Expression of Lewis(a), Lewis(b), and sialated Lewis(a) antigens in early and advanced human gastric cancers.

An immunohistochemical analysis of the expression of Lewis(a), Lewis(b), and sialated Lewis(a) blood group antigens was performed on specimens of human non-involved stomach (n = 91), early gastric cancers (n = 41), and advanced gastric cancers (n = 50). In non-involved stomach, Lewis(a) and Lewis(b) were detected mainly in surface epithelium, although sialated Lewis(a) was scarcely expressed. These blood group-related antigens were rarely observed in deep glands. In gastric cancers, Lewis(a) showed a tendency to be expressed in well or moderately differentiated carcinomas, and this was observed to be more marked in advanced tumors. There was no obvious difference in positive ratios of Lewis(b) between degrees of differentiation of the cancers. Sialated Lewis(a) was not detected in early cancers, and its frequent expression in well- or moderately differentiated, advanced carcinomas suggested an association with both cancer differentiation and progression. These results indicate that knowledge of the expression of blood group antigens may help interpret the antigenic alterations occurring in the course of carcinogenesis, differentiation, and progression of gastric cancers.

Hypocalcemic hyper-CK-emia in hypoparathyroidism.

A 15-year-old boy with increased serum creatine kinase (hyper-CK-emia) due to hypocalcemia in turn caused by idiopathic hypoparathyroidism (HP) is presented. Hyper-CK-emia was incidentally noted while managing a patient, aged nine, with mental retardation, epilepsy and mild hypocalcemia. Neurological examination showed normal deep tendon reflexes and no muscle weakness; electromyogram was normal. The hyper-CK-emia normalized during treatment for the hypocalcemia. Previously reported patients with hypocalcemic hyper-CK-emia or myopathy together with HP are discussed, as well as the degree of hypocalcemia and the wide spectrum of the muscle dysfunction.

[A case of massive hemorrhagic enteritis due to rotavirus].

We described a 8-month-old boy of massive hemorrhagic enteritis due to rotavirus complicated by shock. As far as we know, this case is the first patient who developed vascular collapse from the gastrointestinal tract bleeding by rotavirus infection. Clinicians should be alerted to the possibility of the occurrence of severe hemorrhagic enteritis as a life threatening complication in rotavirus induced enteritis.

Tissue biocompatibility of cellulose and its derivatives.

Tissue biocompatibility of cellulose and its derivatives was examined in two in vivo tests, one for absorbance by living tissue and one for foreign body reaction. The samples examined were regenerated celluloses and cellulose derivatives: methyl cellulose, ethyl cellulose, aminoethyl cellulose, hydroxyethyl cellulose, and cellulosic polyion complexes. The in vivo absorbance by living tissue was found to depend on the degree of crystallinity and the chemical structure of the sample. The foreign body reaction was relatively mild for all the samples examined, showing that cellulose can be converted to biocompatible materials by physical and/or chemical transformation.

[Healing process of bone fractures examined with X-ray radiography and xeroradiography].

The healing process of bone fractures produced in rat mandibulae was examined with conventional X-ray radiography and xeroradiography. Sixty rats of about 200g were offered to this examination and fractures were experimentally produced in the right mandibulae. Radiological observations were carried out immediately, 1 day, 4 days, 7 days, 14 days, 21 days, 28 days, 35 days, 42 days, 49 days and 56 days after fracturing. The conventional X-ray radiograms showed no noteworthy changes until the 4th day after fracturing. The radiopacity gradually increased from the 7th Day until the 56th day. The xeroradiograms obtained on the 4th day showed some early changes which appeared on the periosteal regions of the right mandibulae. The calcification of fractured regions on the xeroradiograms gradually increased until the 56th day, similar to that observed on the conventional X ray radiograms.

[Risk factors for peptic ulcers treated surgically].

To elucidate the risk factors of peptic ulcer which require surgical treatments, we examined the influences of following factors such as sex, age, smoking, and alcohol in 308 peptic ulcer patients. In the group with gastric ulcers, high age group was likely to be operated. On the contrary, young patients with duodenal ulcers were likely to be treated surgically. Sex, smoking and alcohol were not regarded as risk factors. The correlations between the degree of neurosis and the amounts of smoking and alcohol were analyzed in the cases with duodenal ulcers. In the cases treated surgically, the degree of neurosis showed no or little correlations. In the cases treated conservatively, however, the degree of neurosis showed positive correlation with the amounts of smoking and alcohol.

[Morphological studies on estrogen-induced PRL cell hyperplasia in rat pituitary gland--immunocytochemical and scanning electron microscopic studies].

It is well known that the tumorous lesion in rat anterior pituitary gland occurs after continuous administration of estrogen, which is mainly composed of PRL secreting cells. In this study the changes in rat PRL cells by estrogen treatment were studied histologically and immunocytochemically. Furthermore, intracellular structures in PRL cells were observed by high resolution scanning electron microscopy using the aldehyde prefix osmium-DMSO-osmium method. These rat pituitary tumors were established in Fischer-344 female rats by weekly injections of 2.5 mg estradiol valerate. The estrogen-induced, autonomous and transplantable rat pituitary tumors (MtT/F84) were also studied in the same manner and the findings were compared with estrogen-induced tumors. The increase of weight of pituitary gland and of serum PRL level correlated significantly with the number of estrogen administration. On the basis of histological findings, estrogen-induced pituitary tumors can be classified into two different stages, that is, hypertrophic hyperplasia and adenomatous hyperplasia. The former corresponds to the period until 6 weeks after the beginning of estrogen treatment and the latter to the period thereafter. In the former, PRL cells were significantly increased in number and volume, and were diffusely immunoreactive for PRL in the cytoplasm. Cell atypism was rarely found. The extent of acidophil staining was well preserved in hematoxylin-eosin staining. Ultrastructurally, organelles were well developed, especially rough endoplasmic reticulum showed whorl formation. Immature, irregularly shaped secretory granules were frequently observed within the Golgi area. Mature granules were richly stored at the periphery of the cell.(ABSTRACT TRUNCATED AT 250 WORDS)

Aldolase C is localized in neuroendocrine cells.

To elucidate the localization of the subunit C of aldolase (aldolase C) in peripheral neuroendocrine cells, we made an immunohistochemical study with monospecific antibodies against human aldolase C. Aldolase C was found to be localized in various types of neuroendocrine cells; in the pituitary gland, thyroid, pancreas, adrenal gland, bronchus, and gastrointestinal tract.

Role of hypovolemia in dialysis-induced hypotension.

Blood volume was measured continuously during hemodialysis using the authors' hematocrit continuous measurement system in an attempt to determine any relationship between dialysis-induced hypotension with acute onset and blood volume. Hypotension resulted approximately 3 h into dialysis in the present study. This point in time was that at which blood volume was more than 5% above the minimum level during the dialysis treatment, and in some patients it was more than 10% over the minimum level of blood volume. At the time dialysis-induced hypotension occurred or before, there was no sharp decrease in blood volume nor any change in the plasma refilling rate. This suggested that this hypotension is caused by a sudden breakdown of the blood pressure support mechanism compensating for decreased blood volume.

Foreign particles contaminating hemodialyzers and methods of removing them by rinsing.

Foreign particles contaminating hemodialyzers constitute a risk of microembolism and allergic reactions in hemodialysis patients. We investigated the size distribution of particles, and the effects of striking headers of dialyzers and flow rates of rinsing saline on the elimination of foreign particles from dialyzers. Saline used for rinsing was filtered through a Millipore filter, and the particles thus removed were observed microscopically. We also checked changes in particle counts in a closed circuit consisting of a dialyzer and blood lines during 5 h of continuous circulation with a blood pump. The number of contaminating particles increased exponentially as their size decreased. For the sake of reproducibility we counted particles having diameters larger than 3.0 micron to monitor the rinsing efficiency. During rinsing, particles in the dialyzer were effectively eliminated by striking the headers of the dialyzer. We found that striking should be started immediately after the blood line is filled with saline, and that variation in saline flow rates in the range of 350-700 ml/min of saline does not affect the rinsing efficiency. By filtration of saline used for rinsing, particles, mostly ranging from 5 to 200 micron in length, of many shapes and colors were found. Among them were fibers having a length of as much as a few millimeters. Once dialyzers were rinsed effectively, there was almost no change in particle counts in a closed circuit consisting of a dialyzer and blood lines. To rinse dialyzers effectively, at least 1,000 ml of saline are necessary, and striking the headers of dialyzers throughout the rinsing procedure is important.

Effects of treadmill exercise on platelet function, blood coagulability and fibrinolytic activity in patients with atrial fibrillation.

The effects of treadmill exercise on platelet function, blood coagulability and fibrinolytic activity were evaluated in 20 patients with lone atrial fibrillation (AF) and 15 age-matched normal controls (normals). Multistage treadmill exercise up to 85% of the predicted maximal heart rate was performed, and blood for measurements was obtained pre-exercise, and immediately and 6 min post-exercise. There was an increase in the platelet sensitivity to ADP-aggregation after exercise in both groups. Pre-exercise plasma beta-thromboglobulin (beta-TG) levels were higher in AF than in normals. Beta-TG increased after exercise in both groups (immediate post-exercise; 35.1 ng/ml for normals and 62.8 ng/ml for AF), and the increase was greater in AF than in normals. PT and APTT shortened, and plasma fibrinogen levels increased after exercise in both groups. Pre-exercise levels of plasma ATIII and protein C were lower in AF than in normals. These two proteins increased after exercise in both groups. However, the increase was greater in normals. Plasma alpha 2-PI increased after exercise in both groups; the level was lower in AF than in normals at each exercise stage. In conclusion, enhanced platelet activity, and lower levels of anticoagulant and antifibrinolytic activity were observed in AF not only at rest but also after treadmill exercise. These changes might reflect the hypercoagulable state in patients with AF. It is speculated that the risk of thromboembolic complications may be enhanced with exercise in AF patients.

Influence of modes of ACNU administration on tissue and blood drug concentration in malignant brain tumors.

The water-soluble nitrosourea compound ACNU is also lipid-soluble at normal physiological pH levels. It lacks toxic effects on vision that nitrosoureas occasionally produce following intra-arterial administration. In 28 cases of both primary and secondary malignant brain tumors, ACNU was administered at surgery or angiography by three different modes: intravenous injection in Group I (10 cases), intra-arterial injection via the carotid artery in Group II (11 cases), and intra-arterial injection via the carotid artery after opening the blood-brain barrier (BBB) by means of mannitol in Group III (seven cases). Tumor tissue and blood samples were taken serially at various time intervals after ACNU injection, and ACNU was measured by high-performance liquid chromatography. The time-concentration curve for ACNU was calculated in each case by the two- and one-compartment open models for determination of ACNU levels in blood and tissue, respectively. Pharmacokinetic parameters including biological half-life, blood and tissue levels (0-t minutes and 0-infinity minutes), total plasma clearance, and distribution volume of the beta phase were compared. Statistical analysis of tissue ACNU levels at 0-t minutes revealed higher concentrations in Group III patients than in Groups II and I: levels in Group II were significantly higher than in Group I. Mean biological half-life was 30.3, 23.0, and 38.5 minutes in Groups I, II, and III, respectively. Levels of ACNU were significantly increased in tumor tissue as well as in peritumoral tissue in one Group III patient with multiple metastatic anaplastic adenocarcinoma. In this series, treatment of malignant brain tumor by intra-arterial administration of ACNU produced significantly higher tissue levels of ACNU than did the systemic intravenous route.

Non-linear pharmacokinetics of aprindine hydrochloride in oral administration.

The pharmacokinetics of oral aprindine hydrochloride (in the following briefly called aprindine; Aspenon) were studied in 38 patients with ventricular premature contractions following single or multiple administration. Oral administration of aprindine in a single dose of 100-150 mg resulted in a mean maximal plasma concentration of 0.77 microgram/ml and a mean elimination half-life of 26.5 h. With multiple oral administration in 10 mg and 20 mg doses at intervals of 8 h, plasma concentration reached a steady state in 1-2 weeks with either dosage rate. Stable plasma concentrations were maintained with little diurnal or day-to-day fluctuation. The mean steady-state minimal plasma concentration with a 10 mg dosage was 0.28 microgram/ml. With a 20 mg dosage, however, this was more than tripled to 0.89 microgram/ml. The elimination process after the final administration of the drug was slower than with single dosage, and deviated from the first-order kinetics to produce a convex curve on a semilogarithmic graph paper. From these results it was apparent that aprindine shows non-linear pharmacokinetic behavior within the therapeutic dosage range.

Production and characterization of rat monoclonal antibodies with an apparent specificity for certain lung cancers and their potential clinical application.

This paper describes two rat monoclonal antibodies (MAbs) (1A6 and 2H7, both IgG2a) directed against a human adenocarcinoma of the lung (A549). Enzyme-linked immunosorbent assay (ELISA) and immunocytochemistry (Avidin-Biotin-complex method) as the screening of the hybridoma cells revealed that 1A6 and 2H7 MAb mainly reacted with adenocarcinoma of the lung, breast and pancreas, and 2H7 MAb also recognized large cell carcinoma, epithelia of the lung, trachea, tongue or small cell carcinoma of the lung. The antigen molecules recognized by 1A6 MAb was not detected by immunoprecipitation but the molecular size of the antigen by 2H7 was ca. 40Kd. The antibody-producing heterohybridoma cells could be transplanted into peritoneal cavities of nude mice to form ascites. We found these monoclonals useful for immunohistodiagnosis of surgical specimens from patients with lung cancer. Furthermore, we utilized 125I-labeled 2H7 MAb, aiming at localizing A549 tumor mass transplanted into nude mice subcutaneously. This antibody clearly localized the tumor mass. It is suggested that they may be useful as both diagnostic and therapeutic agents.

Human interferon-gamma (IFN-gamma) containing cells bear various surface phenotypic markers.

We explored the population of Interferon-gamma (IFN-gamma) containing cells in order to clarify their cell surface phenotypic markers. Here we define gamma-IFN containing cells as gamma-IFN plaque forming cells (PFC). By this method, it was found that IFN-gamma containing cells consist of two cell fractions, i.e., OKT3+, OKT4+, and OKT8- cells and OKM1+ cells. Effective IFN-gamma production seems to require participation of plastic-adherent cells (presumably monocytes), while the addition of cyclosporin A (CyA) almost completely blocked generation of human IFN-gamma. To characterize Con A-stimulated IFN-gamma containing cells, we performed two-color flow cytometry using FACS IV. Most of the IFN-gamma containing cells have surface phenotypic markers for Leu3, Leu8, Leu15, HLA-DR, and IL-2 receptors, but most lack markers for Leu2 and Leu7. Interestingly, most of Leu3+ and IL-2 receptor+ cells belong to the dimly illuminating cell fractions of the IFN-gamma containing cell population. Our results indicate that IFN-gamma containing cells are heterogeneous with respect to surface phenotypic markers but the predominant IFN-gamma containing cell type is the helper T cell (OKT4+). Lastly, OK432, glycyrrhizin, and CCA (lobenzarit disodium) increase the number of IFN-gamma containing cells and are thought to be immunomodulators.
