[A case of identification of two persons from heavily burned teeth by age estimation].

A man murdered his two daughters, one five years and two months old and one seven months old. He placed them into a 750 degrees C aluminum liquefying furnace. Teeth and many fragments of the severely burned bones were recovered. However, at first, the bone fragments did not offer proof that two children were put into the furnace. The age estimation of the teeth burned in liquefied aluminum was carried out based on the maturity of the teeth and the degree of absorption of deciduous teeth roots. As a result from the admixture of permanent teeth and deciduous teeth, we presumed them to be derived from more than one person.

[Clinical experience of ileal neobladder for bladder cancer].

Between June 1993 and July 1996, an ileal neobladder was created in 20 patients after total cystectomy for bladder cancer. The mean post operative follow-up period was 32 months, with a range of 9 to 47 months. Ureteroileostomy was performed using the Le Duc-Camey procedure. There were 3 (15%) early postoperative complications, which were all of transient urine leakage from the neobladder. Late complications were encountered in 6 patients (30%), which were of stenosis of ureteroileal anastomosis in 2 (10%), stone in the neobladder in 2 (10%), neobladder-cutaneous fistula in 1 (5%) and neobladder-ureteral reflux in 1 (5%). Reoperation was necessary in 4 patients (20%); 2 for stenosis of ureteroileal anastomosis and 2 for removal of a stone in the neobladder. No urethral recurrence has been noted. Local recurrence occurred in 2 patients, who died of tumor progression 16 and 27 months postoperatively. All 20 patients were continent during the day time (100%), while 2 (10%) had nocturnal incontinence.

[New drug delivery systems for the therapy of bronchial asthma].

Inhalation therapy gives us ideal topical therapy for the airway diseases without significant major side-effects. As new century approaches, a major goal for asthma therapy will be to provide more effective therapeutic drugs to prevent airway inflammation and its spasmus. It is also important to produce excellent inhalant devices that are easy to use and do not contain substances that are harmful to the environment. Future inhalers should target the drug to the correct sites in the airways and thus enable optimal therapeutic effects to be achieved. There are many exciting new developments available in this field. And we should finally obtain special therapeutic drugs to prevent the process to the sudden-onset of asthmatic death.

[A case of malignant lymphoma presenting as epididymal tumor].

A 56-year-old man visited our hospital with the complaint of painless swelling of right scrotal contents. Right orchiectomy was performed. Histopathological diagnosis was malignant lymphoma of the epididymis, without involvement of the testis. Bone marrow biopsy and Ga scintigram showed a normal pattern, while a swollen retroperitoneal lymph node (3 cm) was found by computed tomography (CT). The CT also showed a small mass lesion in the left upper lung field. After 2 courses of chemotherapy for malignant lymphoma, left upper lobectomy was performed at another clinic. Pathological diagnosis of the surgical specimen was moderately differentiated adenocarcinoma. The patient died without known causes, 8 months after the orchiectomy.

Immunohistochemical properties of lipid peroxidation and prognosis in adenocarcinoma of the lung.

Glutathione-related enzymes are thought to influence the prognoses of patients with adenocarcinoma of the lung. In this study, the localization of these enzymes was examined immunohistochemically in the primary lesions and metastatic lymph nodes of 61 patients with primary adenocarcinoma. Strong immunoreactivity for glutathione peroxidase (GSH-PO) and superoxide dismutase (SOD) was found in tissue from patients with poor prognoses, while tissue from patients with favorable prognoses demonstrated only immunoreactivity for these enzymes. Therefore, believe that glutathione-related enzymes may serve as predictors of tumor resistance in patients with adenocarcinoma, hence measuring these enzymes may be useful in determining the need for adjuvant therapy.

Finite element analysis of the lower lumbar neural arch under facet loading.

To evaluate the mechanical effect of the direction of facet load on the isthmus stresses of the lower lumbar neural arch, stress analyses were performed by using three-dimensional finite element models of isolated L4 and L5 vertebrae with particular emphasis on accurate discretization of the posterior elements. The bilateral symmetric or unilateral facet loads, with a constant magnitude, were applied within the sagittal and transverse planes. The largest maximal principal stress in the neural arch (the largest sigma(max)) tended to be qualitatively similar in L4 and L5. At the physiologic range, the largest sigma(max), observed on the anterior surface of the isthmus, was relatively insensitive to the sagittal plane loadings but sensitive to the transverse plane loadings. The values of the largest sigma(max) of L4 and L5 were low at the more frontally directed facet load. The magnitude and direction of the largest sigma(max) depended upon the facet loadings mainly on the ipsilateral side and partly on the opposite side. The results suggest that the direction of facet load, which may be characterized by the facet orientation, can have mechanical effects on the occurrence and direction of isthmic crack.

[Inter-institute variations in International Normalized Ratio and thrombotest].

Oral anticoagulant therapy is effective for reducing the risk of thromboembolic events in patients with atrial fibrillation or other heart diseases. However, the intensity of oral anticoagulation therapy required in high risk patients, especially in Japanese patients, to achieve the best balance between the prevention of thromboembolic events and bleeding complications remains unclear. The multicenter study of Toyama Warfarin Rational Dosage (TOWARD) was started in 1996 to determine the optimal level of anticoagulant therapy. This study investigated the relationship between values of thrombotest (TT) and International Normalized Ratio (INR) measured from the same samples to clarify inter-institute variations. The relationship between TT and INR was not linear but hyperbolic. Changes of INR to TT are relatively small in the TT range of more than 20% as compared with the range of 20% or less. There were considerable inter-institute variations of TT, and the coefficient of variation (CV) was 0.16 and 0.24 in the low level and high level anticoagulation samples, respectively. However, the variations became significantly small when the same reference was used. The CV of INR was 0.12 and 0.08 in the high level and low level anticoagulation samples, respectively, and very similar with the control samples without anticoagulation (0.11). The variation was small when INR was obtained from the international sensitivity index (ISI) of thromboplastin less than 1.5. TT is widely used for monitoring oral anticoagulant therapy in Japan, and is an excellent system with little inter-institute variation when a standard reference is offered. Since INR has been established as an international monitoring system, the use of INR measured with thromboplastin of small ISI is recommended for monitoring.

[Susceptibilities of bacteria isolated from patients with lower respiratory infectious diseases to antibiotics (1996)].

The bacteria isolated from the patients with lower respiratory tract infections were collected by institutions located throughout Japan, since 1981. Ikemoto et al. have been investigating susceptibilities of these isolates to various antibacterial agents and antibiotics, and characteristics of the patients and isolates from them each year. Results obtained from these investigations are discussed. In 16 institutions around the entire Japan, 557 strains of presumably etiological bacteria were isolated mainly from the sputa of 449 patients with lower respiratory tract infections during the period from October 1996 to September 1997. MICs of various antibacterial agents and antibiotics were determined against 98 strains of Staphylococcus aureus, 93 strains of Streptococcus pneumoniae, 84 strains of Haemophilus influenzae, 84 strains of Pseudomonas aeruginosa (non-mucoid strains), 17 strains of Pseudomonas aeruginosa (mucoid strains), 31 strains of Moraxella subgenus Branhamella catarrhalis, 21 strains of Klebsiella pneumoniae etc., and the drug susceptibilities of these strains were assessed except for those strains that died during transportation. 1) S. aureus S. aureus strains for which MICs of oxacillin (MPIPC) were higher than 4 micrograms/ml (methicillin-resistant S. aureus) accounted for 67.3%. The frequency of the drug resistant bacteria increased comparing to the previous year's 52.7%. Arbekacin (ABK) and vancomycin (VCM) showed the highest activities against both S. aureus and MRSA with MIC80s of 1 microgram/ml. 2) S. pneumoniae Imipenem (IPM) and panipenem (PAPM) of carbapenems showed the most potent activities with MIC80s of 0.063 microgram/ml. Faropenem (FRPM) showed the next potent activity with MIC80 of 0.125 microgram/ml. The other drugs except erythromycin (EM), clindamycin (CLDM) and tetracycline (TC) were active against S. pneumoniae tested with MIC80s of 8 micrograms/ml or below. 3) H. influenzae The activities of all drugs were potent against H. influenzae tested with MIC80s of 4 micrograms/ml or below. Cefotiam (CTM), cefmenoxime (CMX), cefditoren (CDTR) and ofloxacin (OFLX) showed the most potent activities with MIC80s of 0.063 microgram/ml. 4) P. aeruginosa (mucoid strains) Tobramycin (TOB) showed the most potent activity against P. aeruginosa (mucoid strains) with MIC80 of 1 microgram/ml. Ceftazidime (CAZ), cefsulodin (CFS), IPM, gentamicin (GM), ABK and ciprofloxacin (CPFX) showed the next potent activities, with MIC80s of 2 micrograms/ml. The MIC80s of the other drugs ranged from 4 micrograms/ml to 16 micrograms/ml. 5) P. aeruginosa (non-mucoid strains) TOB and CPFX showed the most potent activities against P. aeruginosa (non-mucoid strains) with MIC80s of 1 microgram/ml. The MIC80s of piperacillin (PIPC) and cefoperazone (CPZ) were 16 micrograms/ml in 1995, and they were 64 micrograms/ml in 1996. 6) K. pneumoniae All drugs except ampicillin (ABPC) were active against K. pneumoniae. CMX, cefpirome (CPR), cefozopran (CZOP) and carumonam (CRMN) showed the most potent activities against K. pneumoniae with MIC80s of 0.125 microgram/ml. The MIC80s of the other drugs ranged from 0.25 microgram/ml to 2 micrograms/ml. 7) M.(B) catarrhalis Against M.(B.) catarrhalis, all drugs showed good activities with MICs of 4 micrograms/ml or below. IPM and minocycline (MINO) showed the most potent activities with MICs of 0.063 microgram/ml. Also, we investigated year to year changes in the characteristics of patients, their respiratory infectious diseases, and the etiology. Patients' backgrounds were examined for 557 isolates from 449 cases. The examination of age distribution indicated that the proportion of patients with ages over 60 years was 71.0% of all the patients showing a slight increase over that in 1994. Proportions of diagnosed diseases were as follows: Bacterial pneumonia and chronic bronchitis were the most frequent with 35.9% and 30.3% respectively. They were followed by bronchiectasis with a proportion of 10.

Genetics of type II diabetes.

Defining the genetic basis of Type II or non-insulin dependent diabetes mellitus (NIDDM) will accelerate our progress toward understanding its etiology and will provide new therapeutic targets for treatment of this common disease. Here we present a brief overview of the history of the search for diabetes genes and report current strategies employed by our laboratory and by others in this effort. Isolation and subsequent mapping of candidate genes involved in insulin production and action has been a major effort in this field. Our lab has focused on pancreatic islet beta-cell genes, since the insulin lack of NIDDM is often the result of resistance to the action of insulin that is superimposed on a limited ability to produce insulin. A number of islet genes have been evaluated, including those involved in glucose metabolism, islet K+ channel genes, and transcription factors. For each gene, human cDNAs and genomic clones have been isolated and simple sequence repeat polymorphisms (SSRPs) identified. The SSRPs were used to map the genes by linkage in CEPH pedigrees, or sequence-tagged sites (STSs) were used to map the genes to radiation hybrids (RH) or to YAC clones containing SSRPs. The SSRPs have then been used as markers for linkage analyses in families with NIDDM. Mutation screening by single-strand conformational polymorphism analysis and by sequencing has revealed variants that have been tested in association studies. A strategy was devised to generate novel expressed sequence tags (ESTs) from human pancreatic islet genes by differential display of islet mRNA. In the first phase of this project we identified 42 cDNAs that were preferentially expressed in pancreatic islets relative to exocrine tissue. When compared to sequences in GenBank, novel genes were represented by 69%. Enhanced islet expression was confirmed by Northern analysis of RNA. Sequence-tagged sites were synthesized for a number of islet ESTs and used to map these genes to human chromosomes. This strategy provides an effective means to selectively identify and map genes transcribed in human pancreatic islets and to identify novel islet candidate genes for NIDDM. Positional cloning of NIDDM genes in families of various racial groups is being conducted by a number of labs. Although regions of genetic susceptibility are being identified, finding the genes within these regions will be difficult because of the polygenic nature of the disease As an alternative strategy, we have begun to map genes responsible for monogenic disorders of carbohydrate metabolism. Familial hyperinsulinism (HI, OMIM #256450) is a rare recessive disease associated with neonatal hyperinsulinism and life-threatening hypoglycemia. To determine the molecular basis for HI, we mapped the gene in multiplex families to chromosome 11p14-15.1. A candidate gene, the sulfonylurea receptor (SUR1), was mapped to the region and shown to harbor mutations in HI patients. Analysis of 21 identified mutations has revealed the role of SUR1 as a nucleotide regulator of the islet ATP-sensitive K+ channel. The challenge for the future will be to utilize the information provided by the Human Genome Project (i.e., the complete nucleotide sequence and expression maps of the genome) to find diabetes-predisposing genes. Our immediate goals include collecting families with NIDDM for phenotyping and for DNA analysis and continuing to identify suitable candidate genes to be studied in these families.

Clinicopathologic features of local and metastatic recurrences in primary lung adenocarcinoma.

We attempted to construct the contour of recurrence in primary lung adenocarcinoma with clinicopathologic variables based on data of 131 patients with completely resected primary lung adenocarcinoma. In univariate analysis, tumor size (more or less 3 cm in diameter), p-T, p-N, pathological stage, differentiation, ly factor and v factor were chosen for prognostic predictors. In multivariate analysis, v factor and p-N were independent variables of local recurrence and metastatic recurrence, respectively. The examination of significant correlation among clinicopathologic variables in terms of 5-year survival rates of patients showed that tumor size, p-T, ly factor and v factor were profoundly related to local recurrence, whereas ly factor, differentiation and p-N were linked to distant metastasis. We therefore examined an additive effect of tumor size, differentiation and vascular invasion on recurrence. The results demonstrated that neither local nor metastatic recurrences were found in patients with well differentiated adenocarcinoma less than 3 cm in diameter if vascular invasion was negative. We conclude that vascular (ly factor and v factor) is central to lung adenocarcinoma recurrence. The vascular invasion is a powerful predictor of recurrence in less than 3 cm diameter, well differentiated adenocarcinoma of the lung.

A new device for thoracic access: the Thoraco Holder.

BACKGROUND: We present a novel invention (Thoraco Holder; Fuji Systems Corporation, Tokyo, Japan) for thoracoscopic surgery. Made of silicone, automatically stops bleeding from the chest wall, increases range of motion for manipulation, and decreases postoperative pain. METHODS: Under general anesthesia using the Univent tube (Fuji Systems Corporation, Tokyo, Japan), two Thoraco Holders were used in each of 300 thoracoscopic procedures for lung cancer, mediastinal tumor, pneumothorax, chest trauma, and emphysema, most using the Two Windows Method). RESULTS: No patient experienced more than 10 mL of bleeding from the chest wall. In addition, analgesics were not needed by any patient for over 1 week. No patient experienced trouble caused by the Thoraco Holder. CONCLUSIONS: The Thoraco Holder is made of silicone. Silicone rubber causes less pain postoperatively. The silicone cuff (balloon) causes less bleeding from the chest wall. This Thoraco Holder is useful access for video assisted thoracoscopic surgery.

Hepatic and renal expression of senescence marker protein-30 and its biological significance.

A novel rat hepatic protein was detected and isolated, the amount of which is downregulated in an androgen-independent manner with ageing. This protein was designated as senescence marker protein-30 (SMP30). Senescence marker protein-30 turned out to be identical to a hepatic calcium-binding protein called regucalcin (RC). This review gives an overview of SMP30 in its structure, expression and possible physiological function(s). A hypothetical role of SMP30 in ageing and calcium homeostasis is also discussed.

[The electrocardiogram in the diagnosis of acute coronary syndrome].

The 12-lead ECG remains a simple and noninvasive technique to diagnose myocardial ischemia in patients with acute coronary syndrome. Although the diagnostic accuracy of the ECG depends upon the extent of myocardial ischemia and its severity, the initial ECG is the most rapid and readily available tool for the evaluation of patients presenting with suspected acute myocardial infarction. This paper discussed classic ECG changes which can be seen in acute myocardial infarction and unstable angina. A brief review is also made of the prognostic significance of the various patterns of the ECG in the era of thrombolytic therapy and invasive revascularization.
