[HBsAg screening in pregnant females].

In 1985-1986, the sera of 2,634 pregnant women in the 26th week of gestation were examined for HBsAg by means of counterelectrophoresis and haemagglutination test. 10 sera (0.38%) yielded a positive test result. Only one serum came from a nurse who was occurred in the Public Health Service, suggesting a general HBsAG screening in pregnancy. Thus, hepatitis-B carriers can be detected and, by simultaneous immunization of the newborns the materno-fetal infection prevented.

[Use of a computer system in perinatal data collection].

We present a computer system for obstetrical and perinatological data collection. All deliveries of the years 1984, 1985, and 1986 have been collected with this data collection program, so that there is how a data base containing 7,975 data records. A relational data base was chosen for data collection and storage. Either off-line or on-line data collection is possible by medical users via menu monitoring. Various possibilities of use are described.

[Determination of immunoglobulins IgG, IgM and IgA in maternal serum in patients with intrauterine growth retardation].

Immunoglobulin IgG, IgM, and IgA were analysed in maternal serum of 33 patients with intrauterine growth retardation and 30 patients without hypotrophic newborns. The estimations of the immunoglobulins were carried out by means of single radial immunodiffusion according to Mancini and coworkers. There were no significant differences between both groups of patients. Therefore IgG, IgM, and IgA are no additional parameters for estimation of the placental function of the 3rd trimester of pregnancy.

[Fetal tachycardia--a cause of nonimmunologic hydrops].

A case of nonimmune hydrops fetalis is demonstrated. The cause of this disorder was a severe fetal tachycardia. Prenatal findings, obstetric management, and neonatal outcome are described. Our data have been discussed with reference to the literature.

[Massive corpus luteum hemorrhage in a dialysis patient].

A 17-year old patient who was continuously dialyzed due to Fanconi-anemia and bilateral renal insufficiency developed an acute and severe bleeding from a ruptured Graafian follicle (corpus luteum). The bleeding was life-threatening, most likely due to the continuous treatment with heparin necessary for hemodialysis procedure. Differential-diagnostic considerations and interdisciplinary therapeutic measurements are reported.

[Twin pregnancy--a special problem of modern obstetric medicine].

Basing on an exemplary organized health protection incorporating up-to-date technique, progressive management of delivery and optimal neonatal care, we were able to reduce the perinatal mortality of all pairs of twins at the UFK Jena in the periods between 1977/79 (n = 204) and 1980/82 (n = 174) from 8,33% to 3,30%. The difference was statistically significant (p less than 0.05). The late morbidity of these children also showed a statistically significant decline (p less than 0.05).

[Management of twin pregnancy with different development].

By means of a case report of a dizygotic twin pregnancy with extreme difference of weight our diagnostic and therapeutic procedure is reported. Beside ultrasound biometry it was made use of the placental perfusion measurement as a functional parameter of the fetoplacental unit. In the case reported a separate judgement of the placental areas of the two fetuses was possible.

[Influence of na-salicylate on uterine motility in labour (author's transl)].

A dosage of 10 mg/kg body weight Na-salicylate has no depressive effect on uterine motility. The examination carried out intra partum in the condition of normal uterine activity support our opinion that relatively high doses of 3--5 g per day or the simultaneous administration of beta-adrenergical substances are necessary to obtain an effective tocolysis.

[Effectiveness of supplemental bleomycin therapy in vulvar cancer].

It is reported about the application of bleomycin within a randomized prospective investigation. Bleomycin was used for additional chemo-therapy at the vulva-carcinoma. There were treated with Bleomycin 27 of 55 women from two gynaecological hospitals. -- After a period of two years and a half there can not be shown by the "life-table-method" advantage neither in comparison to the only surgical therapy not to the cryo-surgical treatment.

[Clinical studies on the connection between preceding abruptio placentae and abortus and subsequent complications during the afterbirth phase (author's transl)].

2176 vaginal deliveries at Jena University Hospital were selected at random to check whether in these patients it is possible to establish the connection postulated by some authors between preceding premature termination of pregnancy and the mass incidence of afterbirth haemorrhages, as well as of necessary surgery in the afterbirth phase. There was no significant increase of afterbirth complications following abruptio placentae and abortus, whereas there are partly statistically significant differences in the incidence of surgery reauired in children of normal weight of underweight.

[Computer evaluation of tocograms following administration of Spasmalgan (denaverin)].

Computer analysis of tocograms after administration of Spasmalgan (Denaverin). The influence of spasmolyant Spasmalgan on 5 parameters of labour has been examined in 50 intraamniotic derived tocograms by on line fetal monitoring. Only a little increase of the basal tone has been shown after administration of Spasmalgan. The method is recommended for testing the uterotropic influence of pharmacons in labour.

[Clinical investigations relating to a causal connection between abruptio placentae and subsequent birth of an underweight child (author's transl)].

The question was studied as to whether there is a causal connection between abruptio and birth of an underweight child in the subsequent pregnancy. While the phenomena involved are still largely unexplained, though clearly manifold, which can lead to the birth of "low birth weight children", it is shown that abruptio cannot be regarded as a prime risk factor. A comparison with the international literature shows that the specific features of the patient group investigated must always be taken into account when answering this question.

[Infection hazards and complications following the use of IUDs].

Own experiences on IUD-complications especially irregular bleedings and infections underline that modern plastic intrauterine devices are second-rate contraceptives. Compared to hormonal contraceptive drugs intrauterine devices have a higher pregnancy risk and increased rates of infections. Therefore the IUD is in our opinion not suited primary contraceptive method used without indication. Because of the relatively often occurring ascendant genital infections the application of intrauterine devices should not be carried out in nulliparae.

[Results of high dosage progesterone therapy in carcinoma corporis uteri].

The authors report their results on the treatment of progressive endometrial carcinomas of cases not suitable for conventional therapy by means of long term progestogen application in high doses. It is demonstrated the effectiveness of this method of treatment if the progestogen is given in necessary doses without stopping the application. An additional short term treatment in low doses in all cases of operated endometrial carcinomas is assesed not to be useful.

[Relationship between clinical parameters and labor (author's transl)].

In 200 cases of childbirth supervised by intra-amniotic tocometry the relations between various clinical parameters such as age, parity, gestosis, rise of temperature, birth weight, small circumference of the head, duration of the cervical dilatation and the expulsion period, and the exactly defined motility of the uterus were investigated during different phases. The results confirmed the very limited influence of selected clinical phenomena upon the multifactiorially regulated total complex of labour.
