within the first two hours after axillary plexus block in healthy adults and patients with renal insufficiency (author's transl)].

The plasma level of etidocaine was studied within the first two hours of axillary block in 7 patients with renal insufficiency and 7 healthy adults. After 15-20 min the maximum of plasma concentration was found in the group with renal failure and after 30-45 min in the healthy adults. 30 min after the block the plasma level was 1,26 +/- 0,62 mug/ml in the ill patients and only 0,73 +/- 0,31 in the normal group. Using a mathematical model the ratio of permeation into the blood is significantly higher in the group with renal failure than in the healthy adults. The main reason for these results seems to be the acidosis, which is often combined with renal insufficiency. The meaning of these results for regional anaesthesia in patients like these is discussed.

A discriminant analysis for qualitative data with interactions.

Two linkable computer programs have been developed for a discriminant analysis with qualitative variables, which permit taking into account 1st order interactions. The first program is used to estimate the parameters required in the second program. The second program allows a classification of new cases as well as an unbaised estimation of error rates.
