[Causes of death and some etiologic aspects of acute and chronic pancreatitis].

The study gives a survey of the causes of death of 106 patients suffering from acute pancreatitis and of 125 chronic pancreatitis cases. They account for 1.41% and 1.69% respectively of the autopsies performed by us. In 76.4% of the 106 cases of acute pancreatitis the disease was the main cause directly responsible for the death. Chronic pancreatitis was the main underlying disease or a significant secondary condition in the chronic group. The results of etiologic analysis are in acute pancreatitis: 67.9% biliary tract changes, 7.5% alcohol abuses and 20.8% postoperative damages. Alcoholism (44 cases) was important by patients with chronic pancreatitis.

[Intraductal juvenile papillomatosis of the breast].

Papilloma of the female breast is rare before and during puberty. Hence, positive findings recorded from a girl, eleven years of age, encouraged the authors to make these comments on pathogenesis, operability, and prognosis of epithelial proliferations in juvenile patients. Reference is also made to the need for delimitation of this pathological complex from other papillary tumours of the mammary gland.

[Pathogenetic correlation between chronic pancreatitis and pancreatic carcinoma].

The autopsy material from the Institute of Pathology of the Faculty of Medicine of the Humboldt University (Charité) in Berlin for the years 1970 to 1984 was analyzed with respect to the presence of pancreatic carcinoma and a long history of chronic, non-obstructive pancreatitis. A total of 20,515 adult sections were reviewed. 331 (1.6%) of these had carcinoma of the exocrine pancreas. 75 pancreata were dissected in tail-to-head direction into 10 blocks. In 12 (16%) of them a chronic non-obstructive pancreatitis or pancreatic scarring as a result of pancreatitis could be demonstrated histologically. The possibility of a relationship between pancreatitis and pancreatic carcinoma is considered, in particular with respect to pre-malignant changes and latency of the latter. Comprehensive analysis and evaluation of the present study material, together with a critical evaluation of the literature, support the viewpoint that long-term, chronic pancreatitis and pancreatic carcinoma have a common pathogenic basis and that chronic pancreatitis may be regarded as an antecedent event for the neoplasm.

[Pancreatic donor operation for islet cell isolation and transplantation].

Preservation of B-cells, following organ collection and cold ischaemia, was defined on 19 donor organs by means of histological tests and testing for insulin content in pancreatic tissue. The degree of preservation declined by 50 per cent after two hours of cold ischaemia. The Velcro technique and uncrushed organ digestion were used for isolation. Islet yield was calculated by determination of insulin levels in isolated tissue (nmol/ml) and in the islet (pmol/islet). The average islet yield was 2 076 islets to one gram pancreas, that is ten per cent of the original amount. Studies conducted into in-vitro insulin secretion from isolated islets have revealed damage to organs due to handling, especially to organs which had undergone extended cold ischaemia.

[The development of endoscopic papillotomy in East Germany].

UNLABELLED: Up to the middle of 1985 a survey of endoscopic papillotomy (EPT) had been carried out in the GDR resulting in 3124 patients from 8 centers. INDICATIONS: choledocholithiasis 90.6% (13.1% with a T-drain, 16.2% with a gallbladder in situ); papillary stenosis 4.4%; tumors of the papilla 2.2%; other indications 2.2%. The common bile duct got free of stones in 89.3% of 2210 documented cases after successful EPT. In comparison to former results in 1982, 1983 and 1984 (cumulated data) the technical success rate of EPT rose slightly, but steadily to 95.3%, and the complication rate fell to 5.8%. Bleeding was the most common complication followed by cholangitis, pancreatitis, basket impaction and perforation. 30 patients (0.96%) died due to complications, 14 with pancreatitis, 7 cholangitis, 7 bleeding, 2 perforation.

[Visualization of the pancreatic ducts by ultrasound tomography in pancreatic diseases with bile duct obstruction].

The sonographic findings in 274 patients with biliary obstruction and/or dilatation of the pancreatic duct of more than 4 mm were evaluated (128 common duct stones, 82 carcinoma of pancreas, 64 chronic pancreatitis). Carcinoma of the pancreatic head caused a greater degree of dilatation of the common bile duct (average diameter 18.3 mm) than did chronic pancreatitis (10.5 mm) or biliary stones (14.2 mm). Dilatation of the pancreatic duct greater than 4 mm occurred with approximately equal frequency with carcinomas (59%) and chronic pancreatitis (64%); it was found in only 1.6% of biliary duct stones. Dilatation of intra-and extra-hepatic bile ducts and hydrops of the gall bladder was significantly higher with carcinoma of the pancreas than in chronic pancreatitis.

Morphological investigations in human islets of Langerhans isolated by the Velcro-technic.

The lower yield of viable, isolated islets from one donor pancreas and the immunogenicity are responsible for disappointing results in human islet transplantation. Our preliminary results in the application of Velcro-technic (Lacy, 1982) studied by morphological investigations demonstrate the feasibility of this technic in human islet isolation. If we compare the influence of Velcro-technic with regard to the first step of Velcro-technic, the duct perfusion, we can conclude that A-cell granules, stained by Grimelius, are reduced during the collagenase perfusion. On the other side, we recognized B-cell granules up to 60 min after duct perfusion. Connective tissue is reduced by the isolation technic and absent completely up to the Ficoll separation. The exocrine tissue of the pancreas is damaged continuously, and after Ficoll separation we detected singular acinar cells only. The yield from the used lienal human pancreas were about 80 000 islets. Islets after Ficoll separation, studied by electron microscopic investigation, showed non-damaged A and B cells with granules in the typical manner.

[Endoscopic polypectomy of the stomach].

As a result of selective biopsy of gastric polyps 77 hyperplastic, 36 hyperplasiogenous and 11 adenomas were found. By the introduction of endoscopic polypectomy 42 polyps were resected in toto and 39 could be removed. The histological result of the whole polyp showed that the most frequent form is the hyperplastiougenous type with 42.5%. Comparisons of the histological results of the whole polyp with the particles of the biopsy (selective biopsy) differ in 40%. The combination of the results of antral polyposis (type II) with those of an adenocarcinoma in the material affirmed our thesis that the hyperplasiogenous polyp is an indicator for a higher tendency of the gastric mucosa for developing a carcinoma. Follow-up observations of pseudopolypoid folts showed a spontaneous regression of about 50% after one year and of about 75% after four years. In this article tendencies regarding the relation between the patients age, the growth speed and the histological structure of the gastric polyps are discussed.
