Molecular cloning and expression in Escherichia coli of the Bacillus licheniformis bacitracin synthetase 2 gene.

The structural genes for the entire bacitracin synthetase 2 (component II) and for a part of the putative bacitracin synthetase 3 (component III) from Bacillus licheniformis ATCC 10716 were cloned and expressed in Escherichia coli. A cosmid library of B. licheniformis DNA was constructed. The library was screened for the ability to produce bacitracin synthetase by in situ immunoassay using anti-bacitracin synthetase antiserum. A positive clone designated B-15, which has a recombinant plasmid carrying about a 32-kilobase insert of B. licheniformis DNA, was further characterized. Analysis of crude cell extract from B-15 by polyacrylamide gel electrophoresis and Western blotting (immunoblotting) showed that the extract contains two immunoreactant proteins with high molecular weight. One band with a molecular weight of about 240,000 comigrates with bacitracin synthetase 2; the other band is a protein with a molecular weight of about 300,000. Partial purification of the gene products encoded by the recombinant plasmid by gel filtration and hydroxyapatite column chromatography revealed that one gene product catalyzes L-lysine- and L-ornithine-dependent ATP-PPi exchange reactions which are characteristic of bacitracin synthetase 2, and the other product catalyzes L-isoleucine-, L-leucine, L-valine-, and L-histidine-dependent ATP-PPi exchange activities, suggesting the activities of a part of bacitracin synthetase 3. Subcloning experiments indicated that the structural gene for bacitracin synthetase 2 is located near the middle of the insert.

An analogue of thyrotropin-releasing hormone, DN1417, decreases naloxone binding in the rat brain.

We explored whether thyrotropin-releasing hormone may affect opioid receptors in the rat brain. Adult male rats were intraperitoneally injected twice a day with varying doses of DN1417, a potent analogue of thyrotropin-releasing hormone, for 2 days, and opioid receptors of the brain (hypothalamus, striatum, hippocampus, midbrain, ponsmedulla, and cortex) homogenates were determined using [3H]naloxone. Intraperitoneal injection of DN1417 in a dose of 0.3 mg/100 g body wt resulted in a significant reduction in naloxone binding of the striatum as compared with the saline-injected group, whereas naloxone binding of other brain regions was not affected by DN1417. DN1417 produced a dose-dependent decrease in naloxone binding of the striatum. The affinity constant of naloxone binding was similar between the saline- and DN1417-injected groups. In vitro addition of DN1417 did not interfere with the brain naloxone binding. The distribution of 3H-labeled DN1417 injected peritoneally did not differ among the brain regions. The present data imply that the opioid antagonistic action of thyrotropin-releasing hormone may be due, at least in part, to the significant decrease in the striatal opioid binding in the rat brain.

[Anti-tumor effect of LAK (lymphokine activated killer) cells against human bladder tumors transplanted to nude mouse].

The effect of lymphokine-activated killer (LAK) cells on bladder tumor was examined in vivo and in vitro. In the in vitro experiment, 51Cr-cytotoxic assay was performed for which PBL were used as effector cells. A LAK activity of 26.6% was observed in PBL cultured with IL2 for 4 days, whereas OK-432-induced LAK activity was 22%. Furthermore, in the in vivo experiment, the anti-tumor effect of LAK cells was evaluated in human bladder tumor transplanted into nude mouse. IL2, OK432-induced LAK cells were injected intratumorally. In the LAK-treated group, inhibition of tumor growth was seen. Histologically, it was demonstrated that infiltrating lymphocytes were scattered around tumor cells. The augmentation of NK activity in spleen cells was observed in the LAK-treated group. Although further studies are required to establish its full significance, these findings suggest that immunotherapy against bladder tumors is hopeful.

Calcium-independent activation of contractile apparatus in smooth muscle by calyculin-A.

Calyculin-A (CL-A), a novel marine toxin isolated from Discodermia calyx, caused contraction in the smooth muscle of guinea pig taenia ceci and rat aorta in the presence or absence (with 1 mM ethylene glycol bis(beta-aminoethyl ether)-N,N'-tetraacetic acid) of external Ca++ at concentrations ranging from 1 X 10(-8) to 1 X 10(-6) M. In the presence of external Ca++, the contraction induced by CL-A was accompanied by an increase in the cytosolic free Ca++ concentration [( Ca++]cyt) as measured by the fluorescence indicator fura-2. Verapamil (3 X 10(-6) M) inhibited the increase in [Ca++]cyt, but not tension development caused by CL-A. In the absence of external Ca++, CL-A still caused contraction without changing [Ca++]cyt. Thus, from studies with intact smooth muscle it was demonstrated that, in the absence of external Ca++, CL-A can induce a contraction that was not accompanied by an increase in [Ca++]cyt. In permeabilized taenia, CL-A caused contraction in the absence of Ca++ (with 2 mM ethylene glycol bis(beta-aminoethyl ether)N,N'-tetraacetic acid) at concentrations similar to those required to contract intact tissue. This contraction was inhibited by the nonselective kinase inhibitors such as amiloride (1 X 10(-3) M) and K-252a (2 X 10(-5) M). Low concentrations of Ca++ (approximately 1 X 10(-6) M) augmented the CL-A-induced contraction in the permeabilized taenia. In native actomyosin prepared from chicken gizzard CL-A induced phosphorylation of the 20 kDa myosin light chain (MLC) in the absence of Ca++.(ABSTRACT TRUNCATED AT 250 WORDS)

Prevention of limb ischemia in surgical patients by intra-aortic balloon pumping.

Intra-aortic balloon pumping (IABP) was recently performed during the perioperative period in seven patients in whom balloon insertion was difficult because of vascular complication. Prophylactic measures for the prevention of limb ischemia were taken when the balloon was inserted. These measures included (1) balloon insertion through the femoral artery and continuous infusion of heparin into the femoral artery in three patients, (2) balloon insertion through the left femoral artery and femoro-femoral arterial crossover bypass in two patients, and (3) balloon insertion through the left common iliac artery and axillo-femoral bypass on the same side in two patients. Most patients recovered satisfactorily without any evidence of ischemia of the limbs either clinically or in serum biochemistry examination. One patient died of prolonged cardiac failure.

[Interest of percutaneous transvenous mitral commissurotomy in the management of rheumatic mitral stenosis].

During from April 1987 to October 1988, 13 cases of mitral stenosis (MS) were treated with percutaneous transvenous mitral commissurotomy (PTMC) among 24 cases of MS. The indications of PTMC were determined by surgeons in 4 cases with several reasons described as follows: 1. Patient, having been treated with closed mitral commissurotomy, who refused to be operated with open heart technique. 2. Patient associated with early gastric cancer. 3. Patient associated with severe hyperthyroidism and cardiac cachexia. 4. Patients suffering from acute renal failure following left heart failure due to association of aortic stenosis. Other 9 cases were determined by cardiologist because of the inherent benefit of PTMC being less invasive. After PTMC, the symptomatic improvement, assessed by means of NYHA classification, were observed in 11 cases out of 13. Hemodynamic data such as mitral valvular area, mitral valve gradient and cardiac index showed marked improvement without any inducing of significant mitral regurgitation. The authors thought with these results as follows: 1. The effect of PTMC may be appreciable for mild or moderate stenotic lesion of mitral orifice, however, severely affected valves should be treated by open heart techniques in order to perform radical procedures. Otherwise, significant MR may occur by overloading of balloon size beyond some extent. 2. On account of the appearance of PTMC, the indicational determination for the treatment of MS will be more controvertial than now.

[A case of complete disruption of right truncus intermedius treated by operation].

A case of right bronchial disruption caused by blunt chest trauma was successfully operated. Patient is a 35-year-old woman who was transferred to our intensive care unit with markedly dyspnea and subcutaneous emphysema. Rupture of right bronchus was suspected by chest x-ray film and bronchial fiberscopy. An emergency thoracotomy was performed with the aid of selective lung ventilation by the use of double lumen endobronchial tube. Right bronchus was completely transected and small part of upper lobe was lacerated. Right bronchus was anastomosed directly with interrupted 3-0 vinyl sutures and laceration of upper lobe was sutured. Postoperative course was uneventful, but postoperative bronchoscopy confirmed slight stenosis of the anastomotic position. Prompt diagnosis by bronchoscopy and emergency thoracotomy followed by selective lung ventilation are mandatory in this patient survival.

[Intrapleural administration of recombinant interleukin-2 in a patient with pleural effusion due to non-Hodgkin's lymphoma].

A 79-year-old female was admitted with dyspnea. Chest roentgenogram showed massive left-side pleural effusion. Chest CT scan and abdominal CT scan revealed marked swelling of mediastinal and para-aortic lymph nodes. Diagnosis of non-Hodgkin's lymphoma was made by pleural fluid cytology. Recombinant interleukin-2 (1000 units/day) was administered into the pleural cavity for 14 days continuously. Clinically, pleural effusions and malignant cells in the effusions disappeared. Immunologically, levels of IL-2 receptor positive cells, soluble IL-2 receptors and CD4 positive cells in the pleural effusion increased 7 days after recombinant IL-2 administration with subsequent decrease after completion of treatment. On the other hand, levels of IL-2 receptor positive cells, soluble IL-2 receptors and CD4 positive cells in the peripheral blood increased with no subsequent decrease after completion of treatment.

[Dental health status of junior high school students in the suburbs of Chiba city].

The Population of 12-15 years is a key age group for oral health because the permanent dentition and periodontal tissues are almost complete at this stage. The purpose of this study was to evaluate oral health status and to develop an effective school dental health program for junior high schools students. The survey was performed on 527 persons aged 12-13 at 2 junior high schools in the suburbs of Chiba city. Periodontal disease was evaluated on the basis of the CPITN, and analyzed for severity and region of periodontal disease. Gingivitis was examined with PMA index, using color photo slides taken of anterior dentition at the time of inspection. At the same time, caries experience and oral hygiene index were also estimated. The results were as follows: (1) The mean DMFT was 3.89. Caries prevalence in females was higher than males. Ratio of treated teeth rate was 77.03%; that of untreated teeth rate was 22.65%. Females revealed less treatment for caries than males. Oral hygiene index showed significant difference between males and females. Labial and buccal parts of the maxillary and molar region on the mandibular showed remarkable plaque deposition. Calculus accumulation in this population was greatest on the lower incisor. (2) The mean PMA index was 7.36. Gingivitis was highly prevalent in both sexes, although it was more severe in males. There was correlations between PMA index and OHI. (3) From the results of CPITN, 10.6% of the subjects showed no signs of periodontal disease (Code 0), and 5.7% exhibited gingival bleeding (Code 1).(ABSTRACT TRUNCATED AT 250 WORDS)

A functional new experimental biventricular model of heterotopic cardiac transplantation.

A heterotopic cardiac transplantation model in which the donor heart was able to maintain the systemic and pulmonary circulation of the recipient was devised, which did not require extracorporeal circulation or heparinization. All donor hearts in five pairs of adult mongrel dogs were resuscitated, and the systemic and pulmonary circulations maintained by the donor hearts alone were studied hemodynamically up to 3 hr after resuscitation, although long-term survival was not achieved.

[Soluble interleukin-2 receptor levels in patients with pulmonary tuberculosis].

Utilizing an enzyme-linked immunosorbent assay, we detected markedly elevated serum levels of soluble interleukin-2 (IL-2) receptors in patients with untreated pulmonary tuberculosis. In these patients, we also found that serum levels of soluble IL-2 receptors were closely correlated with serum adenosine deaminase levels (r = 0.869, p less than 0.001). Therefore, serum soluble IL-2 receptors appear to reflect the existence of active cell-mediated immunity in pulmonary tuberculosis and may prove to be a useful immunological marker for pulmonary tuberculosis.

[The relationship between gestational weeks and fetal breathing movements in each pattern of the fetal heart rate].

Fetal breathing movement (FBM) was analysed in each fetal heart rate (FHR) pattern and the gestational 2 weeks. FHR pattern was divided in automated FHR analysis into 3 groups; active, intermediate and resting patterns. The patients consisted of 102 pregnant women at 30-41 weeks, including 13 complicated pregnancies. 1) The active pattern percentage was 52.7% at 32-33 weeks and it decreased gradually. That of the resting pattern was 25.5% at 32-33 weeks and it increased gradually. 2) The percentages of positive FBM in the active pattern were significantly larger than those in the resting pattern in each 2 weeks. 3) The duration of continuous FBM ranged from 3 to 202 sec. FBM of long duration tended to exist in the resting pattern rather than in the active pattern. 4) The percent time ratio of FBM ranged from 1.0 to 92.7% and showed no definite relationship with the FHR patterns or gestational weeks. 5) The FBM rates in each FHR pattern showed no significant differences. Their sequential differences after 36 weeks were significant; that is, FBM in the resting pattern was more regular than that in the active pattern. 6) In high risk pregnancy, the resting pattern percentage was 52.3% and the ratio of positive FBM in the resting pattern was significantly smaller than that in active pattern.

[A case of pericardial cyst associated with atrial septal defect].

A 51-year-old woman with an abnormal shadow in the right upper mediastinum and systolic heart murmur was diagnosed as mediastinal cyst and atrial septal defect (ASD). Before the operation, the cyst was suspected as bronchogenic cyst or cystic thymoma based on the findings by CT and NMR. Both cystectomy and closure of ASD was successfully performed and the cyst was diagnosed as pericardial cyst by pathological examination. A brief of literature on the pericardial cyst is given.

[A study of peripheral blood lymphocyte subsets in patients with bladder tumors].

The analysis of peripheral blood lymphocytic subpopulations was conducted with 16 bladder tumor patients. Lymphocytes were collected centrifugally from the peripheral blood of preoperative patients with bladder tumors, and measured by means of flow cytometry. Among the stages of the bladder tumor patients, there were no clear relationships in the ratio between T cells and B cells. However, with the progression in stages a rise in OKT 8, as well as the lowering of the OKT 4/OKT 8 ratio were discerned. In relation to pTis, Leu 7 showed a higher value than that in other groups with advanced invasion. This suggests that in measurements of lymphocyte subsets in peripheral blood, it will to a certain extent be possible to estimate the preoperative immunological competence of bladder tumor patients.

[A case report of infective endocarditis with acute pulmonary edema and renal dysfunction treated by emergent mitral valve replacement and postoperative extracorporeal ultrafiltration].

A 40-year-old woman with staphylococcus aureus endocarditis of the mitral valve associated with acute pulmonary edema and renal dysfunction is presented. The patient was admitted to Hiroshima University Hospital with infective endocarditis. On the 14th day after admission, she suffered from severe cardiac failure and oligouria, then she was transferred ICU. Chest X-ray film showed pulmonary congestion and echocardiogram revealed 4th grade of mitral valve regurgitation. Emergent mitral valve replacement was performed and rupture of anterior mitral chorda was found as the cause of acute pulmonary edema. Postoperative care was difficult because of advanced renal failure and cardiac failure not responded to diuretics. Extracorporeal ultrafiltration method was effectively used on the 1st and the 2nd postoperative days and 3000 ml of water was filtered without hemodynamic change. Symptoms of renal and cardiac failure recovered promptly after ultrafiltration. Emergent operative and postoperative use of ultrafiltration method is effective in some cases of infective endocarditis complicated with cardiac and renal failure.

[A case report of successful aortic and mitral balloon valvotomy in patient with acute renal failure].

An effective percutaneous transvenous mitral commissurotomy (PTMC) and aortic valvotomy (PTAV) was performed upon a 61-year-old male patient suffered from acute left heart failure due to combined valvular disease (mitral and aortic stenosis) and had complicated with acute renal failure following low cardiac output. Hemodynamic study being examined through Swan-Ganz catheter and echocardiography after PTMC and PTAV showed marked releaf from these two valvular stenosis. We consider that although this new treatment can't exceed surgical commissurotomy in an effect, it can be applied to those who are thought to be riskfull for the surgical intervention because of its inherent characteristics being simple and less invasive.
