[Early diagnosis and treatment of Alzheimer's disease. Implementation in the doctor's office].

The efficacy of antidemential agents proven in comprehensive studies and by clinical experience, now justifies an active and positive approach by the general physician to the diagnosis and treatment of patients with dementia. The proposals on how to implement diagnostic and therapeutic measures in the doctor's office comply both with medical quality criteria and the requirements for appropriateness of treatment and considerations of economy stipulated by German law. They therefore provide the basis for a modern diagnostic work-up and treatment strategy, which will also meet economical demands.

[Delusional depression as differential dementia of the Alzheimer type diagnosis].

For several years, a 68-year-old patient had been treated for a dementia of Alzheimer's type and finally admitted to a closed gerontopsychiatric nursing home. According to information from his relatives, he first developed psychotic symptoms 3 years prior to admission in our psychiatric department. Several months later, he developed a lack of drive, lack of interests, and reduced emotional reaction. After a standard diagnostic work-up (psychiatric, neurological, and general medical examination, CSF examination, laboratory analyses, cranial computerized tomography), we began electroconvulsive therapy (ECT) with a working hypothesis of major depression with psychotic symptoms. The ECT was able to ameliorate rapidly the psychiatric status of the patient. After 10 weeks of inpatient treatment, he could be discharged from the hospital and was able to take up his usual professional activities on a voluntary basis. This article describes the need for an early differential diagnosis of dementias. Standardized and differential diagnoses of dementias are necessary for an antidementive therapy as well as for the detection of potentially curable diseases.

[Early and differential diagnosis of 1,000 patients examined in a memory clinic].

The results from 1000 patients included in a consecutive sample of older persons showing signs of "age-related memory deficits" clearly demonstrate the necessity for a wide spectrum of differential diagnostic competence. The patients included in the study were diagnosed in succession by an interdisciplinary team of psychiatrists, neurologists, geriatric medical specialists, psychologists and gerontologists. The diagnostic process for clarification of DSM-III-R and ICD-10 criteria are discussed in detail. In all, 49.6% of the patients were diagnosed as suffering from dementia of the Alzheimer type, 31% from vascular dementia and 10% from a mixed form. In all, 12.5% of the patients were somatically ill and 31.4% displayed other psychiatric conditions, 50% of which were evaluated as being treatable with psychotherapy. The results are primarily discussed for their relevance to the reality of current treatment.

[Euphoric effect of lidocaine].

A 42 year old drug addict reported on his experiences with intravenous application of scandicaine and lidocaine. Lidocaine led to short-lasting euphoria comparable with former cocaine-induced effects. Pharmacological hypothesis to explain these mental alterations are discussed.

[Guidelines for integrated management of demented patients in nursing homes for the elderly].

Currently, 30% and above of residents of geriatric care centers are suffering from dementia, most of them in an advanced stage of disease. In 1987, the "Landschaftsverband Rheinland" introduced the so-called "Zuschlagsverfahren" (i.e., additional nursing fees) to promote an integrative care concept for demented people in these institutions. This procedure includes expert rating of the residents, "screening" of the quality of care and teaching of the institutional staff in the field of gerontopsychiatry. The introduced guidelines for integrated care of demented residents of geriatric care centers emerged from practical work in and for such institutions. The guidelines emphasize the inmate-oriented care and housing concept, focusing on the individual biography and on qualified personnel management. An organizational structure is needed that supports the revised care and housing concept.
