[The expression of MMPs and TIMPs in human breast cancer tissues and importance of their balance in cancer invasion and metastasis].

Tumor cells degrade extracellular matrix components (ECM) to invade surrounding tissues. Malignant tumor cells are known to produce various ECM-degrading enzymes including matrix metalloproteinases (MMPs), serine proteinases and cathepsins. Among them, MMPs may play a key role in cancer invasion and metastasis. To study the role of MMPs in the progression of human breast carcinomas, we examined production and tissue localization of MMP-1, MMP-2, MMP-3, MMP-9 and their common inhibitors, tissue inhibitors of metalloproteinases (TIMP-1 and TIMP-2). The data suggest that the imbalance between MMPs and TIMPs produced by tumor tissues may be a determinant of the progression in breast carcinoma.

[Combination chemotherapy with FP versus FEP in patients with advanced gastric cancer. Research group of gastric cancer chemotherapy].

Forty-eight patients with unresectable gastric cancer were enrolled for a randomized trial of FP (n = 24) vs FEP (n = 24) combination chemotherapy with respect to their effects on survival period. Of these, excluding 7 patients, 21 in FP group were treated with 400 mg/m2 UFT po daily and 50 mg/m2 CDDP i.v. on days 1 and 8 every 4 weeks, and 20 in FEP group were treated with 400 mg/m2 UFT po daily, 50 mg/m2 etoposide iv and 50 mg/m2 CDDP i.v. on days 1 and 8 every 4 weeks. In FP group, 10 PR in primary lesions were observed with an overall response rate of 52.4%. In FEP group, 5 PR in primary lesions were observed with an overall response rate of 35.0%. Toxicities over Grade 3 were anorexia (35%), vomiting (25%), and leukopenia (12%), which were manageable. The 50% survival time in FP and FEP group was 233 and 176 days, respectively. Because of the high response rate and prolongation of the survival period, combination chemotherapy with FP seemed to be useful for the treatment of patients with advanced gastric cancer.

Production of monoclonal antibody reactive with leukotoxin and its immunohistochemical application.

A sensitive enzyme-linked immunosorbent assay (ELISA) using a monoclonal antibody (ALM-1) was developed for the determination of leukotoxin (9,10-epoxy-12- octadecenoic acid), which was reported to exist in human burned skin and neutrophils, and was regarded as a toxic and/or defensive substance in living beings. The leukotoxin was conjugated with bovine serum albumin (BSA) by means of the mixed anhydride technique as immunogen, and BALB/c mice were immunized over 6 months. The detection limit of leukotoxin was at least as low as 5 ng in this ELISA. This antibody had a strong specificity to leukotoxin and no cross-reactivity with the other analogs tested, and was usable for an immunohistochemical application. By using the antibody, leukotoxin was immunohistochemically observed not only in neutrophils, but also in alveolar macrophages in an oxygen-exposured rat lung.

Quantitation of the glycation intermediate 3-deoxyglucosone by oxidation with rabbit liver oxoaldehyde dehydrogenase to 2-keto-3-deoxygluconic acid followed by high-performance liquid chromatography.

A simple and sensitive method for the detection of 3-deoxyglucosone was developed using oxidation with crude oxoaldehyde dehydrogenase to 2-keto-3-deoxygluconic acid followed by high-performance liquid chromatography (HPLC). Oxoaldehyde dehydrogenase was prepared from rabbit liver and partially characterized. 2-Keto-3-deoxygluconic acid produced from 3-deoxyglucosone by oxoaldehyde dehydrogenase was derivatized with 1,2-diamino-4,5-methylenedioxybenzene, and the fluorescent products were detected and quantitated by HPLC using a solvent containing borate. In the presence of borate, 2-keto-3-deoxygluconic acid was completely separated from N-acetylneuraminic acid. The detection limit of 3-deoxyglucosone was 2.5 pmol/injection (10 microliters) at a signal-to-noise ratio of 3. This method was used to confirm the inhibitory effect of aminoguanidine on glycation.

Recurrent Breast Cancer at 21 years after Resection Detected by Serum Tumor Markers and Manifested as Dermatomyositis.

A 52-year-old Japanese woman developed dermatomyositis. She had undergone a standard radical mastectomy for left breast cancer 21 years earlier. Though no physical sign of recurrent breast cancer appeared clinically, levels of tumor markers were abnormally elevated. Therefore, tamoxifen and CAF therapy were given. Further, the clinical course of dermatomyositis almost paralleled the level of serum tumor markers and the clinical course of her recurrent breast cancer. These markers were useful for detecting the recurrence, following the metastatic disease, and monitoring her response to therapy.

Determination of ascorbic acid in human plasma by high-performance liquid chromatography with electrochemical detection using a hydroxyapatite cartridge for precolumn deproteinization.

Ascorbic acid (AA) was determined in human plasma by high-performance liquid chromatography with electrochemical detection using a hydroxyapatite cartridge for plasma deproteinization. The proposed method is simple, rapid (deproteinization time, within 1 min; analysis time, ca. 15 min), sensitive [detection limit, ca. 240 ng/ml plasma (at a signal-to-noise ratio of 2:1)], highly selective and reproducible [relative standard deviation, ca. 2.8% (n = 3)]. The calibration graph for AA was linear in the range 0.1-10 ng per injection (20 microliters). The recovery of AA was over 90% by the standard addition method.

Novel precolumn deproteinization method using a hydroxyapatite cartridge for the determination of theophylline and diazepam in human plasma by high-performance liquid chromatography with ultraviolet detection.

The deproteinization of human plasma was carried out using a hydroxy apatite cartridge as a precolumn. After human plasma had been passed through the cartridge followed by suitable elution, protein-free eluate was obtained within only 1 min without the need for centrifugation. Deproteinization was evaluated by the determination of albumin, gamma-globulin and transferrin in the eluate by high-performance liquid chromatography (HPLC) with UV detection. Determination of theophylline and diazepam in human plasma was performed by HPLC with UV detection. The proposed method was suitable for the determination of these two drugs in human plasma, because it is simple and rapid (retention time of each drugs approximately 15 min) and microamounts of sample are required (50-100 microliters). The calibration graphs for theophylline and diazepam were linear in the range 0.1-10 micrograms and 0.1-65 ng, respectively. Recoveries of both drugs were over 90% by the standard addition method.

Proliferative cell nuclear antigen expression in follicular tumours of the thyroid with special reference to oxyphilic cell lesions.

The expression of proliferative cell nuclear antigen (PCNA) in follicular tumours of the thyroid was examined by immunohistochemistry. Both usual nonoxyphilic cell follicular tumours (non-OCT) and oxyphilic cell tumours (OCT) were subdivided into benign, indeterminate, encapsulated carcinoma, and widely invasive carcinoma types. Among non-OCT the percentages of PCNA-positive cells in benign tumours, encapsulated carcinomas, and widely invasive carcinomas was 2.5%-8.6%, 11.8%-39.1%, and 18.6%-20.0%, respectively. There was a statistically significant difference between benign tumours and encapsulated or widely invasive carcinomas, as in previous studies. A value of 10% was appropriate to distinguish benign from malignant lesions. PCNA-positive cells in indeterminate-type non-OCT were not significantly different from those in benign tumours, ranging from 4.3%-19.6%, and occurring at more than 10% in three of six tumours. Among OCT the positivity was less than 10% in benign tumours (4.5%-7.8%) and more than 10% in malignant tumours (14.1%-35.9%) and all the eight indeterminate tumours (12.5%-27.3%), with a statistically significant differences between the benign tumour and each of the latter types. These results indicate that the examination of PCNA is valuable in diagnosis of thyroid follicular tumours and that the use of similar diagnostic criteria may be warranted in both non-OCT and OCT.

Expression of adhesion molecules and HLA-DR by macrophages and dendritic cells in aphthoid lesions of Crohn's disease: an immunocytochemical study.

The phenotypes and ultrastructure of macrophages and dendritic cells in aphthoid lesions of the colon were immunocytochemically observed in patients with Crohn's disease. Biopsy specimens were endoscopically obtained from both aphthoid and advanced lesions in Crohn's disease patients. Biopsy specimens obtained from patients with infectious colitis and from normal individuals served as controls. Aphthoid lesions contained densely aggregated CD68+ macrophages, which were surrounded by numerous ID-1+ dendritic cells. In the normal controls and infectious colitis patients, however, a few scattered CD68+ macrophages and ID-1+ dendritic cells were noted beneath the surface epithelium. CD3+ lymphocytes were significantly increased in both aphthoid and advanced lesions of Crohn's disease, but the CD4/CD8 ratio was similar in all groups studied. The double immunoperoxidase staining method revealed that both CD68+ macrophages and ID-1+ dendritic cells in the aphthoid lesions simultaneously expressed ICAM-1 and HLA-DR antigens. Electronmicroscopic observation revealed that CD68+ macrophages had numerous vesicles and lysosomal granules and few projections, and that ID-1+ dendritic cells had appreciable cytoplasmic protrusions with a few vacuoles. These findings suggested that the colonic mucosa in Crohn's disease contained two types of macrophage/dendritic cells in the same lineage that expressed intercellular adhesion molecules and class-II MHC antigens. It also appeared that the aphthoid lesions of Crohn's disease featured an increase in macrophages and dendritic cells consistent with immunological activation.

Interleukin 6-producing gastric carcinoma with fever, hypergammaglobulinemia, and plasmacytosis in bone marrow.

A 42-year-old man with a remittent fever was found to have both para-aortic and hepatic tumors with generalized lymphadenopathy. The pathological findings from biopsy specimens from the para-aortic lymph node and hepatic tumor by laparotomy and from left supraclavicular lymphadenectomy showed undifferentiated carcinoma. However, the location of the primary lesion could not be determined. Chemotherapy temporarily reduced the size of the metastatic lymph nodes and the hepatic tumor and also suppressed the remittent fever. Fifteen months after onset, massive polyclonal hypergammapathy developed together with plasmacytosis (30% plasmacytes) in bone marrow, consisting of normal mature plasmacytes. Among cytokines, including interleukin (IL) 1 beta, IL-3, IL-4, IL-6, and tumor necrosis factor alpha, a high value of IL-6 was detected in the serum. Postmortem pathological examination showed the scirrhous type of gastric carcinoma with specific staining by polyclonal antibody against human IL-6. To our knowledge, this is the first report of a gastric carcinoma producing IL-6 associated with fever, hypergammaglobulinemia, and plasmacytosis in bone marrow.

Endoscopic injection sclerotherapy for esophageal varices during pregnancy.

Pregnancy is uncommon in patients with portal hypertension, and the correct treatment for pregnant women with esophageal varices has not yet been established. There have been only a few reports on endoscopic injection sclerotherapy (EIS) during pregnancy for the control of variceal bleeding. Here we report two women with esophageal varices who underwent EIS. One was a pregnant woman with variceal hemorrhage who was saved by emergency EIS and was delivered safely. The second woman underwent prophylactic EIS for esophageal varices before becoming pregnant. This woman later gave birth uneventfully. Thus EIS appears to be a useful treatment for pregnant women with esophageal varices.

Endoscopic ultrasonography with color Doppler function in the diagnosis of rectal variceal bleeding.

Extraesophagogastric variceal bleeding in patients with portal hypertension is rare, but has been reported after endoscopic injection sclerotherapy (EIS) for esophageal varices. Here, we report rectal variceal hemorrhage following EIS. Endoscopic ultrasonography with color Doppler function (EUSCD) was useful for the diagnosis of rectal varices and complete hemostasis was obtained with EIS. A review of the literature since 1980 revealed a total of 15 patients, including ours, with extraesophagogastric variceal bleeding following EIS. This suggests EIS is the principal emergency treatment for bleeding rectal varices.

Identification and dynamics of paraquat in the bone marrow, thymus and spleen in rats using immunohistochemical techniques.

We reported the immunohistochemical identification of paraquat in the immune and hematopoietic systems such as bone marrow, spleen and thymus. Twelve paraquat-poisoned rats were sacrificed in 3 h, 12 h, 3 days, 7 days and 10 days after intravenous administration of paraquat. In the bone marrow, paraquat was localized in several types of blood cells (granulocyte, erythrocyte and megakaryocyte) and their precursors between 24 h and 7 days after the injection. In the thymus, paraquat was mainly localized in the medulla between 12 h and 7 days after the injection, whereas in the spleen, it was mainly localized in the red pulp between 12 h and 10 days after administration of paraquat.

Clinical value of enzyme immunoassay of epidermal growth factor receptor in human breast cancer.

Epidermal growth factor receptor (EGFr) levels were analyzed in 140 primary breast cancer specimens by immunohistochemical assay (ICA), competitive binding assay (BA), or enzyme immunoassay (EIA). Thirty-nine of 118 specimens (33.1%) were scored as positive by ICA, 30 of 116 (25.9%; cut-off level 10 fmol/mg protein) by BA, and 31 of 80 (38.9%: cut-off level 5 fmol/mg protein) by EIA. Agreement on EGFr status was 72.3% (68/94) between ICA and BA, 77.0% (57/74) between BA and EIA, and 73.8% (59/80) between EIA and ICA. These discrepancies are based on assay differences and the heterogeneous distribution of cancer cells within specimens. Regardless of the assay method used, EGFr status had a significantly negative correlation with estrogen receptor status. Although EGFr-ICA and BA status had no relationship with prognosis, patients with medium and high EGFr-EIA level tumors (over 5 fmol/mg protein) had shorter relapse-free periods than those with low level tumors. However, the prognostic value of positive EGFr-EIA status was weaker than that of c-erbB-2 overexpression.

Analysis of oncogenes and tumor suppressor genes in human breast cancer.

Oncogenes (c-erbB-2, c-myc, and some genes linked to the 11q13 lesion), tumor suppressor genes (retinoblastoma gene, p53) and an antimetastatic gene (nm23/nucleoside diphosphate kinase) play important roles in breast cancer progression. Amplification of c-erbB-2, c-myc, and int-2, and expression of RB, p53(mutant), and NDP kinase were determined in 77 primary breast cancer specimens. nm23-H1 allelic loss was also studied. c-erbB-2 and c-myc amplification, loss of RB expression, p53(mutant) expression, and nm23-H1 allelic loss were also found in non-invasive carcinoma. int-2 amplification was significantly correlated with lymph node status (P = 0.02) and a significant association was found between p53(mutant) expression and tumor size (P = 0.04). c-erbB-2 amplification was strongly associated with disease-free and overall survival in multivariate analysis (P = 0.002). All of the c-erbB-2 amplified cases and all but one of the int-2 amplified cases in node-positive patients had relapsed within 2 years post resection. The cancer cells may acquire new proliferative pathways sequentially as a result of multiple genetic alterations which enable them to bypass the estrogen-dependent proliferation.
