Microvascular architecture of the palatine mucosa in the common squirrel monkey (Saimiri sciureus).

Detailed observations were made of the structure and microvasculature of the palatine mucous membrane of the common squirrel monkey (Saimiri sciureus) by means of the plastic injection method under a scanning electron microscope. The findings obtained were compared with those of the Japanese monkey and other mammals. The osseous palate was flat horizontally and a pair of incisive foramina were apparently open at the anterior end of the hard palate. At the posterior end of the osseous palate, the pterygopalatine incisurae were found bilaterally. The incisive papilla was not obvious in form and size, and a pair of openings of the incisive canal were always situated on both sides of the papilla. The transverse palatine plicae or ridges numbered 7 or 8 symmetrically. They arched posterolaterally with an anterior protrusion near each median end. Posterior plicae were underdeveloped. Numerous openings of the palatine glands were found in the soft palatine mucosa. The arteries supplying the palate were the major palatine artery passing through the major palatine foramen and the soft palatine artery passing through the pterygopalatine incisura. The major palatine artery extended forwards giving off numerous medial and lateral branches, and its end on the respective side entered a small foramen located lateral to the incisive foramen. Medial and lateral branches formed the submucous arterial network. Arterioles diverging from this network were directed to the epithelial surface and formed an arterial network in the lamina propria. Further, capillaries diverging from the latter network built up the subepithelial capillary network immediately beneath the epithelium. Capillary loops sprouting from the capillary network were found in the form of a simple hair-pin without locational differences in their heights. The microvascular architecture thus displayed some similarity with that of the Japanese monkey. However, the vascular networks in the lamina propria and submucous layer were not distinct in size and scale as compared to those of the Japanese monkey. These differences may be related to the stature, living environment and food habits of this species.

[Neuro-otological study of Laurence-Moon-Biedl syndrome].

Two siblings of Laurence-Moon-Biedl (L-M-B) syndrome with nystagmus and/or sensorineural hearing loss were reported. One sibling had a slight hearing impairment as well as gaze and rebound nystagmus. The auditory brainstem responses (ABR) showed increased latencies of wave V on both ears. The optokinetic nystagmus (OKN) was limited toward the both horizontal sides. Nystagmus was superimposed on the eye tracking movement. Ocular hypermetria was observed, with bilateral weakness of caloric responses and even a failure of visual fixation suppression. Another sibling had a left-beating spontaneous nystagmus. The latencies of wave V of ABR were prolonged in both ears. The leftward OKN was limited and bilateral saccadic pursuits observed on eye tracking test. These results suggest that cerebellar of brain-stem lesions may be involved in L-M-B syndrome.

Extraskeletal osteosarcoma in the axilla associated with breast carcinoma.

A case of extraskeletal osteosarcoma arising in the axilla in a 77-year-old woman is described. Because of its association with an ipsilateral breast carcinoma, the axillary mass was at first assumed to be a lymph node with metastatic breast carcinoma. The occurrence of extraskeletal osteosarcoma in the axillary is rare and association with breast carcinoma has never, to our knowledge, been described.

[Adrenocortical carcinoma: report of a case].

A 24-year-old man visited our hospital complaining of hypertension and headache. Endocrinological findings revealed no abnormalities except for a slight decrease in serum adrenocorticotropic hormone (ACTH), a slight increase in urine 17-ketosteroid (17-KS), and a marked increase in serum pregnenolone. Computed tomography and magnetic resonance imaging revealed a 3 x 3 cm mass in the right adrenal area and I131-aldosterol scintigraphy demonstrated a high absorption of the isotope in the right adrenal area. Vena cavography suggested a 2 x 2 cm tumor thrombus originating in the right adrenal. Under the diagnosis of the right adrenocortical carcinoma, adrenalectomy and removal of the tumor thrombus were performed. Both serum pregnenolone and urine 17-KS returned to the normal level within a week after the operation and blood pressure was well controlled without any medication 3 months after the operation. Thus, the tumor seemed to be endocrinologically active.

[Long-term prognosis of adenoid cystic carcinoma of the trachea].

A study was made on the prognosis of 26 tracheal adenoid cystic carcinoma surgery cases reported in Japan between January 1978 and January 1984. The study was based on a questionnaire sent to doctors who reported prognosis as of May 1991. Twelve cases out of the 26 cases underwent postoperative irradiation. Seven out of the 12 cases survived 9 to 13 years after surgery without any recurrence. Two out of the 12 cases survived 8 and 10 years after surgery with multiple pulmonary metastases. Three out of the 12 cases died of a local recurrence 3, 3 and 9 years after surgery. Four cases received irradiation after a local recurrence was revealed postoperatively. Three out of the 4 cases survived 9, 10 and 11 years after surgery, but 1 out of the cases died 6 years after surgery. Four cases received no irradiation not only after surgery but also after detection of a local recurrence. All of them died within 9 years after surgery. There were six cases who underwent no postoperative irradiation and showed no local recurrence. One out of the 6 cases died of pancreas cancer 7 years after surgery, but the other cases survived 8 to 11 years after surgery without any recurrence. A study was also made on the prognosis of 9 cases with adenoid cystic carcinoma of the trachea who underwent surgery at Keio University and Tokai University until April 1982 (4 cases overlap with the above-mentioned cases). Six out of 9 cases received postoperative irradiation. Four cases survived 9 to 12 years after surgery without any recurrence.(ABSTRACT TRUNCATED AT 250 WORDS)

[Vasoactive intestinal peptide inhibits vagal neuro-effector transmission in feline airway smooth muscle but not in humans].

It has been reported that low concentrations of vasoactive intestinal peptide (VIP) suppress the release of acetylcholine (ACh) from vagal nerve terminals. However, there is little documentation of the neuro-effector transmission of VIP in human airways. In the present study, the effects of VIP on excitatory neuro-effector transmission in the human bronchus were investigated, in comparison with feline trachea. In feline trachea, VIP (10(-10) to 10(-7) M) reduced the amplitude of contractions evoked by field stimulation in the presence of indomethacin (10(-6) M). By contrast, VIP (10(-10) to 10(-9) M) had no effect on ACh-sensitivity of smooth muscle cells. These results indicate that VIP in low concentrations has a pre-junctional action inhibiting excitatory neuro-effector transmission, in addition to a post-synaptic action, presumably suppressing transmitter release from the vagal nerve terminals. On the other hand, in human bronchi, VIP (10(-9) to 10(-7) M) did not affect either contractions evoked by field stimulation or those evoked by ACh. These results indicate that VIP does not exhibit any pre-junctional effects in the human bronchus at these concentrations.

Possible roles of the peripheral vagal nerve in histamine-induced bronchoconstriction in guinea-pigs.

Although the importance of the vagal nerve in the pathogenesis of bronchial asthma has been reported, its precise contribution is still not fully understood. To shed more light on this area, we evaluated the possible contribution of vagal reflex in histamine-induced bronchoconstriction (HIB), and decided the site of action of histamine on the vagal nerve. For this purpose, we studied the effects of the bilateral cervical vagotomy, hexamethonium (2 mg.kg-1) or tetrodotoxin (0.5 mg.kg-1) on HIB (8 micrograms.kg-1, iv) in anaesthetized and mechanically-ventilated guinea-pigs. We also studied whether or not atropine (1 mg.kg-1) decreases HIB after vagotomy, including either the treatment of hexamethonium or tetrodotoxin. Airway responses were assessed by measurement of pulmonary resistance. The following results were obtained; 1) the response to histamine was significantly enhanced by the vagotomy, hexamethonium or tetrodotoxin; 2) propranolol increased HIB, and HIB was further enhanced by the vagotomy in the animals treated with propranolol; 3) atropine significantly suppressed HIB after the vagotomy, hexamethonium or tetrodotoxin. These results suggest that the postganglionic vagal nerve plays an excitatory role in HIB through the release of acetylcholine from the nerve terminals. It is also suggested that the vagal reflex mainly exhibits an inhibitory role in the HIB of guinea-pigs, presumably by the action of the nonadrenergic inhibitory nervous system.

[The relationship between the deep body temperature and hemodynamics after open-heart surgery in children].

The relationships of the forehead-sole deep body temperature difference with the cardiac index (CI), and with the systemic vascular resistance index (SVRI) were studied in 10 pediatric patients (TOF 5, ASD 3, VSD 2) for 24 hours after open-heart surgery. A correlation between the deep body temperature difference (X) and CI (Y) was expressed as, Y = -0.49X + 4.51 (r = 0.72), and that between the deep body temperature difference (X) and SVRI (Z) as Z = 152X + 1039 (r = 0.62). The deep body temperature difference was 1.0 degrees C on the average in patients whose CI was more than 3.0 l.min-1.m-2, and was 3.3 degrees C in patients whose CI was less than 3.0 l.min-1.m-2. Conversely CI was less than 3.0 l.min-1.m-2 in patients whose deep body temperature difference was more than 3.0 degrees C. We conclude that the measurement of deep body temperature difference is useful as a circulatory monitor, and that a critical level of deep body temperature difference is 3 degrees C in children.

Induction of ornithine decarboxylase by sialagogues in a human parotid gland adenocarcinoma cell line.

Ornithine decarboxylase in a human parotid gland adenocarcinoma cell line was induced by both cholinergic (carbachol) and beta-adrenergic (isoproterenol) sialagogues. The enzyme protein level, measured with anti-peptide antiserum, as well as the enzyme activity, was found to be high in unstimulated cells and to increase approximately 2-fold on stimulation, while the mRNA level increased 3-4 fold, as revealed by Northern hybridization. The rise in activity was completely blocked by the simultaneous addition of antagonists or actinomycin D. These results suggest that receptor-mediated stimulation of ornithine decarboxylase activity by sialagogues involves alterations in the level of mRNA and that the proliferative responses of human parotid cells to these sialagogues resemble those of the murine parotid gland.

[Inhibition of airway smooth muscle contraction by airway epithelium in human bronchus].

To clarify the mechanism underlying the inhibitory effect of epithelial cells on smooth muscle contraction, we studied two types of human bronchial tissue preparations: (1) "acceptor" bronchial strip without epithelium, (2) "donor" bronchial ring with or without epithelium. We measured the contractile responses of acceptor bronchial strip surrounded by donor bronchial ring to increasing concentrations of acetylcholine (ACh). Removal of the epithelium of the donor bronchial rings significantly enhanced the contractions of the acceptor bronchial strips. Thus, airway epithelium decreases the airway smooth muscle contraction to ACh in human bronchus. The mechanism of this inhibitory effect of airway epithelium was not due to a change in mechanical property of the airway, nor to a change in diffusion path for the chemical mediators. These results suggest that human airway epithelium may have an important role in modulating airway smooth muscle tone, possibly by the release of an epithelium-derived relaxing factor.

Vital reactions in rat skin wounds by quantitation of fibrin.

The fibrin contents in contused skin wounds that were experimentally inflicted on rats were measured by a new method which was based on the determination of solubilized fibrin degradation products (FDP) using a latex agglutination test after plasmin digestion of tissue homogenates. The levels of FDP in ante-mortem wounds began to increase within 10 min after wounding and continued to increase for up to 6 h. The FDP levels in post-mortem wounds remained under the detection limit of the method used and were identical to those of intact tissues. The fibrin contents in the ante-mortem wounds prepared 2 days after death did not differ significantly from those prepared immediately. The results suggest that this method may be applicable to the identification of the vital reaction of the wound.

[Blind ending bifid ureter with stone in the blind branch: report of a case].

Blind ending bifid ureter is a rare anomaly in the urinary tract. This anomaly may result from failure of a premature branch of the ureteral bud to join with the metanephric blastema. A 21-year-old man was admitted with macroscopic hematuria and colic pain in the left flank region. Urinalysis demonstrated hematopyuria and excretory urography suggested bifid ending accessory ureter with a stone on the left side. Surgical exploration showed that the accessory ureter was bifurcated from the left ureter at about 5 mm from the bladder wall and ran parallel with the left ureter. Although dense adhesions to the surrounding tissue existed, the accessory ureter was resected at the site of the junction. It measured 3 cm in length and 1 cm in greatest diameter. The stone found at the tip of the accessory ureter was composed of calcium oxalate (24%) and calcium phosphate (76%). Histological examination revealed that the ureter had all layers of normal ureteral structure and no renal tissue was identified in the specimen resected. During a follow-up period of 22 months after the operation, he was free of urinary tract infection and abdominal pain. Of 77 cases with blind ending bifid ureter reported in the Japanese literature, a ureteral stone was found in the blind branch in only 5 cases.

[Reevaluation of the unilateral pulmonary artery occlusion test--hemodynamics after lobectomy and pneumonectomy for lung cancer].

After lobectomy, it is recognized that functional as well as absolute reduction occurs in residual lobes of the operated side. So whether lobectomy is indicated or not is determined by the same criteria as those for pneumonectomy, namely, by the unilateral pulmonary artery occlusion (UPAO) test. However, is it really appropriate to use the same criteria for both lobectomy and pneumonectomy? To answer to this question, in patients with lung cancer we compared the hemodynamics after lobectomy (13 cases) and pneumonectomy (14 cases) with that at the UPAO test. After pneumonectomy, the mean pulmonary arterial wedge pressure (mPWP) was significantly lower than that on the preoperative day and at the test. It seemed that hypovolemic change occurred in the hemodynamics after pneumonectomy. After pneumonectomy, the pulmonary arteriolar resistance index (PARI) was significantly higher than the preoperative value. It was the same as that as at the time of the UPAO test. The total pulmonary vascular resistance index (TPVRI) at the time of the test was significantly higher than the preoperative value, but the TPVRI after pneumonectomy was not significantly higher. The TPVRI tended to decrease after pneumonectomy, compared to the value predicated by the test. These results indicated that some of the cases judged inoperable on the basis of the UPAO test might be operable. On the day of lobectomy, the PARI was significantly higher than the preoperative value, but significantly lower than that at the time of the test. The cardiac index (CI) was significantly higher and the mPWP was significantly lower than each preoperative value.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of cyclooxygenase inhibitor on excessive sputum.

Indomethacin suppository and aspirin inhalation reduced the amounts of sputum in the patients with bronchiectasis and chronic bronchitis. A 67-year-old woman with bronchiectasis received indomethacin suppository to relieve the lumbar pain. After the start of indomethacin, a reduction in the amount of sputum was observed and the volume of sputum returned to pretreatment levels after the cessation of indomethacin. Levels of cyclooxygenase products in the sputum during indomethacin was also much lower than their levels after discontinuance of indomethacin. A 64-year-old man with chronic bronchitis was also administered inhaled aspirin to control the sputum volume. Both indomethacin suppository and aspirin inhalation reduced the amounts of sputum. In addition to these cases, a 31-year-old woman with bronchial asthma was tried to control the sputum production with indomethacin, but it had no effect on the sputum volume. It is suggested that these drugs may be useful in the treatment of excessive sputum in bronchiectasis and chronic bronchitis.
