A case of congenital internal mammary artery-to-pulmonary artery fistulas is reported. The patient was a 7-year-old boy in whom this condition was suspected from an abnormal shadow in the left upper lung field upon chest X-ray examination and continuous murmur. A definite diagnosis was made by selective angiography of the internal mammary artery. This condition is extremely rare. Our patient is the youngest among the 16 cases which have been reported so far in the English literature. Surgical treatment may be indicated because there is a risk of rupture of the fistula, endarteritis, and congestive heart failure.
A patient with a functioning intrapericardial paraganglioma (pheochromocytoma) that was located at the root of the aorta overlying the right coronary artery and adherent to the right ventricular wall is reported. The tumour was successfully removed under total cardiopulmonary bypass without inducing cardiac arrest.

Intraarterial digital subtraction angiography (DSA) was applied 44 times to 39 neurosurgical cases. We emphasize the advantage of DSA in relation to conventional angiography. The examination consisted of 25 aortography and 19 selective arteriography. By single small dose injection of contrast medium using DSA, aortic arch and thoracic major vessels were well visualized, so that we could obtain useful information for catheterization to the selective artery. High time resolution of DSA made possible to recognize dynamics of circulation in very short interval, especially in the case of subclavian steal syndrome and right aortic arch. Major four vessels in the cervical region could be visualized by single injection of contrast medium using DSA. Stenosis of carotid artery was recognized by DSA as well as conventional angiography. By aortic injection using DSA, circulatory dynamics of intracranial vessels including collateral circulation were recognized especially in the case of moyamoya disease. Gradual injection of small dose contrast medium reduced jet-effect of the catheter tip and enabled the selective arteriography. Real time display of DSA reduced the time required for examination and enabled repeated angiography to recognize the change of hemodynamics under the different limb position in the case of thoracic outlet syndrome.
Recently, operative results of intramedullary spinal cord tumors have been greatly improved since the introduction of microsurgery. It is very important to know the precise size and location of the tumor prior to the operation so that we can approach the tumor with a minimum of damage to the spinal cord. However, it is not always possible to demonstrate the precise localization of the tumor preoperatively. In this report, we emphasize that intraoperative spinal sonography is very useful in determining the extent of the tumor and differentiating solid component from cystic component of the tumor. Methods and Materials We performed intraoperative spinal sonography on ten patients with intramedullary spinal cord tumor. This series included three cases of hemangioblastoma, three cases of astrocytoma, two cases of ependymoma, one case of subependymoma, and one case of mixed glioma. Eight out of ten cases were associated with cysts. The intraoperative spinal sonographic examinations were performed after laminectomy. The linear scanning probe of 5 or 7.5 MHz transducer was used. Results 1) Solid components The acoustic pattern of the solid tumor was either hyperechoic or iso-echoic. Six cases (three hemangioblastomas, two ependymomas, and one astrocytoma) were hyperechoic. Other four cases (two astrocytomas, one subependymoma, and one mixed glioma) were iso-echoic. 2) Cystic components The cysts associated with the tumor were anechoic in six out of eight cases, which were confirmed at surgery, and multiple cysts were identified.(ABSTRACT TRUNCATED AT 250 WORDS)

Plasmacytoma often forms an intramedullary mass in the vertebrae with absorption of trabecula and cortex of the bone. However, occasionally, it forms a mass in the extramedullary space of the vertebrae. The authors report such a rare case with plasmacytoma which formed a mass in the thoracic epidural space without evidence of involvement of the adjacent vertebra. On May 22, 1985, a 80-year-old man was admitted to our clinic with chief complaints of gait disturbance and hypesthesia below the umbilical level. Difficulty of walking developed approximately four months prior to admission with gradual aggravation and hypesthesia added thereafter. Neurological examinations at admission showed paraparesis with positive Babinski's and Chaddock's reflexes, hypesthesia and disturbances of vibration and position senses below the 9th thoracic nerve level. Myelography and CT scan using metrizamide indicated a presence of epidural mass at the 8th to 9th thoracic vertebrae. There was no abnormal bony change in the spine on plain X-ray and CT scan. On May 30, 1985, total removal of epidural tumor was performed by removing the laminae from the 7th to 10th vertebrae. Histological examinations including immunological stainings showed a plasmacytoma which produced monoclonal immunoglobulin of IgG-lambda type. Radiation therapy was not carried out. The serum protein fraction, immunoglobulin, immunoelectrophoresis, Bence-Jones protein and CSF immunoglobulin examined after operation, supported a histological diagnosis of plasmacytoma. Also, slight proliferation of plasma cells was noted in the bone marrow and peripheral blood.(ABSTRACT TRUNCATED AT 250 WORDS)

In Hokkaido, 113, 612 women were subjected to mass screening of breast cancer from 1973 to 1983. Out of them, 74, 285 (56.3%) were examined once and the remainder twice or more: 4,798 (4.2%) were advised to receive further precise examinations and 99.8% of them underwent the second examination. Breast cancer was detected in 377, in which 291 (77.2%) were detected by the first examination and 33 (3.8%) were so-called "interval cancer" with the mean interval of 19.3 months. The detection rate was 0.3%. Histologically non-invasive cancer was seen in 26 (6.9% of 377). Despite a recent significant increase in detection rate of the earlier stage of breast cancer, the present study showed no evidence of a decrease in advanced cancer. The prognosis of the patients who had curable operation revealed 86.9% in 5 year survival rate and 40% in 10 year survival rate.

A 14 year-old woman was admitted to Tokyo Women's Medical College Daini Hospital complaining of lumps in bilateral breasts and right supraclavicular region. An excisional biopsy of right breast lump was done. Light- and electron-microscopic examination demonstrated abnormal cells containing many PAS-positive granules shich disappeared after diastase treatment. The diagnosis of Ewing's sarcoma was made immunohistologically using NSE, S-100, keratin, prekeratin, cytokeratin, desmin, epithelial membrane antigen and neurofilament. Whole body CT scan revealed large mass in the pelvic cavity considered as primary lesion. CYVADIC therapy was very effective. Breasts and supraclavicular lesions almost disappeared and pelvic lesion decreased in size.
Two kinds of mussel, Septifer virgatus and Mytilus edulis, were collected from Kyushu island, Japan, in order to elucidate a background level of 60Co, which is one of the most significant radionuclide for environmental monitoring around a nuclear power plant. The mussels were collected from 7 locations in 1983 and classified 2 or 3 groups depending on their shell size at each location. Activities of 60Co were measured by a low-background beta counter after purified by means of chemical separation and electrodeposition. Stable cobalt concentrations were determined by colorimetric method. The concentrations of cobalt in Septifer virgatus are one order higher level than that in Mytilus edulis. There are not so large difference in cobalt content depending on shell size so long as comparing them at the same location. The radioactivities in mussels show the same trend as stable cobalt. It has become apparent that Septifer virgatus has a tendency to concentrate cobalt with growing but Mytilus edulis is opposite. The cobalt-60 introduced to sea from nuclear explosions seems to be relatively constant in coastal seawater since specific activities are distributed in a narrow range in spite of kind, shell size and location.
