The long-term survival of patients with carcinoma of the main hepatic duct junction.

Carcinoma of the main hepatic duct junction tends to invade extensively the bile ducts and hepatic parenchyma, although dissemination is rarely seen. Therefore, extensive resection of the bile ducts combined with hepatic resection is the procedure of choice for treating this disease. From January 1973 to January 1987, 23 of 41 patients underwent resection, giving a resectability rate of 56%. One patient died postoperatively, yielding an operative mortality rate of 4.3%. The 5-year actuarial survival rate is 29.8%. Three patients are now alive and well 6 years and 9 months, 5 years and 10 months, and 5 years and 5 months after the operation. One additional patient who underwent resection in an affiliated hospital has done well for 8 years and 8 months. The results from these four patients treated by curative resection support a strategy featuring curative resection with aggressive surgery.

Identification of testicular atypical germ cells by an immunohistochemical technique for placental alkaline phosphatase.

The identification of atypical testicular germ cells is often difficult by by routine histologic examination. By immunohistochemical detection of placental alkaline phosphatase (PLAP) and by periodic acid Schiff staining of glycogen, atypical germ cells were easily identified in testicular samples. Forty-one fetal and adult testes were used for a preliminary study, and 121 testes from infants and adults with either cryptorchidism or germ cell tumors were studied for the presence of atypical germ cells. Two types of clear germ cells were differentiated histochemically, and one with PLAP-positive cell surfaces and glycogen-rich cytoplasm was considered to be atypical. The alkaline phosphatase of atypical germ cells appeared to be similar to that found in a few germ cells of early fetal testes. The atypical germ cells seemed to be multi-potential malignant cells capable of developing not only into seminoma but also into other germ cell tumors. Only in yolk sac tumor of infants were the atypical germ cells absent from tumor-adjacent seminiferous tubules.

Small cell carcinoma (non-oat cell type) of the esophagus concomitant with invasive squamous cell carcinoma and carcinoma in situ. A case report.

A case of double primary invasive carcinoma of the esophagus, consisting of well-differentiated squamous cell carcinoma and non-oat cell small cell carcinoma without squamous differentiation, is presented. This is the first reported case of a double or multiple primary invasive carcinoma of the esophagus in which one component is small cell carcinoma (oat cell or non-oat cell). Furthermore, the mucosal epithelium around the non-oat cell small cell carcinoma revealed multiple dysplasia and carcinoma in situ. These lesions were definitely separated from the invasive carcinoma and from each other. The results suggest that pure non-oat cell small cell carcinoma of the esophagus without squamous differentiation is derived from the esophageal squamous epithelium, and is a variant of squamous cell carcinoma.

Histogenesis of alveolar soft part sarcoma. An immunohistochemical and biochemical study.

In order to clarify the histogenesis of alveolar soft part sarcoma (ASPS), an immunohistochemical and biochemical study was performed on three cases. The immunohistochemical study indicated the presence of actin, desmin, vimentin, and Z-protein in all cases. On the other hand, intermediate filaments other than desmin and vimentin were not detected immunohistochemically. The presence of desmin and Z-protein strongly suggests the myogenic character of this tumor. As to whether ASPS shows striated muscle differentiation or smooth muscle differentiation, the immunohistochemical absence of myoglobin in the three cases suggests that the tumor does not differentiate in the direction of striated muscle. However, biochemical assay of subunits of enolase revealed significantly high amounts of beta-enolase, which is known as a marker for striated muscle, in all three cases. The determined values--735, 426, and 584 ng/mg of protein --are indicative of striated muscle differentiation. In addition, the immunohistochemical study of all cases revealed the presence of beta-enolase in tumor cells. These data definitely show the myogenic character and rhabdomyoblastic differentiation of ASPS.

Expression of neurofilament triplet proteins in human neural tumors. An immunohistochemical study of paraganglioma, ganglioneuroma, ganglioneuroblastoma, and neuroblastoma.

Intermediate filaments which are specific to neural cells, ie, neurofilaments, consist of three subcomponents--68, 150, and 200 kd. Thirty human neural tumors were examined for the presence of these three subcomponents by means of their monospecific antisera. All 8 paragangliomas contained cells that were positive for the 68-kd component, but only 5 of them had cells positive for the 150-kd and 200-kd components. All 4 ganglioneuromas and 11 ganglioneuroblastomas contained cells that reacted with antibodies to all three components. All 7 neuroblastomas had cells reacting with antibody to 68 kd, but only 3 of them had cells that reacted with antibodies to 150 kd and 200 kd. In each case, the number of positive cells depended on the antibody used. The largest number reacting with antibody to 68 kd and the smallest with antibody to 200 kd. Furthermore, it was possible to detect tumor cells in which the 68-kd subcomponent existed by itself, but no tumor cells in which the 150-kd or 200-kd subcomponent existed alone could be detected. These results seem to indicate that antibody to the 68-kd component is sufficiently discriminating to be applied diagnostically.

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1984). I. Susceptibility distribution].

Our research group was engaged for 3 years (1979-1981) in a study on sensitivities to antibiotics of 4 bacterial groups including representative pathogenic bacteria found in cases of urinary tract infections; i.e. E. coli, Klebsiella spp., Citrobacter spp., and Proteus spp. Since 1982, all the bacterial strains isolated by our group from patients with urinary tract infections and deemed by doctors in charge as pathogens were sent to the Laboratory of Clinical Pathology of Juntendo University, where they were refixed and subjected to MIC determination. This is the third year of the new study. E. coli was detected most frequently from patients with urinary tract infections and the detection frequency was 28% (323/1,153) this year (1984), whereas it was 35.3% (304/860) last year, showing a 7% decline from last year to this year. E. faecalis was next frequent organism (12.7% or 147/1,153) followed by P. aeruginosa (10.8% or 124/1,153). This order, however, was reversed from last year. Other pathogens, in a decreasing order of isolation frequencies following the above three, were as follows: Proteus spp. (9.5% or 109/1,153), S. marcescens (6.2% or 71/1,153), S. epidermidis (5.4% or 62/1,153), K. pneumoniae (4.9% or 56/1,153), Enterobacter spp. (2.4% or 28/1,153) and Citrobacter spp. (2.3% or 27/1,153). The results of the determination of the sensitivity of bacterial strains to the antibiotics are described below. Of all the oral antibacterial and antibiotic agents used against E. coli, mecillinam (MPC), cefaclor (CCL) and pipemidic acid (PPA) proved to have high antibacterial potency, and their MIC90 (the concentration to inhibit growths of 90% of the objective bacteria) was 3.13 micrograms/ml. The MIC90's of cefotiam (CTM), cefotaxime (CTX), ceftizoxime (CZX), cefmenoxime (CMX) and latamoxef (LMOX) were less than 0.39 microgram/ml. The MIC90's of cefmetazole (CMZ) and cefoperazone (CPZ) were invariably 1.56 micrograms/ml. K. pneumoniae was not sensitive to ampicillin (ABPC) and did not show much sensitivity to other oral antibacterial and antibiotic agents also. Of all the injectable preparations of antibiotics, cephem antibiotics of the third generation showed the most potent antibacterial effects against K. pneumoniae, and their MIC90's were lower than 0.10 microgram/ml for CZX, 0.20 microgram/ml for CTX, 0.39 microgram/ml for CMX, and 0.78 microgram/ml for LMOX, while MIC90's of CPZ was 6.25 micrograms/ml, which was equal to that of CMZ. The MIC90 of CTM was 0.78 microgram/ml which was identical to that of LMOX.(ABSTRACT TRUNCATED AT 400 WORDS)

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1984). III. Secular changes in susceptibility].

Cases with infections of urinary tracts were divided into 3 groups of the simple infections, and complicated infections without indwelling of catheter, and complicated infections with indwelling of catheter. Susceptibilities to antimicrobial agents of Escherichia coli, Klebsiella spp., Proteus spp., Citrobacter spp., Enterobacter spp., Pseudomonas aeruginosa and Serratia marcescens which were isolated from patients with these infections were determined. There was no tendency of decline in the susceptibilities of E. coli isolated from the patients with simple urinary tract infections (UTI). Susceptibilities of E. coli isolated from the patients with complicated UTI without and with indwelling of catheter to cephem antibiotics of the third generation were examined. The susceptibility of E. coli strains isolated from patients with complicated UTI without and with indwelling of catheter remained the same. More specifically, cefmenoxime (CMX) at a concentration of less than 0.10 microgram/ml inhibited the growth of E. coli isolated from cases without: with catheter at 74.1%: 78.3% in 1982, 75.4%: 73.3% in 1983, and 81.3%: 84.8% in 1984. Also, ceftizoxime (CZX) at a concentration of less than 0.10 microgram/ml inhibited the growth at 83.3%: 95.7% in 1982, 89.2%: 86.7% in 1983, and 91.7%: 97.0% in 1984. Latamoxef (LMOX) at less than 0.10 microgram/ml inhibited the growth at 59.3%: 43.5% in 1982, 47.7%: 40.0% in 1983, and 47.9%: 42.4% in 1984. The antibacterial effect of penicillin against Klebsiella spp. was found to be poor, while those of oral cephem antibiotics, cephalexin (CEX), cefaclor (CCL), and cefazolin (CEZ) which is the cephem antibiotics of the so-called first generation and cefotiam (CTM) among other cephem antibiotics of the so-called second generation were relatively good. A study of susceptibilities of Klebsiella spp. isolated from patients with complicated UTI without and with indwelling of catheter revealed inhibition of growth by CTM at a concentration of 0.39 microgram/ml at 84.0%: 75.9% in 1982, 70.6%: 75.0% in 1983, and 95.8%: 77.8% in 1984. Cefmetazole (CMZ) at a concentration of 0.39 microgram/ml showed a relatively lower rate of growth inhibition of Klebsiella spp., while at 0.78 microgram/ml it inhibited the growth at 88.0%: 72.4% in 1982, 52.9%: 50.0% in 1983, and 70.8%: 66.7% in 1984. The antibacterial effects of both CTM and CMZ against Klebsiella spp. isolated from patients with indwelling of catheter were found to be poor, and some of the bacterial strains showed a MIC over than 100 micrograms/ml.(ABSTRACT TRUNCATED AT 400 WORDS)

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1984). II. Background of patients].

Groups of bacteria were isolated from patients with infections of urinary tracts, and backgrounds of these isolates and patients have been studied in terms of the sexual distribution, and distributions of sexes versus age of the patients relationships between bacterial types and sexual and age distribution of the patients from whom the isolates were obtained correlation ages of patients and types of infections, the association of certain types of bacteria with certain infections and the effect of antibacterial treatments on these types of bacteria, approximately 20% of simple infections of urinary tracts occurred in males and approximately 80% in females on average from 1981 to 1984. This ratio showed hardly any changes during this period. Likewise, male patients comprised approximately 60-70% of all the cases with complicated infections of urinary tracts regardless of indwelling or without indwelling of catheters. In terms of age, the ratio of urinary tract infections was almost universally observed, e.g., for patients in 40's: 10.0% in 1981, 11.0% in 1984, while in those in 70's: 25.5% and 21.2%, respectively. Consequently, background factors of the cases with infections of urinary tracts, such as sexes, age, or simple or complicated, remained almost stable, whereas bacteria involved showed considerable variations. In 1982 and 1983, the frequency of isolations of Enterococcus spp. was approximately 8% in both males and females. However, in 1984, the frequency of isolation of Enterococcus spp. was 22.5% in the males 17.9% in the females. The frequency of isolation of Enterococcus spp. increased in all age groups, but the frequency was not particularly high among the aged. A study on variations and isolation frequencies of bacteria before the administration of antibiotic agents revealed that isolation frequencies of Enterococcus spp. sharply increased in 1983, and this high frequency remained in 1984. The frequency of isolation of E. coli tended to decrease year after year. This finding represents that the frequency of isolations of E. coli from materials of cases with simple infections of urinary tracts has decreased, showing that pathogenic bacteria from simple infections of urinary tracts were becoming similar to that of the complicated infections of urinary tracts. These findings seem to be worth mentioning here. In cases receiving administrations of antibacterial and antibiotic agents for more than 8 days. Infantile digital fibromatosis. An electron microscopic and immunohistochemical study.

Three cases of infantile digital fibromatosis were studied by electron microscopy and immunohistochemistry. The tumor was made up equally of myofibroblasts containing long narrow bundles of microfilaments with dense bodies. Another striking feature was the intracytoplasmic electron-dense inclusion. The inclusions were constituted of packed fibrils similar to those of the long bundles. At the periphery of these inclusions were seen prominent felt-like filamentous structures; the diameter varied from 5-12 nm. The 10 nm filaments were seen most abundantly in the cytoplasm surrounding the inclusions, but sometimes they were also found within the inclusions themselves, mainly at their periphery. These findings suggest that the inclusions may represent an abnormal accumulation of cytoskeletal proteins. On the other hand, various kinds of immunohistochemical study showed that actin, actomyosin, myosin, and vimentin were distinctly observed in cytoplasms of tumor cells. They were located fringing the intracytoplasmic inclusions, but the inclusion themselves showed negative findings, thus indicating a hollow-like staining pattern. These data can be synthesized as follows. The inclusion results from an accumulation of cytoskeletal proteins (actin-myosin complex and vimentin, at least), and antigenicity seems to be lost by for a certain mechanism or in the degenerative process.

Long-term survival of patients with hepatocellular carcinoma combined with liver cirrhosis. Report of two patients.

Two patients with hepatocellular carcinoma combined with liver cirrhosis lived more than 5 years after hepatic resection. One patient with liver cirrhosis combined with schistosomiasis japonica survived 5 years and 2.5 months after left lobectomy and died of liver failure. Two recurrent tumors, each 1 cm in diameter, were present in the remaining lobe but were not responsible for death. Another patient is well 9 years and 2 months after extended right lobectomy. The tumor was bulky, and the resected specimen was 2800 g in weight. These facts show that patients with hepatocellular carcinoma combined with liver cirrhosis can survive for a long period even if a large tumor is involved.

Cellular differentiation of epithelioid sarcoma. An electron-microscopic, enzyme-histochemical, and immunohistochemical study.

For the purpose of clarifying cellular differentiation of epithelioid sarcoma, studies based on various methods were performed. Enzyme histochemical studies showed that epithelioid sarcoma tumor cells have characteristics intermediate between epithelial cells and the large plump cells of synovial sarcoma-incomplete epithelial differentiation. For alkaline phosphatase and adenosine triphosphatase particularly, positive cells and negative cells coexisted, as in the large plump cells of synovial sarcoma. Immunohistochemical studies for alpha 1-antitrypsin, alpha 1-antichymotrypsin, vimentin, and keratin also showed that epithelioid sarcoma tumor cells are very similar to the large plump cells of synovial sarcoma and have incomplete epithelial differentiation. For example, the examinations of serial sections and double staining methods revealed that keratin-positive cells are always vimentin-positive in epithelioid sarcoma and in the monophasic area of synovial sarcoma. Electron-microscopically, bundles of intermediate filaments and filopodia toward the intercellular lumen were observed, as in the monophasic area of synovial sarcoma. The results of enzyme-histochemical and immunohistochemical studies of non-neoplastic synovial lining cells, performed here for the first time, are also discussed.

Numerous mast cells in an 11-deoxycorticosterone-producing adrenocortical tumor. Histologic evaluation of benignancy and comparison with mast cell distribution in adrenal glands and neoplastic counterparts of 67 surgical specimens.

Pathologic study of a rare 11-deoxycorticosterone-producing adrenocortical tumor causing primary aldosteronismlike signs and symptoms, revealed several characteristic features as follows: (1) fairly large size with histologic features corresponding to those of benign zone glomerulosa-type aldosteronoma, (2) lack of spironolactone (S) bodies despite S administration, and (3) heavy mast cell infiltration. In order to explain this rare histology, the localization of mast cells in the adrenal glands and functioning adrenocortical tumors of 67 surgical specimens were investigated. The results of the study supported the view that detection of mast cells helps in the differentiation of mineralocorticoid-producing tumors from cortisol-producing ones, and that the observed mast cell infiltration was due, in part, to its production of 11-deoxycorticosterone.

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1983). III. Secular changes in susceptibility].

The in vitro antimicrobial activities of antibiotics against causative pathogens isolated from patients with urinary tract infections (UTI) in the 8 institutions in Japan during the period from 1980 to 1983 were compared. A number of new beta-lactam antibiotics with broad spectrum of activity have become available for clinical use in recent years. Some of them, in particular so-called third generation cephems, are reported to be responsible for developing microbial-cross resistance to multiple beta-lactam antibiotics. We have been making survey in recent years to explore changes in susceptibility to various antimicrobial agents of clinical isolates. All bacterial isolates from clinical specimens were submitted to the Department of Clinical Pathology, Juntendo University School of Medicine, where they were tested for antimicrobial susceptibility with MIC 2000 apparatus. Of all pathogens from patients with simple UTI, the majority of the isolates was E. coli and Klebsiella spp. In cases of complicated UTI, on the other hand, Pseudomonas spp. were most frequent, followed in order by Proteus spp., Klebsiella spp., Serratia spp., Citrobacter spp. and E. coli. Conspicuous changes in antimicrobial activity of antibiotics against E. coli and Klebsiella spp. from simple UTI have not been found in our survey. Against strains of Citrobacter spp., even the third generation cephems proved to be not remarkably active and there was a significant decrease in susceptibility of isolates to the drugs test-showed MIC values of 50 micrograms/ml and the proportion increased to 50% (22/44) with isolates obtained in 1982. The antimicrobial activity of cefsulodin and gentamicin against P. aeruginosa was decreased in 1982 compared with that in 1980 and 1981. However, resistant strains were slightly more frequent with gentamicin. In 1983, the antimicrobial activity of third generation cephems against Serratia spp. was significantly reduced from that in 1982.

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1983). I. Susceptibility distribution].

During 1979-1983 in vitro activities of antimicrobial agents against causative bacteria isolated from patients with urinary tract infections (UTI) were investigated by Microbiological Research Group of UTI including the 8 institutions in Japan. Of all strains (219) isolated from patients with simple UTI, 65.3% were E. coli, and 9.6% Klebsiella spp.; these species accounted for about 75% of all isolates in 1983. MPC and CCL among the oral antimicrobial agents have showed potent activities against E. coli, MPC at 1.56 micrograms/ml and CCL at 3.13 micrograms/ml inhibited 80% of E. coli from simple and complicated UTI. CTM, CTX, CZX, CMX and LMOX among the parenteral antimicrobial agents, at concentrations of 0.20 micrograms/ml or less inhibited 90% of E. coli isolated from simple and complicated UTI. The frequency of isolates from patients with complicated UTI, without catheter, was as follows; E. coli 27.6%, P. aeruginosa 20.1%, Streptococcus spp. 12.6%, Serratia spp. 8.8% and Klebsiella spp. 8.0%. The frequency of isolates from patients with complicated UTI, indwelling catheter, was as follows; P. aeruginosa 22.6%, Streptococcus spp. 18.0%, Serratia spp. 15.0%, Proteus spp. 12.4%, and Enterobacter spp. and Klebsiella spp. are 6.0%, respectively, in 1983. The antibacterial activity (MIC80) against E. coli, Klebsiella spp., P. mirabilis, Citrobacter spp. and Serratia marcescens from simple and complicated UTI was compared, for example, among third generation cephem antibiotics. Four drugs such as CMX, CZX, CTX and LMOX showed virtually comparable activities while CPZ was slightly less active against the strains tested. There have been many studies reported concerning the antibacterial activity of various drugs against clinical isolates from patients with infections, but it seems that, to our knowledge, no article has dealt with yearly surveys on antimicrobial susceptibility of bacterial isolates from defined clinical specimens with the same period of each year at the same series of institutions.

[Comparative studies of antimicrobial agents against causative organisms isolated from urinary tract infections (1983). II. Background of patients].

We have been collected the causative isolates from patients with urinary tract infections (UTI) from the 8 institutions in Japan during 1980-1983. All strains isolated from UTI and recognized as etiologically responsible in each institution were sent to Bacteriology Laboratory of Juntendo University Hospital, Tokyo and the species of these strains were reidentified. We classified the UTI into 3 categories, simple, complicated without catheter and complicated with catheter. Of all strains isolated as etiologically responsible from cases of simple UTI, 65.3% were E. coli and 9.6%, Klebsiella spp. in 1983: these species accounted for about 75% of all isolates. Most frequently isolated from patients without catheter were E. coli 27.6% in 1983, followed in order by Pseudomonas spp. (20.9%), Gram-positive bacteria (16.7%), Serratia spp. (8.8%), Klebsiella spp. (8.0%) and Proteus spp. (7.1%). In complicated UTI with catheter, Pseudomonas spp. were most frequently isolated (25.6%), followed in order by Gram-positive bacteria (22.9%), Serratia spp. (15.0%), Proteus spp. (12.4%), Enterobacter spp. (6.0%) and Klebsiella spp. (6.0%). A remarkable difference, that is, Gram-positive bacteria, especially S. aureus, showed conspicuous increase of isolation between 1982 and 1983. Furthermore, we discussed the detailed analysis of patient background to clinical bacterial isolates from UTI in respect of patient distribution by sex, types of infections, distributions by age in male and female, interrelations between the species of bacterial isolates and types of infection and antimicrobial chemotherapy.

Clinical episodes of granulomatous pneumonitis. Repetition during four consecutive summers.

From summer to mid-autumn for four consecutive years (1974 to 1977), a 50-year-old woman barber experienced cough and exertional dyspnea with sputum production. Rales were audible at the posterior lung bases bilaterally. Her chest x-ray film showed diffuse nodular shadows, and pulmonary function studies revealed markedly decreased DCO. Open chest lung biopsy was performed and granulomatous lesions with multinucleated giant cells without central necrosis were observed. Having experienced multiple episodes for four years, she ceased to show symptoms after demolishing and reorganizing her house. This case is thought to represent the typical summer-type hypersensitivity pneumonitis, which we have subsequently found to be the most prevalent form of hypersensitivity pneumonitis in Japan.
