Wolcott-Rallison syndrome: diabetes mellitus and spondyloepiphyseal dysplasia.

In 1972, Wolcott and Rallison described three siblings with a combination of infancy-onset diabetes mellitus and multiple epiphyseal dysplasia. We have observed a brother and sister with the same disorder. The chondro-osseous lesions are those of a spondylo-epiphyseal dysplasia. The diabetes mellitus is relatively mild. Histologic and electron microscopic studies of chondro-osseous tissue show findings similar to those in other epiphyseal and spondylo-epiphyseal dysplasias. In addition, however, atypical collagen-like fibres are found inside and outside chondrocytes. Collagen production seems to be normal in cultured fibroblasts. From the available data it appears that the association of characteristic chondro-osseous and endocrine abnormalities is non-random and that the lesions are independent manifestations of a pleiotropic gene. We propose to call this disorder the Wolcott-Rallison Syndrome.

[Obliteration of the bulla of the guinea pig with a Kieler bone chip].

The obliteration of the sinus frontalis after removal of the chronically inflamed and irreversibly transformed mucous is a common surgical treatment. There is no definite answer to the question of the most suitable material for obliteration. The suitability of the Kieler Knochenspan was, therefore, tested in animal experiments. After trepanation and removal of the mucous of the bulla of 30 guinea pigs, the cave was filled up with Kieler Knochenspan. The animals were killed 2 weeks, 1, 2, 3, and 4 months after implantation. The implanted nonvital spongy substance of the Kieler Knochenspan was first enclosed in granulation tissue. Then, depending on the duration of implantation, this granulation tissue turns into bony substance first peripherically and then centrally. Immunologically conditioned reactions were never observed. Thus, the transformation of the Kieler Knochenspan in the pneumatic system of the guinea pig is the same as in human tubular bones. So the conclusion is drawn that Kieler Knochenspan might be used in the obliteration of the human frontal sinus.

[Anastomotic technique in tracheal resection. Animal experiments (author's transl)].

In 24 rabbits and 6 dogs, two subtotal tracheal transsections each were reanastomosed using Dexon , Vicryl, Prolene and Mersilene in running or interrupted suture technique. In order to avoid traumatisation of the perichondrium, sutures included the marginal cartilagenous ring and all layers of the tracheal wall. By putting every second stitch in a 8-figure, an ideal adaptation could be achieved. Half of the anastomoses were secured by additional application of fibrin tissue adhesive. Macroscopical and histological examinations were carried out 5, 10, 21 and 63 days postoperatively. Mersilene resulted in remarkable inflammatory tissue reaction - increasing from the 5th day on - leading to extensive granulomas, abscesses, ulcerations of the mucosa and erosion of the cartilage. In contrast, by using synthetic absorbable materials as well as prolene, minimal tissue reaction occurred with an even and delicate scar formation. The findings do not support the view, that an extramucosal stitch technique is essential. The additional sealing with fibrin tissue adhesive increased the safety of the anastomoses during the early postoperative period.

[Cadmium-induced vertebral-column ankylosis in whitefish ].

In two healthy and two diseased whitefish (Coregonus Wartmanni) taken from Lake Constance (FRG), ankylosis of the vertebral column was investigated both roentgenologically and histologically. Subsequent to the collapse and necrosis of the "residual" spinal cord within the intervertebral spaces, the outside edges of the vertebral bodies come into direct contact. The compression and tensile forces that occur to an increased extent as a result of the instability, lead not only to a remodelling of the vertebral bodies, but also to the formation of spondylotic osteophytes at the edges of the vertebrae and, as a result of periosteal stimulation, to the development of cellular hyaline cartilage, which fills the intervertebral spaces. Finally, as a result of perichondral ossification, a bony ankylosis develops. The humping of the spine of the fish due to the stiffening and shortening of the vertebral column, is accompanied by a restriction in the animal's freedom of movement. Muscular atrophic processes and disordered food uptake give rise to poor growth and a reduction in the weight of the diseased fish. These remodelling processes in the spine resulting from instability are specific to the periosteum and may be equated with the changes seen in man in spondylosis deformans. The possible cause of this vertebral column ankylosis is cadmium poisoning. The accumulation of this heavy metal obviously leads primarily to an irreversible toxic degeneration of the cells of the chorda dorsalis.

[General partial resection of the mandible in para-osteal cancers of the mouth and oropharynx].

The mandibles of 24 patients with a squamous cell carcinoma of the floor of the mouth or the tongue or the tonsils (all were T2N1b and T3N1b), were histologically examined. It could histologically be shown, that the bone will be destroyed by 2 modalities: 1. Atrophy of the mandible, caused by the pressure of the tumor, leaving the periosteum intact 2. Infiltration of the tumor into the bone. Tumor cells could not be found in the lymphatic system of the periosteum. Since the periosteum is a barrier against tumor growth, the indicator for the resection of the mandible was made after shaved off the periosteum. Resulting from our findings we propose the following concept: 1. If the tumor reaches the mandible (distance tumor to bone = 0 mm) or is fixed to the periosteum and the compacta shows no errosion, after shaving off the periosteum, the mandible can be preserved. 2. If the tumor grows through the periosteum, a marginal resection of the mandible is necessary. 3. If the bone shows errosion, a partial resection of the anterior part of the mandible or a hemimandibulectomy is indicated.

[The use of fibrin tissue adhesive in parenchymal organs of the abdomen. An experimental study in rabbits and dogs (author's transl)].

In rabbits and dogs standardized lesions of the liver, kidney, and spleen were treated by application of human fibrinogen tissue adhesive. In all cases this procedure resulted in a complete control of bleeding as well as a stable and lasting closure of the wounds. During the postoperative observation time of one month, the fibrinogen tissue adhesive was initially infiltrated by leucocytes and macrophages and subsequently resorbed by a granulating tissue, characterized by the early appearance of numerous fibroblasts. Our clinical experience, until now, confirms these promising experimental results.

[Deviations of septum cartilage after unilateral separation of Mucoperichondrium in rabbits (author's transl)].

In 49 rabbits the mucoperichondrium of their nasal septa was strictly unilaterally separated according to the Cottle procedure. In addition, in some rabbits only the surface of the septal cartilage was incised and in others the cartilage was cut vertically or horizontally. Four months after the operation the following alterations of the septa were seen: The separation of the mucoperichondrium from the cartilage or superficial incision of the cartilage led to a deviation to the operated left side. If in addition parts of the cartilage septum were mobilized, thus swinging horizontally or vertically, a deviation to the other nonoperated side was seen. Similarly, if two parallel vertical stripes of cartilage were excised, the ends of the remaining cartilage plate bent and the cartilage plate itself deviated to the nonoperated side. The postoperative deviations are due to released unbalanced forces of the cartilage itself and to the disequilibrium of scar tension resulting from the unilateral separation of the mucoperichondrium. To avoid these postoperative deviations the bilateral tunneling of the mucoperichondrium is required.

[Long-term behavior of aluminumoxide ceramic implantations in the region of pneumatized cavities in animal experiments].

After bilateral resection of the facial wall of the maxillary sinus of five Göttingen minipigs and coverage of the defect with perforated Al2O3 ceramic material, the initial response to the implant was examined clinically and histomorphologically for periods of up to eight months. Except for a wound infection in the initial stages of healing in the case of one animal, no complications were observed after the operation. Three months after the operation, the implants were fixed in the connective tissue. After the fourth month, newly formed compact bone had developed and stabilized in the region of the attachment. The basically uncovered surface of the implant, which was turned toward the maxillary sinus, was covered with a non-irritating respiratory mucous membrane. No immunologic responses were observed. Bio-inert reaction, stable tissue-bony anchorage, and coverage with functioning mucous membrane justify the insertion of Al2O3 ceramic implants to cover defects in pneumatized cavities.
